1.Study on biosynthesis of silver nanoparticles using fagopyri dibotryis rhizoma extract and optimization of synthesis conditions.

Silver nanoparticles were synthesized from the extract of Fagopyri Dibotryis Rhizoma and the optimization of synthesis was studied. The absorbance of UV-visible spectroscopy was determined under the different influencing factors such as extracting time of Fagopyri Dibotryis Rhizoma powder, reation temperature of synthesis, volume of Fagopyri Dibotryis Rhizoma extract and concentration of AgNO3 to seek the optimization conditions. By means of FT-IR, TEM, DLS and XRD, the silver nanoparticles were characterized. The results showed that when the boiling time of Fagopyri Dibotryis Rhizoma powder was 5 min, resultant temperature was 25 degrees C, the volume ratio of 0.1 g x mL(-1) Fagopyri Dibotryis Rhizoma extract and 1 mmol x L(-1) AgNO3 was 1 to 10, and the reaction time was 3.5 h, the obtained silver nanoparticles had mean size about 27 nm and Zeta potential about -34.3 mV with good uniformity and dispersivity. Therefore, the green synthesis method of silver nanoparticles using extract of traditional Chinese medicine is stable and feasible.

2.Meta analysis for relationship between exposure of free silicon dioxide and lung tumor.

OBJECTIVEMeta-analysis is applied to process a systematic, comprehensive evaluation on the relationship between silica exposure and lung cancer in order to provide scientific evidences for controlling measures and offer a case for EBOM.
METHODSAccording to established entrance standards, the collected original articles on silica exposure and lung cancer were classified and put in order, then fixed effects model and random effects model were used which suited to analyze occupational epidemiology and occupational tumor materials, and their correlation intensity were analyzed. First, combined SMR and 95% CI values for case-control study and cohort study were calculated respectively. Then all case-control studies and cohort studies were combined according to different study designs, mixed factors adjusted and stratification analysis processed, and their SMR and 95% CI calculated respectively. In order to lessen bias or errors, fail safe number was calculated.
RESULTSWhen combining all the cohort study and all the case-control study respectively, corresponding combination SMR(cohort) = 1.93, 95% CI is 0.55 - 6.92; SMR case = 1.73, 95% CI is 1.01 - 2.99; SMR(sum) = 2.19, 95% CI is 1.45 - 3.31. Statistical test showed a statistically significant relationship lied between silica exposure and lung cancer, with P < 0.05.
CONCLUSIONMiddle intensity relation is drawn between silica exposure and lung cancer incidence at worksite. This result is consistent with a lot of public reports.

3.Clinical analysis for fixation failure of 13 patients with intertrochanteric fractures.

OBJECTIVETo analysis the reasons of fixation failure for intertrochanteric fractures, so as to select correct operation indications and fixation methods.
METHODSRetrospective analysis the clinical data of 13 patients with failed internal fixation of intertrochanteric fractures from September 1997 to September 2008, and the failure reasons were summarized. There were 7 males and 6 females,ranging in age from 58 to 93 years,averaged 71 years. Two patients were treated with intramedullary fixation, 4 patients with anatomical proximal femoral plate, 3 patients with DHS fixation, 2 patients with hollow compression screws, and 2 patients with external fixation. According to Evans types: 1 patient was type II, 7 patients were type III, and 5 patients were type IV.
RESULTSEight patients with unstable fractures and malreduction had no grafted bone. Six patients had bad position of neck screws in the femur neck. Postoperative collodiaphyseal angle: 3 patients were under 90 degree, 7 patients 90 to 110 degree, and 3 patients 110 to 130 degree. Five patients had internal fixed screw exited, 6 patients had neck screws cutting to superior lateral, 3 patients had early weight bearing, and 10 patients were osteoporosis occurred after operation from 6 weeks to 11 months, averaged 4.5 months.
CONCLUSIONThe fixation failure of intertrochanteric fractures was concerned with fractures types, reduction, fixation methods, osteoporosis and the time of weight bearing.

4.The recent clinical efficacy study of photoselective vaporization of prostate (120 W) for treatment of large gland benign prostatic hyperplasia.

OBJECTIVETo explore the recent clinical safety and efficacy of photoselective vaporization of prostate (120 W) for the treatment of large gland benign prostatic hyperplasia.
METHODSThe clinical data of 112 cases who accepted photoselective vaporization of prostate (120 W) for the treatment of severe benign prostatic hyperplasia (the weight of prostate > 75 g) from July 2010 to January 2012 was statistical analyzed. Relief symptoms and complications were observed around surgery, and the recent clinical efficacy was analyzed.
RESULTSAll the operations were smooth. There were not transurethral resection syndrome. No cases need transfusion intraoperative and postoperative. The operation average time was (52.6 ± 12.1) minutes, and the average amount of bleeding was (27.4 ± 18.5) ml. The postoperative bladder irrigating time was (19.4 ± 7.3) hours, the mean postoperative indwelling catheter time was (3.2 ± 0.6) days, and the mean postoperative hospital stay was (4.8 ± 1.3) days. Postoperative international prostate symptom score (t = 52.24 - 59.10), quality of life (t = 48.42 - 53.63), maximum flow rate (t = -31.01 - -24.23) and residual urine volume (t = 9.85 - 12.53) compared with preoperative are significantly improved (P < 0.01).
CONCLUSIONSWith the safe operation of photoselective vaporization of prostate (120 W) for the treatment of large gland benign prostatic hyperplasia, it is less bleeding, recent efficacy is significant, and it is especially appropriate to elderly patients at high risk of large gland.

