AlM:To investigate the effects of pirfenidone ( PFD) on the proliferation and transfomring growth factor-β1 ( TGF-β1 ) expression in vitro culture rat corneal stromal cells.METHODS: Corneal stromal cells from 8 to 10wk SD rats were isolated, cultured and treated with different concentrations of PFD 0mg/mL (control group), 0. 15mg/mL (experimental group▏), 0. 3mg/mL (experimental group‖), 1mg/mL (experimental group Ⅲ) for 48h. CCK-8 assay was performed to assess cell proliferation, while immunocytochemistry and Western Blot were used to detect the expression of ki-67 and TGF-β1 expression, respectively. RESULTS: Compared with control group, PFD significantly inhibited the proliferation in a dose -dependent manner ( all P < 0. 05 ), so was protein expression of ki-67. PFD significantly down-regulated the expression of TGF-β1 in a dose-dependent manner (P<0. 05).CONCLUSlON: Pirfenidone can significantly inhibit the proliferation of rat corneal stromal cell by down regulating TGF-β1 expression, therefore, it has potential prospect in lightening the corneal wound healing reaction.

Objective To study the efficacy of low-dose daytime ambulatory peritoneal dialysis (DAPD) and low-dose CAPD in diabetic end-stage renal disease (ESRD) patients with better residual renal function (RRF). Methods Forty stable diabetic ESRD patients with better RRF (rGFR ≥ 5 ml/min and urine volume ≥ 750 ml/d) were enrolled. They were randomly divided into two groups: low-dose DAPD group (n=20) and low-dose CAPD group (n=20). DAPD group received three 1.5 L to 2 L daily exchanges with a nocturnal empty belly, dwelling for 3 to 4 hours. CAPD group received three 1.5 L to 2 L daily exchange or four 1.5 L daily exchange regimens and dwelled during the night. At the beginning of the study and 6 months later, total weekly Kt/V and Ccr (peritoneal+renal), rGFR were calculated. Meanwhile 24-hour urinary protein,serum albumin (Alb), hemoglobin (Hb), fasting plasma glucose, glycosylated hemoglobin and insulin dosage were measured. Nutritional status was assessed by SGA. Results Thirty-five patients fulfilled the study. There were no significant differences between two groups in age, gender, BMI,PD time, D/Pcr, etc. At the end of the 6th month, the insulin dose[(33.6±10.9) U/d] and 24-hour dialysate protein [(11.13t4.95) g] in CAPD group were significantly higher as compared to DAPD group [(20.6±6.2) U/d, P＜0.05 and (5.66±2.88) g, P＜0.01 respectively]. Alb in CAPD group [(29.7±4.2) g/L] was significantly lower than that in DAPD group [(36.5 ±3.9) g/L, P＜0.05].While the net ultrafiltration [(554±187) ml vs (309±177) ml], 24-hour urine volume [(1090±361)ml vs (750±258) ml] and rGFR [(8.21±2.40) ml/min vs (4.88±2.11) ml/min] in DAPD group were all significantly higher than those in CAPD group (all P＜0.05). Conclusion For the diabetic ESRD patients with better RRF, the low-dose DAPD regimen is more effective to control plasma glucose, improve nutritional status and protect RRF than the low-dose CAPD.

Objective To investigate the potential value of MR dynamic contrast enhanced imaging (DCE)in predicting short-term effects of(chemo-)radiation therapy on cervical cancers in vivo.Methods Thirty-two patients pathologically diagnosed as cervical cancers received MR-DCE scan before therapy.The following scans were taken at the 2nd to 3rd week after the radiation or(and)chemotherapy initiated,to calculate early size change.The fourth scan was performed within one month after the therapy to judge residues.DCE derived parameters include mean time to enhancement(MTE),positive enhancement integral (PEI),time to peak(TTP),maximum slope of increase(MSI)and maximum slope of decrease(MSD).Size and DCE parameters were compared and analyzed by Mann-Whitney test and Spearman teat Results There are 20 cases in non-residue group and 12 cases in residue group.In non-residue group,the maximal lengths in both horizontal and craniocaudal directions[(47.45±13.76)mm and(38.75±10.69)mm]were smaller than that in residue group [(61.08±13.73)mm and(53.67±16.05)mm],with statistical significance(Z=-2.455 and-2.650 respectively,P＜0.05).PEI in low-enhance areas of non-residue group(0.62±0.25)was significantly higher than that in residue group(0.39±0.14)(Z=-2.919,P＜0.05).At early time after radiation started,length changing rate and volume changing rate had medium to strong negative correlations with MSI in hish-enhance areas(r=-0.577 and-0.723 respectively,P＜0.05).Conclusions For cervical cancers,those with larger lengths and lower PEI in low-enhanced areas tend to have tumor residue when radiation therapy completed.Those low MSI in high-enhance areas tend to shrink quickly.It is suggested that volume and parameters of DCE before(chemo-)radiation therapy have the potential in predicting short-term therapeutic effect.

