2.Vitamin D status among neonates and its impact on lung maturation

Recent studies show that vitamin D plays a role in lung development and regulation of lung maturation in the fetus. The AAP and the Institute of Medicine( IOM) both define vitamin D insufficiency as ser-um 25-OH-D concentrations <50 nmol/L in the pediatric population. The surveys in our country show the prev-alence of vitamin D insufficiency is higher according to the criteria defined by AAP. Evidence from animal study suggests that vitamin D may increase alveolar count,alveolar septal thickness,lung volume and decrease airway resistance,immune cell aggregation leading to the maturation of lung. The cohort studies suggest low 25-OH-D level in infants at birth is associated with greater need for assisted ventilation,increased rate of ARDS,increased duration of oxygen requirement.
3.Association of 344-35C/T Polymorphism of Growth Arrest-Specific Gene 6 with Cerebral Infarction

Objective To investigate the correlations of the single nucleotide polymorphism (SNP) 344-35C/T of growth arrest-specificgene 6 (GAS6) gene and cerebral infarction in Han population in the western Guangdong province. Methods The restriction fragment lengthpolymorphism (PCR-RFLP) was used to determine the 344-35C/T polymorphism in the intron region of GAS6 gene in the case group (n=180) and healthy control group (n=150). Results There was no significant association between GAS6-344-35C/T and cerebral infarction (P<0.05). After stratified by gender in chi-square test in women population, the frequency of C allele was significantly higher in cases (77.9%)than that in controls (66.4%) (P=0.040); The frequency of the CC genotype was significantly higher in cases (61.8%) than that in controls(43.1%) (P=0.036). Conclusion In Han women in western Guangdong province, C allele of GAS6-344-35C/T polymorphism is a risk factorof cerebral infarction, CC genotype is a susceptible genotype of cerebral infarction.
4.Cost-effectiveness evaluation of the strategies for prenatal diagnosis of Down's syndrome

35) and the third strategy of providing diagnosis to those tested positive at screenings in mid-term pregnancy could, respectively, lead to the diagnosis of 0.93 and 1.47 cases of DS per 10 thousand pregnant women, but they could also induce, respectively, 1.58 and 0.28 cases of miscarriage of normal fetuses due to diagnosis procedures. The costs of one DS diagnosis were, respectively, 108890.98 yuan and 443683.11 yuan. Conclusion Taking into consideration both cost-effectiveness and safety factors, priority ought to be given to the third strategy.

Brain injury is one of the most serious diseases in neonatal period,which can cause cerebral palsy,motor development delay,cognitive dysfunction and learning difficulties and other sequelae,and severely affects the health development and quality of life of the newborn.Neonatal brain injury (NBI) is a wide range of diseases caused by a variety of causes,its clinical manifestations lack specificity,clinically,it is difficult to judge the severity,duration and the time of prenatal injury,and it has been paid much attention to by scientific researchers and clinicians.At present,imaging method is a major means of NBI diagnosis,but imaging examination is usually a lag and limitations.Levels of humoral biomarkers change early after brain injury,and early brain injury can be predicted by detecting their changes.In recent years,a variety of sensitive brain damage biomarkers have been detected in various body fluids of newborns,mainly including neuron-specific enolase (NSE),ubiquitin carboxyl hydrolase L1 (UCH L1),S100B protein,tau protein,myelin basic protein (MBP),glial fibrillary acidic protein (GFAP) and activin A and so on.the application and research progress of these commonly used biomarkers in NBI are reviewed in this paper.
6.Extracorporeal shock wave therapy, joint mobilization and short wave therapy in treating scapulohumeral periarthritis

