BACKGROUND: Tongxinluo acts on benefiting qi, promoting circulation in collateral, activating blood circulation and resolving stasis, which is well effective in treatment of ischemic cerebral vascular disease. Single-photon emission computed tomography (SPECT) provides reliable reference evidence objectively and sensitively for clinical evaluation of therapeutic effects.OBJECTIVE: SPECT and clinical neurofunctional defect score (NDS)were used to observe the therapeutic effect of tongxinluo capsule on cerebral infarction.DESIGN: Case analysis was designed.SETTING: Internal Department of Neurology of First Affiliated Hospital of Chongqing Medical University.PARTICIPANTS: Totally 22 cases of acute cerebral infarction were admitted in Internal Department of Neurology of First Affiliated Hospital of Chongqing Medical University from March 2002 and July 2003, in which 12 cases were male and 10 cases were female, aged varied from 40 to 78 years, sick duration in range from 3 hours to 3 days.METHODS: Voluntarily, 22 cases of acute cerebral infarction were randomized in tongxinluo capsule group (capsule group)(11 cases) and the control (11 cases). In the control, danshen tablet was administrated,4 tablet/time, 3 times/d. In capsule group, tongxinluo capsule group was administrated, 4 pills/time, 3 times/d. The drugs were administrated for 15 days in two groups. Before and after treatment, SPECT ROI technique was used to assay ROI average radioactive counts on the affected and healthy sides to understand local blood flowing volume of brain in two groups. Neurofunctional defect evaluation was done before and after treatment on each patient.in two groups.RESULTS: Totally 22 cases of cerebral infarction all entered result analcontrol after treatment [(49.182±5.344 5), (28.364±4.610 3) score P < 0.001].the affected side of the control and treatment group before treatment was decreased. Fifteen days after treatment, the volume in original decreased flowing area of brain was improved after taking the capsule (66.536±18.676,ter administration of the capsule reflected in SPECT radionuclide, but it was not improved remarkably in the control.CONCLUSION: Tongxinluo capsule improves definitely cerebral blood flow in patients with cerebral infarction. With improvement of blood flowing in brain, the neurofunctional defect score is promoted clinically, too. It is explained that the capsule promotes the recovery of nerve function.

The relationship between acupuncture and stress is discussed from three aspects, including is it possible for acupuncture stimulation to be a stressor, whether acupuncture will start stress reaction, and whether acupuncture effects contain some stress factors. It is believed that correct acupuncture manipulation will not cause stress response, however, under some circumstances, such as inaccurate manipulation, improper treatment or patients who are very nervous but do not receive effective intervention, acupuncture is likely to cause stress response. Acupuncture-induced stress response is totally different from acupuncture anti-stress. The possible stress factors in acupuncture effect are explored, which can provide a new angle for the research on action mechanism of acupuncture. From the view of stress to review acupuncture treatment, there are three enlightenments: emphasizing on communication between doctors and patients, avoiding over-pursuit of deqi and focusing on analysis of the body constitution.

Objective To investigate the polyploid induction and identification of Nerviliae fordii for harvesting the polyploid plants. Methods The materials and methods for polyploid induction of Nerviliae fordii were screened separately by comparing the induction rates of rhizomes and bulbs under natural conditions and tissue culture environment, and by comparing soaking method with agar method. The effects of colchicine concentration ( 200, 300, 400, 300 mg/L), colchicine action time ( 7, 14, 21, 28 d), DMSO concentration ( 0, 10, 20, 40 mL/L) and KT concentration ( 0, 1.0, 2.0, 4.0 mg/L) on induction rate were observed by orthogonal design method. The polyploid induction in the treated plants was identified by morphology, cytology and chromosome methods. Results After the tissue culture rhizomes were treated with 300 mg/L colchicine, 10 mL/L DMSO, and 2.0 mg/L KT by agar method for 28 days, the polyploid induction rate arrived to 50%, showing better induction effect. The morphology of polyploid plants was characterized by giantism, and the leaf length, leaf width and plant height were respectively 152.17%, 158.67%and 60.90%of those of the diploid plants. The length, width and density of stoma of leaf epidermal cell as well as the number of chloroplast in the treated plants were 138.46%, 153.00%, 59.09% and 109.09% of those of the untreated plants. The results of chromosome identification showed that the amount of the tetraploid ( about 40) was 2 times of the diploid chromosome ( about 20) in the treated plants, proving that the achieved Nerviliae fordii was a tetraploid plant. Conclusion Polyploid plants of Nerviliae Fordii have been successfully obtained, which will supply evidence for improving species, richening seed-breeding resources, and selecting of improved seeds of Nerviliae fordii.

