1.Characteristics and the application status of tissue repair of small intestinal submucosa

Small intestinal submucosa has been applied in a variety of areas of basic and clinical study with its Characteristics of anti-infective and Low immunogenicity and so on.However,it is common to find the shrinkage and other problems in some tissue repair,and affects the functional recovery of repaired tissue to a certain extent.Thus,it is still a question to be resolved that how to avoid the shrinkage and other adverse effects after tissue repair.This paper provides a reliable basis for further correct understanding of the SIS's capacity of tissue repair by reviewing and summarying its characteristics and present situation of tissue repair,especially put forwarding the shrinkage problem.

3.Apolipoprotein E gene polymorphisms in patients with primary osteoporosis

BACKGROUND: In recent years, foreign scholars have found that apolipoprotein E gene was associated with osteoporosis and its complicated hip fracture. National scholars adopted polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) to primarily study the correlation of apolipoprotein E gene polymorphism with bone mineral density (BMD) and osteoporosis.OBJECTIVE: To observe the polymorphism of apolipoprotein E gene, and analyze its correlation with primary osteoporosis and osteoporotic fracture.SETTING: Department of Orthopaedics, Nanchang First Hospital.DESIGN: A clinical controlled observation.PARTICIPANTS: Sixty patients with primary osteoporosis (31 males and 29 females), were enrolled, including 30 cases of hip fracture, who were newly fractured and diagnosed according to radiogram, and 30 cases of simple osteoporosis; Thirty healthy subjects (16 males and 14 females)were also involved.METHODS: Sixty outpatients and inpatients with primary osteoporosis were selected from the Department of Orthopaedics, Nanchang First Hospital from January to December 2002, and they were divided into hip fracture group (n=30) and simple osteoporosis (n=30); Another 30 healthy physical examinees were taken as the healthy control group. All the subjects were fasted for more than 12 hours, and the samples of venous blood were collected on the next morning. DNA was extracted from the separated leucocytes. Gene site DNA amplification was analyzed with polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP). The individual DNA templates (provided by Daan Gene Diagnosis Center of Sun Yat-sen University) of identified apolipoprotein E2/2, E3/3 and E4/4 homozygotes were analyzed with PCR-SSCP, and the band showed that E3 was in the front, E2 in the middle, followed by E4. They were in electrophoresis with DNA templates of the samples at the same time, and the apolipoprotein E genotype of the template could be identified by the migration rate of the band.MAIN OUTCOME MEASURES: Apolipoprotein E genotypes were observed.RESULTS: All the enrolled subjects were involved in the analysis of results. ① Six genotypes were detected with PCR-SSCP in the 60 patients with primary osteoporosis and 30 healthy controls, including apolipoprotein E2/2, E3/2, 3/3, E4/2, 4/3, 4/4. ② There were very significant differences in the allele frequency of apolipoprotein E4 among the three groups (χ2=17.520, P < 0.01). The allele frequencies of apolipoprotein E4 in the simple osteoporosis group and osteoporotic fracture group were obviously higher that that in the normal control group (χ2=4.904, 16.681, P < 0.01),it was also markedly higher in the osteoporotic fracture group than in the simple osteoporosis group (χ2=4.658, P < 0.01).CONCLUSION: Apolipoprotein E4 is closely associated with primary osteoporosis and osteoporotic fracture. Apolipoprotein E is a useful marker for primary osteoporosis, especially for osteoporotic fracture.
4.Microsurgical treatment of large acoustic neurinoma and facial nerve preservation

Objective To explore the microsurgical technique and results of the large acoustic neurinoma and the facial nerve preservation.Methods 35 cases of large acoustic neurinoma treated microsurgically were analyzed retrospectively.Results Total resection was achieved in 33 patients,subtotal in one patient and partial in one patient.The facial nerve was preserved completely in 32 cases (91.4%).One died postsurgery.Conclusion The application of microsurgical techniques and rational selection of operational approach can remarkably increase the total removal rate and facial nerve preservation rate.
5.Establishment of a human large cell lung cancer multi-drug resistance cell line H460/cDDP and its biological characteristics

