2.Comparative Study of the Unilateral Hemilaminectomy Approach and the Total Laminectomy Approach in the Treatment of Subdural Extr-amedullary Tumors

Objective To compare the clinical results of the unilateral hemilaminectomy approach and the total laminectomy ap-proach in the treatment of subdural extramedullary tumors.Methods One hundred cases with subdural extramedullary tumors from Sep-tember 2010 to September 2013 in the General Hospital of Ningxia Med.Univ.underwent surgery intervention were prospective analyzed. Among them,50 cases( the observation group) received tumor resection by the unilateral hemilaminectomy approach,and another 50 cases ( the control group) received the treatment by the total laminectomy approach.All received electrophysiological monitoring in the opera-tion.Operation time,intraoperative bleeding,postoperative hospitalization duration and postoperative recurrence rate in 6 months to 1 year etc.were recorded and compared.Results Operation time,intraoperative bleeding,postoperative hospitalization duration and postoperative fields time in the the observation group were less than the control group(P<0.05).There was no significant difference in curative effect aspects.In the postoperative follow-up(6 months to 1 year) ,the spinal instability cases in the observation group(0 case) was significantly less than the control group (6 cases).Conclusion Compared with the total laminectomy approach,the unilateral hemilaminectomy ap-proach in the treatment of subdural extramedullary tumors has more advantages as follows:better curative effect, smaller trauma, quicker recovery, fewer complications, great maintenance in spinal mobility and stability etc.
3.Study on Internet of things-based oxygen supply system monitoring technology in hospital

Objective:To study a set of system for supplying oxygen monitoring and control in networking. Methods: Oxygen monitoring devices, which are laid up the entrance of clinical department, include signal monitoring and control. Electromechanical actuator consists of oxygen valve and oxygen solenoid valve;Sensor group consists of a pressure sensor, an oxygen concentration sensor and a gas flow sensor;The design circuit includes signal acquisition and data processing, communication and the master circuit board, oxygen dedicated solenoid valve control driver circuit board. Results:The center monitoring technology for oxygen supplying in hospital can monitor oxygen pressure, concentration and flow of all clinical departments. Meanwhile, the monitoring center can be timely warned when oxygen pressure and concentration are lower than normal value. If firing appeared in ward, oxygen supplying can be closed by remote automatic or manual oxygen monitoring control device. Conclusion:This monitoring system can ensure quality and security of oxygen supply, and achieve remote monitoring and alarm of oxygen supply, and improve the level of oxygen supply management.
4.Effect of ultrasonic scaling combined with vinegar for treatment the dental calculus

Objective To investigate the clinical efficacy of ultrasonic scaling combined with the vinegar-for treatment the patients with dental calculus.Methods 120 patients with dental calculus dental were randomly divided into two groups.Control group used ultrasonic scaling treatment,observation group was given ultrasonic scaling combined with the vinegar treatment.Results The clinical cure rate of the observation group was 96.67% (58/60),the incidence rate of complications was 6.67% (4/60).The clinical cure rate of the control group was 68.33% (41/ 60),the incidence rate of complications was 21.67% (13/60).There were statistically significant differences between the two groups about the cure rate and the incidence of complications (x2 =14.37,3.61,P < 0.05).After treatment,periodontal pocket depth of the observation group was significantly lower than that of the control group (P < 0.05).Sulcular fluid volume of the observation group was significantly lower than that of the control group (P < 0.05).The difference of oral sample results of bacterial culture after treatment was statistically significant between the two groups (P < 0.05).Conclusion The clinical effect of ultrasonic scaling combined with the vinegar in treatment patients with dental calculus is certain,which is worthy of further promotion.
5.Application of pemetrexed for the treatment of non-small-cell lung cancer

