1.Analysis on related factors in 1 712 cases of psychological sub-health state

Objective To explore the general characteristics of the psychological sub-health state and to analyze the factors pos-sibly leading to the psychological sub-health state in order to provide the scientific basis for the treatment and prevention of the psy-chological sub-health state .Methods The individuals with the physical examination in our hospital were performed the psychologi-cal heath survey .Among them ,1 712 cases of psychological sub-health were taken as the research group and contemporaneous 1 902 cases of normal health were taken as the control group .The Symptom Check List-90(SCL-90) was adopted to compare the two groups and the logistic regression analysis was adopted to analyze the risk factors possibly leading to the psychological sub-health occurrence .Results The average scores of 20 factors in SCL-90 in the research group were higher than those in the control group , the difference was statistically significant (P<0 .05) ,in which ,the scores of interpersonal relationship sensitivity (22 .1 ± 5 .7) ,para-noid(14 .1 ± 5 .2) and the others(17 .5 ± 6 .3) in the research group were significantly higher than the critical scores with obvious symptoms .The logistic regression analysis results showed that :the sleep quality(OR=1 .843) ,accidental injury(OR=2 .634) ,eco-nomical status(OR=3 .032 ,and professional type(OR=4 .684) were the independent risk factors of the psychological sub-health occurrence .Conclusion The local patients with psychological sub-health were more prominent in the more sensitivity of the inter-personal relationship ,paranoid and poor sleep ,especially the intellectuals ,individuals with poor sleep and the crowd with accidental injury are prone to develop psychological sub-health .
2.Effect of atorvastatin pretreatment and ischemia preconditioning on I/R injury in rats

Objective To study the effect of atorvastatin pretreatment and ischemia preconditioning on I/R injury in rats .Methods Thirty‐two Wistar rats were randomly divided into sham opera‐tion group ,I/R injury group ,ischemia preconditioning group ,and atorvastatin treatment group (8 in each group) .A reversible focal I/R injury model of rats was established by Longa occlusion of middle cerebral artery .Cerebral tissue samples w ere taken ,cut into 3 μm‐thick sections w hich were stained with HE to observe the morphologic changes in frontal lobe ,parietal lobe and hipp‐ocampal CA1 .Results The karyopyknosis in frontal lobe and cytoplasm acid degeneration in hip‐pocampal CA1 were more significantly in I/R injury group than in sham operation group ,ischemia preconditioning group and atorvastatin treatment group (P<0 .01) .Conclusion ischemia precon‐ditioning and atorvastatin pretreatment can protect rats against I/R injury .
3.Optimization of hydrolysis process of linarin using response surface methodology and research about ARI activity of glycosylation-acacetin.

OBJECTIVETo optimize the hydrolysis process of linarin by response surface methodology, and to use the model of aldose reductase to study the acacetin's activity of aldose reductase inhibitory.
METHODThe model of acacetin enzyme in vitro was established by the determination of fluorescence absorption of NADPH, the inhibition rate of acacetin aldose reductase was calculated, and then the IC50 of hydrolysis was obtained. The hydrolysis process of linarin hydrolysis condition was optimized by using response surface method.
RESULTThe results indicated that the IC50 of acacetin (2.74 mg x L(-1)) was less than the IC50 of linarin (3.53 mg x L(-1)). Hydrolyzation time of 7.4 h, sulphuric acid concentration of 0.54 mol x L(-1) and the ratio of material to liquid of 3 : 1 were the optimum conditions.
CONCLUSIONHydrolyzate acacetin has preferable inhibitory activity of aldose reductase. The optimized hydrolysis condition of linarin is convenient to use with good predictability.

4.Study on Syndrome Development Rules before and after Treating Primary Nephrotic Syndrome with Hormone

