2.Expression and clinical significance of serine-threonine kinase/mammalian target of rapamycin/p70 S6K signal path- way in oral squamous cell carcinoma.

OBJECTIVETo investigate the expressions of serine-threonine kinase (Akt)/mammalian target of rapamycin (mTOR)/p70 S6K in oral squamous cell carcinoma (OSCC) and provide references for early diagnosis and prognosis evalua- tion of OSCC.
METHODSA total of 51 cases of OSCC, 10 cases of paracancerous mucosa, and 10 cases of normal oral mucosa were collected. The expressions of Akt/mTOR/p70 S6K in these cases were detected using the SP method of immunohisto- chemistry. The correlation between their expressions in OSCC was also analyzed.
RESULTSThe positive expressions ofp-Akt, p-mTOR, and p70 S6K in OSCC were significantly higher than those in normal oral mucosa and paracancerous mucosa. The expressions of p-Akt, p-mTOR, and p70 S6K in OSCC were not correlated with age, gender, and clinical stage; by comparison, these expressions were correlated with lymph node metastasis and pathological grade. Strong positive correlations were also observed between the expressions ofp-Akt, p-mTOR, and p70 S6K in OSCC.
CONCLUSIONAkt/mTOR/p70 S6K signaling molecules exhibit active expressions in OSCC and may be implicated in the occurrence and development of OSCC.

3.Quantitative Assessment of Left Atrial Function in Patients with Hypertrophic Cardiomyopathies Using Two-dimensional Strain Echocardiography and Left Atrium Volume Index

Objective To evaluate the functions of the left atrium using two-dimensional strain(2DS)echocardiography and left atrium volume index(LAVI)in the patients with hypertrophic cardiomyopathy(HCM)and explore its clinical value.Methods There were 37 patients with HCM and 35 healthy subjects enrolled in our study.Left atrial passive ejection fraction(LAPEF)and left atrial active ejection fraction(LAAEF)were calculated using Simpson's method.The analysis for atrial longitudinal peak systolic strain(S),peak systolic stain rate(SRs),peak early diastolic stain rate(SRe)and peak late diastolic strain rate(SRa)on the interatrial septum and the lateral wall of the left atrium were performed using 2DS echocardiography.Results LAPEF and LAAEF significantly decreased compared with those in the control group(P 0.05,P 0.01).LAVI in HCM group increased compared with that of the control group(P 0.01).S,SRs,SRe and SRa of the interatrial septum and the lateral wall were lower in patients with HCM than those in control group(P 0.05).LAVI was negatively correlated to 2DS(r =0.73,P 0.05).Conclusion The left atrial functions decreased in patients with HCM.2DS and LAVI are readily obtained parameters that provide unique data about global and segmental atrial functions.
4.Regulation of retinoic acid on permeability of retinal pigment epithelial barrier in lens-induced myopia

Background Retinal retinoic acid (RA) plays an important role in the formation of the lensinduced myopia.However,it is not clear how RA transfer the myopic signal to choroid throughout the retinal pigment epithelium (RPE) barrier.Objective The aim of this study was to investigate the effect of all-trans retinoic acid (atRA) on the barrier of RPE in lens-induced myopic eye of guinea pig.Methods Thirty left eyes of 30 21-dayold clean guinea pigs were randomized into normal control group and the model group.The models of out of focus were induced by covering of-6.00 D concave lens on the left eyes for 15 days.Radius of corneal curvature was measured using corneal curvimeter,and diopeter of the guinea pig was examined by mydriatic optometry.The length of ocular axis was detected by A-sonography.The animals were sacrificed and the retinas of the left eyes were isolated for the culture and passage of RPE cells.The third generation of cells were incubated Millcell-PET microporous film,and atRA at the concentration of 1 × 10-6,1 × 10-7,1 × 10-8 and 1 × 10-9 mol/L was added to the micropore respectively for 12 hours,and the micropores with equal-solvent served as negative control group.Methyl thiazolyl tetrazolium (MTT)colorimetric method was used to detect the survival rate of the cells.Subsequently,the transepithelial electrical resistance (TER) of the monolayer cells was determined with CN10-EVOM2 resistance measuring meter.The vesicular transport change of RPE membrane in different groups was evaluated by FM1-43 fluorescence staining.Results The mean diopter was (-2.20±0.95) D in the models,and that in the controls was (+ 1.15 ±0.30) D,with a significant difference between them (t =14.57,P< 0.01).The axial length was (8.24 ± 0.09) mm in the models and it was significantly longer than (7.81±0.05) mm in the controls (t=17.20,P<0.01).RPE cells grew well to form a monolayer in Millcell culture pool after one week.After 24 hours of the atRA treatment,the survival rate of RPE cells reduced gradually with the increase of atRA concentration with the highest rate in the 1 × 10-9 mol/L atRA group (93.3 %) and followed by the 1 × 10-8 mol/L atRA group (88.2%).More than half of the cells dead in the 1 × 10-6mol/L and 1 × 10-7mol/L atRA groups (53.8% and 47.1%).Significant differences in the TER value and fluorescence staining intensity of the cells were seen among the various groups (F =43.89,P =0.00 ; F =26.13,P =0.00),with the maximal values in the 1 × 10-8mol/L atRA group.The FM1-43 fluorescence located on the cellular membrane and cytoplasm.Conclusions AtRA can increase the functional state of tight junction and vesicular transport,which regulated the RPE cell barrier in the guinea pig.
5.Changes of lacrimal film after phacoemulsification