Objective To evaluate the effects of different concentrations of chloroprocaine on KCNQ2/Q3 channel currents in HEK2936 cells. Methods Human embryonic kidney (HEK293) cells served as an expression system.KCNQ2 and KCNQ3 cDNAs and green fluorescent protein were transfected into HEK293 cells by using lipofectamine. The KCNQ2/Q3 currents were recorded by using the whole-cell patch-clamp technique.Part Ⅰ The transfected HEK293 cells were randomly divided into 4 groups (n =11 each):control group, and 1,10 and 100 mmol/L chloroprocaine groups. The KCNQ2/Q3 channel currents produced by different concentrations of chloroprocaine were recorded under different holding potentials (-40,0 and 40 mV) and the action time was 1 min.Part Ⅱ The transfected HEK293 cells were randomly divided into 2 groups (n =5 each):control group and 10 mmol/L chloroprocaine. The KCNQ2/Q3 channel currents were recorded under different holding potentials (-80-30 mV)and the action time was 1 min.Different test potentials were normalized and fitted to Boltzmann function, and KCNQ2/Q3 channel Ⅰ-Ⅴ curve was then obtained. The activation and deactivation currents were both fitted to a single exponential function and the time constants for current activation and for current deactivation were calculated. Results Part Ⅰ When the holding potential was 40,0 and-40 mV,the suppression rate of KCNQ2/Q3 channel currents in HEK293 cells was higher in 1,10 and 100 mmol/L chloroprocaine groups than in control group (P <0.05 or 0.01).Part Ⅱ Compared with control group, the time constant for the current activation at 0 mV of holding potential was prolonged, the time constant for the current deactivation was shortened when the holding potential was-80 mV,and the half-activation voltage of KCNQ2/Q3 channels was increased, the activation curve shifted to the depolarized potentials, and KCNQ2/Q3 channel Ⅰ-Ⅴ curve slope was decreased in 10 mmol/L chloroprocaine group (P < 0.05).Conclusion Chloroprocaine concentration-dependently suppresses KCNQ2/Q3 channel currents in HEK2936 cells. The KCNQ2/Q3 channel is closed in advance due to KCNQ2/Q3 channel opening delay induced by chloroprocaine thus decreasing the activity of KCNQ2/Q3 channels.

Chinese Journal of Integrated Traditional and Western Medicine 2011;31(11):1466-1470
OBJECTIVETo study the effect of Shenfu Injection (SI) on the post-operative cognitive dysfunction in senile patients receiving the orthopedic surgeries after general anesthesia.
METHODS100 patients, 60-89 years old, waiting for selective surgery at the orthopedics department, were randomly assigned to two groups, the SI group (50 cases) and the control group (50 cases), who met the standard of the American Society of Anesthesiologists (ASA) II-III. After surgical incision, patients in the SI group were transfused with SI 200 mL (50 mL SI was added in 200 mL normal saline), while those in the control group were transfused with 200 mL normal saline at the same time. The mean arterial pressure (MAP), heart rate (HR), and oxygen saturation (SpO2) were compared between the two groups before anesthesia, after intubation, 30 min and 60 min after anesthesia, 5 min and 30 min after extubation. The time for surgery, the time of intra-operative hypotension, the volume of hemorrhage, the time for post-operative consciousness recovery, the liver function, the kidney function, and the fasting blood glucose of patients before surgery, 3 days and 7 days after operation were compared. The cognitive functions of patients were tested before surgery, 3 days and 7 days after operation. The incidence of post-operative cognitive dysfunction of patients was compared between the two groups.
RESULTSThere was insignificant difference in MAP, HR, and SpO2 between the two groups at each time point (P > 0.05). There was insignificant difference in the pre-operative cognitive function score, the time for surgery, the time of intra-operative hypotension, the volume of hemorrhage, the liver function, the kidney function, and the fasting blood glucose between the two groups ( P > 0.05). The time for post-operative consciousness recovery was obviously shorter in the SI group than in the control group, showing significant difference (P < 0.05). The post-operative 3-day and 7-day cognitive function scores were obviously better in the SI group than in the control group, showing significant difference (P < 0.05). The incidence of post-operative cognitive dysfunction of the SI group [3 days: 12% (6 cases); 7 days: 8% (4 cases) ] was significantly lower than that of the control group [3 days: 20% (10 cases); 7 days: 14% (7 cases) ] ( P < 0.05).
CONCLUSIONSI could improve the post-operative cognitive function of senile patients receiving the orthopedic surgeries after general anesthesia, lower the incidence of post-operative cognitive dysfunction rate, and shorten the time for post-operative consciousness recovery.