Objective To compare the immediate effects of extracorporeal shock wave therapy with that of joint mobilization plus short wave therapy in treating scapulohumeral periarthritis, and to elucidate the mechanism of extracorporeal shock wave therapy and how it should be used. Methods Forty-seven patients diagnosed with scapulohumeral periarthritis were selected and randomized into two groups, a treatment group (n = 26) treated with extracorporeal shock wave therapy, and a control group ( n = 21 ) treated with joint mobilization plus short wave therapy. The short form of the McGill pain questionnaire (SF-MPQ) and range of motion (ROM) measurements were used to evaluate shoulder function. Results Compared with baseline, both groups improved significantly af-ter treatment, but the immediate effect in the treatment group was superior to that in the control group. Concln-sions As a safe, time-saving and effective therapy, extracorpnreal shock waves could be used to treat scapulo-humeral periarthritis.
7.Effect of propofol post-conditioning on expression of GluR1-containing AMPA receptors during cerebral ischemia-reperfusion injury in rats

Objective To evaluate the effect of propofol post-conditioning on the expression of GluR1-containing AMPA receptors during cerebral ischemia-reperfusion (I/R) injury in rats.Methods Sixtyfour healthy male Sprague-Dawley rats,aged 7-8 weeks,weighing 250-280 g,were randomly divided into 3 groups (n =18 each) using a random number table:sham operation group (group S),I/R group and propofol group (group P).Focal cerebral ischemia was induced by occlusion of the middle cerebral artery for 1 h followed by reperfusion in I/R and P groups.Propofol was infused at 20 mg · kg-1 · h-1 for 2 h starting from the onset of reperfusion in group P,and the equal volume of normal saline was given in S and I/R groups.Fear conditioning test was performed on 7,14 and 28 days after operation.The time spent freezing induced by context and condition was recorded,and the percentage was calculated.The modified neurological deficit score (NDS) was measured at 24 h of infusion.The cerebral infarct size was measured by TTC.The hippocampal samples were isolated for determination of the expression of protein kinase A (PKA),A-kinase anchoring protein 150 (AKAP150) and AMPA receptor GluR1 subunit and phosphorylation of GluR1 at serine 845 (Ser845) site (pGluR1 Ser845).Results Compared with group S,the NDS and cerebral infarct size were significantly increased,the expression of PKA and pGluR1 Ser845 was down-regulated,and the percentage of time spent freezing induced by context and condition was decreased on 7 and 14 days after operation in I/R and P groups.Compared with group I/R,the NDS and cerebral infarct size were significantly decreased,the expression of PKA and pGluR1 Ser845 was upregulated,and the percentage of time spent freezing induced by context and condition was increased on 7 and 14 days after operation in group P.Conclusion The mechanism by which propofol postconditioning reduces cerebral I/R injury is related to phosphorylation of AMPA receptor GluR1 subunit in rats.
8.Protective effect of p38 mitogen-activated protein kinase inhibitors on zebrafish larvae brain after hypoxia/ reoxygenation injury