Purpose:To establish a human large cell lung cancer multi-drug resistance cell line H460/cDDP and explore its biological characteristics. Methods:A resistant human larget cell lung cancer cell line (H460/cDDP) was established by intermittent high dose cisplatin selection from the parental cell line H460. Drug sensitivity was detected by MTT assay. The changes of its biological characteristics were determined using light microscopy, Trypan Blue staining rejection, cell counting, chromatosome analysis; Neoplasia formation test in nude mouse was performed to investigate its in vivo characteristic. Results:H460/cDDP cell line was developed after about 6 months and the resistance index to cisplatin was 10.21. H460/cDDP cells exhibited cross-resistance to 5-Fuorouracil, adriamycn, etoposide and methotrexate. Compared with the parent cells, the morphology wac changed; doubling time prolonged (from 20.78h to 36.46h), while the chromatosome number and caryotype were similar. After being frozen, deposited and resuscitated repeatedly, its biological characteristics remained stable. It had neoplasia formation ability in vivo, but the forming time was longer than its parental cell. Conclusions:The newly established multi-drug resistant large cell lung cancer cell line H460/cDDP cell line possessed the typical multi-drug resistant phenotype. It was stable and had neoplasia formation ability in vivo,suitable for research.
6.Comparison of EPBD and EPBD combined with SEST in treatment of large choledocholithiasis

Objective To compare the clinical efficacy and safety of endoscopic papillary balloon dilation (EPBD) and endoscopic papillary balloon dilation (EPBD) combined with small endoscopic sphincterotomy (SEST) in treatment of large choledocholithiasis. Methods 78 patients with large choledocholithiasis from January 2014 to December 2015 were randomly divided into EPBD group, and combination treatment group. The level of bilirubin, transaminase, alkaline phosphatase (ALP) before and after the operation, the success rates of stone removal, serum amylase of 24 h after the operation, the operation times of endoscopic retrograde cholangiopancreatography (ERCP) and whether complicated with postoperative pancreatitis were compared between the two groups. Results There was no significant difference of the success rates of stone removal, serum amylase of 24 h after the operation, the operation times of ERCP between the two groups. The level of bilirubin, transaminase, alkaline phosphatase (ALP) was declined after EPBD or EPBD and SEST, and the results of the two groups had no statistical significance (P > 0.05). Also there was no significant difference of the incidence of postoperative pancreatitis, postoperative bleeding and postoperative hyperamylasemia between the two groups (P > 0.05). Conclusion EPBD is worthy of promoting because it has a similar clinical efficacy and safety to EPBD and SEST in treatment of large choledocholithiasis.
7.Expression of hypoxia-inducible factor-1α within rat cerebral cortical neurons after hypoxia

Objective To investigate the expression of hypoxia-inducible factor 1α (HIF-1α)gene in rat cerebral cortex neurons under hypoxia conditions,and provide more experimental basis for clinical treatment of hypoxic brain disease.Methods The rat cerebral cortical neurons in primary culture and hypoxia model were prepared and identified by immunocytochemistry analysis.The expression of HIF-1α in normal and hypoxic neurons was detected at 4 time points (12 h,24 h,48 h and 72 h) by immunohistochemical analysis.Results The expression of HIF-1α positive cells in normal control (NC) group was less at each time point,and it showed no statistically significant within groups.The weak expression of HIF-1α was found at 12 h in hypoxic group,and the expression augmented along with the time extended,it increased obviously at 24 h,reached the peak at 48 h(IOD=0.27±0.02,F=35.703,t=11.795,P<0.01),and declined until 72 h,it showed statistical significance between two groups at each time point.Conclusions The expression of HIF-1α is increased after hypoxia.HIF-1α has a protective effect on neurons after hypoxia.
8.One-staged release and reduction by posterior approach to treated basilar invagination with irreducible atlantoaxial dislocation

