2.Clinical research of canalicular intubation combined external dacryocystorhinostomy for chronic dacryocystitis and upper lacrimal duct stenosis

AlM: To evaluate the clinical efficacy of canalicular intubation combined with external dacryocystorhinostomy ( ext-DCR ) for treatment of chronic dacryocystitis and upper lacrimal duct stenosis.
METHODS:Thirty-three patients (33 eyes) with chronic dacryocystitis and upper lacrimal duct stenosis who underwent canalicular intubation combined ext-DCR were retrospective analyzed. The silicon tube was indwelt for 6mo. All cases were re-examined 1wk;1, 3, 6, 9mo post-operation, flow of tears, pus excretion and lacrimal duct clearance were observed.
RESULTS: Clear lacrimal duct ratio was 100% in all cases during 1wk~6mo post-operation; Silicon tube was removed 6mo post-operation, 3mo after tube removal, rechecking reported 32 cases with clear lacrimal duct (97%) and 1 case with resistant duct (3%). Two cases ( 6%) with minor lacerations, no other complications were observed.
CONCLUSlON: Canalicular intubation combined ext-DCR is an effective treatment for chronic dacryocystitis and upper lacrimal duct stenosis.

4.Effect of multi-disciplinary team on management of multidrug-resistant organisms

Objective To investigate the detection of multidrug-resistant organisms (MDROs)in a hospital, evaluate the efficacy of multi-disciplinary team(MDT)on management of MDROs,and provide guidance for effective control on MDRO infection.Methods From October 2013 to September 2014,compliance to comprehensive inter-vention measures in clinical departments in different stages as well as detection of MDROs from patients were com-pared respectively.Results Compliance to comprehensive intervention measures showed an overall upward trend from the fourth quarter of 2013 to the first,second,and third quarters of 2014,difference was statistically signifi-cant (all P <0.001 ).From the fourth quarter of 2013 to the third quarter of 2014,the percentage of the major MDRO strains in the same species of bacteria were:methicillin-resistant Staphylococcus aureus (MRSA)52.34%, 45.45%,48.95%,and 26.25% respectively;carbapenem-resistant Acinetobacter baumannii (CRAB)64.42%, 63.07%,59.87%,and 43.09% respectively;multidrug-resistant Pseudomonas aeruginosa (MDRPA)42.11 %, 41 .82%,29.33%,and 17.52% respectively;the detection rate of MRSA,CRAB,and MDRPA showed an overall downward trend,difference among different stages were statistically significant (all P <0.001 ).Detection rates of carbapenem-resistant Enterobacteriaceae (CRE)and vancomycin-resistant Enterococcus (VRE)were both low (<5%),difference among different stages were not statistically significant (all P >0.05).Conclusion MDT on man-agement of MDROs is helpful for reducing the emergence and spread of MDROs.
5.Effects of the overexpression imprinted gene PHLDA2 on radiosensitivity of osteosarcoma

Objective To study the effects of PHLDA2 overexpression on radiosensitivity and the underlying mechanisms in human osteosarcoma U2OS cell line.Methods To obtain the subclone,cells were exposed to G418 persistently after transfection of pEGFP-C3-PHLDA2 vector into U2OS cells.Three groups of blank control (U2OS),negative control (U2OS-neo) and transfected group (U2OS-PHLDA2) were used.The expression of PHLDA2 in the subclone cells was determined by Western blot.After exposure to X-ray irradiation,cellular growth activity and survival were detected by CKK-8 assay and colony formation assay,respectively.The cell apoptosis was measured by the Annexin V/PI staining,and the apoptotic protein was analyzed by Western blot.The in-vivo effects of PHLDA2 on irradiation were evaluated by xenografts.Results Compared with U2OS group and U2OS-neogroup,the sabclone cells were successfully obtained by G418 selection,in which the expression of PHLDA2 was upregulated(t =13.73,16.28,P < 0.05).In vitro,PHLDA2 overexpression significantly enhanced the response to radiation in U2OS cells with a reduction of colony survival and proliferation with the increase of doses (t =5.00-8.23,P <0.05;t =-2.52--1.26,P < 0.05).In vivo,PHLDA2-upregulated xenografts had more radiosensitivity than control groups with a significant inhibition of tumor growth (t =3.27,2.91,P < 0.05).After 8 Gy irradiation,the apoptosis was significantly increased (t =10.11,9.61,P < 0.05),accompanied with the activation of Caspased-3 in U2OS-PHLDA2 cells,which was presented by upregulation of cleaved Caspase-3 (t =11.26,10.72,P < 0.05).Conclusions Exogenetic expression of PHLDA2 could significantly enhance the radiosensitivity of human osteosarcoma cells,which may be attributed to the activation of Caspase-3 that increases irradiation-induced apoptosis.
6.Analysis of Clinical Symptoms and Blood Lipid Components in Simple Obesity Children

