1.Factors affecting the health-related quality of life among urban adults in Hangzhou City

Objective:
To investigate the health-related quality of life and its influencing factors among urban adults in Hangzhou City, so as to provide the evidence for formulation and assessment of health policy.
Methods:
A total of 1 800 permanent residents at ages of 18 to 64 years were sampled from Gongshu and Xihu districts, Hangzhou City using a stratified cluster random sampling method from May to August 2022. The health-related quality of life was measured with the EuroQol five-dimensional questionnaire (EQ-5D-5L), and factors affecting health-related quality of life were identified with a Tobit regression model.
Results:
A total of 1 624 valid questionnaires were recovered, with a response rate of 90.22%, and the respondents included 693 men (42.67%) and 931 women (57.33%). The proportions of difficulty in mobility, self-care, usual activities, pain/discomfort, and anxiety/depression were 1.23%, 0.43%, 0.74%, 11.15% and 8.56%, and the median (interquartile range) of health state utility value and EuroQol Visual Analogue Scale (EQ-VAS) score were 1 (0) and 90 (15) points, respectively. Female (β=-0.050), age of 45 to 64 years (β=-0.067) and development of chronic disease (one chronic disease: β=-0.036; two and more chronic diseases: β=-0.090) were factor affecting of health state utility values, and age of 35 to 44 years (β=-1.945) and 45 to 64 years (β=-3.459), unemployment (β=-1.913), development of chronic disease (one chronic disease: β=-3.444; two and more chronic diseases: β=-8.529), high-level physical activity (β=2.355) and overweight/obesity (β=-1.456) were factors affecting the EQ-VAS score.
Conclusions
The overall health related quality of life is relatively good among urban adults in Hangzhou City. Gender, age, employment, physical activity, presence of chronic diseases and overweight/obesity may be associated with health-related quality of life among urban adults in Hangzhou City.

The external medicine is an indispensable treatment of traditional Chinese medicine (TCM) surgery.The application of Hg is one of the characteristics of TCM surgery medicine for external use.This paper analyzes the present situation of clinical application of Hg and summarizes the common adverse reactions in the literature.The focus of the research on the development of new drugs containing Hg in traditional Chinese medicine is discussed.
4.Analysis in the result of Keshan disease surveillance in Hebei province from 1990 to 2007

Objective To observe the dynamic changes of prevalence of Keshan disease (KD) in Hebei province from 1990 to 2007, to provide scientific basis for its prevention and treatment. Methods The surveillance data of KD was analyzed according to "the National Scheme of KD Surveillance and the Surveillance of KD" (W/T 78-1996) in Hebei province from 1990 to 2007 by the Institute for Prevention of Endemic Disease in Hebei Province Center for Disease Control and Prevention. The data included physical examination, electrocardiogram (ECG), the chest X-ray film of KD patients and the suspected patients, as well as selenium contents of hair collected in 1990, 1992 and 1999. Results No new cases of acute and subacute types of KD patients were found at the surveillance sites from 1990 to 2007. Thirty-five cases of new latent KD and one case of spontaneous chronic KD were identified respectively. Prevalence of chronic and latent KD ranged from 1.12% (8/713) to 8.18% (27/330) and 2.29% (19/831) to 8.20% (45/549) in Hebei province from 1990 to 2007, respectively. The prevalence of KD in children aged 3 - 14 years old and childbearing woman aged 20 - 45 years old decreased year by year, however population over 45 years old were more likely suffering from KD. The major abnormal changes of ECG in KD) patients were complete fight bundle branch block, ST-T change frequent premature ventricular contraction, and left anterior faseicular block. The prevalence of the heart enlargement in KD patients was 47.00% (211/449) averagely, and the prevalence of heart enlargement of medium grade increased remarkably after 2005 [28.57%(8/28) - 48.39%(15/31)]. The average mortality in chronic KD patients was 18.0%(18/100) from 1990 to 2007. Conclusions The prevalence of KD decreased slowly in Hebei province. Hebei province is still the region with higher prevalence of KD around the country, and the tasks of prevention and treatment of KD is still urgent. Enhancing the surveillance of of KD and carrying out management and treatment of KD patients should be emphasized in the future.
5.Gene express and antibody preparation of Neurocan protein

