1.Lacrimal passage probing combining injection of TobraDex eye ointment for treating chronic dacryocystitis

AIM: To evaluate the effect of probing of lacrimal passage combining injecting of US-produced TobraDex eye ointment for the treatment of chronic dacryocystitis.METHODS: For 127 cases (129 eyes) of chronic dacryocystitis, first the pyoid secretion gathered in the lacrimal sac was dashed out, and some TobraDex was injected in the middle of the lacrimal passage once per day, repeated for several times. The lacrimal passage was probed when the secretion of lacrimal sac disappeared essentially. The lacrimal passage and immit TobraDex eye ointment was used once every two days, repeated for 3-4 times.RESULTS: In 126 cases (128 eyes) the lacrimal passage was dredged. Only one case (1 eye) the youthful patient did not recover for the opening ectopia of lacrimonasal duct.During the 3mo-1a random visit the chronic dacryocystitis did not recrudesce in the cases of lacrimal passage dredging.CONCLUSION: It is simple and safe to use the probing of lacrimal passage combining injecting of US-produced TobraDex eye ointment to treat the chronic dacryocystitis. This method has good curative effects and can keep the normal physiological structure of lacrimal passage. It does not need any expensive medical equipment and cost less, therefore is advisable for patients.
2.Serotype distribution of non-polio enterovirus in patients with acute flaccid paralysis during 2011-2012 in Hebei Province, China.

This study aims to investigate the serotype distribution of non-polio enterovirus (NPEV) isolated from patients with acute flaccid paralysis (AFP) during 2011-2012 in Hebei Province, China and to analyze the relationship between these viruses and AFP. NPEV strains were isolated from the stool specimens from AFP cases in Hebei using human rhabdomyosarcoma cells (RD) and the mouse cell line expressing the gene for the human cellular receptor for poliovirus (L20B) according to the WHO requirements. The nucleotide sequence of VP1 region was determined, and the serotypes of NPEV were identified by molecular typing. The results showed that among the 82 strains of NPEV isolated from the AFP cases during 2011-2012, 42 isolates (55.3%) were identified as human enterovirus A (HEV-A), which were classified into 4 serotypes, 34 (44.7%) as human enterovirus B (HEV-B), which were classified into 13 serotypes, 2 as adenovirus, and 4 were untyped; human enteroviruses C and D were not found in these cases. Enterovirus A71 (EV-A71) was the main type of HEV-A, accounting for 85.7% of all HEV-A strains. HEV-A, especially EV-A71, was predominant among the NPEV strains isolated from AFP patients during 2011-2012 in Hebei Province.

3.Diagnostic process in 99 cases with amyotrophic lateral sclerosis

ObjectiveTo explore the factors that delayed the diagnostic process and resulted in misdiagnosis of amyotrophic lateral sclerosis (ALS),in order to look for solution. MethodsThe records of 99 cases with ALS from 1999 to 2003 in our hospital were reviewed retrospectively. Clinical characteristics and diagnostic process on the patients were statistically analyzed.ResultsThe time needed to confirm diagnosis was (13.1±7.5) months. There was a positive correlation between the time when EMG was performed and the time the diagnosis was made. 58.6% of patients were initially misdiagnosed in other hospitals. The most common misdiagnosis was cervical spondylosis. The misdiagnosis more likely occured in the patients of 40-59 years old. The misdiagnosis rate in the patients with initial lower extremities symptoms was higher than that with initial bulbar and upper extremities symptoms. The misdiagnosis more likely occured in patients with early cervical MRI.ConclusionThe major causes of misdiagnosis are unfamiliarity of the physician with the disease,misleading findings or misinterpretation of neuro-radiological or neuro-physiological findings.
4.Rapid identification of compounds in cortex moutan by liquid chromatography-tandem mass spectrometry.

AIMTo analyze the chemical constituents of cortex moutan by liquid chromatography coupled with electrospray ionization mass spectrometry.
METHODSAn on-line optimized HPLC-DAD/MS2 technique was employed.
RESULTSIn the negative ion detection mode, 38 components such as monoterpene glucosides, galloylglucoses and acetophenones were isolated. Among them, over thirty compounds were identified, including paeonol, paeonilflorin, oxypaeoniflorin, benzoylpaeoniflorin, benzoyloxypaeoniflorin, galloylpaeoniflorin, galloyloxypaeoniflorin, mundanpioside A, C, D, E, H, etc. by the high performance liquid chromatography with diode-array detection in parallel with electrospray ionization and quadrupole-time of flight tandem mass spectrometry (HPLC-DAD/ESI-MS2).
CONCLUSIONThis method can be used to rapidly determine the constituents of cortex moutan.

