AIM: To evaluate the safety and efficacy of one transscleral sutured fixation intraocular lens implanted in the capsular treating traumatic lens dislocation.
METHODS: Twelve eyes with lens subluxation from 12 patients during Mar. 2013 to Mar. 2015 were reviewed. The stopping and chopping method combined with manual nuclear extraction was performed in extent of lens subluxation less than 1/2 quadrant and transscleral sutured fixation intraocular lens implanted in the capsular. Visual acuity, best - corrected visual acuity, intraocular pressure, corneal endothelial cell count and the position of IOL were observed and recorded.
RESULTS: All the surgeries were performed successfully. Patients were followed up for 6 - 12mo. During the follow-up period, the number of eyes for BCVA ≥0. 8, 0. 4 - 0. 6 and ≤0. 3 was 2, 7 and 3, respectively. It meant 66. 67% of the eyes showed BCVA in 0. 5-0. 6. Intraocular pressure and the position of all intraocular lens were normal. Effects of operation on corneal endothelial cells were slight. No complications took place in and after surgery.
CONCLUSION: Without implanting capsular tension ring ( CTR ) , we successfully use the intraocular lens ( IOL) single loop suture fixation in the capsular bag for the treatment of the patient with traumatic lens dislocation. It indicates that the pressure and place shift from the use of IOL avoided by this method without implantation of CTR. This method is safe and effective for the treatment of eyes with traumatic lens dislocation.

Objective:To discuss the oncologic safety, aesthetic outcome, and upper extremity function of single-port insufflation endoscopic nipple-sparing mastectomy (SIE-NSM) in the treatment of early breast cancer. Methods:From January 2014 to August 2019, a total of 80 patients with stage Ⅰ and Ⅱ breast cancer underwent SIE-NSM, at Beijing Friendship Hospital, Capital Medical University were retrospectively analyzed and followed up. Estimated the oncologic safety, aesthetic outcome, and upper extremity function.Results:SIE-NSM was performed successfully on all 80 patients. There was no serious complication after surgery. The follow-up time was 16-82 months and the median follow-up time was 42 months. Local recurrence occurred in two patients, and there was no distant metastases. Four (5%) patients developed grade 1-3 nipple-areola complex ischemia. There were no cases of subcutaneous effusion. The satisfaction with breasts, chest well-being, psychosocial well-being, and sexual well-being scores were confirmed to be highly rated by Breast-Q scale. Upper extremity function score confirmed that good upper limb function was preserved after surgery. The mean length of incision was (3.6±0.8) cm, and the blood loss was (24.7±19.3) mL.Conclusions:SIE-NSM can achieve a higher cosmetic score and a better recovery of upper limb function on the premise of ensuring the safety of the tumor. This novel method is an appropriate surgical option for patients with early breast cancer.

Myeloid-derived suppressor cells (MDSCs) play a crucial role in T cell dysfunction, which is related to poor outcome in patients with severe trauma. Cyclooxygenase-2 (Cox-2) contributes to immune disorder in trauma and infection via production of prostaglandin E2. However, the role of Cox-2 in the accumulation and function of MDSCs after traumatic stress has not been fully elucidated. In the present study, we treated murine trauma model with NS398, a selective Cox-2 inhibitor. Then the percentages of CD11b+/Gr-1+ cells, proliferation and apoptosis of CD4+ T cells were determined. Arginase activity and arginase-1 (Arg-1) protein expression of splenic CD11b+/Gr-1+ cells, and delayed-type hypersensitivity (DTH) response were analyzed. The results showed that Cox-2 blockade significantly decreased the percentages of CD11b+/Gr-1+ cells in the spleen and bone marrow 48 and 72 h after traumatic stress. NS398 inhibited arginase activity and down-regulated the Arg-1 expression of splenic CD11b+/Gr-1+ cells. Moreover, NS398 could promote proliferation and inhibit apoptosis of CD4+ T cells. It also restored DTH response of traumatic mice. Taken together, our data revealed that Cox-2 might play a pivotal role in the accumulation and function of MDSC after traumatic stress.

Objective To evaluate the effect of operation on aged women with breast cancer. Methods The clinical data of the 166 patients recruited from 1989 to 2004 were retrospectively analyzed. Results One hundred and sixty six patients were all treated by operation. The modified radical mastectomy was performed in 98 cases(59.0%), radical masteetomy in 21 cases(12.7%), mastectomy in 29 cases (17.5%), palliative operation in 18 cases (10.8%). After operation, one hundred and twenty six patients received chemotherapy, twelve radiotherapy and 73 endocrine therapy. The rates of postoperative complications were 10.8%, no patients died during operation. The overall postoperative 3-and 5-year survival rates were 82.2% and 65.2% respectively. Conclusions The effect of surgical treatment and its combination with other adjunct therapies is satisfactory for aged women with breast cancer. Different types of operations should be performed for different patients. Ageing is not a contraindication of the operation. Surgery should be combined with medical therapy to reduce the concomitant diseases and postoperative complications.
