Objective To investigate the clinical significance of dynamic changes of CD4~+ CD25~+ regulatory T cells(Treg)in patients subject to allogeneic hematopoietic stem cell transplanta- tion(alIo-HSCT).Methods Forty-five patients received allo-HSCT.The graft-vesus-host disease (GVHD)was prevented by cyclosporine A and short-term MTX regimen in 31 patients.Fourteen of all the patients received Zenapox(CD25MAb)at the day of transplantation and day 4 after transplan- tation. The levels of Treg in peripheral blood were detected by flow cytometry from 45 patients at 2nd,4th,8th and 12th week after allo-HSCT and the time of aGVHD development,respectively.Re- suits Anti-CD25 could suppress the peripheral blood levels of Treg significantly. The Treg levels were significantly higher in patients with grade 0-1 aGVHD than those with 2-4 aGVHD at 8th and 12th week after transplantation.Among patients with 2-4 aGVHD,Treg levels were significantly low- er after development of aGVHD than before. Conclusions Treg are important for the aGVHD preven- tion and can be a useful clinical surveillant index for the development of aGVHD.It can significantly decrease the levels of Treg in the peripheral blood with anti-CD25.

Journal of the Korean Society of Emergency Medicine 1997;8(2):185-192
BACKGROUND: The Sampung Collapse took place at 17:55 June 29, 1995. The rescue operation encountered 458 dead and 477 multiply traumatized patients and 464 non traumatized peoples. We wanted to present basic data for disaster planning for example of Sampung Collapse. METHODS: We made a survey of accident field immediately after accident and interviewed with victims, victim's famaly, and rescuer. We analysed the medical record and SCL-9 (Hopkins symptom check list 90R) which was checked several list with victims, victim's family and control group. RESULT: The mean RTS was 11.9 and the mean ISS was 4.3.96 long bone fractures occurred in 76 patients. 127 case of vertebral injury occurred in 63 patients. Total number of surgical operations were 237. There are also many crushing injury and acute post-traumatic stress diseases. CONCLUSION: There are many injury types which are crushing injury, vertebral injury, blunt trauma and acute PTSD(post traumatic stress diseases). Thus, The field management, especially field triage, is very important. And, the urgent patients were 72 patients, about 5% of total patients, witch needed intensive care in tirtiary hospital. Therefore, we need to plan out the expeditious proper management. and prevention of disaster propagation.

Journal of Applied Clinical Pediatrics 1986;0(01):-
Objective To discuss the clinical diagnostic method of postural orthostatic tachycarda syndrome(POTS) in children. Methods Thirty-six children with POTS were selected for the research.Among them 15 were boys,21 cases were girls. The age ranged from 5.9 to 16 years, average age 12.3 years.Among them 28 patients(78%) were in between 11-16 years. The age distribution, clinical courses and hemodynamic indexes were analyzed and also the incidence of clinical manifestations and investigation reports were observed. Results Among 81 patients of orthostatic regulation disturbance,36 patients were diagnosed POTS,which was 47% of total. The clinical courses ranged from 1 day to 5 years, average clinical course 10.2 months. The clinical courses of more than half of the total patients were within 6 months(56%).The common clinical features of POTS were chest tightness on standing, vertigo, fatigue, palpitation, syncope, orthostatic regulation disturbance.Ten patients were also associated with gastrointestinal symptoms like nausea, vomiting. The most common feature of POTS patients was tachycardia(HR increased by ≥30 times/min) within 10 min after head-up tilt test(HUT).Average HR increased by 38 times/min.In some patients HR increased up to ≥120 times/min.There were no significant changes in blood. In 23 cases(64%),the T waves were descended by ≥0.2 mV in 2 or more than 2 leads in ECG reading.Investigations reports showed that there were 12 cases whose urine specific gravity was increased. In 11 cases HCO_3~-decreased.Conclusions POTS is commonly seen in schooling female children. The common symptoms are vertigo, chest tightness, fatigue, palpitation.HUT is an important method for the diagnosis of POTS.

To investigate the therapeutic effects and associated complications of allogeneic peripheral blood stem cell transplantation (allo-PBSCT). 40 patients with various malignant hematopoietic diseases received allo-PBSCT. The preparative regimens were based on BUCY2 or modified BUCY2. The acute graft-versus host disease (aGVHD) was prevented by cyclosporin A and short-term MTX regimen in all patients. Two patients from donors with one fully mismatched HLA on DRB1 locus and 4 from unrelated donor also administered Zenapox (CD25 MAb) at dosage of 1 mg/kg every day on the day before transplantation and day 4 after transplantation. These 6 patients were also treated with mycophenolate mofetil (MMF). Transfusion of the donor cells: The median of the transfused nucleated cells was 5.38 x 10(8)/kg and that of the CD34+ cells was 7.8 x 10(6)/kg respectively. All the patients gained hematopoietic reconstruction except one who died of infection before engraftment. Seven patients got II degrees-IV degrees aGVHI) and the incidence was 17.5%. Fourteen patients got cGVHD and the incidence was 53.8% in the patients who survived over 6 months. Twenty-eight patients had fever or other characteristics of infection. The median follow-up time was 13.8 months. The incidence of transplantation related mortality (TRM) was 17.5% and 2 patients relapsed (5.0%). It was concluded that allo-PBSCT can reconstruct hematopoiesis quickly and is a favorable therapeutic method for leukemia.