Objective]Based on 346 cases of PNS patients using glucocorticoids observed during treatment, the main symptoms, syndrome differentiation of syndrome type of traditional Chinese medicine(TCM) by induction, and discusses its the evolution regularity of TCM Syndromes.[Methods]Develop a unified observation table, then clinicians recorded symptoms, signs, tongue picture and type of pulse to log on to observe truthfully and un-mistakbly table, after the end of observation, comprehensive analysis by the four examination methods, using a unified standards for TCM syndrome type determination. Discussion after glucocorticoids treatment of patients with PNS TCM syndrome objective laws of evolution.[Result]First ,TCM syndrome evolution:the results show that:before hormone treatment, Qi deficiency of spleen and kidney and Yang deficiency of spleen and kidney have become the most important type of syndrome;hormone use initial stages 1~4 weeks, Qi deficiency of spleen and kidney, deficiency of Qi and Yin, when the hormone used for 5~8 weeks. Yin deficiency of liver and kidney and deficiency of Qi and Yin occupied the main position; during the hormone withdrawal stage, the deficiency of Qi and Yin increased significantly; in the hormone maintain stage it was mostly deficiency of Yin and Yang and Yang deficiency of spleen and kidney. As for demonstration, in hormone therapy before a lot of patients suffering from retention of water and turbid damp;After the treatment of 1~4 weeks it was mostly humid heat ,turbid damp and blood stasis;the accompanying symptoms were mostly humid heat, blood stasis and pathogenic factor of wind after the treatment of 5-8 weeks ;Miscellaneous accompanying symptoms were very conspicuous except retention of water in the treatment of hormone withdrawal stage; In the hormone maintaining stage, the highest proportion of blood stasis compared with others. The use of hormone therapy, humid heat and blood stasis were more prominent than others. Second, after the hormone treatment(1~8weeks) the relationship between efficacy and TCM syndromes: A total of 130 patients enrolled, the 96 patients with Yin deficiency syndrome, the rest of the patients had no deficiency of Yin syndrome. The results indicated that Yin deficiency syndrome group serum albumin and 24-hour urinary protein excretion efficacy than those without Yin deficiency syndrome group, with statistical significance.[Conclusion] Glucocorticoids should belong to pure Yang medicine in traditional Chinese medicine, the body under the action of pure Yang drugs are prone to Yin deficiency symptoms, suggests that the higher the sensitivity to drugs, the clinical remission rate is also higher. This can be applied in clinical glucocorticoids and observe the change of the patients with syndrome, early in the patients' sensitivity to the glucocorticoids.
5.The relationship study between the changes of the frangible lymphocyte and the degree of destroyed liver function in fulminant viral hepatitis

Objective To study the changes and the significance of the frangible lymphocyte in patients with fulminant viral hepatitis and investigate the relationship between the degree of frangible lymphocyte and the severe of fulminant viral hepatitis. Methods 32 cases confirmed with fulminant viral hepatitis, 66 cases with chronic hepatitis have been studied. 15 cases children from 3 to 5 years old and 46 cases adults have been used with control groups. 100?l lymphocytes (1?106/ml) and 100?l tumor cells(1?105/ml) mixed with 100?l self-serum at 37℃ for 5min to 30min. The fragmental lymphocyte can defined as a frangible lymphocyte. The lymphocyte attached with tumor cell is undestroyed anything can be regarded as negative frangible lymphocyte. Less than 30 percent of lymphocyte attached with tumor cells is destroyed will be conformed as low-grade frangible lymphocyte, between 30 percent to 60 percent regard as mid-grade, and more than 60 percent as severe-grade mode. Results The equiform frangible lymphocyte is detected in fulminant viral hepatitis and other groups like that in SARS in 2003 years. There is none of children have frangible lymphocyte. Only 3 person’s lymphocytes show severe-grade frangible and 9 people display mid-grade in 46 adult individuals. Contrarily There is high of 46.9 percent of cases with fulminant viral hepatitis show severe-grade frangible lymphocytes, only 4 cases is negative and 3 cases is recovered in one months of them. The frangible lymphocytes in fulminant viral hepatitis are significantly lower than those in chronic hepatitis patients not only in mid-grade frangible but in severe type. There is no significant recovery of frangible lymphocyte from acute to recover in fulminant viral hepatitis, but the degree of the lymphocytes are closely relate to the state of an illness. Conclusion The percent of frangible lymphocytes in children is lowest among adult people, chronic hepatitis and fulminant viral hepatitis. The proportion of frangible lymphocytes in fulminant viral hepatitis is highest, and significantly higher than that in chronic hepatitis. The phenomenon of frangible lymphocytes may be an important characteristic of an innate immunology reaction of the body.
7.Comparative study of duloxetine and psychological intervention in the treatment of somatoform disorders

ObjectiveTo observe the clinical efficacy of duloxetine combined with psychological intervention in patients with somatoform disorder.Methods67 patients with somatoform disorder were randomly divided into two groups:study group (34 cases) treated with duloxetine and psychological intervention and control group (33 cases) treated with duloxetine only.And all the 67 patients in the study were assessed by symptom checklisth-90-R (SCL-90),hamilton anxiety scale(HAMA) and global assessment scale(GAS).ResultsAfter 8 weeks treatment,the results of SCL-90 showed that the scores of somatization,obsessive-compulsive,depression,anxiety and phobia factors in the study group ( 1.39 ± 0.45 ; 1.66 ± 0.52 ; 1.83 ± 0.69 ; 1.73 ± 0.66 ; 1.30 ± 0.48 )were notably lower than that in the control group ( 1.78 ± 0.61 ; 1.69 ± 0.47 ;2.39 ± 0.54 ; 2.1 1 ± 0.49 ; 1.49 ±0.78 ).And there were significant statistical differences (P< 0.01 ).At the end of the 2nd and 4th week the,scores of SCL-90 somatization,depression,anxiety and phobia factors were lower in the study group than the control group with statistical significance (P<0.05).At the end of 8th week,the HAMA score in the study group( 10.20 ±3.70) was significantly decreased than the control group (13.07 ± 3.20 ),and with statistical significant (P <0.05 ).The response rate was 85.29% in study group and 60.60% in control group.And there was a statistically singnificant difference in complete remission ( χ2 =4.08,P < 0.05 ) between the two groups.ConclusionThe study indicates that the combination of duloxetine and psychological intervention is more effective in treating somatoform disorder than using duloxetine only.So this combination treatment is worth generalizing and applying.
8.Therapeutic effects of catheter balloon dilatation therapy for neurogenic cricopharyngeal achalasia

