Objective To evaluate the safety and efficacy of epirubicin (EPI) and gemcitabine (GEM) alternating sequential intravesical chemotherapy after transurethral resection in the treatment of non-muscle invasive bladder cancer.Methods 240 patients with primary non-muscle invasive bladder urothelial carcinoma were randomly divided into 2 groups.There were 120 cases for each group,EPI group was given EPI 50 mg (once a week),bladder perfusion,while the EPI+GEM group was given EPI 50 mg (once every other week),GEM 1000 mg (once every other week),alternating sequential perfusion.The follow-up time ranged from 6 to 24 months when the time of tumor recurrence and adverse reactions of chemotherapy were observed and recorded.Results The 2-year tumor free survival rate for EPI group recurrence was 60.0 ％ (72/120),and 75.0 ％ (90/120) for EPI+GEM group.There were statistical significance between the differences of the 2 groups (x2 =5.489,P ＜ 0.05).3 cases in EPI group and 2 cases in EPI+GEM group progressed to muscle invasive bladder cancer,and there was no statistical significance between the differences of bladder cancer progression rates for the 2 groups (2.5 ％ and 1.7 ％) (x2 =0.000,P ＜ 0.05).The main adverse reaction during the treatment was gastrointestinal discomfort (15 cases and 6 cases respectively),no serious haematological toxicity and other adverse reactions were frequent urination,urgency,dysuria and hematuria.The differences between the occurrence rates of adverse reactions for the 2 groups were 25.0 ％ and 11.7 ％,which were statistically significant (x2 =6.252,P ＜ 0.05).Conclusion For non-muscle invasive urothelial bladder carcinoma,the curative effect is better to use EPI and GEM sequential intravesical chemotherapy than to use EPI alone,which can not only reduce the 2 years recurrence rate after the operation,but also reduce the incidence rate of adverse reactions.Yet,this method cannot change the progress of bladder cancer.

Objective To investigate the relationship between the ankle-brachial index (ABI)and the extent of coronary artery stenosis and the left ventricular function in elderly patients with coronary heart disease (CHD)with metabolism syndrome (MS). Methods A total of 140 patients with coronary artery disease was study subjects. In which, 66 cases complicated MS (MS group), 74 cases without MS (non MS group). ABI and the left ventricular function was examined in an patients. Results ABI was 0.90±0.32 in MS group, and 1.03±0.26 in non MS group, there was significant difference between two groups(P=0.015). Compared with non MS group, significant lower ratio of ABI in MS group [43.9%(29/66) vs 27.0%(20/74)] (P=0.038); whereas much more 3-vessel disease in ABI≤0.9 patients compared with those of ABI > 0.9 patients [86.3%(25/29) vs 37.8%(14/37) ], significant lower of the left ventricular function in ABI≤0.9 patients compared with those of ABI >0.9 patients (P<0.05). Conclusion There is more severe of coronary artery stenosis in ABI≤0.9 patients for CHD with MS, more attention should be payed to CHD patients combined with MS.

Objective To explore the value of transvaginal color doppler ultrasound(TVS) in the differential diagnosis of endom-etrial hyperplasia(EH) and endometrial cancer(EC) .Methods The clinical data concerning TVS in 48 patients with EH and 37 pa-tients with EC were reviewed retrospectively ,then we compared the ultrasonic diagnosis with the pathological diagnosis to observe the preoperative diagnostic accuracy of ultrasonic diagnosis .Results In 48 patients with EH ,the inspection results of 44 cases were accordant with the pathological diagnosis .The rest of 4 cases were misdiagnosed by TVS as EC before operation ,and the accurate rate is 91 .67% (44/48);In 37 patients with EC ,the checking results of 34 cases were consistent with the pathological diagnosis . The rest of 3 cases were misdiagnosed by TVS as endometrial atypical hyperplasia ,and the accurate rate is 91 .89% (34/37) .Con-clusion TVS has diagnostic significance in the differential diagnosis of EH and EC .

