1.Therapeutic observation on lung-clearing and spleen-strengthening tuina in children with exogenous cough

Objective: To observe the clinical efficacy of lung-clearing and spleen-strengthening tuina on exogenous cough in children. Methods: A total of 77 children with exogenous cough that met the inclusion criteria were randomly divided into a treatment group and a control group according to the random number table, with 39 cases in the treatment group and 38 cases in the control group. The routine lung-clearing manipulation was used in the two groups, while the spleen- strengthening manipulation was added in the treatment group. Both groups were treated once a day, 5 times as a course of treatment. After one course of treatment, the symptom score and clinical efficacy were observed. Results: There were one dropout in the treatment group and 2 dropouts in the control group. Therefore, a total of 74 cases were finally included in the analysis, with 38 cases in the treatment group and 36 cases in the control group. After treatment, there was a statistically significant difference in cough score between the two groups (P<0.05). There was a statistically significant difference in the score of poor appetite between the two groups (P<0.05). After treatment, the total effective rate was 89.5% in the treatment group versus 75.0% in the control group, showing a statistically significant difference (P<0.05). Conclusion: The therapeutic efficacy of lung-clearing spleen-strengthening manipulation is better than that of lung-clearing tuina manipulation alone for children with exogenous cough; regulating spleen and stomach can improve the curative efficacy of exogenous cough in children.
3.Comparative study of complications after D1 and D2 gastrectomy for gastric cancer

Objective To analyze the early mortality and morbidity of early postoperative complications of gastric cancer patients submitted to D1 or D2 gastrectomy for improving the surgical effect.Methods All consecutive patients who underwent D1 or D2 radical gastric resection between January 2006 and December 2007 were evaluated.Clinicopathologic features of the tumor,the extent of lymphadenectomy,the postoperative mortality and the early morbidity of complication were analysed.Results There were 130 patients admitted for the treatment of gastric cancer.34 underwent D1 dissection and 96 underwent D2 dissection.The early morbidity of D2 dissection was higher than that of D1 dissection (20.6% vs.39.6%,P<0.05).When complications were analyzed individually,there was no significant difference.Although the postoperative mortality was higher in the D2 group,no significant difference was observed(4.2% vs.0,P>0.05).4 patients underwent D2 dissection died.Considering the causes of the 4 died patients,respiratory complication related to anaatomotic and pancreatic leakage was the most important.Conclusion This study indicates that D2 dissection is a safe and effective procedure and the extent of lymphadenectomy may be related with high postoperative complication morbidity and mortality.Improving the surgical dissection skill and standardizing the gastric lymphadenectomy are the key ways to decrease the morbidity and mortality.
4.Reconstruction of the cistern magna for the treatment of Chiari Ⅰ malformation associated with syringomyelia

Objective To evaluate surgical performance of reconstruction of the cistern magna for the treatment of Chiari Ⅰ malformation associated with syringomyelia.Methods Suboccipital craniotomy and laminectomy of C1 segment were performed.The superior edge of bone window was limited to the inferior nuchal line.The dura mater and arachnoid were opened Y-shaped.The cerebellar tonsils were removed by electrocoagulation or subpially.A communication of cerebrospinal fluid between the fourth ventricle and subarachnoid space was achieved.The pseduomembrane,which is a very thin membrane over the opening of the central canal,should be cut away,with fascia grafting.Then a new enlarged cistern magna was established.Results The procedure was successfully completed in all the 36 patients with Chiari Ⅰ malformation and syringomyelia.The pseduomembrane was found in 30 patients.Postoperative follow-up checkups for 3 months ~ 5 years showed various degrees of recovery of sensation and muscle strength.Re-examinations with MRI revealed that the spinal cord cavity disappeared in 6 patients and decreased in size in 30 patients.Conclusions The decompression of the posterior cranial fossa,the communication of cerebrospinal fluid between the fourth ventricle and subarachnoid space,and the removal of pseduomembrane are key processes for treating Chiari Ⅰ malformation associated with syringomyelia.

6.Observation of the effect of percutaneous transforaminal endoscopic surgery for lumbar prolapse upwards type disc herniation

