Objective To study the early effect of arthroscopic bipolar radiofrequency technique in the treatment of patellofemoral malalignment. Methods 42 patients(47 knees) who were confirmed to be patellofemoral malalignment by radiographic methods, underwent arthroscopic bipolar radiofrequency chondroplasty and lateral retinacular release. The cartilage abnormality were classified according to the Outerbridge grade. 34 cases(37 knees) were followed up and mean period was 18.3 months. Patients were assessed before and after surgery using the Kujala patellofemoral score. Patellofemoral alignment were evaluated through MRI in 23 of 37 knees. The change of Cac (congruence angle of cartilage), Tac(tilt angle of cartilage), LPDc (lateral patellofemoral distance of cartilage) and LPFac(lateral patellofemoral angle of cartilage) were measured. Results The patellofemoral joint score was improved significantly in OuterbridgeⅠ,Ⅱand Ⅲ grade patients(6 knees ofⅠgrade: 60.83?3.54 preoperatively and 82.50?9.22 postoperatively, P0.05). To study the significance of detection of K ras mutation in stool samples in the diagnosis of large intestinal cancer, stool and local tissue samples were collected from consecutively diagnosed patients, among them 23 cases suffered from large intestinal cancer, 20 cases from colorectal adenomatoid polyps and 20 healthy subjects. DNA was extracted from both the stool and the tissue, K ras gene mutations at the first and second base of codon 12 were studied by allele specific mismatch method in both of them. The results showed that K ras mutation rate in large intestinal cancer was 56 52%(13/23), which was much higher than 5%(1/20)in healthy subjects( P 0 05). There was a 92 31%(12/13)incidence of K ras mutation at the second base of codon 12 in large intestinal cancer. Idiopathic thrombocytopenic purpura(ITP) as an autoimmune disease,is the most common clinical hemorrhagic disease.But,its pathogenesis is not completely clear yet.Low platelet count can easily repeate in chronic ITP patients,there are poor efficacy in refractory ITP patients,too.In recent years,a series of important progress on the pathogenesis of ITP had been made.In the humoral mechanism,there were significant progress on the emergence of autoantibodies;the theory on anomaly of the megakaryocyte quantity and quality caused by autoantibody were put forward.In the cellular mechanisms,the theory on reduction in the number of Tr cells,Co-stimulative signal and platelets directly dissolved by cytotoxicity T cells were put forward also.This review will be made to sum up the study progress on the pathogenesis of ITP.

Objective To explore the significance of the changes of serum B cell activating factor(BAFF) in children with idiopathic thrombocytopenic purpura(ITP).Methods The concentrations of BAFF were detected by ELISA in 42 children with ITP(test group) before and after treatment in the Children′s Hospital Affiliated to Chongqing Medical University from Apr.to Nov.2008,the blood platelet count(BPC) was detected also.And 40 children who were operated selectively were selected as control group.The variation of concentrations of BAFF were analyzed among children with ITP before,after treatment and control group.Moreover,the relationship between serum BAFF expression and BPC were analyzed by the Pearson test.Results The concentrations of serum BAFF were higher in children with ITP before treatment compared with that in control group [(0.943 3?0.583 5) ?g?L-1 vs(0.538 9?0.234 7) ?g?L-1,P0.05).There was negative correlation between serum BAFF expression and BPC(r=-0.305,P

Objective To compare the efficacy of arthroscopic synovectomy between early and middle stage rheumatoid arthritis. Methods A total of 34 cases of rheumatoid arthritis (42 knee joints) either in early stage (22 cases, 24 joints) or in meddle stage (12 cases, 18 joints) underwent arthroscopic synovectomy. The bipolar radiofrequency was used for the synovial debridement and hemostasis in 32 knee joints. Postoperatively, routine anti-rheumatoid drugs were administrated. Follow-up observations in the two groups included the evaluation of knee function, the erythrocyte sedimentation rate (ESR), the C-reaction protein (CRP), and the rheumatoid factor (RF). Results The rate of excellent or good results was 91.7% in early stage (22/24) and 66.7% in middle stage (12/18), without statistical significance between the two groups ( ? 2=2.705, P =0.100). Conclusions Arthroscopic synovectomy is effective for the treatment of rheumatoid arthritis in both early and middle stage. Bipolar radiofrequency is helpful for the complete elimination of the synovium, the prevention of joint hematoma, and the rehabilitation of joint function.

Objective To explore the effect of the ?-blocker —metoprolol on the treatment of vasovagal syncope(VVS) in children.Methods Twenty-nine children with unexplained syncope and positive responses to head-up tilt test(HUT) were included in the study.Sixteen of them took metoprolol(treatment group) and 13 of them took vitamin B or oryzanol (control group) at least 2 weeks and HUT were repeated and syncope episodes were observed.Results In treatment group,9 of 16 patients had no syncope episode while 5 of 16 patients had fewer syncope episodes,1 case had more syncope episodes,and 1 case remained the same.HUT were repeated and 6 of 9 cases had negative outcome.In control group,1 of 13 patients had no syncope episode while 5 of 13 patients had fewer syncope episode,3 cases had more syncope episodes and 4 cases remained the same.HUT were repeated and 3 of 7 cases had ne-(gative) outcome.Conclusion These results indicate that adminstering metoprolol orally may be effective for VVS in children.