5.Studies on HPLC fingerprint of cortex moutan.

OBJECTIVETo establish a HPLC fingerprint for quality evaluation of Cortex Moutan.
METHODThe HPLC chromatographic fingerprinting of 30 lots of Cortex Moutan was established and major peaks were identified by LC-MS and MS-MS.
RESULTThe HPLC fingerprint of Cortex Moutan was established, showing 15 characteristic peaks. The areas of these peaks were found to complied with the following rule: paeonol > 1, 2, 3, 4, 6-penta-O-galloyl-glucos > methyl gallate > galloylpaeoniflorin > gallic acid > oxypaeoniflorin > other compounds.
CONCLUSIONThe chromatographic fingerprinting of Cortex Moutan with high specificity can be used to control its quality and monitor lot to lot consistency.

6.The"modification"Type Microsatellite Change in High Frequency Microsatellite Instability Colorectal Cancer Closely Relating to MLH1 and KRAS Mutation

Microsatellite instability(MSI)was defined according to the frequency of positive findings in a panel of MSI markers.High frequency MSI(MSI-H)was the phenotype in which repeat sequences were extraordinarily unstable, and was considered to be the bona fide phenotype of DNA mismatch repair defection. However base substitutions in some well studied oncogenes or tumor suppressors were reported to be uncommon in MSI-H tumors. To explore this obvious contradiction, the relationship between MSI and KRAS gene mutations were studied in a panel of 76 human colorectal carcinomas, the whole exon of MLH1 and MSH2 were sequenced for MSI-H tumors. KRAS gene mutation was confirmed by similar frequencies in tumors of different MSI status. Intriguingly, all of the KRAS mutant MSI-H tumors harbored sequence alterations in MLH1gene, which was a key player in DNA mismatch repair system. This implied that in MSI-H tumors carrying MMR mutations, KRAS mutation were frequently and almost exclusively occurred. Furthermore, these MMR mutants were uniformly carrying a unique "modification" + "jumping" type MSI, which was different to MSI-H tumors without MLH1 or MSH2 gene mutations. This study shaded lights on the heterogeneity of MSI-H tumors, and implied the connection between "modification" type MSI and DNA mismatch defection.
7.Screening and sero-immunoscreening of ovarian epithelial cancer associative antigens

Objective To explore epithelial ovarian cancer(EOC)antigens that are potentially useful for cancer early detection and therapy.Methods A high quality cDNA library derived from ascites tumor cells of EOC patients(3 cases of serous EOC,1 case of mucinous EOC,and 1 case of endometrial carcinoma of ovary)was constructed,and the method of combining serological analysis of recombinant cDNA expression libraries(SEREX)and suppression subtractive hybridization(SSH)was used for screening cDNA library.All of the positive clones were sequenced and bioinformatics analysis with BLAST software in GenBank was performed.Serological mini-arrays of recombinant tumor antigens(SMARTA)was used to investigate the prevalence of autoantibodies to these antigens in both 96 ovarian cancer patients and 96 cancer-free controls.Results Fifty-five positive clones encoding different antigenic genes of EOC recognized by IgG and(or)IgM were obtained.It showed that these 55 clones derived from 45 distinct genes and these genes could be grouped into 6 classes as following according to homology with known expressed sequence tag(EST):(1)known ovarian carcinoma related genes:BARD1,et al;(2)homologous genes with other tumors:TM4SF1,et al;(3)homologous genes with special tissues:ILF3,FXR1,et al;(4) homologous genes with special function:TIZ,C1 D,et al;(5)embryo originating genes:PKHD1,et al; (6)novel genes:OV-189,et al.SMARTA results showed that the positive ratio of five EOC antigens TM4SF1(28% vs 9%),CID(21% vs 6%),BARD1(23% vs 5%),FXR1(23% vs 8%),OV-189 (31% vs 13%)which reacting with their IgG autoantibodies,three antigens TIZ(26% vs 8%),FXR1 (28% vs 11%),and OV-189(18% vs 7%)which reacting with their IgM autoantibodies in patients was higher than in controls(P
8.Modulation of NRF2 and UGT1A expression by epigallocatechin-3-gallate in colon cancer cells and BALB/c mice.