Objective To investigate the effects of catheter balloon dilatation on patients with dysphagia caused by neurogenic cricopharyngeal achalasia.Methods Thirty-two patients with dysphagia caused by neurogenic cricopharyngeal achalasia were divided randomly into a treatment group (n =16) and a control group (n =16).The treatment group received catheter balloon dilatation therapy and routine dysphagia rehabilitation training; the control group was treated with routine dysphagia rehabilitation training only.Both groups were tested before treatment,after 3 weeks of treatment (after treatment) and at 6 months after treatment (follow-up).All cases were evaluated using Saitoh's 7-grade swallowing assessment and videofluoroscopy.Results After treatment,5 cases in the treatment group had been cured (31%) and 8 cases showed marked effectiveness (50%) ; in the control group,1 case was cured (7%) and 5 cases showed marked effectiveness (31%).These differences were statistically significant.At follow-up,5 cases in the treatment group had been cured (31%) and 9 cases showed marked effectiveness (56%) ; for the control group,1 case was cured (7%) and 6 cased showed marked effectiveness (38%).Those differences were also statistically significant.After treatment and also at follow-up,the average time for a bolus to pass the pharynx in the treatment group was significantly shorter than that in the control group.Conclusion Catheter balloon dilatation therapy is effective for treating neurogenic cricopharyngeal achalasia in both the short and long term.
9.The correlation among HBV DNA load, HBeAg/Anti-HBe and Alanine Aminotransferase in patients with chronic hepatitis B infection

Objective To explore the relationship among HBV DNA load, HBeAg/Anti-HBe and ALT in patients with chronic hepatitis B infection. Methods HBV markers in 134 patients with chronic HBV infection were detected by ELISA,and patients were divided into group Ⅰ (HBeAg +/Anti-HBe-,43 patients) ,group Ⅱ (HBeAg-/Anti-HBe + ,69 patients)and group Ⅲ (HBeAg-/Anti-Hbe-,22 patients)according to the results of presence of HBeAg/Anti-HBe. HBV DNA load and ALT were tested respectively by fluorescence quantitative polymerase chain reaction technology and successive monitor reaction. Results Both the positive rate and the HBV DNA load quantification in group Ⅰ were higher than in group Ⅱ and Ⅲ (P<0.01). Abnormality rate of ALT in HBV DNA positive patients was higher than the patients with HBV DNA negitive(P<0.01). There was no relationship between HBV DNA load and ALT in HBV DNA positive patients. Conclusion There was a certain corelation among HBV DNA load, HBeAg/Anti-HBe and ALT,but the active degree of HBV replication could not to be assessed by HBeAg/Anti-HBe or ALT alone. It was necessary for the padient with chronic HBV infection to examine them together.
10.Application of self- management programme in home care of oncohematologic patients with peripherally inserted central catheters

Objective To investigate the effect of self- management programme in home care of oncohematologic patients with peripherally inserted central catheters(PICC). Methods Seventy oncohematologic patients with PICC were divided into the control group and the intervention group with 35 patients in each group by random digital table method. The control group received the routine care. The intervention group received the self- management programme. Self- care ability, anxiety, catheter- related complications and maintenance cost were observed before and on the 10th day and 30th day after intervention. Results On the 10th day and 30th day after intervention, the total score and scores of each dimension of self- care ability in the intervention group were significantly higher than those in the control group, P<0.05 or 0.01, the differences were significant. The score of anxiety in the intervention group was significantly lower than that in the control group [for the patients on the 10th day, 47.64±4.40 vs. 51.14±6.28,on the 30th day,44.36±3.71 vs. 48.64± 5.54, t=3.40, 5.18, P<0.01; for the family members of patients on the 10th day, 47.36±2.12 vs. 50.57±3.99, on the 30th day,43.57±2.04 vs. 47.57±2.79, t=4.35, 7.42, P<0.01. The incidences of catheter- related complications were not significantly different between two groups, P>0.05. Conclusions Self- management programme can effectively improve the self-care ability, relieve the anxiety and prevent the catheter- related complications in home care of oncohematologic patients with PICC.
