1.The efficacy and safety of Prostant~(TM) in the treatment of chronic prostatitis:a multi center, randomized, double blinded, placebo controlled trial

Objective To evaluate the efficacy and safety of Prostant TM in the treatment of chronic prostatitis. Methods A multi center, randomized, double blinded, placebo controlled trial was carried out during March 2001 and May 2001. 72 cases of patients who had been diagnosed as chronic prostatitis were separated into tow groups: 36 cases in the trial group treated with Prostant TM and the other 36 cases in the controlled group using placebo. The efficacy was evaluated by the WBC in EPS and NIH Chronic Prostatitis Symptom Index after a one month follow up. The efficacy of the therapy was divided into four levels:cure,the symptom index score being reduced over 90%, and the number of WBC in the EPS less than 10/HP; effective, the symptom index score being reduced 60%~89% and the number of WBC in the EPS lowered over 50% or less than 15/HP;improved,the symptom index score being reduced 30%~59% and the number of WBC in the EPS lowered over 25%; no effect, the symptom index score being reduced less than 30% or the number of WBC in the EPS lowered no more than 25%. Results All but two cases had completed the follow up.One case (2.8%) was completely cured,remarkable effective in 7 cases (20.0%) of the trial group;and improred in 16 cases (45.7%).Only 2 cases (5.7%)seemed effective and 8 cases (22.8%) improved in the controlled group. 2.Preliminary clinical application of SilverHawk directional atherectomy device in arteriosclerosis obliterans of lower extremity

Objective To evaluate the clinical efficiency and seeurity of SilverHawk directional atherectomy device in treating arteriosclerosis obliterans of lower extremity.Methods Seyen patients with lower extremity ischemia were treated with SilverHawk directional atherectomy.The lesions length varied from 1.8 to 14.5 cm.Clinical symptoms(Fontaine classification)included 4 cases of grade Ⅱ b,1 case of grade Ⅲ,2 cases of grade Ⅳ.The ABI varied from 0.28 to 0.65.Patency was evaluated with color duplex sonography or CTA besides clinical examination during follow-up.Results Seven occlusive lesions were recanalizated successfully.The technical success(residual stenosis<30%)rate was 100%. Clinical symptoms were obviously improved or disappeared.The ABI varied from 0.82 to 1.18(P=0.002),and Fontaine classification included 6 cases of Ⅰ:1 case of Ⅱ a.Patency rate was 100%and the ABI varied from 0.75 to 1.14(P=0.938).Fontaine classifications keep consistent l-6 nonths after operation.Conclusion SilverHawk drectional atherectomy Was a safe and effective way for treatment of lower arteriosclerosis obliteran.
3.The clinical manifestations of olivopontocerebellar atrophy and the diagnostic value of MR

Objective To investigate the clinical manifestations and MR imaging features of olivopontocerebellar atrophy(OPCA).Methods The clinical data and MR findings of 8 patients with OPCA (OPCA group) were retrospectively analyzed,and 75 cases of healthy physical examination people were chosen as control group (they were divided into 21-30 years group,31-40 years group,41-50 years group,51-60 years group,61-70 years group,and 15 cases in each group),all subjects under went pons measurement.Results All 8 patients with OPCA were sporadic cases.The cerebellar symptom,autonomic symptoms and extrapyramidal symptoms were common manifestations.The head MR imaging of OPCA presented with cerebellum and brain stem atrophy,cross sign was the characteristic appearance.Vertical diameter and anteroposterior diameter of the pons in OPCA group patients was (20.63 ± 2.16) and (16.74 ±4.37) mm,compared with control group [(27.14 ±1.73) and (22.28 ± 1.49) mm],there was significant difference (P < 0.01).In control group,vertical diameter and anteroposterior diameter of the pons maximum in 21-30 years group and 31-40 years group,increased with age,the value had been reduced,but there was no significant difference (P > 0.05).Conclusion MR is the first choice for the diagnosis of OPCA,combine with clinical manifestations may improve the diagnostic accuracy.
4.Design and Application of the Database for Hospital Drug Electronic-Directions

5.Study on the Ex Vivo Adsorption of Montmorillonite Powder to Ofloxacin

OBJECTIVE:To study the ex vivo adsorption of montmorillonite powder to ofloxacin.METHODS:Different dosages of ofloxacin and montmorillonite powder were mixed with artificial gastric juice and artificial intestinal juice,respec?tively,which were filtrated after warmed at(37?0.5)℃in water bath for1hour,the content changes of ofloxacin were de?termined by ultraviolet spectrometry.RESULTS:The adsorption rates of montmorillonite powder to ofloxacin in artificial gastric juice and in artificial intestinal juice were(99.76?0.01)%and(99.55?0.02)%,respectively.CONCLUSIONS:Montmorillonite powder has strong adsorption to ofloxacin in both artificial gastric juice and artificial intestinal juice,there?fore,which should not be administered simultaneously in the clinic.