Objective To offered some prophase works by preparing Neurocan protein, antiserum, and assaying their characteristics, in order to construct the isopropy-β-D-thiogalactoside (IPTG)-participated DNA vaccine, which can neutralize the inhibitors in the injured CNS following the immune administration and then promote the nerve regeneration. Methods Neurocan gene was syntbetized with HisTag label in beginning and enzyme-cut sites at amphi- of the sequence. The prokaryotic expression plasmid, PET30a-Neurocan, was constructed as usual, converted into the Escherichia coli, and induced by IPTG to express positively. The interest protein was identified by SDS-PAGE and Western blot respectively. The preimmune serum was as the negative control during the ELISA assay of antiserum valency. The immune serum was as the first antibody, and the goat-anti-rabbit labeling with alkaline phosphatase (AP) was employed as the second one. Coloration was with NBT/BCIP method. Results The correct sequence of the synthetic Neurocan gene was clearly showed by identification with enzyme-cut, PCR and sequencing. The Neurocan protein expressed by prokaryotic showed its molecular weigh as 55 000 following the SDS-PAGE identification, and it could specifically bind with anti-HisTag, which implied the interesting protein just as the expression product of Neurocan gene. The valency of antiserum was shown by ELISA as 1:1 000 000, the purpose strap of which was confirmed by Western blot. Conclusions Neurocan protein could be successfully expressed in prokaryotic, the antibody of which could be specifically obtained by immune administration to the rabbit. The Neuroncan antibody could bind with the Neurocan protein specifically.
6.Discussion of clinical research on new Chinese herbal medicine is used to treat diabetic foot ulcers

Diabetic foot ulcer (DFU) is one of the serious complications of diabetes.With high disability,high amputation rate,serious harm to the physical and mental health of diabetes patients.Traditional Chinese medicine drugs for external use has a certain effect and advantages for treatment of DFU.In this paper analyzes the clinical characteristics of DFU and the status of external drug application,and the necessity of research and development of traditional Chinese medicine for DFU treatment.In the article pointed out the difficulties and of the concerns in process of clinical trials for the DFU traditional Chinese medicine research and development.The preliminary recommendations are given in this article.
7.Ultrasonic destruction of albumin microbubbles enhances gene transfection and expression in cardiac myocytes

Background It has been proven that ultrasonic destruction of microbubbles can enhance gene transfection efficiency into the noncardiac cells, but there are few reports about cardiac myocytes. Moreover, the exact mechanisms are not yet clear; whether the characteristic of microbubbles can affect the gene transfection efficiency or not is still controversial.This study was designed to investigate whether the ultrasound destruction of gene-loaded microbubbles could enhance the plasmids carried reporter gene transfection in primary cultured myocardial cell, and evaluate the effects of microbubbles characteristics on the transgene expression in cardiac myocytes.Methods The β-galactosidase plasmids attached to the two types of microbubbles, air-contained sonicated dextrose albumin (ASDA) and perfluoropropane-exposed sonicated dextrose albumin (PESDA) were prepared. The gene transfection into cardiac myocytes was performed in vitro by naked plasmids, ultrasound exposure, ultrasonic destruction of gene-loaded microbubbles and calcium phosphate precipitation, and then the gene expression and cell viability were analyzed.Results The ultrasonic destruction of gene-loaded microbubbles enhanced gene expression in cardiac myocytes compared with naked plasmid transfection ((51.95±2..41) U/g or (29.28±3.65) U/g vs. (0.84-0.21) U/g, P <0.01), and ultrasonic destruction PESDA resulted in more significant gene expression than ASDA ((51.95e2.41) U/g vs. (29.28±3.65)U/g, P <0.05). Ultrasonic destruction of microbubbles during calcium phosphate precipitation gene transfection enhanced 3-galactosidase activity nearly 8-fold compared with calcium phosphate precipitation gene transfection alone ((111.35±11.21) U/g protein vs. (14.13±2.58) U/g protein, P<0.01). Even 6 hours after calcium phosphate precipitation gene transfection, ultrasound-mediated microbubbles destruction resulted in more intense gene expression ((35.63±7.65)U/g vs. (14.13±2.58) U/g, P<0.05 ).Conclusions Ultrasonic destruction of microbubbles might be a promising method for the delivery of non-viral DNA into cardiac myocytes, and the gene tranfection is related to the characteristics of microbubbles.
8.An experimental study on the influence of hypoxia induction factor-1alpha on the glycolysis of the rat myocardial cell under hypoxic condition.

