BACKGROUND:Numerous studies have focused on the clinical efficacy of alogeneic bone graft and humeral head replacement for the treatment of proximal humeral fractures, but their comparative studies are rarely reported. OBJECTIVE:To investigate the effect of alogeneic bone grafting in the treatment of proximal humeral fractures in the high risk group. METHODS:Clinical data of 120 cases of proximal humeral fractures aged≥ 60 years were retrospectively analyzed. Sixty of the 120 cases underwent alogeneic bone grafting combined with locking plate fixation as experimental group, and the other 60 cases were subjected to semi-shoulder joint replacement as control group. Al patients were folowed up for 8-24 months. Fracture healing, colodiaphyseal angle, humeral head height and shoulder joint function were observed and measured. RESULTS AND CONCLUSION:During the postoperative 8-24 months, al the fractures were healed by first intention, and there were no rejection reactions, large/smal nodules, humeral head displacement, necrosis, and screw loosening. Loss of the humeral head height at the last folow-up and the active flexion angle of the shoulder at postoperative 12 weeks were significantly higher in the experimental group than the control group (P < 0.05). Scores on forearm, shoulder and hand dysfunction were significantly lower in the experimental group than the control group (P < 0.05). However, no significant difference was observed in the colodiaphyseal angle and SF-36 scores between the two groups. These finding indicate that alogeneic bone grafting can strength the internal fixation of proximal humeral fractures in the high-risk group, and improve patient’s upper limb function.

Objective To survey village doctors'knowledge, demand and behavior of health management,to provide scientific evidence for village people's health self-management.Methods Prepared a self-designed question-naire and make it completed by 106 village doctors chosen at random from 106 different villages in the eight towns in Guangzhou's two mountainous districts. Results The average age of the village doctors was(50.9 1 ±7.84)years, 36.79% of them had the ultimate diploma of a health technical school, and the average knowledge rate of health man-agement was 30.09%.There was remarkable difference among the 3 age groups which disclosed statistic value(χ2 =112.900,P =0.000).The 35-44 age group was the highest in the knowledge rate of health management (52.61%). The doctors with a ultimate technical diploma vary in the knowledge rate (the highest was 47.44%),which also had statistic value(χ2 =112.900,P =0.000).Only 2.83% of the village doctors believed that their health management skill can completely meetthe current demand and 3.77% can.18.87% believed that training about health management was very meaningfuland 25.47% meaningful.63.21% (most required by the health bureau) expected to receive training.16.98% didn't receive such training in the recent year because they thought it was un-necessary or not available.11.32% of the village doctors were willing to pay for the health management training, while 97.17% had never taken such training on their own expenses. Conclusion Village doctors are mostly elderly,low-educated,and insufficient in the professional knowledge of health management.Without the corresponding motivations and regulations, they are not enthusiastic about the training. It is suggested that the local authorities pay more attention to the buildup of the village health stations and improve the structure, knowledge, and earning of the village doctor group so as to ensure the effective health management service for the masses in villages.

Chinese Journal of Digestion 2008;28(11):764-766
Objective To study the potential role of nerve growth factor(NGF)on visceral hypersensitivity in rats. Methods Visceral hypersensitivity was induced by intraperitoneal injection of NGF with or without water avoidance stress in N group(n=8)and NW group(n=8),respectively.The rats in controI group(n=8)were intraperitoneaIly injected with normal saline and received shame water avoidance stress.Electromyogram activity of the external oblique musculature was measured by means of the visceromotor response(VMR)during colorectal distention(CRD).Statistical analysis was made by ANOVA.Results There was,nb statistical difference among three groups in the VMR at pressure of 20,40 or 60 mmHg(1 mmHg=0.133 kPa).When the pressure at 80 mmHg,the VMR was higher in NW group(3.54)than that in control group(2.12)(P=0.016).while it was 2.62 in N group with no difference to control group. Conclusion The colorectal visceral sensitivity in rats may be promoted by NGF in presence of water avoidance stress.

Chinese Journal of Organ Transplantation 2005;0(11):-
Objective To analyze the causes and outcome of conversion from tacrolimus (FK506) to cyclosporine-A (CsA) in recipients after liver transplantation. Methods 317 consecutive liver transplantation recipients in our department received anti-CD25 monoclonal antibody,FK506, mycophenolate mofetil and corticoid for prophylaxis of cellular rejection. The blood FK506 trough level was (10-15)?g/L within the first 30 days,(8-12)?g/L within next 60 days, and (5-8)?g/L was kept during 90 to 180 days after transplantation. All recipients reveived a follow-up of 6 months. Remits Sixteen out of 317 recipients (5.05%) required conversion from FK506 to CsA.The clinical indications for conversion included:neurological adverse effect of FK506 in 5 cases (31.25%),hema- tological adverse effect in 2 cases (12.5%),gastrointestinal effect in one case (6.25%),not capable of reaching therapeutic window concentration in 3 cases (18.75%),refractory hyperglycemia in 2 ca- ses (12.5%),and economic factor in 3 cases (18.75%).The majority of recipients demonstrated clinical improvement after the switch, except 2 of 16 patients (12.5%) had to be reconverted to FK506 due to renal disadvantage. No dead recipient and adverse effect correlated to immunosuppres- sive agent conversion were seen. Conclusion If necessary, conversion from FK506 to CsA in patients undergoing liver transplantation is safe and effective.

Chinese Journal of Endocrinology and Metabolism 1985;0(02):-
In adipose tissues from sreater omentum of patients with type 2 diabetes, the mRNA and protein expressions of human rearranged immunoglobulin?light chain (HSIGVL) 022 were measured by the fluorescent quantitative RT-PCR and immunohistochemistry respectively. The results showed that mRNA and protein levels of HSIGVL022 were up-regulated in patients with type 2 diabetes. The mRNA level of HSIGVL022 was linearly correlated with insulin resistance index, suggesting that this gene may play a role in the pathogenesis of insulin resistance and type 2 diabetes.
