1.Pathological Features of Myocardial Contraction Band Necrosis in Sudden Cardiac Death.

OBJECTIVE:
To investigate the pathological features and significance of myocardial contraction band necrosis in sudden cardiac death.
METHODS:
Using HE and PTAH staining, the distribution sites and pathological features of myocardial contraction band necrosis were observed. The data were analyzed according to the extent of necrosis.
RESULTS:
The locations, pathological features and the extent of necrosis were similar in different sudden death cases. The locations were related with sites of myocardial damage. The papillary muscles of left ventricular were most occurred, followed by the anterior wall of left ventricular. In the sudden death cases caused by stress cardiomyopathy, the extent of myocardial contraction band necrosis was lighter than the others. Most cases were predominantly level 1, the others were level 2.
CONCLUSION
Used as the diagnosis basis of acute myocardial ischemia, the myocardial contraction band necrosis has important significance to determination of death.

4.Comparison of NT-proBNP in the diagnosis of acute heart failure in different age groups

Objective To analyze the influence of age on the diagnosis of acute heart failure (AHF) with amino terminal brain natriuretic peptide (NT‐proBNP) ,and to explore the best cut‐off value of NT‐proBNP in the diagnosis of AHF .Methods Totally 457 inpatients suspected with AHF obtained a blood NT‐proBNP measurement within 24 hours when in charged ,including 194 patients with AHF .Patients were divided into three groups according to their age :≤50 years ,>50-70 years and >70 years . The relationship between NT‐proBNPand AHF was analyzed .The diagnostic value of NT‐proBNP was evaluated for AHF in differ‐ent age groups .Results The median NT‐proBNP levels of AHF in all age groups were significantly higher than those without (P<0 .01) .AUC of NT‐proBNP for AHF in ≤50 years ,>50-70 years and >70 years groups were 0 .95 ,0 .91 ,0 .88 ,the best diagnos‐tic cut‐off points were 1 100 ,1 800 ,3 000 pg/mL ,respectively .The diagnostic specificity was 88% ,76% and 73% ,respectively .O‐verall ,the specificity of NT‐proBNP cut‐off values for age group was higher than non‐age groups .Conclusion NT‐proBNP has a good value in the diagnosis of AHF in patients with different ages .According to the age group ,the different NT‐proBNP cut‐off values can be used to improve the specificity of diagnosis .
5.Strategies of sperm collection from men with temporary penile erectile dysfunction on the day of oocyte retrieval and the outcomes of IVF-ET.

OBJECTIVETo search for the optimal strategies for sperm collection from the patient with temporary penile erectile dysfunction (ED) on the day of oocyte pick-up ( OPU) in in vitro fertilization embryo transfer (IVF-ET).
METHODSWe retrospectively analyzed 93 cases of temporary ED on the OPU day of IVF-ET from January 2011 to May 2014, with fresh semen for 45 cases (group A), cryopreserved sperm before oocyte retrieval for 30 cases (group B), and frozen oocytes for 18 cases (group C). Group A was again subdivided into A1 (n = 18) and A2 n = 27) , the former intervened with oral sildenafil while the latter left untreated. We compared the rates of fertilization, high-quality embryo, and pregnancy among different groups.
RESULTSNo statistically significant differences were found among groups A, B and C in the age of the males and females, duration of infertility, numbers of obtained and mature oocytes, and rates of cleavage, or in the percentages of normal fertilization (80.78% vs 80.43% vs 84.77%), high-quality embryo (53.27% vs 52.97% vs 47.69%) and pregnancy (60.00% vs 56.77% vs 44.44%) (all P > 0.05). The rate of 3PN was markedly lower in group C (0.63%) than in A (9. 61%) and B (4.34%) (P < 0.05). There were no significant differences between groups A1 and A2 in the age of the males and females, duration of infertility, numbers of obtained and mature oocytes, and the rates of fertilization, cleavage, high-quality embryo, and pregnancy (all P > 0.05).
CONCLUSIONOn the OPU day of IVF-ET, oral sildenafil can help temporary ED men to achieve penile erection and ejaculation without affecting the outcomes of assisted reproduction. Cryopreserved sperm can be used in case of predicted temporary ED and frozen oocytes can also be employed if sperm retrieval fails. However, to avoid puncture injury to the epididymis or testis, fresh semen should be the first choice.

6.Analysis of 27 death cases of the abandoned babies in Shanghai.

OBJECTIVE:
To analyze and summarize the characteristics of the death cases of the abandoned babies in Shanghai, and to explore the main points and features in forensic investigation.
METHODS:
Twenty-seven death cases of the abandoned babies were selected from 2007 to 2011 in the Institute of Forensic Science of Shanghai Public Security Bureau. The cases were retrospectively analyzed in the respects of scene of abandonment, cause of death and manner of death, etc.
RESULTS:
Among the death cases of the abandoned babies in Shanghai, most of the infants were alive with young unmarried mothers. The main cause of death was mechanical asphyxia, and homicide occurred more commonly.
CONCLUSION
It is significantly difficult to determine the cause and manner of death in such cases, needing to be a comprehensive analysis on the case background, the investigation of scene and the examination of injury, etc.