Objective To evaluate the safety and effectiveness of one stage surgery of release and bone reduction by posterior approach to treat basilar invagination (BI) with irreducible atlantoaxial dislocation (IAAD),and to explore the indications and crucial techniques of posterior approach.Methods All of 17 Consecutive patients (8 males and 9 females) with BI and IAAD who underwent release and reduction by posterior approach from July 2000 to June 2015 were enrolled in the present study,the mean age was 35.2±13.8 years with a range of 12-56 years.The clinical symptoms and signs was recorded,and preoperative imaging examination,including anteroposterior,lateral,dynamic films,MRI and CT of cervical spine,were performed to identify the series.There were 14 cases with atlanto-occipital fusion,7 cases with C2,3 fusion,6 cases with Chiari malformation,6 cases with Syringomyelia,and 8 cases with myelomalacia.The clinic symptoms include occiput/neck pain in 15 cases,cervical movement limitation in 13 cases,short neck in 9 cases,torticollis in 12 cases,Paresthesia in 14 cases,weakness in 13 cases,tendon reflexes hyperfunction in 16 cases and ataxia in 13 cases.The postoperative X-rays,MRI or CT were used to observed the results of decompression,fixation and fusion.Neurological function was assessed by JOA scale and Ranawat's score before,after surgery and at final follow-up.Pre-and post-operative Chamberlain (CL),Wackenheim (WL),McGae (ML),atlantodental interval (ADI) and cervico-medullary angle (CMA) were analyzed by student t-test.Results The average operation time was 145 mins (90-210 mins) and blood loss was 175 ml (150-350 ml).The average follow-up was 44.47 months (9-94 months).JOA score was increased from 8.06 preoperatively to 15.20 postoperatively,the improvement rate was 77.2%.Preoperative Ranawat's score was Ⅱ in 1 case,Ⅲla in 12 cases,ⅢB in4 cases.Postoperative score was Ⅰ in 13 cases,Ⅱ in 4 cases.The preoperative CL,WL,ML,ADI and CMA were (12.52±5.17) mm,(6.59±3.04) mm,(6.96±4.32) mm,(9.88± 1.93) mm,115.35°± 12.40°,respectively.and the postoperative CL,WL,ML,ADI and CMA were (2.0±3.67) mm,(-3.06±1.85) mm,(-1.76±2.88) mm,(1.17± 1.18) mm,136.76°±11.44°,respectively.The perioperative complications were discovered in 2 cases,including 1 case of infection and1 case of cerebrospinal fluid(CSF) leakage.Conclusion Primary surgery of nerve release and bone reduction by posterior approach may be safe and efficient for the treatment of BI and IAAD.Preoperative evaluation,proper surgical indications and advanced surgical techniques are important for treatment results.
9.Integrative treatment of multiple trauma patients with severe craniocerebral injury in emergency center (with 148 cases report)

Objective To explore the significance and important measure of integrative treatment of multiple trauma patients with severe craniocerebral injury in emergency center. Methods One hundred and forty-eight multiple trauma patients with severe craniocerebral injury in emergency center from November 2002 to February 2006 were analysed retrospectively. Result In total 148 cases, 72 were cured and 26 dead, 7 were in status of plant man, 17 experienced severe deformity, and 26 did mild deformity. Conclusion Multiple trauma patients with severe craniocerebral injury usually experience a serious situation and rapid development. Therefore emergency doctors are required for organizing salvage of such patients, at the same time treatment and diagnosis are implemented. Firstly the most important key to successful salvage is appropriate disposal of fatal injury and early elimination of shock. Proper surgical choice, especially at the first time, inspection and protection of visceral functions, and attention to nutritional support are other vital methods to gain more successful salvage. ICU is also emphasized for its essentiality.
