1.A Case of Chronic Mucocutaneous Candidiasis.

A 6-year-old male patient had been suffering from angular cheilitis and paronychia with fragmentation and dystrophic change of the finger nails. Laboratory findings showed low serum iron level and anemia. Immunologic studies revealed defects in cell mediated immunity. KOH examination and culture of specimens from the lesions showed hyphal elements and growth of Candida albicans respectively. Concomitantly itraconazole and iron sulfate were administered. Four months after treatment he was free of any clinical evidence of the disease.

2.A Case of Multiple Basal Cell Epithelioma in the Nevus Sebaceus of Jadassohn.

Nevus sebaceus of Jadassohn is a hamartoma of the skin with the potential to develop benign and malignant neoplasms. This case was characterixed by multiple basal cell epitheliomas, clinically one reddish nodule and multiple pigmented papules, arising in the nevus sebaceus. Histologically, epithelial papillomatous hyperplasia and high-positioned hyperplastic sebaceous glands were found, and tumor nests consisting of basaloid cells with peripheral palisading arrangements were mainly situated in the upper dermis without significant infiltrative growth. We report a rare case of nevus sebaceus with multiple basal cell epitheliomas in the right cheek of a 49-year-old woman.

3.Recurrence of Left Atrial Myxoma.

We present a case of recurrent left atrial myxoma which occurred 7 years and 10 months after initial operation despite resection of originaltumor and adjacent atrial septum. This is the first case report of recurrence in Korea. Among the possible causes of recurrence, regrowth from pretumorous focus seems to be the most suggesting one in this case. Histopathologic findings of recurrent myxoma showed increased cellularity with active proliferation and nuclear hyperchromasia. These findings, together with the rapidity of regrowth, suggest that recurrent myxoma may have 'wilder' behavior. Prolonged postoperative observation is important, even if ample resection was done at the initial operation including atrial septum.

4.The Irritancy of Hand Dishwashing Liquids on the Skin.

BACKGROUND: The regular use of soaps and synthetic detergents has been associated with skin dryness and aggravation of dermatological conditions. OBJECTIVE: Our purpose was to investigate the irritancy of 10 hand dishwashing liquids on normal skin. METHODS: The skin changes, responding to repeat patch testing with 10 hand dishwashing liquids, were measured by visual scoring. RESULTS: The results are summarized as follows; 1. Of the 16 healthy volunteers 8 had reactions. 5 women and 3 men, but there was no significant difference in irritancy between men and women. 2. The mean irritancy scores of the 10 hand dishwashing liquids ranged from 0.06 to 0.88. 3. A prominent increase in the degree of erythema in the first 24 hour patch test did not persist beyond exposure day 5, but after day 5, augmented responses of the erythema and scales, even fissuring occurred. 4. The pH of the test solutions of the hand dishwashing liquids ranged from 5.0 to 6.0, but there appeared to be no correlation between pH and irritancy in the dishwashing liquids tested. CONCLUSION: Our data suggests that regular exposure to hand dishwashing liquids may cause disturbances of the skin in a substantial proportion of individuals, and workers should use hand dishwashing liquids in lower concentrations than recommended and only for short periods with protective measures.

6.Three Cases of Baboon Syndrome.

The Baboon syndrome is used to denote a characteristic distribution pattern of systernic allergic contact dermatitis. Diffuse erythema of the buttock, upper inner surface of the thighs and axillae are characteristic features. We report three cases of Eaboon syndrome developed after contact with mercury through breaking a clinical thermometer in a 30 year-old, 16 year-old, 32 year-old females, respectively. Mercury sensitivity was confirmed by a patch test in all three patients.

7.A Case of Maffucci's Syndrome Associated with Spindle-cell Hemangioendothelioma.

A case of a 21-year old female with Maffucci's syndrome is presented where the patient developed multiple cutaneous spindle-cell hemangioendotheliomas. She had multiple pea to walnut sized, non-tender, normal skin colored or slight bluish nodules and bony mass-like lesions on the left upper extremity and hand. A histological examination of the lesions showed that they were composed of irregularly dilated, thin walled cavernous blood spaces containing phleboliths and collapsed vascular spaces separated by spindled fibroblastic cells. Radiologically, the bony lesions showed radiolucent densities with calcified spots within the second proximal phalanx and metacarpal bone of the left hand, which were consistent with enchondromas.

9.A 10 Year Period ( 1984 - 1993 ) of Clinical Observation of Cutaneous Malignant Tumors.

