1.Retrospective Analysis of Chemotherapy-related Death from Human Malignant Neoplasms

Objective:To investigate the cause and preventive measures of chemotherapy-related death from human malignant neoplasms.Methods:A retrospective study was carried out to evaluate the clinical data of 15 patients with chemotherapy-related death of human neoplasms in 2008.The cause,treatment,survival time after chemotherapy and preventive measures were analyzed.Results:The average age of the 15 patients was 52.4 years,and 5 of them were treated with docetaxel.The mean survival time after chemotherapy was 16.8 days.5 cases presented severe sehematological toxicity, and 2,breath failure.Conclusion:Chemotherapy-related death from human malignant neoplasms was correlated with disease history,age and chemotherapeutic agents,etc.;therefore,no use of docetaxel for patients aged over 60 years or with hepatic and lung diseased history could decrease the chemotherapy-related death.
2.Role of epithelial-myofibroblast transdifferentiation under injury in the development of airway remodeling

Objective:To investigate if injury-related myofibroblast induction could occur in bronchial epithelial cells and the role of TGF-?_1(rhTGF-?_1) in epithelial-myofibroblast transdifferentiation. Methods:Cultured bronchial epithelial cells, 16HBE-14o, were exposed to poly-L-arginine(PLA). Using a immunohistochemical method, ?-smooth muscle actin (?-SMA) expression was detected in those cells after being treated with PLA . Relative activity of chloramphenicol acetyltransferase(CAT) was assessed in 16HBE-14o cells, which were transiently transfected with VSMP8 plasmid containing ?-SMA promoter linked to CAT gene, at different time points after rhTGF-?_1(10 ?g/L) treatment. Results:①?-SMA positive, spindle-shaped cells emerged among the regenerated 16HBE-14o epithelial cells following damage. The number of ?-SMA positive 16HBE-14o cells increased and cell repair was delayed under rhTGF-?_1 stimulation.② CAT activity increased significantly in VSMP8 plasmid transfected 16HBE-14o cells 24 h after TGF-?_1 stimulation. 4.Descriptive analysis on military training injuries in a special force corps

[Objective]To explore the incidence, causes, training courses at high risk and location of the body of training injury in a special force corps, provide evidence for effective prevention of the injury.[Method]A questionnaire on training injuries and relative situations, and review of medical records from Nov 2005 to Oct 2006 were performed in 823 soldiers and officers in a special force corps at Nov 2006. Data were input into computers after quantization, and descriptive analysis was conducted with SPSS 13.0 software. [Result]Of 823 soldiers and officers, 738 (89.67%) experienced one or more injuries in that year, 525 (63.79%) were affected in normal training, 221 (26.85%) lost more than one training day due to the injuries. Confirmed with the medical records, 213 (25.90%) lost more than one training day due to injuries. A total of 1696 injuries occurred in 823 soldiers and offices in that year, of them, 1263 injuries interfered with normal training. Injury rate was 12.78 per 100 per month. 24.12% of injuries were due to over-exertion, 15.86% due to accident and 13.33% due to environmental field. Injury occurred mainly in runing related training courses including 5km running (22.17%) and 400m obstacle running (20.87%), followed by weight lifting (9.67%). The most frequent site of injury was the lower extremities, accounted for 65.74 % of the inj urie s including the foot ( 18.75 %),the ankle (13.50%), the leg (13.56%) and the knee (14.39%), followed by low back (14.21%). [Conclusion]Persistent over-exertion basic training, the courses at high risk of injury, should be improved in focus. Lower extremities and low back, the most frequent locations of injury, should be taken as key points of prevention and treatment of training injury.
5.Early Management of Complication in Acute Cervical Spine Injury

Objective To recognite the importance of early emergent management of and prevention therapy of complication in acute cervical spine injury. Methods early emergent support and 8 types and 715 early complications and their prevention and management in 194 cases with acute cervical spinal cord injury admitted through 1989 to 1995 were reviewed.Results Mortality in early stage can be markedly reduced with routinely and standardization of early emergency treatment obviously reduced the mortality procedure,pulmonary complication being the main cause of death, while early complications can delay surgical treatment,increase the cost and the staying in hospital and reduce difficulty of rehabilitation.Conclusion Early complication occurring in the first days with its own feature, prevention or treatment properly will be benefit for the treatment of the neural lesion and following rehabilitation.
6.Orthotopic spelenectomy and selective pericardial devascularization for portal hypertension

Objective To evaluate orthotopic splenectomy and pericardial devascularization for the treatment of portal hypertension.Methods The modified anterior approach splenectomy includes dissection of the peri-splenic vessels and ligments before division of short gastric vessels.During pericardial devascularization,the dessection panel was close to the esophagus and the stomach,leaving intact both the anterior and posterior vagus trunks.Results 63 patients underwent this modified operation.The free portal pressure decreased from (38 ±4) cmH2O to(28 ±4)cmH2O.The average blood loss was (530 ± 37)ml.There was no mortality,nor perioperative gastric paralysis and portal venous thrombosis.By 12-36 months follow-up,there was no pancreatic leakage,hepatic coma and recurrence of bleeding.Intrahepatic portal venous thromboses were detected in 4 cases at the sixth postoperative months.Conclusions This modified splenectomy plus selective pericardial devascularization carries less bleeding and is safe and effective for the treatment of portal hypertension.
8.Spectral CT Imaging in Differentiating Pancreatic Carcinoma and Mass-forming Focal Pancreatitis