5.Etiological analysis and significance of anterior knee pain induced by gluteal muscles contracture.

OBJECTIVETo explore causes of gluteal muscle contracture induced anterior knee pain and curative effect of arthroscopic release.
METHODSFrom March 2002 to August 2013,36 patients with gluteal muscle contracture induced anterior knee pain were treated, including 15 males, 21 females, aged from 9 to 40 years old with an average (18.7±7.2) years old; the courses of diseases ranged from 4 to 30 years. The clinical manifestations involved limited to symmelia, positive Ober sign, buttocks touch contracture belts, knee and patella slide to lateral when doing squat activities. All patients were performed gluteal muscle contracture release under arthroscopic. Postoperative complications were observed, Kujala scoring before and after operation was used for compare curative effect.
RESULTSAll patients were followed up with an average of 29 months. The incision were healed well, and no complications were occurred. Postoperative Kujala score were improved more than preoperative.
CONCLUSIONGluteal muscle contracture release could alleviate hypertension of lateral patella, and palys an important role in preventing patellofemoral arthritis.

6.Arthroscopic repair of meniscus injury with Fast-fix under local anesthesia.

OBJECTIVETo evaluate the clinical outcome of arthroscopic repair method of meniscus injury with Fast-fix under local anesthesia.
METHODSFrom October 2005 to September 2012,106 patients with meniscus injuries admitted into our - hospital were studied, including 74 males and 32 females, ranging in age from 13 to 71 years old, averaged 27.6 years old. The duration of the disease ranged from 15 days to 5 years. The main clinical manifestations included knee joint pain after exercise, joint locking, pressing pain of knee joint and positive McMurray signs. The MRI showed meniscus tear or degeneration. Arthroscopic repairing surgeries were performed with Fast-fix under local anesthesia. Each patient was assessed with VAS pain evaluation and Lysholm knee-joint score system before and after operation. All the patients were followed up more than 1 year.
RESULTSOne hundred and 2 patients were followed up by recording subjective symptoms, clinical examinations and questions naires for an average of 2.6 years (ranged, 1.1 to 8 years), and 4 patients lost follow-up. All the 102 patients had no anesthetic complications. Ninety-six patients had normal subjective symptom and clinical examinations. Four patients had a mild ache with activities,2 patients had moderate pain after activities with joint space pressing pain. VAS pain evaluation and Lysholm knee-joint score after operation both were much better than that before operation.
CONCLUSIONonclusion: Local anesthesia can provide nice circumstances for surgeries. Arthroscopic repair using Fast-fix is an idea method for meniscus injury, especially for the posterior horn tear of medial meniscus, which is simple and convenient with less complications, and satisfactory results.

7.Endovascular repair of peripheral traumatic pseudoaneurysm with ViabahnTM self-expanding covered stent

Objective To discuss the feasibility, safety and validity of the use of ViabahnTM self-expanding covered stent in endovascular repair of peripheral traumatic pseudoaneurysm (TPA). Methods During the period from June 2012 to April 2014, a total of 8 patients with peripheral TPA were admitted to the Department of Vascular and Endovascular Surgery of Shanghai Changzheng Hospital . The clinical data were retrospective analyzed. The lesions were located at the common carotid artery (n=4), vertebral artery (n=1), subclavian artery (n=1) and superficial femoral artery (n=2). On the basis of acute and chronic TPA classification and different therapeutic principles, endovascular repair with ViabahnTM self-expanding covered stent was employed in 8 patients. Intraoperative angiographic findings, the size and number of the implanted stent, and the immediate angiographic results after stent implantation were recorded. All the patients were followed up to evaluate the improvement of the symptoms, the effectiveness of the repair of TPA cavity, and the occurrence of endoleak or restenosis. Results Successful implantation of ViabahnTM self-expanding covered stent was accomplished in all 8 patients, with a technical success rate of 100%. Angiography performed immediately after stent implantation showed that complete repair of TPA cavity was obtained in all patients, the distal segment of the parent artery was patent, and no endoleak was observed. The sizes of the stent used in the patients were 8×50 mm (n=2) and 9×50 mm (n=2) in common carotid artery, 6×50 mm (n=1) in vertebral artery, 11×100 mm (n=1) in subclavian artery, and 8×100 mm (n=2) in superficial femoral artery. The patients were followed up for 6-30 months (mean of 14.3 months), and the clinical symptoms disappeared in all patients. CT angiography indicated that there was no endoleak or restenosis. Conclusion Based on the different therapeutic principles, the use of ViabahnTM self-expanding covered stent in endovascular repair of peripheral TPA is technically simple and micro-invasive, and clinically feasible with reliable effect and safety. Therefore, this technique can be employed as a first-line therapy option for peripheral TPA.
8.Anterior cruciate ligament reconstruction with tendon graft enveloped by preserved remnants.

