1.Analysis of the delay diagnosis and treatment of lung cancer initially expressed as solitary pulmonary nodules diagnosed by physical examination——162 cases report

Objective To analyze the delay in diagnosis and treatment lung cancer expressed as solitary pulmonary nodules (SPN) found by physical examination,and to discover the relative reasons and consequence for future improvement.Methods From January 2000 to August 2011,162 patients (93 males,69 females,ranging 33-82 years,median age 63.9 years) with SPN found by physical examination and diagnosed lung cancer by surgical pathology subsequently were enrolled to this study.Depending on the interval between the date when finding SPN and the date of surgery,these cases were divided into 6 groups,including the group with interval less than 1 month (74 patients),1-3 months (48 patients),3-6 months (10 patients),6-12 months (7 patients),12-24 months (8 patients),and more than 24 months (15 patients).Factors which impact the delay interval between SNP finding and surgery were discussed.The change of tumor' s diameter during observation period,and the pathological characters were also analyzed among each group.Results Of all the delay cases,30.2% were attributed to doctor,30.9% attributed to patients themselves,and 38.9% attributed to hospitals.51.0% (25/49) of the cases delayed by doctors were misdiagnosed as inflammation,16.3% (8/49) misdiagnosed as tuberculosis,and 16.3% (8/49)misdiagnosed as old lesions,which were three common reasons.Patients delayed more than 3 months were more likely to be related to doctor's misdiagnosis than those delayed less than 3 months[70.0% (28/40) vs.36.8% (21/57),x2 =10.338.P =0.001].Moreover,the possibility of SPN enlargement was positively correlated with the delay interval.In groups with delay interval more than 24 months or between 12 and 24 months,the percentage of SPN enlargement were 73.3% (11/15) and 87.5% (7/8) separately.The proportion of patients with stage IV disease in groups whose delay interval exceeded 12 months (13.3% for more than 24 months and 12.5% for 12-24 months) also significantly surpassed others groups with shorter delay interval.Conclusion There exists obvious delayed diagnosis and treatment in lung cancer initially expressed as solitary pulmonary nodules (SPN) during physical examination.Nearly 1/3 patients were delayed by doctors and half of them were misdiagnosed as inflammation.Increase of observation time could result in enlargement of SPN and advancement of disease staging.Thus,patients and doctors should pay more attention and clarify the diagnosis through surgery in less than 3 months after finding SPN,which could remarkably benefit early treatment.

3.Design and implementation of private security network for Tianjin disease control and prevention information system

Objective To build a private security network for Tianjin disease control and prevention information system to extend the coverage of Tianjin direct reporting organizations virtual private network (VPN).Methods Load balance and VPN technologies were applied,and the network of Tianjin Centers for Disease Control and Prevention was used as the core node to distribute load balance devices.The disease control facilities at city and district levels were gifted with unified internet protocol address,and IPSect VPN was used to connect the facilities.The accesses of grades of healthcare administration organizations and medical facilities were executed with SSL VPN.Results The private security network adopted proper technologies,and could be used for safe transmission of the information on Tianjin disease control and prevention to fulfill national and Tianjin standards for network security.Conclusion The private security network developed decreases the risks for information security.
4.Simple high position ligation of internal spermatic vein under laparoscope in the treatment of children's varicocele

Objective To explore the method and superiority of laparoscopic management of children's varicocele. Methods High position ligation of varicocele under laparoscope was performed and internal spermatic arteries were reserved intraoperatively. Results 32 cases had smooth recovery after operation, and no complication occurred. The hospital stay was (1~3)days postoperatively. 29 cases had been followed up for 6 months~1 year and no relapse and testis atrophy occurred. Conclusions The method has the advantages of simpleness, minimal invasion and quicker recovery. Reservation of internal spermatic artery can prevent testis atrophy.
5.Mediastinoscopy for diagnosis of disease characteristic with mediastinal mass only

Objective To assess the usefulness video-assisted mediastinoscopy (VM) for diagnosis of disease characteristic with mediastinal mass. Analyze the correlations of clinical features of the disease with the pathologic diagnosis. Methods Between July 2000 and March 2006, 97 undiagnosed patients with mediastinal mass in chest radiography underwent biopsy of mediastinal lesions by either cervical mediastinoscopy (n=69) or parasternal mediastionoscopy (n=28). We also analyzed the correlation of symptoms, location and extent of the lesion with the pathologic diagnosis. Results All 97 patients recovered smoothly without postoperative complications. Histologic diagnosis was obtained in 96 patients. Patients with hoarse, dysphagia and superior vena cava obstruction syndrome indicate malignant lesions (26/26). The most common diseases in the super-anterior mediastinum are lymphoma(7/19), thymoma(4/19) and small cell lung cancer(SCLC) (4/19);The most common disease in the middle mediastinum are metastasis of non small cell lung cancer(11/37), sarcoidosis (9/37)and SCLC(7/37); The most possible disease involved both mediastinum and lung hilar lymph-node are scalerdosis(32/41) and SCLC(5/41). Conclusion Mediastinoscopy is effective and safe methods to establish a histologic diagnosis in patients with mediastinal mass. It is possible to make primary evaluation on some of these patients according to the typical symptoms and location, extent of the mass involvement.

