1.Guide and prediction value of partial pressure of end-tidal carbon dioxide during cardiopulmonary resuscitation

Objective To study the clinical significance of end-tidal carbon dioxide partial pressure (PetCO2 ) during cardiopulmonary resuscitation (CPR) and seek the fixed value according which to decide if we should and when to give up.Methods This was a prospective,observational study.A total of 124 patients with cardiac arrest in or out-of-hospital from may 2003 to March 2009 in emergency department of our hospital were selected.All of them had definite etiological factors.Changes of PetCO2 in 124 cardiac arrest patients during CPR were tracked.Results The gender,age,rescue time in seventy-one patients with the return of spontaneous circulation (ROSC) after endotracheal intubation have a significant difference with that in fifty-three patients without ROSC (P <0.01 ).The PetCO2 of the survival were higher than that of patients without ROSC or with ROSC,but finally died (P <0.01 ).A fixed point 14.4 mmHg of PetCO2 after 20 minutes' CPR can be used as a reference value to guide CPR or predict prognosis.Conclusions Monitoring PetCO2 during CPR has a predictive value on the success of resuscitation.
2.THE INFLUENTIAL FACTORS AND COMPARABILITY AMONG LABORATORIES OF RAPD ON THE IDENTIFICATION AND GENOTYPING OF BACTERIA

To investigate the influential factors of different concentrations of some reagents and the comparability among the laboratories of RAPD. RAPD was carried out with different concentrations of reagents and three different amplified cyclers to amplify 11 strains of pathogenic vibrios. 11 strains of pathogenic vibvios were genotyped into 4 types, and could be easily repeated.The different concentrations of primer and 4 dNTP could influence the result of RAPD,but the fingerprints of RAPD produced respectively with 3 amplified cyclers were consistent. It is suggested that RAPD can be compared among the laboratories with the standard reagents.

The present study shows that mushroom juice exhibits a significanteffect on growth of L. acidophilus in reconstituted skim milk. When 5% - 10% mushroom juice was added in skim milk, they shorten generation time and the rate of lactic acid production was accelerated. After 10h incubation at 37℃ cell counts of L. acidophilus in test groups were much more than in control group, were up to 108/mL.
6.Effect of bag-1 expression on the occurrence of breast cancer and its 5-year survival rate

BACKGROUND: Bag-1 is a multifunctional and anti-apoptotic gene. Its anti-apoptotic ability is enhanced when binding to bcl-2 to form a complex.Now it is considered as a predictive biomarker for the early diagnosis of breast cancer. However, whether it is useful in the assessment of the prognosis of breast cancer is still elusive.OBJECTIVE: To explore the expression of bag-1 in breast cancer and its role for prognosis.DESIGN: A controlled study with breast cancer, benign breast tumor and normal breast tissues as subjects.SETTING: The Vascular and Endocrine Surgery Department of Xijing Hospital of the Fourth Military Medical University of Chinese PLA.MATERIALS:Totally 100 breast cancer specimens were obtained form May 1995 to May 2000. Ten benign breast tumor and 10 normal breast tissues were used as control. All the specimens were paraffin-embedded and came from the Pathological Department of Xijing Hospital Affiliated to Fourth Military Medical University of Chinese PLA.METHODS: Immunohostochemical strept avidin-biotin complex(SABC) method was adopted to detect bag-1 expression in these specimens.pression levels of bag-1.RESULTS: The positive expression rate of bag-1 in breast cancer (85%) was significantly different form those of benign breast tumor (10%) and normal breast (10%) (χ2= 29.98, P = 0.00). While the positive expression rates in breast cancer of different stages (stage Ⅰ, stage Ⅱ and stage Ⅲ ) were 88%, 82% and 88%, respectively, which has no significant difference (χ2 = 0. 61, P = 0.75) . In duct carcinoma, lobular carcinoma and special carcinoma, bag-1 positive expression rate was 86%, 85% and 80%,which was also no significantly different (χ2 =0.16, P =0.95). In the 94followed patients, the 5-year survival rate of positive bag-1 expression was 79% and that of negative bag-1 expression was just 9%. The difference was significant (χ2 = 0. 07, P = 0.04).CONCLUSION: High bag-1 expression exists in breast cancer and its level is not associated with the clinical stages or pathological types of the cancer.Therefore, bag-1 may be used as a predictive marker for the prognosis of breast cancer.
7.Effect of age, body figure and risk information in making treatment decision for female patients with breast cancer