AIM: To observe of cataract phacoemulsification and intraocular lens implantation in patients with postoperative tear film, and to explore its clinical significance.
METHODS: A total of 106 patients ( 140 eyes ) undergone phacoemulsification were randomly chosen. Subjective dry foreign body sensation were observed at six nodes of period 1d, 1, 2, 3wk, and 1mo. Corneal fluorescein ( FSC ) , basal tear secretion ( SIT ) and tear film break-up time ( BUT) were used to detect functional changes of the tear film. And the correlation between tear film stability and corneal sensitivity was analyzed.
RESULTS: Dry eye cumulative score of postoperative 1d, 1, 2wk was higher than preoperative ( t= 8. 53, P=0.000;t=6. 27, P=0. 000; t=9. 02, P=0. 000). There was no significant difference in dry eye cumulative score at postoperative 3wk, 1mo compared with preoperative ( t=1.91, P= 0. 824; t= 1. 27, P= 0. 069). Corneal epithelial fluorescein staining points of postoperative 1d, 1, 2wk were increased compared with preoperative (t=11. 64, P=0. 000;t=9. 61, P=0. 000; t=8. 87, P=0. 001). There was no significant difference in corneal epithelial fluorescein staining points of postoperative 3wk and 1mo compared with preoperative (t=2. 52, P=0. 746; t=1. 16, P=0. 094). Corneal sensitivity detection values of postoperative 1d, 1, 2wk were significantly higher than that of preoperative (t=9.61, P=0.000;t=9.27, P=0.000;t=11.39, P=0.024), and there was no difference postoperative 3wk and 1mo compared with preoperative (t=1. 19, P=0. 562;t=2. 17, P=0. 501).
CONCLUSION: Phacoemulsification with intraocular lens implantation will reduce the tear film stability in the short term, but after a long rest will be improved to a certain extent.

When senescence induction is based on DNA damage, senescent cells display a unique phenotype, which has been termed “senescence-associated secretory phenotype”( SASP ) . SASP, including proinflammatory cytokines, growth factors, chemokines, matrix remodeling enzymes and other cytokines, may be an important driver of chronic inflammation and therefore may be part of a vicious cycle of inflammation, DNA damage and senescence. Senescence-associated secretory products released by such cells can affect the neighboring cells and further exacer-bate their regenerative capacity. SASP is associated with many chronic age-related diseases.