7.Video-assisted thoracoscopic lower esophageal myotomy in the management of achalasia: A report of 21 cases

Objective To evaluate the practicability of video-assisted thoracoscopic lower esophageal myotomy in the treatment of achalasia. Methods A series of 21 consecutive patients with achalasia received video-assisted thoracoscopic lower esophageal myotomy between June 1997 and June 2005. The operation was performed under general anesthesia with double-lumen endotracheal intubation. The patient was positioned in the right lateral decubitus. Four thoracoscopic ports were introduced into the left hemithorax. A 6~11 cm incision at the lower esophageal sphincter with an extension to the gastric wall for 0.5~1 cm in length was made, to the depth of the submucous layer. A flexible gastroscope was inserted to check the integrity of the esophageal mucosa intraoperatively. No anti-reflux procedures were conducted. Results All the procedures were accomplished smoothly. The intraoperative hemorrhage volume was 50~100 ml (mean, 58 ml) and the operation time, 60~270 min (mean, 137 min). Esophageal mucosal perforation occurred in 3 patients, in 2 of whom the perforation was repaired thoracoscopically and in 1 of whom, through open thoracotomy. All the patients were recovered uneventfully, without severe complications. Follow-up checkups for 1~80 months found recurrence of dysphagia in 2 patients at 2 and 4 months after operation, respectively. Out of the 21 patients, the relief of dysphagia was classified as “excellent” results in 8 patients, “good” in 10 patients, “fair” in 1, and “poor” in 2, respectively. Conclusions Video-assisted thoracoscopic lower esophageal myotomy is a surgical approach with simple performance, minimal invasion, quick recovery and good efficacy. It can be employed as the first of choice for the management of achalasia.
8.Safety evaluation of mouse nerve growth factor injection for treatment of children and elderly patients with optic nerve damage

Background The clinical efficacy and safety of mouse nerve growth factor(NGF) for injection have been evaluated by Ⅰ,Ⅱ,Ⅲ stage of clinical trials.But as a clinical drug,its adverse effects for special population should been further studied.Objective This clinical trial was to observe and assess the safety and tolerability of mouse NGF for injection during the application in juvenile and elder patients with optic nerve damage.Methods A multicenter non-randomized controlled trial for the safety evaluation of mouse NGF injection solution was performed in 100 eye centers in China.This study protocol was approved by the Ethic Committee of each drug research center,and written informed consent was obtained from the subject,legal mandatary and guardian prior to this study.Total 2046 patients who met the included criteria of optical nerve damage were enrolled.The patients were divided into the juvenile group(90 cases,< 18 years),youth-middle-age group (1868 cases,1 8-75 years) and older group(88 cases,>75 years).Mouse NGF(30 μg)was intranuscularly injected once per day for 21 days in all the patients.Systolic and diastolic blood pressure,heart rate,intraocular pressure (lOP),laboratory examinations of blood and urine,blood biochemical indexes were recorded before and after injection.All adverse events were evaluated after administration of drug.Results There were 189 (189/2046,9.23%)dropped out cases,including 184 lost cases after 3 weeks of treatment and 5 withdrew cases from adverse responses of drug,with a shedding rate <10% in each group.Total 1857 individuals finished this trial.No significant differences were found in the IOP,heart rate and blood pressure between before and after admninistration of grug in the juvenile group,youth-middle-age group and older group (P>0.05).The incidence rate of the adverse response in the juvenile group,youth-middle-age group and older group was 57.78%,68.18%,60.01% respectively,and the primary local adverse events were pain(48.89%),redness and swelling(59.09%),duration(54.49%),without significant difference in the rate of local adverse events among the three groups(X2 =2.302,P =0.324).The abnormal rates of laboratory results of blood and urine were not significantly different among the three groups(all at P>0.05).The percentage of impaired fasting glucose was 7.46%,23.73%and 7.79% in the juvenile group,youth-middle-age group and older group,respectively,and the percentage in the older group was higher than that in the juvenile group and the youth-middle-age group (x2 =8.685,P =0.005 ;x2 =27.720,P =0.000).Conclusions Injection of mouse NGF is well tolerated in juvenile and elder patients over the age of 75 years.
9.Preliminary investigation of the gene expression of Tbx3 in human breast carcinoma tissues and the related significance

Objective To explore the role of Tbx3 gene in the occurrence and development of breast cancer. Methods 44 cases of the fresh breast carcinoma tissues and peritumor tissues were collected individually, the gene expression of Tbx3 was detected by RT PCR and the relationship between the changes of Tbx3 gene expression and the related clinicopathological factors of breast carcinoma was investigated. Results Obviously, the expression level of Tbx3 gene in breast carcinoma tissues was higher than that in the peritumor tissues. Furthermore, Tbx3 gene expression level in breast carcinoma tissues was not associated with the tumor size and ages of patients. However, the histological grading of breast carcinoma, tumor infiltrating degree, TNM stagings, and axillary lymph node metastasis were positively correlated with the expression level of Tbx3 gene. Conclusion Tbx3 gene with high expression may promote the occurrence and development of breast cancer and it is also likely a symbol of crucial changes in the development of breast cancer