Objective To investigate the effect of percutaneous transforaminal endoscopic surgery for lumbar prolapse upwards type disc herniation.Methods According to the digital table,90 patients with lumbar prolapse upwards type disc herniation were randomly divide into the observation group and the control group,45 cases in each group.The observation group was treated with percutaneous transforaminal endoscopic surgery,the control group was treated with conventional open surgery.The curative effect,operation and adverse reactions were compared between the two groups.Results All patients were satisfied with the location,and they were completely removed from the nucleus.Compared with the control group,the operation time[(75.3±23.1)min vs.(103.4±35.4)min]and hospitalization time[(6.8±1.2)d vs.(8.7±1.8)d]of the observation group were shorter,the bleeding volume[(24.5±8.5)mL vs.(51.6±12.7)mL]was lower,the differences were statistically significant(t=4.453,5.632,4.269,all P<0.05).After one month and three months,the VAS score[(2.53±0.65)pints and(1.33±0.48)points]and ODI[(21.40±5.04)% and(12.93±3.50)%]of the observation group were significantly lower,there were statistically differences between the two groups compared with those before treatment(t=6.755,8.765,all P<0.05).And after one month,the VAS score and ODI of the observation group were significantly lower than those of the control group,the differences were statistically significant(t=4.342,3.985,all P<0.05).There was no significant difference between the two groups in the excellent and good rate(86.7% vs.82.2%,X2=1.011,P>0.05).ConclusionPercutaneous endoscopic discectomy surgery in the treatment of lumbar prolapse upwards type disc herniation is reliable,and it has less trauma and shorter hospital stay.

Biodegradable and biocompatible functional polymers show high potential as novel drug carriers in disease diagnosis and therapy.Recently,protein drugs have brought about major breakthroughs in the treatment of various diseases including cancer,while the development of carrier technology is relatively delayed.This article reviews recent advances in biodegradable functional polymers as protein drug nanocarriers.Additionally,we have discussed the perspective of developing new generations of biocompatible and functional polymers.
8.The effects and mechanisms of vitamin C on intestional mesenteric arterioles responsiveness to vasoconstrictors in patients with cirrhosis and portal hypertension

Objective To study the changes of mesenteric arterioles on the contractile response to norepinephrine in patients with cirrhosis and portal hypertension (PHT) caused by hepatitis B after treatment by high-dose vitamin C.Methods Eleven PHT patients and 6 non-PHT volunteer were those undergoing their respective surgeriesat in our hospital from January 2010 to June 2011.The PHT patients were randomly assigned to vitamin C treatment group and placebo treatment group.After admission and before induction of anesthesia,blood taken to mesure plasma concentration of malonaldehyde (MDA).Intraoperative portal pressure was measured.We took the third level intestinal mesentery vessels with their mesentery next to the jejunum and studied the contractile response to norepinephrine of the intestinal mesenteric arterioles.We also measured RhoA/ROCK pathway associated proteins' expression and activity changes by Western-blot method.Results The average value of the plasma MDA of PHT patients with cirrhosis was much higher than that of non-PHT patients.After treatment by vitamin C,the value decreased.The dose-response curve of isolated mesenteric arterioles to norepinephrine from PHT patients with cirrhosis shifted to the right,and the EC50 value was higher than that of non-PHT patients.But this phenomenon was reversed after the patients treated with vitamin C.The protein expression of RhoA in mesenteric arteries did not change significantly among the three groups.But the protein expression and activity of ROCK-1 was significantly reduced in PHT patients with cirrhosis.After treatment with vitamin C,the expression and activity of ROCK-1 in mesenteric arteries of cirrhosis was significantly increased.Conclusion The PHT patients with cirrhosis are in the situation of oxidative stress.The high-dose vitamin C treatment can reduce oxidative stress level and improve contractile hyporeactivity of mesenteric arterioles of PHT patients with cirrhosis to norepinephrine.The protein expression and activity increasement of ROCK in RhoA/ ROCK pathway is involved in thisphenomenon.
9.Efficacy of Compound Matrine Injection in the Adjuvant Chemotherapy for Advanced Gastric Cancer

Objective:To research the efficacy and adverse reactions of compound matrine injection combined with chemotherapy in the patients with advanced gastric cancer. Methods:According to the order of admission, 68 patients with advanced gastric cancer were divided into the observation group (34 cases) and the control group (34 cases). The two groups of patients were given chemo-therapy, and the observation group was treated with compound matrine injection additionally. After 3 months treatment, the curative effect and KPS score before and after the treatment were compared between the two groups. And adverse drug reactions were recorded for the two groups. Results:The total effective rate of the observation group (44. 12%) was significantly higher than that of the control group (20. 59%,P<0. 05). After the treatment, the KPS score of the observation group was significantly higher than that of the con-trol group (P<0. 05). The incidence of adverse drug reactions in the observation group was significantly lower than that in the control group (P<0. 05). Conclusion: The curative effect of compound matrine injection combined with chemotherapy for advanced gastric cancer is notable, which can significantly improve the KPS score and reduce the incidence of adverse reactions, and is worthy of promo-ted use in clinical practice.

Objective To discuss the application of visualization software of CiteSpaceⅢ to the treatment and research of transfusion medicine.Methods The software of CiteSpaceⅢ and the function of the reference database ISI Web of Science itself were used to the study.Results In the past 16 years,paper quantity and cited frequency on transfusion medicine re-search had the wave-like increasing tendency year by year.The research forces of the field were mainly distributed in Europe and the United States,the research hot spot and frontier around the blood safety,change over time in a dynamic develop-ment.Conclusion The study reveals the progress and development tendency of transfusion medicine,which could provide ef-fective reference for related research.
