1.Therapeutic Observation of Wrist-ankle Acupuncture for Pain after Laparoscopic Cholecystectomy

Objective To observe the clinical efficacy of wrist-ankle acupuncture in treating pain due to laparoscopic cholecystectomy.Method Totally 150 patients who were going to receive laparoscopic cholecystectomy were randomized into group A, group B, and group C, 50 cases in each group. Group A was intervened by wrist-ankle acupuncture prior to anesthesia, with the needles retained for 12 h; group B was by subcutaneous needling at the area nearby the points prior to anesthesia, with the needles retained for 12 h; group C didn’t receive any intervention before anesthesia. For the three groups, general inhalational and intravenous anesthesia was adopted for surgery, and patient-controlled intravenous analgesia for post-operation analgesia. The incision pain and visceral pain in the three groups were recorded by using Visual Analogue Scale (VAS) respectively 1 h, 2 h, 4 h, 8 h, 12 h, 24 h, 36 h, and 48 h after the operation. The total effective rate, analgesics consumption after operation, and incidence rate of adverse reaction were compared.Result There were significant differences in comparing the VAS scores of incision pain and visceral pain between group A and group C 4 h, 8 h, 12 h, 24 h, and 36 h after the operation (P<0.01,P<0.05). Between group A and group B, there were significant differences in comparing the VAS score of incision pain 8 h, 12 h, 24 h, and 36 h after the operation and the VAS score of visceral pain 12 h, 24 h, and 36 h after the operation (P<0.05). The total effective rate was 96.0% in group A, which was significantly different from 84.0% in group B and 86.0% in group C (P<0.05). The consumption of Fentaneyl citrate injection was (52.4±10.8)μg in group A, which was significantly different from (92.2±11.0)μg in group B and (107.2±11.5)μg in group C (P<0.05,P<0.01). The incidence rate of adverse reactions was 12.0% in group A, which was significantly different from 58.0% in group B and 66.0% in group C (P<0.01).Conclusion Wrist-ankle acupuncture plus patient-controlled intravenous analgesia can mitigate pain after laparoscopic cholecystectomy, and thus it can be taken as one of the post-operational analgesic approaches.

3.Simultaneous determination and correlation study on the active constituents in Salvia miltiorrhiza Bge

AIM:To establish a simple and accurate HPLC method for quality evaluation and determination of 8 active constituents, i.e. Danshensu, Protocatechualdehyde,rosmarinic acid,Salvianolic acid B, Dihydrotanshinone, Cryptotanshinone, Tanshinone Ⅰand Tanshinone ⅡA in Radix Salviae Miltiorrhizae, at the same time, in order to study the correlations of the contents of the 8 constituents. METHODS: Detected at 286 nm, analytes were separated on a ODS C_ 18 column(150 mm?4.6 mm, 10 ?m) eluted gradually with methanol and 1% tetrafuran in 1% formic acid water solution(3∶97)on the increasement of methanol from 3% to 70% in the whole analysis. After determination, SPSS 13.0 software was used to calculate the correlation between the 8 constituents. RESULTS: Under the above conditions, HPLC method was constructed to quantify 8 constituents,and their correlation coefficients were calculated. CONCLUSION: The result shows that the contents of the 8 constituents in Salvia miltiorrhiza Bge. vary greatly from different origins; and there are significant correlations between the contents of hydrophilic constituents such as Salvianolic acid B,Danshensu,rosmarinic acid, so do hydrophobic constituents like Cryptotanshinone,Tanshinone Ⅰ,Tanshinone ⅡA,but there is no correlation between the hydrophilic constituents and hydrophobic constituents.
4.The clinical efficacy evaluation of transcatheter hardening treatment for varicose veins of lower extremity

Objective To explore the clinical efficacy and safety of transcatheter absolute ethanol injection treatment on varicose veins of lower extremity.Methods twenty-there patients with 25 varicose veins of lower extremity were treated by puncture of great saphenous vein above 1—2 cm of complicated inner ankle,perforating catheter to the point below the 3—4 cm of the conjunction of great saphenous vein and Femoral vein and pressing the conjunction of these two veins.Under the monitor of DSA,inject the absolute ethenal slowly while retrieve the catheter little by little(one limb with varicose veins injected total volume 15—20 ml),in the mean time,using contrast agent to monitor the level of embolism until the formation of total embolism in the all great saphenous veins.Results All the cases were retrospectively followed up with CDFI examination after 3—12 months of the surgery,No blood flow were seen in the 25 embolismic great saphenous vein.Clinical symptom were alleviated obviously after 2—3 weeks of treatment;varicose veins were collapse after 3 to 7 days.Two eases of leg ulceration were healed after 4 to 6 weeks of operation.20 limbs were found mild swelling in the 2 day after the surgery.However,all the cases were disappeared after 1 to 2 weeks;4 treated limbs developed delayed paresthesia in the 3 day after the surgery,and recovered totally in the 2 weeks.No complications of deep vein thrombosis,lung thrombosis etc al,were found after operation.Conclusions Using transcatheter injection of absolute ethanol to treat varicose veins of lower extremity has the advantage of less invasion,more safety and low appearance of complications.The short term efficacy is solid while the long term effect needs further evaluation.
5.Clinical characteristics and rescuing on severe trauma in patients of various ages

