BACKGROUND: Superior migration of humeral head has been conventionally determined by measuring the acromiohumeral distance (AHD), We sought to devise a novel measurement system more reliably and accurately than AHD. We described a structural landmark called 'C-line'. In this study, we investigated the clinical usefulness of 'step-off of the C-line (SOC)' compared to that of AHD. METHODS: The C-line formed from the medial margin of the proximal humeral head continuing up to the inferior margin of the articular glenoid and then to the lateral border of the scapula. The superior migration of the humeral head triggered by a rotator cuff tear introduces a discontinuity in this C-line. We measured the distance of this discontinuity. We enrolled 144 patients who underwent a rotator cuff repair. We selected 58 controls who didn't have any cuff lesions apparent on magnetic resonance imaging. Using radiographs derived from standardized true anteroposterior views of the shoulder, we measured the SOC and the AHD. We used t-tests for statistical analyses. RESULTS: A rotator cuff tear was associated with an increase in SOC and a decrease in AHD. In control group, the mean SOC was 1.29 ± 1.71 mm and AHD was 9.71 ± 2.65 mm. In cuff tear group, the mean SOC was 3.15 ± 3.41 mm and AHD was 8.28 ± 1.76 mm. The mean SOCs of the patient group in relation to the mean SOC of the control group according to tear size, the SOCs of medium tear and lager groups showed statistically significant increase (p<0.05). CONCLUSIONS: The SOC may be a similarly effective to diagnose cuff tears of medium size and larger compared with AHD.

Clinics in Shoulder and Elbow 2015;18(4):197-205
BACKGROUND: The purpose of this study was to evaluate the postoperative magnetic resonance imaging (MRI) results of minimal-tying (one medial-row tie among 4 medial-row sutures) on the medial-row in double-row suture-bridge configuration (2x2 anchor with 4x4 suture stands). METHODS: From 2011 March to 2012 July, 79 patients underwent arthroscopic rotator cuff repair using 2x2 anchor double-row configuration. The mean age was 61.3 years (range, 31-81 years). Two double-loaded suture anchors were used for medial-row. Four medialrow stitches were made with only one medial-row knot-tying (the most anterior suture). Lateral-row was secured using the conventional suture-bridge anchor technique; all 4 strands were used for each anchor. Repair integrity was evaluated with MRI at mean 6.2 months postoperatively. Retear and the pattern of retear, change of fatty infiltration, and muscle atrophy of supraspinatus were evaluated using pre- and postoperative MRI. RESULTS: Repaired tendon integrity was 38 for type I, 30 for type II, 6 for type III, 4 for type IV, and 1 for type V, according to Sugaya classification. Considering type IV/V as retear, the rate was 6.3% (5 out of 79 patients). Medial cuff failure was observed in 4 patients. Fatty atrophy of supraspinatus was significantly improved postoperatively according to Goutallier grading (p=0.01). The level of muscle atrophy of supraspinatus was not changed significantly after surgery. CONCLUSIONS: Minimal tying technique with suture configuration of four-by-four strand double-row suture-bridge yielded a lower retear rate (6.3%) in medium to large rotator cuff tears.

Our Study was performed to evaluate the effect of anthralin for the treatment of psoriasis. Thirty seven patients with mild form of psoriasis were trested with topical anthralin ointment. 1. Twenty patients(54%) showed clearing of lesion within 8 weeks. 2. Treatment with anthalin was failed in 15 patients(40.5%). 3. Side reaction such as staining, pruritus and erythems was noted in 18 patients (48.6%), among them, 8 patients(21.6%) terminated the therapy with anthralin due to marked side reaction.

A clinical investigation of the patients with pustulosis palmaris et plantaris (PPP) was made on 36 patients who were seen at the Department of Dermatology, Seoul National University Hospital, from January 1986 through June 1987. The results were summarized as follows : 1. In this series of 36 patients, 14 patients(38.9%) were male and 22 patients (61.1%) were female. Females are more subject to the affection than males. 2. The mean age of onset in male was 30.6 years and in female it was 38.4 years, indicating a earlier onset in male. The peak age of incidence in male was between 20 and 29 years and in female, between 40 and 49 years. 3. The sites of involvement were mostly bilateral hands and feet. 4. the season of the year did not influence the severity of the skin lesions in 68.9%. 5. Nine patients(28.1%) had psoriasis-like skin lesions and 14 patients(43.1%) had nail changes out of 32 patients with PPP. 6. No direct relationship could be found between pustular eruptions and focal infections, such as tonsilitis, otitis, dental abscess and sinusitis. 7. Remmission of the pustular eruptions could be achieved in 5 out of 10 patients treated with etretinate and in 4 out of 11 patients treated with ketoconazole for more than 4 weeks' treatment

9.Open, Active-Controlled Clinical Study to Evaluate the Correlation between Whole Body DEXA and BIA Muscle Measurements
Yong-Chan HA ; Shinjune KIM ; Jun-Il YOO
Journal of Bone Metabolism 2024;31(3):219-227
Background:
Dual energy X-ray absorptiometry (DXA) is the gold standard for diagnosing sarcopenia. However, comparative studies using bioelectrical impedance analysis (BIA) would be required in the Korean population. This study aimed to evaluate the correlation between total-body bone density measuring devices (Hologic and GE Lunar) and a bioelectrical impedance measurement device (InBody 970) as well as the correlation between upper body muscle mass.
Methods:
A total of 119 participants were involved in this study, aged 20 to 70 years, with specific body mass index ranges and no severe health conditions used both DXA (or DEXA) and BIA technologies to assess body composition. The participants were scanned using a Hologic QDR-4500W DXA scanner and GE-Lunar Prodigy DXA systems, and the InBody 970 type of multi-frequency BIA machine. Statistical analysis was performed to determine the correlation between the devices, with a coefficient of at least 0.8.
Results:
The muscle mass measurement comparisons between the InBody 970 and Hologic devices demonstrated remarkably high correlation coefficients (exceeding 0.9) across all limbs. Similarly, the muscle mass comparison between the Inbody 970 and GE Lunar devices also revealed substantial correlation coefficients, ranging from 0.83 upwards, across all limbs.
Conclusions
Limb muscle mass measurements using Hologic and GE Lunar whole-body DXA and Inbody 970 BIA demonstrated particularly high levels of concordance. In addition, a conversion formula that bridges limb muscle mass measurements from two widely used whole-body DXA machines and a BIA machine will facilitate sarcopenia research and patient management.