Objective To determine the effect of age,body figure and risk framing in making treatment decision for female patients with breast cancer.Methods Two groups of healthy female:one group of 60 female ages 18-24 from undergraduate student and the other group of 60 female ages 35-60 from the university community.Healthy women imagined that they had been diagnosed with breast cancer and received information regarding lumpectomy versus mastectomy and recurrence rates.Participants indicated whether they would choose lumpectomy or mastectomy and reasons.Results The different concern levels of body appearance between younger and older women influences treatment selection of lumpectomy versus mastectomy.Facing the risk information,older women were unlikely to select the breast conserving operation.Conclusions The factors of age and the attention to body figure would effect the choice of breast conserving operation or mastectomy.Risk information on the selection of treatment would not affected directly,but the effects of risk information on younger and older female is different.Nurses should provide the age-appropriate information who recently have been diagnosed with breast cancer regarding treatment alternatives to ensure their active participation in the decision-making process,female who have different levels of investment in body figure also may have different concerns about treatment,and should be alert on empathetic of such concerns.
8.Cultivation and Identification of Dendritic Cells from Mouse Bone Marrow in Vitro

Objective To establish a method of cultivation of dendritic cells (DC) from mouse bone marrow in vitro and identify their phenotype and function. Methods Under aseptic condition, bone marrow cells were extracted from the tibia and femur bones of BALB/c mice. Bone marrow cells were cultured with recombinant mouse granulocyte-macrophage colony-stimulating factor ( rmGM-CSF) in vitro. The expansion and morphological changes of DC were observed with light inverted microscope. Phenotype was identified with flow cytometry and biological function was studied with antigen phagocytosis test. Results A large number of immature and mature DC with typical dendritic morphological characteristics could be generated from murine bone marrow. Immature DC, which had high expression in CD11c and low expression in CD40, MHC-II and CD86, could phagocytize antigen. Mature DC, which could be induced from immature DC by lipopolysaccharides, had high expression in CD11c, CD40, CD86 and MHC-II molecules. Conclusion Immature and mature DC can be generated from mouse bone marrow cells through cytokine induction in vitro and be used for further study associated with DC.
9.Treatment of Peripheral Arterial Extensive Occlusive Disease by One Stage Arterialization of Posterior Tibial Vein

Objective To investigate the effect of one stage arterialization of posterior tibial vein in treatment of peripheral arterial extensive occlusive disease.Methods Forty-six cases(56 limbs) of patients with peripheral arterial extensive occlusive disease were treated with one stage arterialization of posterior tibial vein.Results The symptom of pain disappeared right after one stage arterialization of posterior tibial vein in all patients.Skin temperature went up.The long-term results were satisfactory during the period of 3 months to 7 years follow-up,except two limbs were amputated and two limbs were reoperated with pedicle omental transplantation.Conclusion The technique of one stage arterialization of posterior tibial vein has advantages of one-stage procedure,various indications,little influence to venous return and rapid relief of ischemic symptoms.
10.Influence of Yes-associated protein-1 on proliferation and metastasis of gallbladder carcinoma

Objective To investigate the expressions of Yes-associated protein-1 (YAP1) in gallbladder mucosal epithelium of normal persons,in patients with simple/calculous cholecystitis,and in patients with gallbladder carcinoma;and to study the mechanism of YAP1 in gallbladder carcinoma development.Methods Immunohistochemistry was used to detect the expression and distribution of YAP1 protein in 50 persons with normal gallbladder,101 patients with simple cholecystitis/calculous cholecystitis and 100 patients with gallbladder carcinoma.RT-PCR and western-blot were used to detect the mRNA and protein levels of YAP1 in normal and malignant gallbladder mucosal epithelium cells.siRNA was used to shut down the expression of YAP1 in SGC996 cells.MTT was used to test cell vitality.Flow cytometry was used to measure cell cycle.Results Immunohistochemistry revealed the expression rates of YAP1 in the gallbladder carcinoma group,the cholecystitis/gallstone group and the control group to be 87.0% (87/100),56.4% (57/101) and 5.0% (1/20),respectively (P < 0.01).The YAP1 protein levels were higher in gallbladder carcinoma tissues and cells when compared to normal tissues and cells.RT-PCR showed the mRNA levels of gallbladder carcinoma cells to be 12.5 ± 1.2 times of normal gallbladder mucosal epithelial cells (P < 0.05).After using siRNA to shut down the YAP1 expression,EMT associated proteins were down-regulated,cell vitality was decreased,and cell cycle was arrested in the S-phase.Conclusions YAP1 is closely related to cell proliferation and metastasis of gallbladder carcinoma.It may promote tumor progression through epithelial-mesenchymal transition.transition;Tumor progress