Objective To analyze the clinical characteristics and the outcome of severe trauma of various ages in emergency department,to improve the therapeutic effect of emergency rescuing.Methods Two thousand and sixty nine patients of severe trauma were treated during a seven-year period from October 1997 to October 2004.The studied patients were divided into 5 groups:(1)adolescent group(Group A,<13 years old,n=106);(2)juvenile group(Group B, 13-18 years old,n=128);(3)youth group(Group C,18-40 years old,n=1518);(4)middle age group(Group D, 40-60 years old,n=215);and(5)elderly group(Group E,>60 years old,n=102).All the patients were evaluated with Injury Severity Score(ISS),and the result was≥16 in all of the studied patients.Results The incidence of severe trauma in male in all 5 groups was higher than that in female,and it was significantly higher in Group C,Group D and Group E when compared with that in the other groups(P<0.01).Traffic accident was the leading cause of injury,and its incidence in Group A,Group B and Group D was higher than that in other groups(P<0.01).However,injury caused by falling from high places was the second cause of injury,being significantly increased in Group A,while armed fighting and injury during work being significantly increased in Group B,Group C and Group D,slip fall injury being significantly increased in Group E(P<0.01).Head injury was mainly found in Group A and Group E,extremities injury and/or spinal injury were increased markedly in Group B,Group C and Group D(P<0.05),abdominal injury was significantly decreased in Group E(P<0.001).The total mortality was 11.9%(246/2069).The mortality within 24 hours (20.6%,21/102)was significantly higher than that beyond 24 hours(7.8%,8/102)in Group E(P<0.01).The time of staying in the emergency department differed significantly between the survived patients and those died in all 5 groups (P<0.01).Conclusion The gender,the incidence and the characteristics of the injury causes and injury sites differed between severely injured patients of various ages.Mortality in the elderly is significantly increased following severe trauma. The idea of“golden one hour”and“platinum ten minutes”,measures of shortening the time of staying in the emergency clinic,early definite operation and damage control operation should be emphasized.
6.Analysis of remedy on senile severe trauma

Objective To study senile severe trauma patients admitted to Emergency Surgical Department of the second affiliated hospital of Guangzhou Medical college from October 1997 to October 2004,to investigate constructive suggestions of emergency treatment for urban senile trauma.Method Retrospective analysis on 102 senile severe trauma patients screened out from trauma patients admitted to the Emergency Surgical Department was carried out.Results There were 76 male and 26 female patients,the ratio of male to female was 2.9:1,mean age was 69.2 years old(range 60~83 years),the mean trauma score(TS)was(14.5?2.6),the mean Glasgo Coma Score(GCS)was(11.8?3.1),the mean Injury Severity Score(ISS)was(24.5?10.7),there were 61 patients of multiple injuries,transportation accident 45 patients(43.3 %),tumbling injury 23 case(22.5 %), falling injury 19 patients(18.6%),shock 65 patients.Definitive rescue surgery was carried out in 68 patients. The time from emergency treatment to operation departments was(58?19)minutes for the survival patients and (111?34)minutes for those died,there was significant difference between the two groups(P
7.Application of quantitative fingerprint to amino acids composition analysis of Xingnao Tongluo injection.

OBJECTIVETo control the quality of the product, quantitative fingerprint was used to evaluate the composition of the amino acids in the Xingnao Tongluo injection.
METHODThe method of the quantitative fingerprint to the amino acids composition was established through AccQ Tag precolumn derivatization. The quality was evaluated by the quantitative test of the amino acids and the similarity in ten batches.
RESULTThe Xingnao Tongluo injection contained 12 amino acids and the contents of these amino acids were stable. All the ten batches of the samples had similarity of more than 0.90.
CONCLUSIONThe method was accurate, feasible and could be a simple and effective way to evaluate the quality of the traditional Chinese medicine.

Objective To investigate the effect of Tenascin-C on remodeling of ventricle in mice with chronic autoimmune myocarditis.Methods Seventy-five specific pathogen-free BALB/c mice were divided into experimental group(n=45)and control group(n=30).Animals in experimental group were immunized with an emulsion of polypeptides(myhc-? 614-629)and complete Freund's adjuvant(CFA)on day 1 and day 8,while those in control group with equal volume of mixture of phosphate buffer solution(PBS)and CFA.Groups of mice were sacrificed respectively on day 21,day 75 and day 120 after the first immunization.Hematoxylin and eosin(H-E)staining was used to identify the areas of inflammation,and Masson staining was used to identify the areas of fibrosis.The Tenascin-C protein expression in myocardium was detected by Western blotting.The content of serum procollagen type-Ⅲ amino-terminal propeptide(PⅢNP)was measured by radioimmunoassay.The correlation between Tenascin-C protein expression and content of serum PⅢNP was analyzed.Results On day 21,inflammatory infiltration in the myocardium was remarkable,with slight deposition of collagen in the interstitial tissue.On day 75 and day 120,inflammatory infiltration in myocardium was markedly reduced,but with more marked deposition of collagen.The Tenascin-C protein expression in myocardium and the content of serum PⅢNP were significantly higher in experimental group than in control group(P
9.Analysis of Distribution and Drug Resistance of Bacteria in Elderly Patients with Vaginal Infection

