1.Misdiagnosis of Pulmonary Cancer Osseous Metastasis with Shoulder and Back Pain:1 Case Report

Objective To analysis the reason why patients with shoulder and back pain caused by pulmonary cancer osseous metastasis were misdiagnosed.Methods A patient of pulmonary cancer with osseous metastasis who was misdiagnosed as herniation of cervical disc,shoulder and back chronic lesion were analysed and reviewed with the literature.Results The involved vertebral bodies were osteolysis or wedging,the tumor encroach in cortical bone or soft tissue could result in shoulder and back pain.Conclusion A complete history and physical examine,systemic differential diagnosis,proper radiographic examination can decrease the incidence of misdiagnosis.

3.Epitope mapping of hepatitis C virus non-structure 3 protein from a 7 peptide phage library by using immobilized specific monoclonal antibody

Objective To screen HCV NS3 mimotopes by using monoclonal antibody and phage peptide library. Methods Using HCV NS3 monoclonal antibody as selective molecule, a 7 mer phage peptide library was biopanned and positive clones were selected by ELISA, competition assay and DNA sequencing. Results 13 positive clones were chosen for DNA sequencing. From the experiment and sequen-cing comparsion results, one epitope was confirmed as the mimotope of HCV NS3. Conclusions HCV mimotope is obtained by phage peptide library screening.
4.Study on differentially expressed genes in Jurkat cells treated with phosphonoformate using DNA microarray

Objective To screen the genes differentially expressed in gene expression in human lymphoma cell line Jurkat cells treated with phosphonoformate (PFA). Methods cDNA microarray technique was employed to detect the mRNA expressed in Jurkat cells treated with PFA or 0.9 percent sodium chloride, respectively. Results Among 1 152 genes there were 94 genes with different expression, of which 38 genes were upregulated and 56 genes were downregulated in Jurkat cells treated with PFA compared with those treated with 0.9 percent sodium chloride. Conclusions cDNA microarray technique was successfully used to screen the genes with different expression in Jurkat cells treated with PFA, and the results brought some new clues for the study of the immune regulation mechanism of PFA.
5.Simultaneous Determination of Campanulin, Paeoniflorin and Hydroxysafflor Yellow A in Buyang Huanwu Decoction by HPLC

Objective To establish a HPLC method for the simultaneous determination of campanulin, paeoniflorin and hydrox-ysafflor yellow A in Buyang Huanwu Decoction. Methods A Kromasil C18(4. 6 mm × 250 mm, 5 μm) column was adopted. The mobile phase consisited of methanol and acetonitrile(26 : 2) (A), and 0. 7 % phosphoric acid solution(B), in gradient e-lution (0~10 min, 25 % →35 % A; 10~25 min, 35 %→40 % A). The flow rate was at 1.0 mL·min-1. The column tem-perature was 35 ℃;, and the detection wavelengths were set at 260nm for campanulin, 230 nm for paeoniflorin and 403 nm for hydroxysamor yellow A. Results The linear ranges of campanuliu, paconiflorin and hydroxysafflor yellow A were 0. 0539~1. 078 μg (r=0. 999 5), O. 4160~8. 320μg (r=0. 999 8), and 0. 0418~0. 8352 μg (r=0. 999 5) respectively; the av-erage recovery was 99. 4 % (RSD=1.5 %), 100. 6 % (RSD=1.7 %), and 101.0 % (RSD=1.9 %) respectively. Conclu-sion The method is simple, feasible and reproducible, and can be used for the quality control of Buyang Huanwu Decoction.
6.Effect of Types of Pinch on Measurement of Pinch Strength in Healthy Young Subjects

