2.Clinical application of color ultrasonic doppler in the diagnosis of benign and malignant tumors of salivary glands

Objective:To evaluate 2D ultrasonography(2DUS), color Doppler flowing imaging(CDFI) and color Doppler energy (CDE) in the differential diagnosis of benign and malignant tumors of salivary glands.Methods:According to the characters of 2DUS and CDFI examination, benign and malignant tumors in 142 cases of salivary glands were distinguished. The results of ultrasound examination and postoperative biopsy were checked and compared. Meanwhile the same tumor's blood flow information and hemodynamics index were compared. Results:In 2DUS examination, the inter echo and post echo of benign and malignant were different. In CDFI examination, the blood flow form could be displayed clearly. Besides Warthin tumors, the blood supply ranking of benign tumors was usually levelⅠor Ⅱ-(78.7%), and that of malignant tumors was levelⅡ+ or Ⅲ(80.0%). The artery's peak systolic velocity(PSV) of malignant tumors was higher than that of benign tumors. The rates(%) of sensitivity, specificity,accuracy,false negative, false positive, positive predicting and negative predicting of 2DUS were 70.2,87.4,81.7,29.8,12.6,73.3 and 85.6,those of CDFI 80.9,65.2,70.4,19.1,34.7,53.0 and 87.3,those of the integrated usage of ultrasonic methods 89.3,90.5,90.1,10.6,9.5,82.4 and 94.5 respectively.Conclusion:With the integrated usage of ultrasonic methods, higher accuracy of differential diagnosis for benign and malignant salivary glands tumors may be obtained.
3.Application of color doppler ultrasound examination for evaluation of blood vessel in transplanted tissue flap in maxillofacial surgery

Objective: To analyse the acuracy of color doppler ultrasound examination in the measure- ments of blood vessels in tissue flaps. Methods: 110 pieces of associated blood vessel in tissue flaps in 64 cases were measured with color doppler ultrasound examination before operation. The quality of blood vessel, its diameter, blood stream filling, the vascular pedicle length and its depth were measured. The results were checked and compared with the findings in operation. Results: Of all 110 pieces of associated blood vessel evaluated by doppler, the accurate ratio confirmed by the finding in operation was 100%. The inner diameter of 38 pieces of blood vessel measured in operation was coincident with that measured by doppler before operation, or with little difference. Conclusion: The color doppler ultrasound examination is a reliable method for evaluating the blood vessel both of the donor and host areas in maxillofacial reconstruction surgery with tissue flap.
4.Up-regulating effect of hepatitis C virus core protein on inducible nitric oxide synthase gene promoter

Objective To investigate the transactivating effect of hepatitis C virus(HCV)core protein on inducible nitric oxide synthase(iNOS)gene promoter and the molecular biological mecha- nisms of HCV pathogenesis.Methods Polymerase chain reaction(PCR)technique was employed to amplify the sequence of iNOS promoter by using HepG2 genomic DNA as template,and the product was cloned into pGEM-T vector.The iNOSp gene was cut from T-iNOSp by KpnⅠand XhoⅠ,and then was cloned into pCAT3-Basic,the constructed vector was named as pCAT3-iNOSp,pCAT3-iN- OSp was transfected into the LO_2 cell line.LO_2 cell was also cotransfected with pcDNA3.1(-)-core and pCAT3-iNOSp by FuGENE 6 transfection reagents.The LO_2 cells transfected with pCAT3-Basic was used as negative control.The activity of CAT in LO_2 cells was detected by an ELISA kit after 48 hours,which reflected the transactivating function of HCV core protein to iNOS gene promoter.Re- sults The expressive vector pcDNA3.1(-)-core and report vector pCAT3-iNOSp had been construc- ted and confirmed by restriction enzyme digestion and sequencing.The expression of CAT in LO_2 cells transfected with pCAT3-iNOSp and peDNA3,1(-)-core was 11 times as higher as that of pCAT3-bas- ic,and 6 times as higher as that of pCAT3-iNOSp.Conclusion It is suggested that HCV core protein can transactivate iNOS gene promoter.
6.Interventional treatment of coronary artery fistulas by using Guglielmi detachable coils:analysis of feasibility and safety with single center experience

Objective To summarize the experience in treating coronary artery fistula (CAF) by using Guglielmi detachable coils. Methods During the period from July 2009 to November 2014 at the Affiliated Changhai Hospital of Second Military Medical University, interventional treatment of CAF by using Guglielmi detachable coils was performed in 40 patients. The clinical data were retrospectively analyzed. The feasibility, safety and effectiveness of this technique were evaluated. Results Successful transcatheter closure of CAF with Guglielmi detachable coils was achieved in all 40 patients; the average Guglielmi detachable coils used in each patient was(2.33±1.38) coils. No procedure-related complications occurred. Intra-operative angiography showed that residual shunt completely disappeared in 12 patients (30%) and blood flow was significantly decreased in 28 patients (70%). All the patients were followed up for 1-65 months, neither complications such as recurrent bleeding and ischemia nor stenosis and occlusion of related arteries, or fistula cavity rupture occurred. Conclusion The use of Guglielmi detachable coil in interventional treatment of CAF is safe and effective, although its long-term effect needs to be further verified.
7.Roles of maspin in biological behaviors of A549 cells