OBJECTIVE:To provide reference for the formulation of clinical therapy plan for elderly patients with vaginal in-fection. METHODS:From Jun. 2014 to Oct. 2016,200 elderly patients with vaginal infection were selected from Wuhan Wuchang Hospital (called our hospital for short). Leucorrhea routine examination,bacterial culture,identification and drug sensitivity test were all performed for the secretions of vaginal side wall. The occurrence of vaginal infection was compared among different age groups. The distribution of bacteria,distribution of cleanliness and drug resistance were observed. RESULTS:The types of vaginal infectious diseases in 200 patients included Candida vaginitis,Trichomonas vaginitis,bacterial vaginitis,nonspecific vaginitis. The incidence of vaginal infection in 66-70 age group was significantly higher than 60-65 age group and 71-80 age group,the incidence of vaginal infection in 60-65 age group was significantly higher than 71-80 age group,with statistical significance(P<0.05). A to-tal of 205 strains of bacteria were detected,most of which were Gram-positive bacteria,except for Escherichia coli. Streptococcus was the most common pathogen,with a total of 76 strains(37.07%),followed by Staphylococci(48 strains,23.41%). The vagi-nal secretion cleanliness was grade Ⅰ,Ⅱ,Ⅲ and Ⅳ respectively in 10,70,90,30 cases. The amount of bacteria increased with the increase of vaginal secretion cleanliness,and Streptococcus and Staphylococcus infection were the most common in patients with Ⅲ to Ⅳ degree. Resistance rate of bacteria to penicillin,gentamicin and erythromycin were relatively higher while most of them to teicoplanin and linezolid were relatively lower;they were all sensitive to vancomycin (resistance rate of 0). CONCLU-SIONS:There are many types of vaginal infectious diseases in our hospital,and most of them are 66-70 years old. The majority of bacteria were Gram-positive bacteria,most of them were Streptococcus. The drugs which are sensitive to pathogen include vancomy-cin,teicoplanin and linezolid. Antibiotics should be rationally selected according to drug sensitivity test.
10.Therapeutic benefit of Yangxue Qingnao Granule on cognitive impairment induced by chronic cerebral hypoperfusion in rats.

OBJECTIVETo observe the therapeutic effect of Yangxue Qingnao Granule (, YXQNG) on cognitive impairment induced by chronic cerebral hypoperfusion and to investigate its impact on oxidative stress, apoptosis, and the cholinergic system.
METHODSAdult male Wistar rats were subjected to chronic cerebral hypoperfusion by permanent occlusion of bilateral common carotid arteries (2-VO). Thirty rats were randomly assigned to one of the five treatment groups in a 1:1:1:1:1 ratio: sham operation plus normal saline treatment, 2-VO plus normal saline treatment, 2-VO plus YXQNG at a dose of 2 g·kg(-1)·d(-1) or 4 g·kg(-1)·d(-1), or 2-VO plus rivastigmine 2 mg·kg(-1)·d(-1). The Morris water maze test was used to assess the spatial memory retrieval. Apoptosis, total antioxide capacity (T-AOC), acetylcholine esterase (AchE) and choline acetyl transferase (ChAT) activities in the hippocampus and the cortex were investigated.
RESULTSIn the chronic cerebral hypoperfusion model, the 2-VO plus saline treatment resulted in impaired special learning as shown by the significantly prolonged escape latency and shorter swim time in the first quadrant as compared to the sham operation. The impairment was associated with apoptosis and significant decreases in T-AOC, AchE and ChAT activities in the hippocampus and the cortex. Treatment with YXQNG at either 2 g·kg(-1)·d(-1) or 4 g·kg(-1)·d(-1) dose, or rivastigmine resulted in significantly shorter escape latencies and longer swim time in the first quadrant. YXQNG at both doses, but not rivastigmine, had significant reduction in apoptosis, and significant increases in T-AOC and ChAT activity in both the hippocampus and the cortex. Unlike rivastigmine, neither dose of YXQNG showed significant reduction in AchE activity.
CONCLUSIONSYXQNG ameliorated cognitive impairment induced by chronic cerebral hypoperfusion. The protective effect may be mediated through its regulation of apoptosis and activities of T-AOC and ChAT in the hippocampus and cortex of the rats in the chronic cerebral hypoperfusion model, a mechanism that is different from rivastigmine.