7.Monitoring Rational Clinical Use of Drugs in Hospital Using Local Area Network

OBJECTIVE: To monitor clinical rational use of drugs,analyze the problems involved and to put forward reasonable suggestions.METHODS: Using the platform of medical information communication established based on the "Military Medical Project No.1" local area network and "Hospital Information Management System",the electronic medical records of the inpatients,ward round records,treatment plans and the relative laboratory test reports from October 2006 to March 2007 were retrieved online.The involved problems were discussed with clinical physicians through the network.RESULTS: The problems in the clinical drug use manifested as repeated medication,improper administration,overdoses and adverse interactions of combined drugs.The irrational prophylactic administration of antibiotics in the surgical department was characterized by improper choice of drugs and prolonged course of treatment etc.CONCLUSION: Great importance should be attached to drug interactions,and the indications for drug combination and course of treatment should be kept under tightly control so as to reduce adverse reactions and delay the onset of drug resistance.
8.Effects of Leukocyte-and Platelet-rich Fibrin on Tendon-bone Healing of Rabbits' Extra-articular Bone Tunnel in the Early Period

Objective To evaluate the early effect of leukocyte-and platelet-rich fibrin (L-PRF)on tendon-bone healing in rabbits' extra-articular bone tunnel.Methods Thirty healthy New Zealand rabbits were randomly divided into an experimental group and a control group,each of 15.The semitendinosus tendon harvested from the hind leg of each rabbit was prepared as free autografts.Each autograft was implanted into the bone tunnel created at ipsilateral proximal tibial metaphysis.The experimental group was added with autologous peripheral blood-derived L-PRF,while the control group did not add anything.Speciments from each group were harvested at 4,8 and 12 weeks after operation.The morphological changes were observed and evaluated at each time point.The fibroblasts on the tendon-bone interface were counted and analyzed under the high magnification microscope.Results The number of fibroblasts between the tendon and bone in the experimental group was significantly higher than that of the control group at each time point (P<0.05).And the connection between tendon and bone was closer and the collagen fibers were more regular in the experimental group than the control group at each time point.Sharpey's-like fibers which marked the early healing of the tendon to bone,were observed at 4th week in the experimental group,while they were observed at 8th week in the control group.As the healing time extended,Sharpey's like fibers continue to increase in both groups and significant differences were observed in the histological morphology (Buark grades)between the two groups (P<0.05).Conclusion Autologous peripheral blood-derived L-PRF can promote early healing of autologous tendon to bone in the extra-articular bone tunnel of model rabbit.
9.Study on the ?-lactamases in eight pan-drug-resistant citrobacter freundi isolates

Objective Study the production of the?-lactamases in eight pan-resistant citrobact- er freandi and their epidemic characteristics.Methods Modified Hodge test and EDTA-disk syner- gy test were used to screen Metallo-?-lactarnases,and isoelectric focusing to estimate the production of ESBLs and AmpC enzymes.ESBLs,AmpC,MBL genes were amplified and analyzed by PCR and DNA sequencing.Molecular epidemics were investigated by PFGE.Results All the 8 strains pro- duced TEM-1,CTX-M-14,4 of them at the same time producing CTX-M-3.No Plasmid mediated AmpC was found.Furthermore,a new subtype of MBL with one amino acid different from IMP-8 was de- tected.All the 8 strains belong to the same PFGE type.Conclusions Pan-drug-resistant citrobacter freundi produced more than one kind of?-lactamases.All these isolates were clonally related.
10.Liver histological changes in chronic hepatitis B patients with alanine aminotransferase lower than two times the upper limit of normal

Objective To assess the liver histopathological characteristics of chronic hepatitis B (CHB) patients with alanine aminotransferase(ALT)lower than 2 times the upper limit of normal (ULN) through liver biopsy, and try to provide subjective evidence for clinical anti-viral treatment.Methods From October 2005 to August 2010, patients accepted liver biopsy in department of infectious disease, Sichuan provincial people's hospital were enrolled. The criteria for liver biopsy was as follow, (1) HBsAg-positive for more than 6 months, (2) HBeAg-positive patients with HBV DNA ≥103 copies/ml or HBeAg-negative patients with HBV DNA≥ 104copies/ml, (3) ALT was lower than 2 times ULN for more than 6 months,and without any hepatic protectants, (4) never accepted any antiviral treatment before, including IFN or nucleoside analogues, (5) willing to accept liver biopsy. Before liver biopsy, routine blood test, prothrombin time, liver function test, hepatitis B antigen and antibody test, HBV DNA quantification were examined. The biopsy position was located under routine ultrasound, liver biopsy were performed to assess the grading of inflammation and necrosis and the degree of fibrosis. The correlation between all the factors and liver inflammation and fibrosis were analyzed. Results Totally 383 cases (240 males and 143 females) met the diagnostic criteria, aged from 16 to 59 years old and the mean age was 28.0 years old. Cases of liver inflammation in G0, G1, G2, G3andG4 grade was 2 cases (0.5%), 165 cases (43.1%), 191 cases (49.9%), 25 cases (6.5 % ) and 0 cases (0 % ) respectively, cases≥G2 grade accounted 56.4 % of total. Meanwhile,stage of fibrosis in S0, S1, S2, S3 and S4 was 103 cases (26.9%), 265 cases (69. 2%), 13 cases (3.4%), 2 cases (0.5%) and 0 cases (0%) respectively, percentage of liver fibrosis in S2stage and over was only 3.9%. The occurrence of serious liver inflammation was associated with age, ALT levels, HBV DNA levels and HBeAg status (P<0.05). There was no obvious association between HBV DNA level and liver fibrosis (P>0.05). Conclusions There were obvious liver inflammation and different degree of liver fibrosis in CHB patients with alanine aminotransferase(ALT)lower than 2 times ULN. The degree of liver injury assessed by liver biopsys is recommended as an evaluation for the necessary of anti-viral therapy.