Purpose To explore the value of spectral CT imaging in differentiating pancreatic carcinoma and mass-forming pancreatitis, and to improve the preoperative diagnosis accuracy. Materials and Methods The dual-phase spectral CT imaging data in patients with pathology-proven pancreatic carcinoma (n=62) and mass-forming pancreatitis (n=30) were retrospectively analyzed. At 40-140 keV (with 10 keV increment), the mean CT values, energy spectrum curves and iodine concentrations were measured and compared between pancreatic carcinoma and mass-forming pancreatitis in arterial phase and portal venous phase. Results The mean CT values of mass-forming pancreatitis at 40-140 keV were higher than pancreatic carcinoma in both arterial phase and portal venous phase with significant difference (t= -7.349- -4.810, P<0.05; t= -6.760- -5.496, P<0.05). The energy spectrum curves were significantly different between pancreatic carcinoma and mass-forming focal pancreatitis, especially at 40-70 keV. The iodine concentrations of pancreatic carcinoma were lower than mass-forming pancreatitis in arterial phase and portal venous phase with significant difference (t=-28.577, P<0.05; t= -28.451, P<0.05). Conclusion Pancreatic carcinoma and mass-forming pancreatitis can be differentiated by measuring the CT value at different energies, energy spectrum curves and iodine concentrations.
9.Role of histone methylation in spinal cord in maintenance of neuropathic pain in rats

Objective To evaluate the role of histone methylation in the spinal cord in maintenance of neuropathic pain (NP) in rats.Methods Sixty healthy male Wistar rats, weighing 200-250 g, aged 6-8 weeks, were randomly divided into 5 groups (n =12 each) using a random number table: control group (C group), sham operation group (S group), NP group, chaetocin group (CH group), and dimethyl sulfoxide (DMSO) group.NP was induced by chronic constriction injury (CCI) to the sciatic nerve in NP, CH and DMSO groups.The sciatic nerve was only exposed in group S.H3K9 methylation methyltransferase inhibitor chaetocin 50 μg was injected intrathecally in group CH, and the equal volume of DMSO was given instead in group DMSO for 4 consecutive days, starting from 2nd day after CCI.The mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured before CCI (T0),before administration on 2nd day after CCI (T1), and at 5, 7, 9 and 14 days after CCI (T2-5).Six rats in each group were sacrificed after measurement of pain threshold at T3 and T5, and the lumbar segment of the spinal cord was removed for detection of the methylated H3K9 expression by immunohistochemistry.Results Compared with C group, no significant change was found in the MWT, TWL and methylated H3K9 expressionat each time point in S group (P>0.05) , the MWT was significantly decreased, and the TWL was shortened at T1-5, and the methylated H3K9 expression was up-regulated at T3 and T5 in NP and DMSO groups, and the MWT was significantly decreased, and the TWL was shortened at T1 in CH group (P< 0.05).Compared with NP group, the MWT was significantly increased, and the TWL was prolonged at T2-5, and the methylated H3K9 expression was down-regulated at T3 and T5 in CH group (P<0.05) , and no significant change was found in the MWT, TWL and methylated H3K9 expression at each time point in DMSO group (P>0.05).Conclusion Histone methylation in the spinal cord is involved in the maintenance of NP in rats.
10.The effects of DR2 on myocardial ischemic postconditioning and its underlying mechanisms.

OBJECTIVETo study the effects of dopamin receptors-2 (DR2) on myocardial ischemic postconditioning and explore its underlying mechanisms.
METHODSThe myocardial ischemic postconditioning (PC) model was established in cultured primary rat neonatal cardiomyocytes which were then randomly assigned in the following groups: Nomial control group, Isehemia/reperfusion (L'R) group, PC (ischemic postconditioning) group, PC + Bro (Bromocriptine, a DB2 antagonist) group, PC + Hal (Haloperidol, a DB2 repressor) and PC + Hal + Bro groups. The lactate dehydrogenase (LDH) and superoxide dismutase (SOD) activity and malondialdehyde (MDA) content in cell medium were analyzed by colorunetry. The cell ultrastructure changes were observed by transmission electron microscope. The cell apoptosis was analyzed using flowcytometiy. The protein expression level of D112 and activity of p-p38 and p-JNK were detected by Western blot.
RESULTSCompared with the nonnal control group, hR increased the protein expression level of DB2, enhanced LDH activity and MDA content, promoted cell injury and apoptosis, decreased SOD activity, up-regulated the activity of p-p38 and p-JNK. Compared with the hR group, although PC further increased the expression of DR2 protein, it decreased LDH activity and MDA content, cell injury and apoptosis, increased SOD activity, down-regulated activity of p-p38 and p-JNK. Bromocriptine treatment further enhanced PC-induced canlioprotective effect, yet Hal addition attenuated this enhancing effect exerted by bromocriptine.
CONCLUSIONThe activation of DB2 is involved in the protective effect of ischemic postconditioning on myocardial ischemia/reperfusion injury through down-regulating the activity of p-p38 and p-JNK.
