1.External Fixation Combined with Limited Internal Fixation in the Treatment of Unstable Distal Radius Fractures

Objective To investigate the efficacy of minimally invasive treatment by combination of external fixation with limited internal fixation for unstable distal radius fractures. Methods From September 2003 to September 2007,43 patients with unstable distal radius fractures were treated with external fixation combined with Kirschner wire fixation. According to AO classification,there were 10 cases of C1,21 cases of C2,and 12 cases of C3. Two external fixation clamp stand screws were inserted into the dorsal radius in operation,then external clamp stand was fixed when manipulative reduction were performed after C-arm perspective. The broken bones were sledged by Kirschner wire when articular facet and bone block were not peaceful and could not achieve reduction by traction. When the bone block was instable,a Kirschner wire fixation was needed. Bone graft through a limited incision were performed for bone defect if necessary. Results All of the 43 cases achieved anatomical reduction after the operation. The mean operaton time was 62.4 min (range from 43 to 121 min). And the mean intraoperative blood loss was 15.3 ml (ranged from 6 to 30 ml). The patients were discharged from hospital in 3 to 14 days with a mean of 5.5 days. After being discharged,they were followed up for 6 to 24 months (mean,12 months),during which,re-examination showed bone union and improved articular activities in all the cases. According to the Dienst scale system,30 patients achieved excellent,8 were good,and 5 were fair. The rate of excellent and good results was 88.4% (38/43). Conclusion External fixation combined with limited internal fixation is a simple,reliable and effective method for treatment of the unstable distal radius fractures.

OBJECTIVETo investigate the relationship between the level of di-2-ethylhexyl phthalate (DEHP) and idiopathic oligoasthenospermia by measuring the content of DEHP in the semen samples of different subjects.
METHODSWe obtained semen samples from 100 infertile men with idiopathic oligoasthenospermia, 50 working all the year round in the plastic greenhouse (group A) and the other 50 constantly dining from plastic meal boxes (group B). We also enrolled 50 normal male volunteers as controls (group C). We conducted semen analyses using a computer-assisted sperm analyzer, measured the DEHP concentration by reversed-phase high-pressure liquid chromatography, and subjected the data to statistic processing by t-test and correlation analysis.
RESULTSThe mean concentrations of DEHP in the seminal plasma were (0.72 +/- 0.48), (0.71 +/- 0.49) and (0.21 +/- 0.18) mg/L in groups A, B and C, respectively, significantly higher in A and B than in C (both P < 0.05). The DEHP concentration was negatively correlated with sperm motility (P < 0.05).
CONCLUSIONThe DEHP level in the seminal plasma is higher in infertile men frequently exposed to plastic products than in normal males and excessive DEHP may be one of the important factors of idiopathic male infertility.

4.Advances in epidemiology, etiology and pathogenesis of Kawasaki disease

Kawasaki disease (KD) is an acute systemic vasculitis that primarily affects young children between 6 months and 4 years old. Coronary arteritis is an important clinical feature of KD because it is associated with aneurysms and thromboembolic events that can lead to severe complications, even sudden death. To date, the etiology and pathogenesis of Kawasaki disease has not been understood completely. In this paper, we will review the recent advances in epidemiology, etiology, pathogenesis and genetic susceptibility of Kawasa-ki disease.
5.Prevalence and risk factors of sexual dysfunction in patients with Parkinson's disease

Objective To investigate the prevalence and related factors of sexual dysfunction (SD) in patients with Parkinson's disease.Methods A total of 411 patients with Parkinson' disease in Nanjing Brain Hospital were analyzed retrospectively.The ICD-10 diagnostic criteria for SD was used to evaluate SD.The Unified Parkinson's Disease Rating Scale (UPDRS) part Ⅲ, Hoehn and Yahr (H&Y) staging, Non-motor Symptoms Questionnaire (NMS-Quest), Parkinson's Disease Sleep Scale (PDSS), Hamilton Anxiety Rating Scale (HAMA), Hamilton Rating Scale for Depression (HRSD) and Montreal Cognitive Assessment (MoCA) were used to evaluate patients.Results SD was found in 145 (35.3 %) patients and the prevalence was higher in male than in female.Patients with SD had older age, longer duration, severer motor symptoms, and higher scores of UPDRS-Ⅲ, HAMD, HAMA and NMS-Quest (all P<0.05).In male patients, SD was positively associated with age, duration of disease, UPDRS-Ⅲ , H-Y stage, HAMD, HAMA and NMS (r=0.127, 0.303, 0.240, 0.236, 0.181, 0.221 and 0.302, all P<0.05).In female patients, SD was positively associated with duration of disease, HAMD, HAMA and NMS (r=0.194, 0.163, 0.189 and 0.178, all P<0.05).The forward binary Logistic regression analysis indicated that female gender (OR=0.539, P<0.05), duration of disease (OR=1.133, P<0.05), NMS (OR=1.104, P<0.05) and history of exposure to heavy metals (OR=5.268, P<0.05) were associated with SD.Conclusions SD is a common complication in patients with Parkinson's disease, and a great attention should be paid to its screening and diagnosis.