Objective To investigate the effect of types of pinch on the measurement of pinch strength in healthy young subjects.Methods All anthropometric factors of 20 right-handed young volunteers(10 males,and 10 females)were recorded.Their pinch strength in 3 types(I.e.Pulp2,Chuck,and Lateral)were measured with digital pinch dynamometer in standardized position.Results For both genders,the pinch strength of Lateral was the largest,followed by Chuck,Pulp2 and the dominant hand was stronger(P<0.01).Men were stronger than women in all types of pinch strength(P<0.01).Correlation among the 3 types of pinch strength were presented(r=0.568~0.707,P<0.01).The correlation were found between the pinch strength of 3 types and weight in women(r=0.764~0.807).No correlation has been found among the pinch strength of 3 types and age,height,the thickness of palm in both the males and the females.Conclusion The sex and types of pinch significantly impact pinch strength in healthy young subjects,but the correlation between pinch strength and anthropometry should be further surveyed.
7.The effect of stellate ganglion block on humoral immunity at perioperative period in patients with breastcancer

Objective To observe the effects of stellate ganglion block (SGB) on humoral immunity func-tion in patients who received modified radical mastectory (MRM). Methods Eigty female patients were randomly divided into control group(group Ⅰ ,n =40) and intervention group (group Ⅱ,n =40). Blood samples were taken pre-intervention (T1), after intervention ( T2 ) and 30 rain after operation ( T3 ) for the determination of lgG, IgA, IgM. Results Compared with T1, IgG, IgA, IgM at T2 and T3 in two groups were lowered, IgG [ ( 11.8±2.2 ) g/L vs (9.5 + 1.5) g/L,t = 5. 4630,P =0.0041 ] and IgA [(1.9±0.3)g/L vs (1.7±0.4) g/L,t =2. 5298,P = 0.0134 ] at T3 in group Ⅱ were significantly different as compared with that of group Ⅰ ; IgM at T2 [ (2.5±0.8 ) g/L vs (2.1±0.6)g/L,t =2.8276,P=0.0072] and T3[ (2.1±0.5)g/L vs (1.6±0.5)g/L,t =4.4721,P = 0.0000 ] was significantly different between the two groups. Conclusions Preoperative-SGB can improve the inhibi-tion degree of humoral immunity caused by modified radical mastectory.
8.Surgical treatment of unstable pelvic fractures combined with acetabular fractures.

OBJECTIVETo discuss the treatment strategy of acetabular fractures and unstable pelvic fracture of the hip and to evaluate its outcome.
METHODSRetrospective analysis of clinical data in 32 patients with unstable pelvic fracture and acetabular fractures from January 2007 to June 2013 were collected. There were 18 males and 14 females aged from 18 to 62 years old (means 38 years old). According to Tile classification of pelvic fracture, 11 cases were type B1, 8 were type B2.1, 7 were type B2.2, 3 were type C1.1, 2 were type C1.2, 1 was type C3. According to Judet-Letournel classification, anterior column fracture was in 1 case, transverse fracture in 8, transverse plus posterior wall fracture in 6, T-type fracture in 1, anterior column plus half transverse fractures in 5, double column fracture in 11. Other combined injuries were treated early, the surgical operation were performed after stable condition. The hip joint function and the fracture reduction were assessed during follow-up.
RESULTSThe operative time was from 1.8 to 6.5 hours (averaged 3 hours). Two fat patients' incision occurred in fat liquefaction and healed after dressing, no incision infection happened. Only 1 case was lost to follow-up, 31 patients were followed up with a mean time of 23 months (6 to 42 months). The healing time of pelvic fracture was from 8 to 18 weeks (averaged in 10.6 weeks). The hip function was evaluated according to the Matta and Tornetta standard postoperatively, the result was excellent in 15 cases, good in 14 cases, fair and poor in 1 case respectively. The Majeed score of the hip function was 83.65? 7.67, the result was excellent in 15 cases, good in 12 cases and fair in 4 cases. The healing time of acetabular fractures was from 8 to 16 weeks (averaged in 10.2 weeks). The fracture reduction was assessed by Matta standard, the result was excellent in 15 cases, good in 12 cases and fair in 4 cases. The heterotopic ossification was evaluated by Brooker standard, 4 cases were grade I, 1 case was grade II . There were no infection, nonunion and necrosis of the femoral head in all patients. The nerve damage symptoms in 5 patients disappeared during 4 to 6 months after operation.
CONCLUSIONPatients with unstable pelvic fractures and acetabulum fractures were in a critical condition early, using the concept of damage control to save lives in a timely manner. Grasp the operation time in the late treatment, acetabulum fractures reach anatomical reduction as far as possible, pelvic fractures are given priority to stable reconstruction. The operation order was fixed pelvic ring first, according to the condition to choose the appropriate surgical approach and fixed mode.

