1.The damage effect of ethanol on the celiac in patients with anesthesia by propfol combined with remifentanil

Objective To investigate the effect of ethanol using by neurolytic celiac plexus block(NCPB)on the Bispectral index(BIS)during anaesthesia with propofol and remifentanil.Methods Thirty-six patients,ASA Ⅰ~Ⅲ,undergoing pancreatic cancer surgery were studied.Patients were maintained with propofol(6~9mg·kg~·h-1)and remifentanil(0.1~0.3μg·kg~·min-1)to keep the BIS between 38 and 50.After 30minutes of stable anesthesia,patients in control group were treated with NCPB by 0.9%NS 30ml,while patients in ethanol group were employed NCPB at the same time by 75%ethanol 30m1.BIS values,mean arterial pressure(MAP),heart rate(HR)were recorded 5minutes before the injection and one,five,fifteen,twenty five,thirty five,forty five minutes after that.Results Therewas no significant change in BIS,emodynamic variables compared to preinjection values in the control group.15min to 45min after injection of four point,BIS values and MAP of ethanol group decreased significantly,as compared with before injection,the same time point compared with the control group had statistically significant difference(P<0.001).No significant changes in HR were observed throughout the observation.Conclusion Ethanol used in the operation of NCPB was additive to the anesthesia depth during the anesthesia of propofol and remifentanil.

3.Interference of lipid compositions and carry-over contamination of lipid reagents on measurement of serum total bile acid

Objective To investigate whether lipids and reagents would interfere the results when serum total bile acid(TBA) was measured by enzymatic cycling assay.Methods The serum TBA was measured by enzymatic cycling assay.The carry-over contaminations of high density lipoprotein(HDL-C),low density lipoprotein(LDL-C),cholesterol(TC),and triglyceride(TG)rea-gents were evaluated.In order to reduce the interference and carry-over contaminations,different washing procedures and detection sequence were set.Results By measuring the levels of TBA in pooled serums with low and high levels of lipids,the results showed that there was statistically significant difference between the groups with and without the addition of cleaning process before and af-ter TBA measurement(P <0.01).Cleaning with water might be more effective on reducing interference than those with acid solu-tion.Moreover,the mean of TBA levels in HDL-C,TC,TG and LDL-C reagents were (476.06 ± 1.88 ),(127.78 ± 1.18 ), (121.05±1.08),and (2.23±0.51)μmol/L,respectively.The stability of TBA level was greatly affected by HDL-C regents,fol-lowing by TC and TG reagents,and was little affected by LDL-C reagent.Setting up proper detection sequence and flushing proce-dures could obviously reduce the interference(P <0.01),but not completely rule out.Conclusion Analysis sequence and flushing procedures of biochemical analyzer as well as exogenous substance from reagents may seriously affect the accuracy of determination results.To ensure the accuracy and reliability of the results,it is necessary not only to set up reasonable irrigation and reaction se-quence,but also to master the instrument operation,to know the principle of test reaction and the components of reagents as well as equipment maintenance.

5.Clinical analyses of emergency neoplasty on 102 patients with corneal penetrating injury

Objective To evaluate the clinical effects of emergency neoplasty on corneal penetrating injury. Methods A total of 102 cases of corneal penetrating injury were investigated.The patients underwent micro-trauma technique suture,optical iridectomy,extraction of anterior segment foreign bodies,nepheloid lens removal,and artificial lens implantation.The vision of pre-and post-surgery was compared and relationship between injury location and recovery was analyzed. Results The injury of cornea repaired completely 1 month after surgery.Cross infection was not found in the patients with one-year long follow-up(80% of all patients).The vision of all patients raised.The injury in center and nasal side affected vision significantly. Conclusion Timely surgery to corneal penetrating injury may be considered in order to improve the recovery of the traumatic eyes.
6.Constraint-induced Movement Therapy on Upper Extremities for Sub-acute Stroke