AIM:To investigate the roles of maspin in the biological behaviors of non-small-cell lung cancer A549 cells.METHODS:Full-length coding region of maspin was amplified by PCR from human normal breast tissue and cloned into MSCV vector .Virus supernatants was produced by Phoenix A packaging system , and target cell line was infec-ted with the virus supernatants .The transfected cells were screened with puromycin .The cell line with maspin over-expres-sion was identified by real-time PCR and Western blot.Cell growth, migration and invasion were investigated by xCelli-gence system .RESULTS:Maspin over-expression vector was successfully constructed .The cell line with maspin over-ex-pression was established .No difference of the growth between A 549-control cells and A549-maspin cells was observed .The migration and invasion were quite different between A 549-control cells and A549-maspin cells.The expression level of inte-grin β1 in A549-maspin cells was decreased compared with A 549-control cells .CONCLUSION:Maspin has no effect on the growth of A549 cells.Maspin suppresses the migration and invasion of A 549 cells, in which integrin β1 is involved.
8.miR-17-92 cluster increases the migration and invasion abilities of DU145 prostate cancer cells and enhances the cisplatin resistance

Background and purpose:miR-17-92 gene cluster overexpression has been observed in various cancers, such as lung cancer, liver cancer, gastric cancer and prostate cancer. In this study, we established the stable cell line overexpressingmiR-17-92 to explore the inlfuence ofmiR-17-92 on the migration, invasion abilities and cisplatin resistance of the prostate cancer DU145 cells.Methods:miR-17-92 overexpression vectors were constructed. DU145 cells were infected with the viral supernatants produced by Phoenix A packaging system. Real-time lfuorescent quanti-tative polymerase chain reaction (RTFQ-PCR) was conducted to detect the expression level of miR-17-92 in the cells. The migration and invasion abilities were measured by a real-time xCELLigence system. The scratch healing assay was carried out to investigate the migration abilities. The expression of integrin β1 was detected by Western blot, and the activities of matrix metalloprotein-2 (MMP-2) and matrix metalloprotein-9 (MMP-9) were measured by gelatin zymography experiment. The cell growth of the two cell lines after the treatment of cisplatin was detected by a real-time xCELLigence system. The mRNA expression ofERCC1 was measured by RTFQ-PCR. Western blot was conducted to investigate the protein expressions of ERCC1, ERK1/2 and pERK1/2.Results:DU145-miR-17-92 cells migrated faster than DU145-control cells during the 24 h continuous monitoring (P<0.01). The scratch healing assay indicated that DU145-miR-17-92 cells migrated from the edge towards the scratch center faster than DU145-control cells. DU145-miR-17-92 cells invaded through matrigel markedly faster than DU145-control cells (P<0.01). The protein expression level of integrin β1 and the MMP-9 activities in DU145-miR-17-92 cells were increased than those in DU145-control cells. After the treatment of cisplatin, DU145-miR-17-92 cells grew faster than DU145-control cells, presenting cisplatin resistance (P<0.01). The phosphorylation of ERK1/2 in DU145-miR-17-92 cells was constantly at a high level regard-less of the treatment of cisplatin. Compared with DU145-control cells, the expression of drug resistance-related gene ERCC1 was dramatically increased in DU145-miR-17-92 cells after the treatment of cisplatin.Conclusion:miR-17-92 overexpression increases the migration and invasion abilities of the prostate cancer DU145 cells, which is associated with the upregulated expression of integrin β1 and the increased activity of MMP-9. Besides,miR-17-92 overexpression enhances the cisplatin resistance of DU145, which is correlated with the increased phosphorylation level of ERK and the upregulated expression of ERCC1 at both the mRNA and protein levels.

Objective Promote the use of medical record information, the depth of excavation,provide strong support for clinical research and hospital management.Methods Medical Record Information lower utilization reasons put forward need to build the whole structure of the paperless electronic medical records, electronic medical records for research concluded that the key to building elements to provide support.Results Pointed out that the construction of paperless electronic medical records from the storage structure of the building medical record systems, and data warehouse technology combined start, outpatient and inpatient medical records while achieving interoperability, building regional health care, improve follow-up system, and finally pointed out the key technical implementation.Conclusions It is to promote the utilization of medical records, medical records for research to improve support efforts to promote development and progress of medicine and enhance the hospital's soft power has great significance.
10.Survival analysis of lupus nephritis with neuropsychiatric systemic lupus erythematosus

Objective To investigate the survival rate and the influencing factors in lupus nephritis (LN) patients with neuropsychiatric systemic lupus erythematosus (NPSLE). Methods Clinical characteristics and biochemical markers of 78 patients including 59 variances were analyzed. Patients were followed up from the onset of NPSLE to death. Patient survival rate was estimated by Kaplan-Meier method. Cox regression model was used to analyze influencing factors. Results Sixteen (20.5%) of 78 patients died of SLE or its complications. Infection was the main cause of death (31.3%). One-, 3-, 5- and 10-year survival rates were 83.2%, 81.7%, 76.7% and 76.7%, respectively. Hypertension (RR =6.965,95% CI:1.578-30.746, P= 0.010), pulmonary infection (RR=8.171,95% CI:1.954-34.177, P=0.004)and acute renal failure (RR=6.978,95%CI:2.063-23.609, P=0.002) were risk factors of mortality, while cyclophosphamide (CTX) impulse therapy (RR =0.130,95 % CI:0.031-0.541, P=0.005) and resolution of NPSLE (RR= 0.169, 95%CI:0.042-0.679,P=O.012)were protective factors. Conclusions Infection is the main cause of death in patients of LN complicated with NPSLE. Survival rate of LN patients with NPSLE in this study is lower than those of LN and NPSLE alone reported by other authors. Hypertension, pulmonary infection and acute renal failure are risk factors of mortality, while CTX impulse therapy and resolution of NPSLE reduce the mortality and improve the prognosis.