Objective:To observe the clinical effects of tuina plus Western medication for functional dyspepsia (FD) due to liver qi stagnation and spleen deficiency.Methods:A total of 72 patients in conformity with the inclusion criteria of FD were randomly divided into an observation group and a control group based upon the random number table,36 cases in each group.The control group was treated with mosapride citrate dispersible tablets,and the observation group was treated with the same tablets plus tuina.Before the treatment and 4 weeks after the treatment,the clinical symptoms,quality of life (QOL) and depression severity were observed by the scale,and were followed up two months later after the treatment for assessment of the clinical effects.Results:After the treatment and at the follow-up,the symptom scores of FD and the sores of Hamilton depression rating scale (HAMD) in both groups decreased,and the scores in Chinese version of quality of life questionnaire for functional digestive disorders (Chin-FDDQL) increased,with statistically significant differences in comparison with the same group before the treatment (all P＜0.05).In comparison between the two groups at the same time point after the treatment,the scores of FD symptoms,HAMD and Chin-FDDQL were improved better in the observation group than those in the control group,with statistically significant differences (all P＜0.05).The total effective rates at the follow-up were 91.7％ in the observation group and 75.0％ in the control group,without statistical difference between the two groups (P＞0.05).The rate of clinical cure and remarkable effect was 66.7％ in the observation group,higher than 41.7％ in the control group,it is higher in the observation group than that in the control group,with a statistically significant difference between the two groups (P＜0.05).Conclusion:Tuina plus Western medication is precise in the therapeutic effects for FD due to liver qi stagnation and spleen deficiency and can effectively relieve clinical symptoms,elevate the QOL and alleviate depression severity of the patients.Moreover,it's better than the treatment by Western medication alone in the long-term therapeutic effects.

Objective To improve the anderstanding of clinical profile of primary plasma cell leukemia.Methods Case report and literature review.Results A rare case of nervous system relapse in primary plasma cell leukemia was reported.Six patients were identified from the literature.The type of immunoglobulin included IgG(3 patients),IgD(2 patients).Clinical manifestations of nervous system were variable.The average interval from initial diagnosis to the development of nervous system relapse was 16.5 months.Plasma cells were found in cerebrospinal fluid in 4 patients.The mean surviaval time was 6.7 months after nervous system relapse.Conlusion Nervous system relapse in primary plasma cell leukemia is rare with poor prognosis.

OBJECTIVE: To study the evidence of adenosine triphosphate (ATP) in the marginal cells of the stria vascularis in the neonatal rat cochlea, namely, whether ATP vesicles could be detected in the marginal cells and ATP release could be detected in the culture solution in vitro. METHOD: The marginal cells of 1-3 day old Sprague-Dawley rats were cultivated, purified and verified. ATP vesicles Were observed in the marginal cells under fluorescence microscope using quinacrine staining technique. The concentration of ATP released from the cells in the extracellular solution was determined through bioluminescence assay. RESULT: The marginal cells were verified in vitro by detecting antibodies of cytokeratin and vimentin using flow cytometry. A large number of anterior-like staining could be observed in cultured marginal cells that were incubated with quinacrine. The concentration of ATP released from the cells in the culture solution could be determined, and the concentration of ATP could be calculated by fluorescent intensity. CONCLUSION The ATP vesicles exist in the marginal cells and can release ATP.

Background The double kissing (DK) crush technique is a modified version of the crush technique.It is specifically designed to increase the success rate of the final kissing balloon post-dilatation,but its efficacy and safety remain unclear.Methods Data were obtained from the DKCRUSH-I trial,a prospective,randomized,multi-center study to evaluate safety and efficacy.Post-procedural and eight-month follow-up intravascular ultrasound (IVUS) analysis was available in 61 cases.Volumetric analysis using Simpson's method within the Taxus stent,and cross-sectional analysis at the five sites of the main vessel (MV) and three sites of the side branch (SB) were performed.Impact of the bifurcation angle on stent expansion at the carina was also evaluated.Results Stent expansion in the SB ostium was significantly less in the classical crush group ((53.81±13.51)％) than in the DK crush group ((72.27±11.46)％) (P=-0.04).For the MV,the incidence of incomplete crush was 41.9％ in the DK group and 70.0％ in the classical group (P=-0.03).The percentage of neointimal area at the ostium had a tendency to be smaller in the DK group compared with the classical group ((16.4±19.2)％ vs.(22.8±27.1)％,P=-0.06).The optimal threshold of post-procedural minimum stent area (MSA) to predict follow-up minimum lumen area (MLA) ＜4.0 mm2 at the SB ostium was 4.55 mm2,yielding an area under the curve of 0.80 (95％ confidence interval:0.61 to 0.92).Conclusion Our data suggest that the DK crush technique is associated with improved quality of the final kissing balloon inflation (FKBI) and had smaller optimal cutoff value of post-procedural MSA at the SB ostium.

OBJECTIVETo explore the relationship between the levels of free triiodothyronine (FT3), free thyroxine (FT4), cortisol (CO), testosterone (T), serum estradiol (E2) and Cold Syndrome and Heat Syndrome in traditional Chinese medicine.

METHODSThe blood levels of FT3, FT4, T, E2, CO in groups of sthenic Heat (SH), sthenic Cold (SC), asthenic Heat (AH) and asthenic Cold (AC) and the healthy control group were measured. The number of cases in each group was 50.

RESULTS(1) FT3 and FT4 were higher in Heat Syndrome than that in Cold Syndrome in the order of AH > SH > control > SC > AC, it revealed that the basal metabolic rate and oxygen consumption in Heat Syndrome were increased, while it in Cold Syndrome, it declined. (2) CO was higher in SH, SC than that in AH and AC, the order was SC > SH > control > AH > AC, it revealed that CO was one of the factors correlated to asthenic and sthenic syndrome, but not relevant to Heat Syndrome and Cold Syndrome. (3) T was higher in Heat Syndrome than that in Cold Syndrome, but E2 showed the opposite, suggesting that T was one of the factors for forming Heat Syndrome, and E2 for Cold Syndrome.

CONCLUSIONThe levels of FT3, FT4, T, E2 are basis of pathophysiology related with forming HS and CS.
