OBJECTIVETo evaluate the clinical outcomes and complications of Triple-Endobutton plates in treating Tossy type III acromioclavicular joint dislocation.

METHODSFrom January 2011 to January 2013,45 patients with Tossy type III acromioclavicular joint dislocation were treated with Triple-Endobutton plates. There were 35 males and 10 females with an average age of 30.5 (ranged from 19 to 60) years old. At the final follow-up, VAS, DASH, Constant-Murley criterion were used to evaluate shoulder function.

RESULTSAll patients were followed up from 15 to 36 months. No neurovascular injury, wound infection and stress fractures were found,but 3 patients had a re-dislocation. At the final follow-up,the mean VAS score was decreased from (5.7±1.6) preoperatively to postoperative (0.2±0.1); DASH score was significantly decreased from (19.6±4.3) preoperatively to (0.3±0.1) postoperatively; Constant-Murley score was improved from (34.4±4.3) before operation to (94.8± 3.5) after operation.

CONCLUSIONClinical outcomes of treating Tossy type III acromioclavicular joint dislocation with Triple-Endobutton plates is satisfactory. However, re-dislocation is still the most common complication. Careful perioperative management is an important factor in preventing re-dislocation.

The main problems and solutions of mineral drugs in clinical use were studied based on the investigation on the quality of Haematitum and Fluoritum pieces in venalicium. The outward appearance and intrinsic quality of Haematitum and Fluoritum pieces in venalicium were studied by the requires which were in the first part of Chinese Pharmacopoeia published in 2010. The outward appearance and intrinsic quality of mineral drugs had large differences, the disqualification rate was 41.67% in 12 batches of Haematitum pieces and the disqualification rate was 53.85% in 13 batches of Fluoritum pieces. The crushing granularity of mineral drugs should be defined, the quality standards should be further improved. The drug adiministration ought to strengthen inspection and supervision in order to ensure the stability and reliability of the clinical efficacy.

Objective To evaluate the treatment methods for subtrochanteric femur fractures in children and adolescents.Methods Sixty-two patients (at age of 4-16 years) with subtrochanteric fe-mur fractures were managed with traction and delayed hip spica (14 patients, set as nonoperative group) ,titanium elastic nails or pin combined with cast (38 patients, set as less invasive group) and plates (10 patients, set as traditional operative group).The radiologic outcomes, hip joint function and complica-tions of the patients were analyzed.Results According to Beaty scoring, five patients in nonoperative group and 44 in operative group obtained satisfactory early radiologic outcomes.Late radiologic outcome was satisfactory in nine patients from nonoperative group and 46 from operative group according to Theolo-gis scoring, with statistical differences between two groups (P<0.05).Sanders scoring showed excellent results in 11 patients and good in three from nonoperative group, and excellent results in 43 and good in five from operative group, with no statistical differences between two groups (P > 0.05).Less invasive group and traditional operative group showed significantly statistical different in aspects of complications and Sanders scores (P < 0.05).Conclusions Operative treatment provides more satisfactory early and late radiologic results than nonoperative methods.Less invasive operations have better outcomes than tra-ditional operative methods.Selection of treatment modalities based on age, body weight, fracture features and soft tissue conditions of children is the prerequisite for a good outcome.

Objective To examine the short-and middle-term efficacy and refracture rate in patients with thoracolumbar fracture having percutaneous kyphoplasty (PKP) and determine their correlation with the severity of osteoporosis.Methods A retrospective analysis was conducted on 247 patients with thoracolumbar fracture undergone PKP from January 2008 to February 2014.Mean age of the patients (94 males and 153 females) was 63.2 years (range,50-81 years).Ground-level falls occurred in 119 patients,traffic injuries in 72 patients and high falls in 56 patients.The patients were assigned to osteopenia group and osteoporosis group based on the preoperative bone mineral density.Visual analogue score (VAS),Roland-Morris disability survey scale (RDQ),vertebral compression rate,kyphotic Cobb angle and refracture rate were recorded before and after operation.Efficacy in correlation with the severity of osteoporosis was evaluated.Results Mean duration of follow-up was 24.3 months (range,18-36 months).Three days after operation,VAS descended from (7.7 ± 2.1) points to (4.3 ± 1.1) points in osteopenia group and from (7.7 ± 2.0) points to (4.2 ± 1.5) points in osteoporosis group (P ＜ 0.05);RDQ descended from (21.2 ± 1.5) points to (10.4 ± 2.5) points in osteopenia group and from (20.8 ± 1.9) points to (11.2 ± 1.7) points in osteoporosis group (P ＜ 0.05);rate of vertebral body compression descended from (42.2 ± 10.5) ％ to (3.2 ± 0.5) ％ in osteopenia group and from (46.2 ± 12.7) ％ to (2.9 ± 0.8) ％ in osteoporosis group (P ＜ 0.05);kyphotic Cobb angle descended from (21.2 ± 9.5) ° to (7.2 ±2.4) °in osteopenia group and from (23.1 ± 8.9) ° to (7.1 ± 2.6) ° in osteoporosis group (P ＜ 0.05).Three years after operation,all the parameters remained better than these before operation,with the value higher in osteoporosis group than in osteopenia group (P ＜ 0.05).Rate of refracture was higher in osteoporosis group after operation,compared to that in osteopenia group (P ＜ 0.05).Conclusions The clinical short-term result is satisfactory,but refracture rate is increased with the osteoporosis aggravation.Targeting osteoporosis therapy is important to obtain a better mid-term results.