Objective To examine the effects of p38 mitogen-activated protein kinase (MAPK) inhibitor on the behavioral response to zebrafish larvae after hypoxia/reoxygenation brain injury and to identify whether the protective effect is mediated by inhibiting apoptosis and protein,and mRNA related to apoptosis.Methods The 5-day post-fertilization zebrafish larvae were randomly assigned to 3 groups:control group,model group and intervention group.Fishes in the intervention group were separated into 3 subgroups according to p38 MAPK inhibitor concentration (5,10,20 μmol/L).The activity levels of the larvae were analyzed by using quantization mode of ZebraLab software,swimming distance and moving speed were recorded.Terminal transferase dUTP nick end labeling (TUNEL) assays of brain assays were performed.The protein levels of phosphorylation of p38 MAPK,apoptosis related proteins of B-cell lymphoma-2 (Bcl-2),Bcl-2 associated X protein(Bax) and Caspase-3 were determined by Western blot.The mRNA expressions of Bcl-2,Bax,and caspase-3 were also analyzed by reverse transcription-quantitative PCR (RT-qPCR).Results The activity movement analysis of the intervention group 2 and 3 demonstrated a significantly increase in the swimming distance compared with the model group(P < 0.05).After irradiation under strong light,all groups showed dramatically increasing in the moving speed.After removal of strong light,a significant decrease in moving speed was found in the control group and intervention group 2 and intervention group 3.The TUNEL assay showed that apoptosis index decreased in the intervention group (21.7 ±2.0,12.8 ± 1.9,17.7 ±2.6) compared with model group (46.8 ±5.3) (all P <0.01).Western blot assays demonstrated a significant increase protein level of phosphorylation of p38 MAPK after hypoxia and reoxygenation,and the inhibitor reduced the p-p38 MAPK expression.Compared with the model group,p38 MAPK inhibitor increased the protein and mRNA expression level of Bcl-2,whereas reduced the Bax and caspase-3 expression in the brain.Conclusions Under the influences of p38 MAPK inhibitor,zebrafish larvae improved the behavioral changes after hypoxia-induced brain injury.The inhibitor (10 μmol/L) optimally reduces hypoxia-induced apoptosis in brain by up-regulating Bcl-2,down-regulating Bax/caspase-3 protein and their mRNA level.
9.Optimize of the fiber optic epidural catheter and study of the epidural cavity surgical anatomical physiology

Objective To discuss the epidural cavity surgical anatomical physiology under fiber optic epidural catheter, and to analyze the technological advantages compared to the the agent under blind. Methods 60 cases who need to epidural abdominal surgery from Jan. to Nov. 2013 were collected. Common duct (30 cases) and fiber optical epidural catheter (30 cases) were respectively used at anesthesia, and the epidural cavity surgical anatomy physiology were observed. Results Compared with indexs before anesthesia, before induction and before intubation, the HR, SPB, and MAP in 5 min after intubation and at the moment of skin incision were of statistically significant differ-ences (P<0. 05). There is a cavity between epidural space and vertebral canal periosteum which is not linked to cranial cavity, but there were a certain amount of connective tissue and venous plexus. The volume of the cavity was about 100 mL, the front and side were narrow and the back was wide, and it began to gradually expand from the neck, and reached its peak at the waist, and then gradually narrowed, un-til the sacral hiatus. Conclusion Fiber optic epidural catheter could make the patient’ s anatomy physiology structure more clear, and it also make it possible to cathetering visually for the anesthesiologist.
10.Near Infrared Spectral Hybrid Model Quantitative Analysison Samples with Different Physical States

While the near infrared spectroscopy (NIRS) is used to measure the inhomogeneous samples with diffuse reflection mode, the accuracy and robustness of the calibration model is not quite good for the variation of spectrum scattering and absorption coefficient in those samples.Therefore, an establishment strategy of hybrid model based on homogeneous sample and calibration transfer method was proposed to solve the problem of detection inhomogeneous samples by NIRS.This work was focused on the tobacco leaf samples aspect.Three common calibration transfer methods, including Shenk′s patented algorithm (Shenk′s), piecewise direct standardization (PDS) and calibration transfer based on canonical correlation analysis (CTCCA), were used to construct two hybrid models of tobacco powder mixed with cut tobacco and tobacco powder mixed with tobacco flake samples to predict nicotine content in the samples of cut tobacco and tobacco flake.Experimental results showed that the hybrid model of adding some cut tobacco and tobacco flake samples to the powder model would get preferred prediction ability.Root mean square errors of cut tobacco and tobacco flake samples were reduced by 1.39% and 2.73%, respectively.This showed that the hybrid model was help for the improvement of the predicted results and the robustness of model.Moreover, CTCCA got the optimal performance between these three calibration transfer methods.Therefore, the scheme of building a hybrid model by NIRS homogeneous model and calibration transfer method to determinate the heterogeneous samples is feasible, which can accelerate the development of on-line near infrared spectroscopy technology and will provide reference for the share of near infrared spectral model.