9.Early diagnostic and prognostic value of procalcitonin in critical ill patients with sepsis

Objective To investigate the diagnostic value of procalcitonin(PCT)as an early indicator for sepsis.Methods Serum levels of PCT and C-creative protein(CRP)and white blood cell (WBC)count were measured in 30 patients in critical condition hospitalized at an intensive care unit(ICU) with diagnosis of systemic inflammatory response syndrome(SIRS).They were divided into two groups, sepsis and non-sepsis,based on their clinical manifestations and results of lab tests.Blood specimen was collected from each patient for measurement of PCT,CRP and WBC count on the 1~(st),3~(rd)and 7~(th)day after hospitalization and bacteriological culture for blood and sputum,and chest X-ray was performed,as well. Acute physiology,age and chronic health evaluation(APACHE Ⅱ)was made on the 1~(st),3~(rd)and 7~(th)day after hospitalization to assess their ill condition.Their prognosis were judged on the 28~(th)day of the follow-up. Results Serum level of PCT increased significantly in the sepsis group(with the highest of 10.13 ng/ml), as compared with that in the non-sepsis group.Sensitivity,specificity and predictive value of a positive test for a cut-off value of serum level of PCT at 0.5 ng/ml were 97.0%,91.7% and 82.1%,respectively, which were all better than those of serum level of CRP and WBC count.Serum level of PCT in the patients was significantly associated with their prognosis,and PCT in those died was significantly higher than that in those survived.Whereas,serum level of CRP and WBC count elevated in both groups,but the difference between the two groups did not reach a level of statistical significance.Conclusion Serum level of PCT can be used as an early indicator for judgment of sepsis for a patient with infection and reflection of severity of illness.
10.Study on preparation and fluoride-relesae properties of fluorinated nanosilica filler

Objective The aim of the study was to develop a kind of new fluoride composite resin .We explored the practica-bility of preparing fluorinated filler by using hollow silica microsphere as a carrier of sodium fluoride (NaF) as well as its fluoride-re-lease properties in composite resin . Methods Hollow silica microspheres with mesoporous shells were prepared by the sacrificial template method.NaF was loaded into hollow silica microspheres by the impregnation method .Scanning electron microscopy ( SEM) was used to determine the morphologies of hollow silica microspheres before and after NaF loading .Hollow nanosilica microsphere con-taining NaF and NaF powder were respectively mixed with resin at the same intial NaF concentration of 13.9, 27.8 and 55.6 mg/mL for comparison, which were taken as the experiment group and the control group .The samples were put into 100 mL artificial saliva at 37℃to caculate the cumulative fluoride release quantities from 1 d to 47 d.Results The morphology of hollow microsphere did not chang significantly before and after NaF loading .In the experiment group ,the cumulative quantities of fluoride release were 0.026, 0.105, 0.21 mg/cm 2on 1 d and increased to 0.088, 0.239, 0.484 mg/cm 2on 47 d according to respective initial NaF concentration . While in the control group, the corresponding figures were 0.006, 0.025, 0.04 mg/cm2on 1 d and 0.022, 0.045, 0.056 mg/cm2on 47 d.The difference between these two groups was of great statistical significance . Conclusion Hollow silica microsphere could be used as a carrier of NaF .Fluorinated nanosilica filler using hollow microsphere promotes the fluoride release in composite resin .