Pemetrexed is a novel antifolate which is approved in the USA and Europe for the treatment of non-small-cell lung cancer (NSCLC).Researches reveal that pemetrexed enhances radiation-induced cell inactivation.A series of clinical trials of pemetrexed combined with radiotherapy for NSCLC also demonstrated that pemetrexed provides similar efficacy with lower toxicity and better tolerability than other third-generation chemotherapy drugs.
6.Efficacy and safety of adjuvant intravitreal injection of anti-vascular endothelial growth factors prior to vitrectomy in the treatment of proliferative diabetic retinopathy: A Meta-analysis

AIM: To investigate the effectiveness and safety of intravitreal injection of anti-vascular endothelial growth factors (VEGF) drugs to the patients with proliferative diabetic retinopathy before vitrectomy treatment.METHODS: A Meta-analysis.A comprehensive retrieval was conducted using the database including EMbase,the Cochrane Library,Pubmed,CBM,WanFang Database,CNKI and so on.The retrieval time was limited from the building time of database to Jan.2017.The randomized controlled trial was adopted with no requirements on languages.The Jadad scale and Cochrance cooperation were used as the tool of the risk and bias evaluation to analyze the literature quality.Quality estimation of evidence-based medicine on the parameters of each evaluation index was made via GRADEpro Software.The publishing biases of enclosed documents were inspected with funnel plot.At last,the Meta analysis was conducted with Review Manager 5.3.RESULTS: Totally 16 literatures published from 2008-2016 were finally put into randomized controlled trial.A total of 923 cases were included,among which 493 cases were grouped as intravitreal injection of anti-VEGF before the combined operation of PPV group (the experimental group),and 430 cases were involved in simple PPV group (the control group).The results of Meta-analysis show: (1) The probability of intraoperative bleeding was remarkably lower than the control group [OR=0.06,95%CI (0.02,0.15),P<0.01],with statistically significant differences.(2) The duration of operation was obviously shorter than that of the control group[WMD=-29.13,95% CI (-36.95,-21.30),P<0.01],with statistically significant differences.(3) The probabilities of both early and late postoperative vitreous hemorrhage were lower than those of the control group[OR=0.34,95%CI (0.20,0.58),P<0.01],with highly statistically significant differences.(4) The best corrected visual acuity after surgery was no better than that of the control group,[WMD=-0.51(LogMAR),95%CI(-1.10,0.08),P=0.09] with no statistical significance.(5) The occurrence of iatrogenic retinal rupture was lower than that of the control group[OR=0.24,95%CI(0.14,0.40),P<0.01],with statistically significant differences.(6)Among the 493 patients included in the 16 literature,no case of ocular and general adverse effects due to anti-VEGF injections was reported.CONCLUSION: It is effective and safe for the patients with proliferative diabetic retinopathy to inject anti-VEGF drugs into vitreous cavity before vitrectomy.And it can reduce the occurrence of complications during and after surgery,improving the general treatment effects.However,the specific implementation plans,such as the injection does,the interval between the injection and the operation and so on,still need further exploration and perfection.

The corneal reconstruction is tosurgial recover the structure integrity and corneal function after suffered from various trauma, inflammation and degenerative diseases.The corneal diseases caused millions of people worldwide suffering from eyesight damages and even blindness.At present, the corneal transplant is the main therapy for corneal blindness.However, the shortage in donor corneal issue is a worldwide problem and the failure due to the immunologic rejection of host is common.Nowadays, with the development of tissue culture and bioengineering technology, the application prospect of autologous stem cell transplantation is becoming more and more popular which might replace the allogeneic transplantation, becoming an important clinical treatment of regenerative medicine.Human dental pulp stem cells (hDPSCs) is a class of adult stem cell divided from the third molar teeth.Both hDPSCs and corneal cell are from the cranial nerve in embryonic ectoderm.Extensive researches show that the hDPSCs have the potentialities in corneal cell differentiation without causing immunologic rejection of the recipient.These findings manifested the potentials of hDPSCs in the clinical applications related to ocular surface reconstruction.In this paper, the features and current investigation status of hDPSCs in ocular surface reconstruction are reviewed.
8.Evaluation of the clinical effectiveness of silicone oil removal in high myopia with macular hole by optical coherence tomography