ObjectiveTo assess the effects of constraint-induced movement therapy(CIMT) on sub-acute stroke patients with upper extremity motor dysfunction.Methods63 patients with hemiplegia after stroke were divided into two groups: control group(31 cases) and CIMT group(32 casese). The two groups received the regular rehabilitation training for 14 d. Then the control group went on the the regular rehabilitation training, while the CIMT group received CIMT for 14 d. All patients were assessed by the Action Research Arm Test(ARAT) and Fugl-Meyer(FMA) on the first day, the 15th day and the 30th day after the treatment.ResultsARAT score and FMA scores were higher in CIMT group than in the control group 30 d after the treatment and in CIMT group 15 days after the treatment(P<0.001).ConclusionCIMT is more effective to improve the upper limb motor function of the sub-acute stroke patients than the regular rehabilitation training.

9.High-performance capillary electrophoresis for determining caffeic acid content in compound Yinhuangjiedu decoction.

OBJECTIVETo establish a method for determining the content of caffeic acid in compound Yinhuangjiedu decoction using high-performance capillary electrophoresis (HPCE).
METHODSThe optimized HPCE for determining caffeic acid content utilized a fused-silica capillary tube (75 microm x 60 cm, effective length of 53 cm) with 20 mmol/L borate as the running buffer (pH=9.18), a constant voltage of 12 kV, a sampling time of 5 s at 25 degrees celsius, and UV detection wavelength at 313 nm.
RESULTSThe linear range of caffeic acid was 20-100 microg/ml.
CONCLUSIONHPCE is simple, rapid, and sensitive for quality control of the compound Yinhuangjiedu decoction.

10.Study on interventional therapy in infants with congenital ventricular septal defect:The feasibility and long-term effects

Objective To study the feasibility and long-term effects in 173 infants with congenital ventricular septal defect who underwent transcatheter therapy by double-disk ventricular septal defect occluder. Methods We analyzed the clinical data of 173 infants with ventricular septal defect who were performed interventional therapy followed by a follow up study from December 2002 to October 2013. Results The procedure was performed in 173 infants (male = 81 cases and female = 92 cases) aged 11 month-3 years[(2.1±0.7)years]. The weight were 6-15 kg[(10.2±3.6)kg]. The diameter of the defects ranged from 2.5-9.0 mm[(5.1±1.7) mm]. The characteristics of septal were classiifed into four types:the simply perimembranous ventricular septal defects (88 patient,50.8%), the perimembranous ventricular septal defects with pseudoaneurysm (52 patients, 30.0%), the perimembranous ventricular septal defects complicated with aortic valve prolapse (13 patients, 7.5%), and the intracristal ventricular septal defect (20 patients, 11.7%). The diameter of the occluders were between 4-12 mm[(6.3±2.2)mm]. The period of follow-up ranged from 1 month-10 years[(6.2±1.3)years]. The 168 defects were completely occluded in 173 patients(97.1%closure rate) except 5 infants, which 3 patients because the occluder hinder the function of aortic valves and the other occurred complete atrioventricular block(CAVB) when the catheter through defect. One patient occurred CAVB on the third day after the procedure, was reversed by dexamethasone after 4 days. 3 patients with complete left bundle branch block(CLBBB) after the procedure and one went to sustained, but did not observed heart failure in this case during 5 years follow-up. One patient observed sustained CLBBB on the fourth year after the procedure, also did not observed heart failure in this case during 3 years follow-up. 8 patients with trivial residual shun and 4 patients with newly appearance of trivial aortic regurgitation after the procedure, the shun all disappear at one year and the regurgitation did not progress during longest follow-up period at 8.5 years. No other complications, including late-onset CAVB, hematolysis, the occluder displacement and detachment, occurred during 1 month-10 years of follow-up. Conclusions Transcatheter closure of congenital ventricular septal defects is an efifcient method that can be safely used in the majority of infants with ventricular septal defects and have signiifcant long-term effects.