OBJECTIVETo investigate the influence of hypoxia induction factor-1alpha (HIF-1alpha) on glycosis of rat myocardial cell under hypoxic condition.
METHODSThe myocardial cells of the rats were routinely isolated and cultured. The cells were divided into single hypoxia (H) and HIF-1alpha inhibiting (I) groups. The cells in H group were cultured in glucose-free medium with mixed low-oxygen gas [1% O2, 94% N2 and 5% CO2 (v/v)]. While the cells in I group were cultured with low-oxygen gas after the cell model of low expression of HIF-1alpha protein constructed by RNAi technique. The cells in both groups were all observed before hypoxia (routine culture) and at the time points of 1, 3, 6, 12 and 24 hours of hypoxia. The LA (lactate acid ) content in the supernatant of the culture and the activity of the key enzymes in glycolysis such as hexokinase (HK), phosphofructokinase (PFK) and lactate dehydrogenase (LDH) of both groups of cells were determined at all the time points.
RESULTS(1) After hypoxia, the HK and PFK activities of the rat myocardial cells in H and I groups were obviously increased at the beginning and decreased thereafter when compared with that before hypoxia. While the activities of HK and PFK in H group at 1, 3 and 6 hours after hypoxia were evidently higher than those in I group (P <0.05 or 0.01), and the peak activity of them in H and I groups was 159 +/- 13 U/g vs 133 +/- 55 U/g, and 298 +/- 44 U/g vs 188 +/- 55 U/g, respectively. (2) Compared with normal control (92 +/- 12 U/g), the LDH activity of the cells in H group after hypoxia increased significantly, reaching the peak at 6 hours after hypoxia (2 568 +/- 125 U/g, P < 0. 01), and it decreased thereafter, while that in I group peaked at 3 hours after hypoxia (2125 +/- 126 U/g, P <0.01). The LA content in the culture supernatant in H group increased significantly after hypoxia with the passage of time, while that in I group increased in smaller magnitude (P <0.01).
CONCLUSIONHigh expression of HIF-1alpha in the rat myocardial cells after hypoxia could directly cause continuous enhancement of cell glycolysis, which was beneficial to the protection of myocardial cells under hypoxic condition.

9.Incidence and mortality of thyroid cancer among permanent residents in Luwan district of Shanghai during 2004-2011

Objective:To analyze the incidence and mortality of thyroid cancer among permanent residents in Luwan district of Shanghai from January 2004 to December 2011.Methods:The data of thyroid cancer in permanent residents of Luwan district were collected from the database of cancer registration and management system in Shanghai.The incidence and mortality of thyroid cancer were calculated.The rates were standardized by the demographic composition developed in the Fifth Nationwide Census in the year 2000.The temporal trend in the incidence and mortality of thyroid cancer was assessed using average annual percent change.Results:A total of 413 cases of thyroid cancer were diagnosed from 2004 to 2011,accounting for 3.96% of the total patients with malignant diseases.During 2004 to 2011,the crude incidence rates ranged from 7.49/105 to 34.86/105,and the standardized incidence rates ranged from 6.02/105 to 23.63/105,displaying a significant increasing trend.The incidence rate in females was significantly higher than that of males.The incidence rate of females peaked at 40-55 years of age.A total of 24 cases died of thyroid cancer during this period.During 2004 to 2011,the crude mortality rates ranged from 0.63/105 to 1.61/105,and the standardized mortality rates ranged from 0.13/105 to 0.46/105.The mortality rate remained stable during the eight year period.Conclusions:There were significant increases of thyroid cancer occurrence from 2004 to 2011 among permanent residents in Luwan district of Shanghai.The mortality rate of thyroid cancer is low.Both the incidence and mortality rates of females were significantly higher than those of males.It is important to identify risk factors of thyroid cancer,so effective control measures can be taken to control its rapid increase.
10.Prevalence and risk factors of peri-procedure electrical storm in acute myocardial infarction patients underwent emergency percutaneous coronary intervention

Objective To determine the prevalence and to identify risk factors of peri-procedure electrical storm (ES) in patients with acute myocardial infarction (AMI) underwent emergency percutaneous coronary intervention (PCI).Methods The clinical data of 228 AMI patients underwent emergency PCI were retrospectively analyzed and patients were divided into ES group (n = 39) and non-ES (n = 189) group.ES was referred to spontaneous ventricular tachycardia or ventricular fibrillation occurring twice or more within 24 h and requiring emergency treatment including anti-arrhythm medicine and/or cardioversion or defibrillation.Results ES was diagnosed in 39 out of 228 patients (17.1%) during peri-procedure stage.The incidence of ES in patients with various infarct related arteries (IRA) was as follows:55.6% with left main artery (LM),23.7% with right coronary artery (RCA),12.4% with anterior descending branch (LAD) and 0 with left circumflex artery (LCX).Older age,lager diameter of IRA,higher concentration of CK-MB and cTnT,higher incidence of reporfusion arrhythmia (RA),lower grade of TIMI after PCI and higher mortality were associated with increased risks of ES (The P value was 0.043,0.012,0.036,0.018,0.001,0.049,respectively).Gender,systolic pressure,diastolic pressure,random blood glucose level,white blood count and concentration of hs-CRP were similar between ES and non-ES patients.Logistic analysis showed that the diameter of IRA (OR 2.381,95% CI 1.127-5.028,P = 0.023),TIMI grade of IRA after PCI (OR 4.744,95% CI 1.773-12.691,P = 0.002) and RA (OR 12.680,95% CI 4.360-36.879,P =0.000)were the independent risk factors of per-procedure ES in AMI patients underwent emergency PCI.Conclusions The AMI patients with LM as IRA had the highest incidence of ES during emergency PCI and the diameter of IRA,TIMI grade of IRA after PCI and RA were independent risk factors for the development of ES during peri-PCI stage.