Objective To investigate the single nucleotide polymorphisms ( SNP) of IL-13G+2044A and IL-13A-1512C and to analyze their relationships with the occurrence of bronchial asthma in chil-dren in Guiyang city. Methods Sequenom MassArray platform, a newly developed genotyping assay, was used to detect the G+2044A locus and the A-1512C locus in 153 subjects with asthma and 103 healthy sub-jects. Then the results were statistically analyzed with chi-square test and t-test. Results Three genotypes of IL-13G+2044A locus including GG, GA and AA were detected in the subjects with or without asthma. There were significant differences in the genotype distribution between the subjects with or without asthma (χ2 =7. 691, P<0. 05). The subjects carrying the variant allele A at IL-13G+2044A locus were more likely to have asthma than those not harboring the variant allele A. There were significant differences in allele fre-quency between the subjects with or without asthma (χ2 = 7. 458, P<0. 01). Three genotypes of IL-13A-1512C locus including AA, AC and CC were detected in the subjects from both of the two groups. There were significant differences in the genotype distribution between the two groups (χ2 = 12. 906, P<0. 01). The variant allele C at IL-13A-1512C locus was associated with lower risk for asthma. Significant differences in allele frequency were observed the two groups (χ2 =10. 407, P<0. 01). Conclusion Both of the alleles at IL-13G+2044A and IL-13A-1512C loci were associated with asthma. The variant allele A at IL-13G+2044A locus might be the gene predisposing children to asthma in Guiyang city. Children carrying the variant allele C at IL-13A-1512C locus showed a lower risk for asthma in Guiyang city.

Objective To investigate the correlation between the single nucleotide polymorphisms (SNPs) of IL-4 gene -590C/T as well as IL-13 gene -1112C/T and the incidence of asthma in children.To address whether the SNPs of two loci have any impact on total serum IgE( TIgE) levels.Methods Polymerase chain reaction-restriction fragment length polymorphism technique( PCR-RFLP) was used to determine the two locus polymorphisms of 250 patients with asthma and 200 healthy subjects in control group.The level of TIgE was tested by enzyme-linked immunosorbent assay(ELISA) in two groups.Results (1)The genotype distribu-tion in each locus was different in two groups(P<0.01).(2)In patient and control groups, the C allele fre-quency at IL-4-590 locus was 23.00%and 35.25%,the T allele frequency at the same locus was 77.00%and 64.75%, respectively.Besides, the C allele frequency at IL-13 -1112 locus was 59.20%and 70.50%,the T allele frequency at the same locus was 40.80%and 29.50%, respectively.The allele frequencies at each locus had the significant difference in asthma and control group(P<0.01).(3)The subjects with variant allele have higher risks for asthma than those without variant allele ( OR=1.82, 95%CI=1.36-2.44, and OR=1.647, 95%CI=1.246-2.178).(4)The TIgE level of asthma group was higher than that of control group in those with the same genotype (P<0.05).(5)No significant difference with the TIgE levels was observed in those with or without variant T allele at IL-13 -1112 locus(P>0.05), which was in contrast with that at IL-4 -590 locus in asthma group(P<0.05).Conclusion The SNPs of two loci were associated with childhood asthma.Variant al-lele T at 590C/T locus in IL-4 gene correlated with higher serum IgE levels.There was no significant correlation between the serum IgE levels and variant T allele at IL-13 -1112C/T locus.

Objective To investigate the effect on apoptosis and extracellular signal regulated kinase (ERK) expression of cholangiocarcinoma RBE cell line induced by paclitaxel and the role of extracellular signal regulated kinase in paclitaxel-induced apoptosis.
