Objective To explore the risk factors and treatment of crush syndrome comlicated in earth-quake injury. Method The demographics and epidemiological characteristics, clinical treatment and the outcome of 35 cases with crush syndrome were reviewed and analyzed. The 35 patients were among 325 victims of "5·12" Wenchuan earthquake, who were admitted in Sichuan Provincial People's Hospital, Chengdu. Results Of 35 pa-tients with crush syndrome, 7 cases injured at forearm, 25 cases injured at cruse, and 3 cases injured at palm, and 3 cases were complicated with acute renal failure. The time from the occurrence of injury to transporting patients into the hospital was 6~92 hours, with mean time 49.3 hours. All the 35 patients had their wound cut open for de-compression,and 6 cases were cured, 2 had dysfunction, and 27 underwent amputation. Young patients under 14 years old had higher rate (25.8%) of crush syndrome than elder patients of 14~59 years old with rate of 8.0%.And patients admitted into hospital later (over 48 hour after injury) had higher incidence of crush syndrome (31.0 %) than those admitted earlier within 48 hours after injury (3.5 %). Conchusions Attaching great impor-tance to crush syndrome after earthquake injury, early diagnosis and treatment, and timely and complete surgical decompression are key points to the successful treatment and prevention of crush syndrome complicated in earth-quake injury. Because the longer interval between injury and rescue, delayed diagnosis and treatment, and the younger age of patients are all the risk factors of crush syndrome.

ERCP has become an important method for diagnosis and treatment of digestive diseases, but the high incidences of post-ERCP complications(PEP),especially for post-ERCP pancreatitis, have limited its application.Prevention of PEP has become a focus of ERCP-related studies.Current drug prevention of PEP is not satisfactory, and there has not been a single drug which can effectively prevent PEP.In contrast,non-pharmacologic preventive techniques such as pancreatic stent placing and guide wire cannulation have been proven to have prominent effects in preventing PEP,which brings new hopes for PEP prevention. This paper reviews the studies on non-pharmacologic prevention of PEP.

Objective: It is difficult to repair large-area Ⅲ?burn wound in children. Here we introduce the advantages of the back skin donor site in repairing Ⅲ?burn wound in children. Methods: This clinical study included 15 children with Ⅲ?burn, ranging in age from 18 months to 11 years(mean 7.6 years),with the Ⅲ?burn wound area averaging at 12.6% of the total body surface. The back skin donor site was used to repair the wound. The functional site, such as the joint, was preferentially considered if the skin graft was not enough for all the wounds. Results: Most of the skin grafts survived and the donor sites on the back healed very well. The patients were followed up for six months to three years, which revealed that the skin grafts remained soft without dysfunction and the scar hyperplasia was mild on the donor site. Conclusion: The back donor site supplies larger-area skin with good quality. The donor site heals fast. After healing, patients could sleep on back and compress the scar so as to prevent scar hyperplasia, and the scar does not affect the patient's appearance. With all these advantages, the back donor site is a good choice for repairing Ⅲ?burn wound in children.

Progress in Biochemistry and Biophysics 2006;0(05):-
It is known that extracorporeal shock wave (SW) may promote healing of fracture. A previous study reported that SW promoted human bone marrow stromal cells (hMSCs) towards osteoblasts in vitro. To study the osteogenesis ability of hMSCs treated by shock wave in porous hydroxyapatite (HA) in vivo, primary hMSCs of SW group and control group were cultured in the porous HA for 2 weeks and then implanted into subcutaneous sites of nude mouse. These implants were harvested and prepared for the biochemical analysis of alkaline phosphatase activity by AKP kit, histological analysis of decalcified and undecalcified sections and morphology by scan electric microscope (SEM), as well as osteocalcin mRNA expression by RT- PCR 4 weeks and 8 weeks after implantation. It showed that cells of SW and control group almost covered the rough surface of HA before implantation and the extracelluar matrix of SW group was abundant by SEM photomicrograph . The histological analysis and SEM photomicrograph showed active bone formation 4 weeks and 8 weeks after implantation, as well as tetracycline labeling under fluoroscopy analysis in SW group. Alkaline phosphatase in supernatants of the implants detected 4 weeks and 8 weeks after implantation in SW group was higher than in control group (P

Chinese Journal of Tissue Engineering Research 2005;9(6):214-216
BACKGROUND: Much attention has been focused on mandibular condyle fracture, which has long been considered as a severe injury to the temporomandibular joint(TMJ). Restricted by diagnostic methods, many studies have been conducted on transverse fracture whereas little attention has been paid to longitudinal fracture of the mandibular condyle. Even less is known about longitudinal fracture in young children.OBJECTIVE: To evaluate the effects of childhood longitudinal fracture of the mandibular condyle on secondary ankylosis of TMJ.DESIGN: A randomized controlled study.SETTING: Department of Stomatology, General Hospital of Chinese PLA.MATERIALS: The experiment was completed in the Center for Experimental Animals, General Hospital of Chinese PLA. Twelve young Chinese experimental miniature pigs bred by the Institute of Experimental Animals, Beijing Agriculture University, aged 2 -3 months and weighing(5 -5.5) kg, were used and raised with mixed feed, and then divided into 3 groups randomly.METHODS: Food and water was forbidden for the miniature pigs 12 hours before operation. Under anesthesia with(5.0 - 15.0) mg/kg ketamine/xylazine abdominally injected, conventional disinfection was performed in the right lateral decubitus position and layer-by-layer preauricular incisions were made on the left side of the miniature pigs. The joint cyst was incised transversely and the lower cavity of TMJ was exposed and dragged downwards. The mandibular condyle was cleaved into two vertically along the inner 1/3 part of it with a 5.0 mm-wide bone chisel to cause inferior and posterior sagittal fractures. The miniature pigs were executed 3 months and 6 months after operation, and their condyles were cut for observation.MAIN OUTCOME MEASURES:①The morphological changes of mandibular condyle specimens in the three groups observed with the naked eyes.②The pathological changes of mandibular condyle specimens under light microscope.RESULTS: The articular disc was adhered to the mandibular condyle after 3months. Temporal fossa adhered to the articular disc and mandibular condyle could still be observed in one specimen. Bifid condylar deformity was found in two specimens and no disc perforation was found. The adhesions became more severe after 6 months and bifid condylar deformity was formed in all the 3 specimens, with distinctly thickened discs. Light microscope observation:Three months after fracture, the adhesion between disc and condyle was obvious. There was no clear boundary between the surrounding tissues. The adhesion was composed of fibro-connective tissues, with a large number of chondrocytes in it. Fibro-connective tissues tightly adhered to the trabeculae were found in some adhesions. The fiber of disc became deranged and blood vessels and fatty cells were seen. Six months after fracture, blood vessels, a large number of fibroblasts and sparse chondrocytes were seen in the adhesions. The collagen fibers of the discs were arranged in whirlpool shape. There were a large number of fatty cells and blood vessels within it. The fiber-cartilage of the condylar surface was found thickened and the fiber deranged in the unadhered tissues. Proliferative changes were seen on disc surfaces.CONCLUSION: Childhood longitudinal fracture of the mandibular condyle causes severe secondary injury to TMJ, suggesting a close correlation between longitudinal fracture of the mandibular condyle and TMJ ankylosis.