Background The recurrent rate of retinal detachment is much higher after silicone oil removal in high myopia with macular hole.How reduce the recurrence is a hot topic.Whether optical coherence tomography (OCT)-assisted examination is helpful for the improvement of surgery successful rate is in clinical observation ObjectiveThis study was to observe macular retinal status before silicone oil removal in eyes with macular holerelated retinal detachment in highly myopic eyes by OCT and evaluate the factors that influence successful silicone oil removal.MethodsFifty-five eyes of 49 consecutive patients who received vitreoretinal surgery for highly myopic retinal detachment caused by macular hole from January 2005 to December 2008 were involved,and 45 eyes of 40 patients who underwent silicone oil tamponade were retrospectively analyzed.Three to six months after vitreoretinal surgery combined with silicone oil tamponade,removal of silicone oil was performed based on the macular appearance on the OCT and the patients were followed up for over 1 year.The OCT manifestation of macular hole and the best corrected visual acuity (BCVA) were compared between before vitrectomy combined with silicone oil tamponade surgery,and after removal of silicone oil.The healing of the macular hole was classified into type Ⅰ and Ⅱ healing according to the OCT results.ResultsThe retina at the macular area completely reattached in 40 eyes(88.89% )before removal of silicone,and the margin of the macular hole had disappeared in 2 of the 40 eyes,showing type Ⅰ heal.Thirty-eight eyes were determined to exhibit type Ⅱ healing,presenting with a visible hole margin and local defection of nervous fiber layer.Two years after the removal of silicone,retinal detachment reappeared in 1 eye with type Ⅱ heal.The BCVAs were 1.93±0.06 and 1.16±0.07 before and 1 year after removal of silicone,respectively,showing a significant difference between them(P =0.00).ConclusionsOCT is a useful tool for the prediction of anatomic outcomes in macular hole-related retinal detachment eyes with high myopia.Silicone oil removal can be performed in macular hole closure eye or attached-well hole edge eye based on OCT examination.However,attachedwell hole edge eye should receive longer follow-up time.

A survey on drug resources and identification or the commercial drugs showed that Heshouwu was derived from Polygonum multiflorum and the roots of another two species of the same family were misused as Heshouwu. The roots of 6 species of Cynanchum genus, Asclepiadaceae were used as Baishouwu in different regions. The index of botanical identification was listed. Botanical origins, distribution, production and sale of "Shouwu" were investigated.
10.The clinical efficacy of inductive chemotherapy with pemetrexed followed by radiotherapy for patients with lung adenocarcinoma

Objective To compare the efficacy and toxicity of inductive chemotherapy with pemetrexed followed by radiotherapy for patients with lung adenocarcinoma .Methods Patients with stage Ⅲa/b or Ⅳ and histologically confirmed lung adenocarcinoma were evaluated from July 2008 to April 2012 ( n=95 ) .Patients re-ceived inductive chemotherapy with pemetrexed or paclitaxel followed by thoracic radiotherapy or chemotherapy with pemetrexed alone,respectively.Primary end point was the tumor response rate (RR),progression-free sur-vival(PFS),1-year survival rate and toxicity.Results Tumor RR were 83.3%,81.8%and 62.5% in three groups of inductive chemotherapy with pemetrexed or paclitaxel followed by thoracic radiotherapy or chemotherapy with pemetrexed alone ,respectively.PFS was 12.0 months,10.9 months and 5.7 months.The 1-year survival rate was 80.0%,78.7%and 46.8%.Tumor RR and PFS were statistically superior in pemetrexed/radiotherapy versus pemetrexed alone ,and toxicity was significantly lower than in paclitaxel /radiotherapy.Conclusion The treatment of pemetrexed/radiotherapy for lung adenocarcinoma could prolong the survival of patients .Compared with the treatment of paclitaxel/radiotherapy ,pemetrexed/radiotherapy provides similar efficacy with better tolera-bility and more convenient administration .
