Objective To evaluate the efficacy and toxicity of weekly docetaxel and cisplatin in previously untreated patients with advanced non-small-cell lung carcinoma. Methods Between January 2002 and December 2003 ,34 patients with pathologically comfirmed advanced non-small-cell lung carcinoma who had not received treatment were enrolled. The mean age was under 66 years. The patients received intravenous infusions of docetaxel(25 mg/m2,dayl ,8,15) with dexamethasone premedication and cisplatin(25 mg/m2,dayl ,8,15) ,followed by a week of rest. The remedies which were less than 6 regimens lasted to disease progression or severe toxicity. Therapeutic effect was evaluated by CT scan every two courses . The patients were followed up for 24 months. Descriptive statistics and SPSSIO. 0 software were used to analyse the results. Results 34 patients finished 90 courses. The mean was 2. 6 courses. All patients were followed up. Two patients achieved complete responses, ten patients achieved partial responses, ten patients achieved stable disease. An objective response rate of 35. 29% (95% confidence interval 19. 25%-51. 33% )was obtained. Patients life quality was significantly improved. The median time to progression was 4. 1 months, and median overall survival was 11 months. The 1-year survival rate was 47. 06% , the 2-year survival rate was 11.76% . Toxicities were mild. Grade 3 to 4 neutropenia (11.76%), anemia (5.88%), hyponatremia (5.88%), alopecie (17.64%) and nausea/vomiting (5. 88% ) were observed. Conclusion Weekly Cisplatin plus docetaxel is an effective and well-tolerated regimen in chemo-naive patients with advanced NSCLC. Well-designed clinical trials should be conducted.

Objective To assess the continent diversion results of sigma rectum pouch after radical cystectomy. Methods The reconstruction of bladder with sigmoid was modified for treatment of 18 cases of bladder tumor. The intestine was incised over a length of 20~24 cm with the junction of sigmoid colon and rectum as the midpoint so as to create a low pressure reservoir for urine and side-to-side anastomosis was performed on the posterior borders of the rectosigmoid wall.Submucosal tunnel modified technique was em- ployed in antireflux urethral implantation,Urination has been controlled by anal sphincter. Results About 80 minutes was spent to finish a new low pressure pouch after radical cystectomy.Among 18 patients with this op- eration, the controlled emiction were good after pull out the anal duct and"J"stent in 1 week to 2 months.Af- ter 2 months, the times of urination is stable,4~5 times in daytime and 1~3 times during nighttime. Two pa- tients had nocturnal enuresis and the symptom vanished after 2 months. One patient had adhesive ileus, two patients had hyperchloremia acidosis and kaliopenia, one patient had urethral stump cancer. There is no com- plication as anastomotic block, renal function lesion and severe upper urinary tract infection. Conclusion This operative method was easy, emiction control was well, and with higher quality of life for patients. It is al- so a better alternative diversion procedure that would be easily accepted.

BACKGROUND: The surface electromyography can evaluate the effect of exercise by recording the parameters of muscle activities, and vibration exercise is the best supplement to traditional weight training. More research focuses on the effect of simple vibration training on the surface electromyography of lower limbs, but the influence of vibration combined with weight-bearing training is poorly understood.OBJECTIVE: To study the effect of vibration combined with load on the surface electromyography of lower limbs at the micro level of muscle working principle.METHODS: Eight healthy college students were recruited, and subjected to four different stimulations: vibration (45 Hz) combined with load (45% one repetition maximum (IRM)); vibration (45 Hz) combined with load (60% 1RM); vibration (50 Hz) combined with load (45% 1RM); vibration (50 Hz) combined with load (60% 1RM), followed by semi-squat exercise with the heel lifting, 10 times/minute, for 3 courses with more than 2 hours in between.RESULTS AND CONCLUSION: Different vibrations combined with loads made significant difference on the root mean square amplitude of the surface electromyography (P < 0.01), and there was a significantly increased root mean square amplitude in the vibration (50 Hz) combined with load (45% 1RM), especially at the medial gastrocnemius. The four kinds of stimulations made significant different effects on the surface electromyography of rectus femoris, tibialis anterior, and medial gastrocnemius, except semitendinosus (P ≤ 0.05). Moreover, the effect showed significant difference among different stimulations except vibration (50 Hz) combined with load (45% 1RM) (P ≤ 0.05). Compared with the other three stimulations, vibration (50 Hz) combined with load (45% 1RM) exerted better effect on the muscular activation. To conclude, different vibrations combined with loads exert different effects on the motor unit of same neuromuscular activity, and a suitable stimulation may produce better effect. Besides, the same stimulus for the motor unit of different neuromuscular activities produces different effects, which may match to the muscle nature.