OBJECTIVETo explore the curative effect of anterior cruciate ligament (ACL) reconstruction with tendon graft enveloped by preserved remnants.
METHODSFrom June 2009 to June 2011,76 patients with ACL injury were treated, and the ACL was reconstructed with tendon graft enveloped by preserved remnants. There were 59 males and 17 females, aged 15 to 46 years with an average of 29 years. The causes of injuries included sports in 32 cases, training in 20 cases, traffic accidents in 16 cases, and other reasons in 8 cases. The average disease duration was 3 weeks (2 to 5 weeks). The results of Lachman test was positive in 72 cases and anterior drawer test were positive in 76 cases. The preoperative IKDC score was 45.9 +/- 3.6. The preoperative Lysholm knee score was 54.3 +/- 4.5.
RESULTSAll incisions got an one stage healing, and no early complications occurred. Sixty-nine cases were followed up, and the duration ranged from 17 to 31 months (mean, 23 months). The result of Lachman test was negative at the latest follow-up. At the latest follow-up, the degree of IKDC was A in 36 cases, B in 30 cases, C in 3 cases, no one in degree D. The postoperative IKDC score was 91.5 +/- 3.4, showing significant difference compared with preoperative score. The postoperative Lysholm score was 92.4 +/- 3.8, showing a significant difference compared with preoperative score.
CONCLUSIONThe clinical effect of anterior cruciate ligament (ACL) reconstruction with tendon graft enveloped by preserved remnants is satisfactory.

OBJECTIVETo evaluate clinical effects of arthroscopic anterior cruciate ligament (ACL) reconstruction with preservation of the remnant bundle.
METHODSFrom January 2002 to December 2009, 57 patients with ACL partial rupture were treated with preservation of the remnant bundle. There were 39 males and 18 females, with an average age of 28.5 years old (ranged, 16 to 49 years old). Thirty-three patients had injuries in the left knees,and 24 patients had injuries in the right knees. Forty patients had injuries caused by sports, 7 patients had injuries caused by traffic accidents and 10 patients had injuries caused by daily sprain. The anterior drawer test showed positive results in 23 patients, weakly positive results in 6 cases; Lachman test showed positive results in 19 patients, and weakly positive in 4 patients. Both anterior drawer test and Lachman test showed positive results in 5 patients; and valgus stress test (medial) showed positive results in 24 patients. The mean value of Rolimeter measurement was 8.7 mm, (ranged, 7.5 to 11.5 mm). The mean International Knee Documentation Committee (IKDC) score was 70.0 +/- 7.5 and the Lysholm score was 68.0 +/- 6.3. The duration from injury to surgery was 1 week to 12 months,with an average of 3.1 months.
RESULTSThere were 32 patients treated with anteromedial bundle reconstruction and 25 patients treated with poterolateral bundle reconstruction. Fifty-seven patients were followed up, and the mean follow-up time was 22.5 months (ranged, 13 to 37 months). The anterior drawer test and Lachman test were all negative at 1 month post-operation. At the end of the follow-up, the range of motion of all the knees was 120 to 130 degree. There were 54 patients with negative results of anterior drawer test and Lachman test, 1 with weakly positive result of anterior drawer test and 2 with weakly positive results of Lachman test. The measurements of Rolimeter showed excellent stability of the knee. The mean IKDC score was 92.0 +/- 4.9 and the mean Lysholm score was 91.0 +/- 3.7, which all improved compared to preoperative scores.
CONCLUSIONArthroscopic reconstruction of anterior cruciate ligament with preservation of the remnant bundle has satisfactory effects on the knee stability, which could increase the angiogenesis and collagen of the ACL and improve the proprioception of the knee.

10.The influences examination and analysis of blink reflex in normal volunteer.

OBJECTIVE:
To study the influence factors on latent values of blink reflex (BR) in normal volunteer.
METHOD:
BRs of 60 normal volunteers were detected bilaterally with Medtronic Keypoint EMG.
RESULT:
The normal values of BRs latencies in our research was R1 (10.27 +/- 0.91) ms, R2 (28.79 +/- 4.08) ms and R2' (29.06 +/- 4.35) ms, respectively. There was no statistical difference between left ear and right ear, as far as the latencies of R1, R2 and R2'were concerned respectively (P > 0.05). The latency of R1 in male was more prolonged than female (P < 0.05), as well as the R2 and R2 in male prolonged significantly (P < 0.01). Furthermore, the latency of R1 has a positive relation with age (P < 0.05) in male group, while the latencies of R2 and R2 have no relation with age (P > 0.05). Latencies of BR in female have no relation with age (P > 0.05).
CONCLUSION
To some extent, age and gender have some influence on the latencies of BR. It is important to establish normal values about BR in different age or gender of people.