BACKGROUNDGreen tea is an important source of flavonoids in human diets and epidemiological data correlate green tea consumption with a reduced cancer risk. Given its complicated properties at effective concentrations, we put epigallocatechin-3-gallate (EGCG) that previously reported on its anti-proliferative activities against several cancer cell lines on our research agenda to further examine the mechanism of its chemopreventive potential.
METHODSRNA interference (RNAi) expression vector pSilencer 3.1-H1 was used to construct recombinant nuclear factor erythroid 2 related factor 2 (Nrf2)-targeting RNAi plasmids. EGCG (5 microg/ml) was added into the culture fluid of cells before and after transfection. RT-PCR and Western blotting were used to detect the expression of uridine 5'-diphosphate-glucuronosyltransferase (UGT) 1A in cells. Forty male BALB/c mice were assigned to four groups: a normal unexposed control and three groups treated with varying doses of EGCG. Four weeks later, the mice were sacrificed, and their colon tissues were subjected to mRNA and protein expression of Nrf2 and UGT1A via RT-PCR and Western blotting analysis.
RESULTSEGCG up-regulated the expression of Nrf2 and increased the level of UGT1A in cells. The blockade of Nrf2 activity via RNA intervention largely attenuated the induction of UGT1A expression by EGCG. In mice, the mRNA and protein levels of Nrf2 and UGT1A detected by RT-PCR and Western blotting increased (both P < 0.05 compared with the control). This increase in Nrf2 expression also had a positive correlation with an increased UGT1A expression.
CONCLUSIONSEGCG mediated its effect in part by inducing the NRF2 signaling pathway and increasing UGT1A expression. Both in vitro and in vivo studies demonstrated the role of NRF2 and UGT1A expression in the potential use of EGCG as a possible chemopreventive agent and supported further study of EGCG for cancer treatment.

9.A survey report on dermatitis patients in hospital in Baoan district of shenzhen city from 1992 to 2006.

OBJECTIVETo analyse the epidemiological distribution of dermatitis patients in hospital and provide basis for studying the basic conditions and the prevention of dermatitis especially TCE-induced dermatitis in Baoan District of Shenzhen City.
METHODSAll dermatitis patients in hospital in Baoan District from 1992 to 2006 were descriptively analysed.
RESULTSThere were 645 dermatitis patients in hospital in Baoan District. The total number of patients is tended to rise in recent years. The main age group of 16 approximately years old, followed by 26 approximately years old, the majority are workers. The clinical diagnosis are allergic dermatitis, drug-induced dermatitis, TCE-induced dermatitis. Compared with other dermatitis, women are more than men in TCE-induced dermatitis (P < 0.05). The risk of hepatic dysfunction in TCE-induced dermatitis was significantly greater than other dermatitis (P < 0.05). It is mainly distributed in electronic, metal, electroplating industry.
CONCLUSIONThe TCE-induced dermatitis was one of the main occupational hazards in Baoan district. To strengthen self-protection awareness of workers, pay more attention to early work observed and occupational screening in key industries and trades workers, reduce opportunities for occupational exposure, all above can effectively reduce the occurrence of TCE-induced dermatitis.

10.Significance of hRad17 mRNA expression in human gastric cancer.

OBJECTIVETo study the relationship between hRad17 mRNA expression and clinicopathologic factors and lymph node metastasis of gastric cancer, and to assess the significance of predicting the extent of lymph node metastasis and prognosis.
METHODShRad17 mRNA expression was examined in matched primary lesions, normal gastric mucosa and lymph node metastatic lesions among 52 gastric cancer patients by reverse transcription polymerase chain reaction (RT-PCR), polyacrylamide gel electrophoresis (PAGE) and silver stain with the relation between hRad17 mRNA expression and clinicopathologic factors analyzed. At the same time, hRad17 mRNA expressions in 5 gastric benign lesions and SGC7901 gastric carcinoma cell lines were also examined.
RESULTSThe primary tumor samples (88.4% positive) showed a significantly higher level of hRad17 expression compared with matched normal tissue (76.9% positive) (P = 0.014), so did the lymph node metastatic samples (94.2% positive) (P = 0.001). The hRad17 mRNA expression showed a low level in benign lesions, but very high in SGC7901 cell line. The hRad17 mRNA expression showed a higher level in patients with the number of lymph node metastasis above 15 than below 15 (P = 0.02), so did the diffused growth than the mass-like growth (P = 0.04).
CONCLUSIONThe method of PAGE and silver stain can improve the sensitivity of RT-PCR. The degree of lymph node metastasis and invasiveness of carcinoma cells are more serious in cases with hRad17 mRNA overexpression, and extensive lymph node dissection should be carried out for these patients. Examination of hRad17 expression by RT-PCR before surgery is indicated to arrive at an optimum treatment scheme and to estimate the prognosis.