Objective To examine the role of adenovirus cytotoxic T lymphocytic associated antigen 4(Ad-CTLA4Ig) in treatment of induced Sjogren syndrome(SS) in mice.Methods SS was induced in 30 BALB/c mice by challenging with the mixture of homologous antigen from submandibular gland tissues and complete freunds adjuvant(CFA).Two hours after challenge,Ad-CTLA4Ig was intraperitoneally injected in the experimental mice(n=10),while thymic peptide in control mice(n=10).Morphological changes of submandibular gland,water intake and static total saliva flow rate of each group were observed.Results There was no obvious pathological change in Ad-CTLA4Ig treated group,in which the static total saliva flow rate was significantly higher than that in control groups(P

OBJECTIVE:To determine serum concentrations of western medicines in patients treated with "traditional Chinese antiepileptic medicine" alone and to evaluate the curative effects.METHODS:A total of 60 epileptic patients who visited our hospital between Feb.1997 and June 2006 were subjected to plasma drug level monitoring and during which the patients were treated with "traditional Chinese antiepileptic drugs" alone.Plasma concentrations of 4 kinds of western medicin-es were determined by FPIA.RESULTS:Of the 60 cases,valproic acid,carbamazepine,phenytoin and phenobarbitone were detected in 18,40,41,and 47 cases/times,respectively.On average,more than two kinds of western medicines were detected in every patient,and the blood concentrations were mostly beyond effective plasma drug concentration.The total curative effects were unsatisfactory.CONCLUSION:The fact that western medicine ingredients detected in these traditional Chinese antiepileptic medicines is inconformity with medication principle of epilepsy.Traditional Chinese antiepileptic medicines should be used with caution in the clinic in the treatment of epileptic patients.

OBJECTIVE:To improve the efficacy and safety of high dose methotrexate (HDMTX)chemotherapy in the treatment of children with acute lymphoblastic leukemia through blood concentration monitoring.METHODS:27 children with acute lymphoblastic leukemia who received high dose of HDMTX (1.5～4.0 g/m2)for 55 times were involved in this study,the blood samples were collected timely and the blood concentration of methotrexate was determined,the efficacy of the chemotherapy was evaluated according to blood MTX concentration at the ending of its intravenous drip (12h),the calcium folinate relief scheme was determined from the MTX serum concentration in terminal elimination phase.RESULTS: The times for 2.0 g/m2,3.0 g/m2 and 4.0 g/m2 different dosage of methotrexate groups with blood concentration maintained above osmotic concentration(2?10-5mol/L)at the end of intravenous drip (12h)were respectively 75%,92.1%and 100%of the total chemotherapy times.Only one patient was observed with large area of impairment of skin and mucosa,and no severe irreversible adverse reaction were observed in the other cases.CONCLUSIONS: MTX serum concentration monitoring is helpful for mastering the rational rescue dosage of MTX and calcium folinate so as to ensure the efficacy and safety of the chemotherapy.

In this paper,a pulsed magnetic fields stimulation instrument is designed and realized,which provides a pulsed magnetic field with the range of maximal intensity from 0.01~2T,frequency from 0.2~100Hz and time width of pulse from 0.01~1ms.The instrument,controlled by the hand,foot or itself,can display stimulus intensity and times and output trigger signals with different waveforms to make measuring devices operate synchronistically.

Pesticides, especially chemistry pesticides with high toxicity, high residue, and difficult degradation are a kind of important environment pollutants and pesticide degradation by microorganisms is one of the powerful means to treat pesticide pollution. Many researchers conducted lots of studies on it. Types of pesticide degraders, construction of genetically engineered microorganisms, degrading mechanisms, degradation characteristics, influencing factors, applying effect and so on were summarized in this article. The research trend of degradation of pesticides by microorganisms and problems to be solved were also put forward.

Objective To investigate the environmental quality and water quality of bathing beach in Haikou,Hainan province and the effects on human health.Methods The investigation of the bathing beach environment and the water quality were conducted according to Bathing Beach Monitoring Regulations(2002).The water samples were collected at PM 3:00-5.00 on May 28th to June 3th in 2007 from three sampling sites,No.1(easily be polluted),No.2(crowded),No.3(less pollution).The questionnaire survey was conducted on the present day and 7 days later,the items included the respiratory,digestive systems infection and eye,ear,nose and skin inflammation,the survey was completed in two days.Results The environment survey of bathing beach suggested that non-point source pollution caused by domestic wastewater emerged on raining days,the salinity was 31‰-33‰,clarity exceeded 30 centimeter and no heat pollution was found.The pH value,chroma,dissolved oxygen,nitrate nitrogen,chemical oxygen demand,inorganic nitrogen,fecal coli group was 8.01-8.10,11.42-15.00 NTU,5.60-6.71 mg/L,0.16- 0.17mg/L,2.70-3.40mg/L,0.19-0.21 mg/L and 35.0-36.0/L respectively.Four hundred and eight questionnaires were eligible, the response rate was over 90%.The results demonstrated that the swimmers were facing to the increased illness risk from the water quality,such as skin pruritus,gastrointestinal allergies,throat sore,eye and ear and nose infections.There was distinct higher proportion in the participants with water contact and complained one or more symptoms than those without water contact(P