Objective Based on the objective test (A type) results of the 2014 comprehensive clinical course graduation test of 3+2 assistant general practitioners training,this article analyzed the differences between different teaching units,so as to provide objective basis for improving the teaching level of each unit.Method We established a database with EXCEL 2000,and put each question's ID and points,and the score of each student into the computer,so we could get the difficulty coefficient,distinction degree and reliability of the test paper,and calculate the difficulty coefficient and difference of each question.Then we analyzed the difficulty coefficient,the difference and the reliability of the 122 students' testing results,and compared the accuracy to the same question of the 6 teaching units.Result The objective test's (A type) difficulty coefficient is 0.77,distinction degree is 0.19,and reliability is 0.99.The highest score of the 122 students is 47 points,and the lowest score is 28 points,the average score is (38.5 ± 3.9) points.In the 50 questions,3 questions' difficulty coefficient is less than 0.4,14 questions' difficulty coefficient is between 0.4-0.7,33 questions' difficulty coefficient is above 0.7,so the difficulty degree of the paper is relatively low.In the 50 questions,23 questions' distinction degree is less than 0.15,17 questions' distinction degree is between 0.15-0.30,10 questions' distinction degree is above 0.30,so the distinction degree of the paper is relatively high.In the 50 questions,20 questions' accuracy appears larger differences between each unit:9 questions' accuracy has decreased significantly among 1-2 units,4 questions' accuracy decreased significantly among 3 units,3 questions' accuracy decreased significantly among 4 units,only 1 unit has high accuracy among 2 questions,and 2 questions' accuracy decreased significantly among all units.These problems are related to the teachers' teaching ability,the difficulty in mastering the key points of the curriculum,the lack of the concept of the general practitioners training.Conclusion The design of the examination paper is basically in line with the study purpose and the objectives of the training course.This examination paper was highly reliable,and suitable for the professional theory and ability test.There are site differences between each unit,which can provide an objective basis for improving the teaching level of different teaching unit,and we will solve the problems in the form of collective preparation next step.

Based on the analysis of survey results and random interviews on medical students , this paper dis-cussed from the perspective of medical students under the present situation of doctor -patient relationship:satisfied with prescriptions basic medical quality , but there are still a large room to improve;to cure medical ethics appraisal is higher , but still need to strengthen medical ethics education; the parties are still lack of trust in the prescrip-tions;the doctor-patient relationship nervous .And put forward the following countermeasures on how to relieve the strain on the doctor -patient relationship , strengthening medical ethics education , strengthen the study and practice of doctor-patient communication , pay attention to legal consciousness propaganda and training , use media power to improve direction of publit opinion .
9.Diagnostic value of 64-slice spiral CT colonography in TN staging of colorectal carcinomas

Objective To assess the value of 64-slice CT colonography (CTC) in preoperative TN staging of colorectal carcinomas. Methods One hundred and sixty-eight patients with suspected colorectal neoplasm underwent 64-slice spiral CTC after bowel cleaning and rectal air insufflating, among them 91 patients were proved with surgical pathology and enrolled into this study, and CTC images were reviewed separately by two experienced radiologists for preoperative TN staging of the tumors. Results For CTC evaluation of T staging, the sensitivity, specificity and accuracy were 98.57% (69/70), 85.00% (17/20) and 92.31% (84/91), respectively. The sensitivity, specificity and accuracy of CTC N staging were 87.80% (36/41), 96.00% (48/50) and 82.42% (75/91), respectively. Conclusion 64-slice spiral CTC plays a significant role in the preoperative TN staging of colorectal carcinoma.
10.Diagnostic value of multi-slice spiral CT colonography in colorectal neoplasms

Objective To assess the value of 64-slice CT colonography (CTC) in diagnosing colorectal neoplasms. Methods One hundred and thirty-two patients with suspected colorectal neoplasm underwent 64-slice spiral CT after bowel cleaning and rectal air insufflating, double contrast barium enema (DCBE) and colonoscopy. CT data were transmitted to the workstation and was performed respectively. All images were separately evaluated by two experienced radiologists. The sensitivity, specificity and accuracy of the CTC and DCBE were evaluated and compared with that of colonoscopy or surgical biopsy. Results The sensitivity, specificity and accuracy of CTC was 96.49%, 90.91% and 95.59%, of DCBE was 91.23%, 72.73% and 88.24% (χ~2=0.566, P=0.319). In neoplasms with diameter ≤5.0 mm, the sensitivity, specificity and accuracy of CTC was 80.95%, 86.96% and 84.09%, of DCBE was 57.14%, 69.56% and 63.64% (χ~2=0.679, P=0.422). In neoplasms with diameter of 5.1-9.9 mm, the sensitivity, specificity and accuracy of CTC was 91.67%, 100% and 94.12%, of DCBE was 75.00%, 80.00% and 76.47% (χ~2=0.631,P=0.374). The sensitivity, specificity and accuracy of both CTC and DCBE for detecting neoplasms with diameter ≥10.0 mm was 100%. Conclusion CTC is valuable in detecting colorectal neoplasm as a noninvasive method.