BACKGROUND: Skin cancers arc the most common malignancies in human. The increasing incidence of skin cancer has been attributed to increasing exposure of the papulation to sunlight and other factors, such as chemicals or. viruses. OBJECTIVE: The purpose of this study is to investigate a recent 10 year period(1984-1993) of clinical observation of cutaneous malignant tumors. METHODS: A statistical study was made with 60 cases of malignant tumors that visited the Departrnent of Dermatology, Chung Ang University during a 10 year period(1984-1993). RESULTS: 1. The mean incidence of cutlneous malignant tumors in the total number of outpatients was 0.16%(60/36,416). The incidence of malignant turnors has tended to increase in the last 4 years. 2. The age of onset was quite varied but the majority of cases, 51 cases(85.0%), were over 40 years old. The ratio of rnen to women was 1.3: 1.3. Among the 60 cases of malignant cutaneous tumors, the most frequently observed tumor was basal cell carcinoma, 20 cases(33.3% ). The next most frequent tumors were squamous cell carcinoma, 9 cases(15.0%), metastatic carcinoma, 9 cases(15.0%) and malignant melanoma, 6 cases(10.0%). 4. Among the 20 cases of basal cell carcinoma, 19 cases(95.0%) were over 40 years old. The most frequent age was the 7th decade(30.0%). The most common predilection sites were the nose (30.0%), the cheek(20.0%) and the eyelid(10.0%). 5. Among the 9 cases of squamous cell carcinoma, 8 cases(88.9% ) were over 40 years old. The ratio of men to women was 1: 2. The most frequent age was the 8th decade(44.4% ). The most common predilection sites were the head(55.6%), lower extremity(22.2%), upper extremity (11.1%) and trunk(11.1%). 6. All patients with malignant melanoma were over 40 years old. The most frequent age was the 5th decade(50.0%). The most cornmon predilection sites were the upper and lower extremities (66.7% ). 7. Among the 9 cases of metastatic carcinoma, 7 cases(77.8%) were over 40 years old. The ratio of men to women was 1: 1.25. The most frequent primary tumor was stomach cancer in men and lung cancer in women. 8. Cutaneous malignant tumor. usually had no detectable metastasis but there were 2 cases of extramammary Pagets disease with metastasis in the regional lymph nodes. CONCLUSIONS : This study revealed that the incidence of malignant tumors has tended to increase over the last few years, the most frequent age was over 40 years old, the most observed turnor was basal cell carcinoma and any type of cutaneous malignant tumors was rarely metastasis.

10.Fracture strength between different connector designs of zirconia core for posterior fixed partial dentures manufactured with CAD/CAM system.

Statements of problem: Zirconia core is used for posterior fixed partial dentures because it's good mechanical properties. Stress is concentrated on connectors in fixed partial dentures, so the proper design of connector areas is needed for adequate mechanical long-term properties of any prosthesis. The area of connector is critical, but tooth size and surrounding soft tissue limit the connector design. PURPOSE: The purpose of this study is to compare fracture strengths between different connector designs of zirconia core for posterior fixed partial dentures manufactured with CAD/CAM system and determining the optimal connector design satisfying strength and hygiene. MATERIAL AND METHOD: The following four groups of 40 posterior fixed partial denture specimens(each group 10) were fabricated as followed; group 1, vertical height of connector is 3mm(control group, all groups have the same condition); group 2, lingual vertical 1mm reinforcement on connector; group 3, lingual vertical 2mm reinforcing on connector and group 4, lingual vertical 3mm reinforcing on connector. Specimens were subjected to compressive loading on the central fossa of pontic by instron. SEM was used to identify the initial crack and characterize the fracture mode. RESULTS: The results were as follows: 1. The mean fracture load of the non-lingual reinforcing group was 1212N and the lingual vertical 1mm reinforcing group was 1510N, the lingual vertical 2mm reinforcing group was 1882N, the lingual vertical 3mm reinforcing group was 1980N. 2. The reinforcing groups were statistically significant compared to non-reinforcing groups(P<0.001). 3. There were 2, 3mm reinforcing groups that were statistically significant compared to 1mm reinforcing groups(P<0.001), and the 3mm reinforcing group was not statistically significant compared to 2mm reinforcing groups(P>0.05) 4. Fractures were initiated in gingival embrasures of connectors and processed to the loading site. CONCLUSION: In this study, lingual reinforcement of connector for improved strength of zirconia based fixed partial denture is nessasary. And long-term study for clinical application is required.