7.Treatment of renal lower caliceal calculi with minimally invasive percutaneous nephrolithotomy

Objective To investigate the clinical outcomes of minimally invasive percutaneous nephrolithotomy(MPCNL)in the treatment of lower caliceal calculi.Methods We retrospectively re- viewed the clinical outcomes and complications of 33 patients who underwent MPCNL for lower caliceal cal- culi from March 2001 to April 2005.The average diameter of the calculi was 2.8 cm.Single tract nephrosto- my was performed in all 33 cases;among them renal access was obtained through a middle calyx in 10 cases and a lower calyx in 23.Nine cases had F14 renal access;and 24 cases,F16.Results Of 33 cases,28 (85%)achieved stone-free at 1 session.A second-look was needed in 3 cases due to intraoperative bleed- ing;ESWL,in 1 case with residual,calculi;no treatment,in 1 case with residual calculi<4 ram.The mean operative time was 93 min;mean blood loss was 113 ml;mean hospital stay was 11 d.Blood transfusion was needed in 1 patient who suffered from hepatic cirrhosis preoperatively;another experienced severe bleeding 7 d after operation and was cured with hyperselective spongia gelatinosa embolization of the renal artery.Fol- low-up was available in 19 cases for 2-48 months,and no recurrence of renal calculi was noted.Conclu- sions Minimally invasive percutaneous nepbrolithotomy has advantages of safety,less invasion,and easy re- covery for the treatment of lower caliceal calculi.
8.Clinical research on relative factors and curative effects of Yi-gan-san modified with attention deficit hyperactivity disorder

Objective To explore the main influence factors of attention deficit hyperactivity disorder ( ADHD) and evaluate the clinical effect of Yi-gan-san modified.Methods Questionnaire survey consisting of home environment, living habits and exposure to environmental toxicant was used to investigate and evaluate children in the two groups, one of them was the study group with 120 cases of ADHD children, and the other was the control group with 120 cases of healthy children.A total of 75 cases with the pi-xu-gan-kang type were found among the ADHD children, and they were randomly divided into 2 groups:the experimental group ( n =38 ) , using drug therapy of Yi -gan -san modified ( 150 mL, twice a day ) and behavioral therapy (3 times a week);the control group, using behavioral therapy alone.The treatment lasted for all 8 weeks.The data of the line integral of traditional Chinese medicine symp-tom assessment was used to evaluate the curative effect before treatment and 4, 8 weeks after treatment.Results The survey found that the family environment such as low education level of parents, parenting methods, family income less than three thousand yuan each month, parenting and feeding patterns, whether both parents were the only child in the family were significantly associated with the occurrence of ADHD.We also found the exposure to DEHP, children′s rest hobbies, sleep habits, eclipse snack times and playing phone were significantly re-lated with the occurrence of ADHD.The CR of the treatment group was 52.63% and the total effective rate was 86.84%, and there was 24.32%and 43.24%in the control group respectively.The TCM syndrome score between the two groups had significant statistical significance ( P<0.01 ) , and the curative effect of the treatment group was better than that in the control group.The TCM syndrome score were significantly lowered after 8 weeks of treatment in the two groups (P<0.01).Conclusion The family environment, lifestyle and children′exposure to environmental toxicants were susceptible to the occurrence of ADHD.Using Yi-gan-san modified intervention for the treatment of ADHD is effective.
9.Clinical analysis of a family with familial hemangioblastoma

Objective To analyze the clinical and imaging features of a family(3 patients)with familial hemangioblastoma,and their diagnosis and prognosis.Methods The detailed data about clinical and imaging features of all patients diagnosed as familial hemangioblastoma,admitted to our hospital from October 2004 to May 2010,were analyzed,and the lesions of other regions,besides the tumor lesion,were observed.Results No lesions of other regions were noted in these 3 patients.Cranial MRI showed that 2 had cystic and solid tumor and 1 had solid tumor;,total removal was performed on these patients under microscope; regular follow-up was given and no recurrence was noted.Conclusion Familiar hemangioblastoma is serious hereditary disease; and MRI is the most important detective method; microsurgical operation is the most important therapy.Early diagnosis and treatment should be given to the patients with familiar hemangioblastoma due to its high recunence rate,having difficulty in operation and its trend to combining with other lesions of the other parts.
10.Effects of L-nitroarginine on the recovery of traumatic facial paralysis.

OBJECTIVETo study the effects of constitutive nitric oxide synthase inhibitor L-nitroarginine on the recovery of traumatic facial paralysis in rats and the changes of the expression of cNOS and OX42 in the facial nucleus.
METHODSL-nitroarginine was intraperitoneally injected into rats and the recovery of facial paralysis was observed at different time point. and the changes of cNOS and OX42 positive neurons were studied in facial nucleus.
RESULTSTreatment of L-nitroarginine could remarkably inhibit the recovery of traumatic facial paralysis. The cNOS immunoactivity was obvious inhibited in facial nucleus, while the OX42 immunoactivity was obvious increased.
CONCLUSIONEndogenous nitric oxide may play an important mediator role on the recovery of traumatic facial paralysis.