Objective To analyze blood lipid components in simple obesity children and to explore the effects of obesity in the lipid metabolism and its clinical significance.Methods A total of 90 children,including 50 simple obesity children(obesity group)and 40 normal children(control group),were enrolled in this study.The age ranged from 2.5 to 16.0 years.Their blood lipid profiles of all the children were analyzed.The blood lipid profiles were examined by biochemical analysis,including triglyeride(TG),total cholesterol(TC),low density lipoprotein(LDL) and high density lipoprotein(HDL),and the livers of all the children were analyzed.The blood lipid profiles were examined by ultrasonograph.Results 1.There were no significant differences in age and height in obesity group and control group,but there were significant differences in body mass index(BMI) and blood pressure(P0.05),which had no statistical meaning.3.Liver ultra sonogram showed that 18 cases had fatty liver(36%) in simple obesity children.Conclusions Metabolic disorder of blood lipid is present in simple obesity children,who have a tendency to get fatty liver.LDL is markedly elevated in obesity group.Arteriosclerotic cardiovascular disease should be prevented at earlier period of childhood.
7.Effect of Traditional Chinese Medicine Practolol Oral Liquid on Water Load Rat Model

Objective:To investigate the influence of practolol oral liquid on the rat model of water load. Methods: Totally 70 qualified rats were selected and randomly divided into seven groups with 10 ones in each, and the rats were with intragastric administra-tion respectively by practolol oral liquid (20,10 and 5 ml·kg-1 , respectively diluted 20 ml, 10 ml and 5 ml to 20 ml liquid with dis-tilled water), furosemide tablets suspension (0. 033 5 mg·ml-1), Qiliqiangxin capsule suspension(0. 03 g·ml-1) and distilled wa-ter with the same voulme( model) . The rats were placed in metabolism cages. After the administration, the urine volume was recorded every 1 h for 5 hours, the total volume of urine was collected during the 5 h, and the concentration of Na+, K+ and Cl- in the urine was measured. Results:Compared with that in the blank group, the urine volume in 2 h, 3 h and 5 h after the administration in the model group was significantly increased(P<0. 01), the total urine volume was also increased significantly(P<0. 01), the K+ con-centration was significantly reduced in the urine(P<0. 05), and there was an increasing trend of Na+ and Cl- concentration in the u-rine, which indicated that the water load model was made successfully. Compared with the model group, furosemide could significantly increase the urine volume in 1 h, 2 h, 3 h, 4 h and 5 h after the administration, and the total urine volume was increased as well ( P<0. 01). Qiliqiangxin capsule group could significantly increase the K+ concentration in the urine(P<0. 05), and furosemide could significantly reduce the K+ concentration in the urine (P<0. 05). High-dose practolol oral liquid could significantly increase the urine volume in 5 h after the administration and the Na+ and Cl- concentration(P<0. 01), mid-dose practolol oral liquid could significantly increase the urine volume in 3 h and 5 h after the administration and the total urine volume (P<0. 05), and improve the Cl- concen-tration in the urine (P<0. 05), and the Na+ and Cl- concentration in low-dose practolol oral liquid group was decreased significantly (P<0. 01). Compared with Qiliqiangxin capsule group, the high, medium and low-dose practolol oral liquid showed no significant in-fluence on the urine volume(P>0. 05), high-dose practolol oral liquid could significantly increase the Na+ concentration in the urine (P<0. 01), medium and low-dose practolol oral liquid could significantly increase the Na+ concentration in the urine(P<0. 05), low-dose practolol oral liquid could significantly reduce the K+ concentration(P<0. 01), high and medium-dose practolol oral liquid could significantly increase the Cl- concentration in the urine (P<0. 01), and low-dose practolol oral liquid could significantly reduce the Cl- concentration in the urine (P<0. 01) . Compared with the high-dose practolol oral liquid group, the influences of medium and low-dose practolol oral liquid showed no significant difference in the urine volumes(P>0. 05), however, the total urine volume could significantly increase(P<0. 01), and the medium and low-dose practolol oral liquid could significantly reduce the Na+ and Cl- con-centration in the urine(P<0. 01). Conclusion:Practolol oral liquid has significant diuretic effect in a dose-dependent manner.
8.Test-retest Reliability and Inter-rater Reliability of ASIA2000 Scale