OBJECTIVE: To investigate the level of H2-Ab1 in the nasal mucosa of mice with allergic rhinitis. METHOD: Twenty-four female 129/sv mice were divided into 2 groups: ovalbumin (OVA) group and control. The allergic rhinitis models were induced by classical method with OVA. After the last challenge, the pathological differences between the two groups were surveyed. The levels of H2-AB1 were measured by ELISA and quantitative real time PCR. RESULT: The expression of H2-Ab1 is higher in subjects with AR than that in controls (P < 0.05). CONCLUSION The levels of H2-Ab1 were highly increased in allergic rhinitis group, which might be associated with the pathogenesis of allergic rhinitis.

Objective To investigate the effects of extracellular ATP on the recovery of motor function after spinal cord injury in rats.Methods Twenty healthy adult rats were randomly divided into two groups after contusion injury was performed by the weight drop method at T10 with Allen impactor.Group A(ATP group) and group B(control group) each had ten rats.At days 1,3,7,14 and 28 after injury,the recovery of motor function after spinal cord injury in rats was assessed with modified Tarlov scores and slanting board test.Results Modified Tarlov scores and slanting board test were better in group A than in group B after spinal cord injury in rats.Significant difference was found in modified Tarlov scores and slanting board test between the two groups at days 14 and 28 after injury(P

Background Orbital fracture in children has its own characteristics.Choosing the suitable plastic materials is the effective measurement to improve the successful rate of surgery.Absorbable plate filling provides a new material for children with orbital fracture.However,few studies compare the effectivity and safety between absorbable plate filling and conventional filling.Objective The aim of this study was to compare the longterm efficacy and safety between absorbable plate and Medpor implants in the repair of pediatric orbital fracture.Methods Clinical data of 72 eyes of 72 pediatric patients with orbital fracture who received surgicals reconstruction in Beijing Tongren Eye Center from 2008 April to 2014 August were respectively analyzed,including 28 patients with absorbable plates filling and 44 patients with Medpor implants.Demography and preoperative clinical signs were matched between the two groups.Operative procedure was same except the implanted materials.The functional training of extraocular muscle was performed since the second day after surgery in both groups.The imaging findings of the orbit were evaluated by computed tomography (CT) before and after surgery.The patients were followed-up for avarage 12.7 months after operation.The therapeutic effectiveness and safety were compared between the different implants,such as incidence of enophthalmos,extraocular movement impairment and diplopia,and the sensory deficit of skin and complications were documented.Results Extraocular muscle movement improved and diplopia alleviated or disappeared in 92.9％ (26/28) in the absorbable plate group and 90.9％ (40/44) in the Medporgroup,without significant difference between the two groups (P=0.57).The sensory deficit of skin was found in 13 eyes (46.4％) and 20 eyes (45.5％ in the absorbable plate group and Medpor group,respectively,and there was no significant difference between the two groups (P =0.56).CT revealed that the anatomical reconstruction of orbits could be achieved in all patients in both groups without the shifting of implants and extrusion.No enophthalmos and postoperative infection were found in absorbable plate group,however,the residual enophthalmos or late infection was found in I eye for each in the Medpor group.Conclusions Both the absorbable plate and Medpor implants show good results for reconstruction of pediatric orbital fracture.Absorbable plate is an ideal material for pediatric orbital blowout fracture because of fewer postoperative complications.