ObjectiveTo investigate the test-retest reliability and inter-rater reliability of ASIA2000 scale.MethodsTwenty-five hospitalized spinal cord injury volunteers were chosen according to the selective criteria. The items were retested by the same examiner one week later to determine the scale's test-retest reliability. Another doctor did the same work 24 h later to determine the scale's inter-rater reliability.ResultsThe Pearson correlation coefficients of the pin prick score, light touch score and motor score of the test-retest reliability were all above 0.90 (P<0.05), and the Kappa values of the ASIS impairment scale (AIS) and completeness of the injury were 0.787 and 0.756 (P<0.05). The Pearson correlation coefficients of the pin prick score, light touch score and motor score of the inter-rater reliability were all above 0.85 (P<0.05), and the Kappa values of the AIS and completeness of the injury were 0.802 and 0.764 (P<0.05).ConclusionThe ASIA2000 scale has good test-retest reliability and inter-rater reliability.
9.Neurological Prognosis of Spinal Cord Injured Patients Using ASIA2000 Scale

ObjectiveTo investigate the application of ASIA2000 classification in prognosis of spinal cord injury patients' neurological function. MethodsConsecutive observation was done to the 42 patients using ASIA2000 classification 1 month, 3 months and 12 months after injury respectively. ResultsTo the incomplete SCI patients, motor score, pin prick score and light touch score were significantly improved 3 months after injury compared with that in 1 month after injury (P<0.05), but no significant improvement was observed in the complete ones, nor did in all the patients between 3 months and 12 months after injury. ConclusionThe Standard Neurological Classification is more sensitive than Standard Impairment Classification in ASIA2000. Individuals within 3 months after injury got better neurological recovery than those more than 3 months after injury. Incomplete injuried individuals may get better neurological prognosis than complete ones.
10.CT evaluation and classification of the Le Fort fracture

Objective To evaluate the CT findings and classification of the Le Fort fracture. Methods Sixty-two cases with Le Fort fractures were studied with thin-slice high-resolution CT scanning and analyzed with three-dimensional(3D)imaging reconstruction.Results Of the 62 patients,10 had Le Fort type Ⅰ fracture,9 had Le Fort type Ⅱ fracture,8 had Le Fort type Ⅲ fracture,and 35 had various combinations of the three types of Le Fort fractures,including 18 Le Fort Ⅰ+Ⅱ fracture,7 Le Fort Ⅰ+ Ⅱ+Ⅲ fracture and 10 Le Fort Ⅱ+Ⅲ fracture.Fifty-five cases had associated multiple fractures in the maxillofacial region.On 2D CT images,Le Fort fracture manifested as multiple and complex fractures. Though 2D image was better than 3D image in accurately defining tiny fractures and fractures of deep structures,the diagnosis of Le Fort fracture could not be correctly made solely on 2D image.3D CT clearly and stereoscopically demonstrated the entire shape and orientation of Le Fort fracture,thus facilitating the correct classification of Le Fort fracture.Conclusion 3D CT image is important in providing information about the space relationship of Le Fort fracture,thus very valuable for the preoperative planning.
