1.A Study on Serum Total Bile Acid in Patients with Various Liver Diseases.

For the purpose to define the clinical usefulness of fasting serum total bile acid, the authors carried out to measure the serum total bile acid in 18 cases of acute hepatitis, 10 cases of chronic active hepatitis, 21 cases of liver cirrhosis, 7 cases of hepatocellular carcinoma and 35 cases of adult healthy control. Diagnosis were confirmed histopathologically in all the patients. Blood samples were obtained usually in the morning before meals from th anterior cubital vein of all the patients and adult healthy control in Pusan National University Hospital during the periods from January to August, 1984. Total bile acid was measured fluorome cally by means of an enzymatic technique. The results obtained were as follows: 1) The mean value of serum total bile acid in adult healthy subject in fasting was 2.6+/-2.3 umol/L. 2) The mean values of serum total bile acid were significantly higher in the patients with various liver diseases, as compared with those in adult healthy subjects. 3) In acute hepatitis and chronic active hepatitis, the percentages of abnormal values were significantly higher or tended to be higher in total bile acid, as compared with those in total bilirubin, ALP, LDH and GGT activity and tended to be lower in total bile acid, as compared with those in AST and ALT activity. 4) In liver cirrhosis and hepatocellular carcinoma, the percentages of abnormal values of total bile acid were significantly higher in the patients with above 100IU/L of AST activity, compared with those in the patients with 50~100IU/L of AST activity. 5) Total bile acid showed a significant correlation with total bilirubin and ALP in acute hepatitis, with total bilirubin in chronic active hepatitis and hepatocellular carcinoma, and with ALP in liver cirrhosis. On the basis of the results obtained, it was suggested the measurement of serum total bile acid is clinically a highly sensitive, useful test in diagnosis of various liver diseases.

3.Evoluaotion of Appropriateness of Blood Order Bosed on Quality Indicator of Crossmatching to Transfusion Ratio.

In spite of vigorous efforts to enhance appropriateness of blood usage in surgery, it is recognized that there are still not a few overuse and misuse of blood products in Korean hospitals. To assure appropriate use of blood, continuous monitoring and controlfling blood orders, particularly for surgical operations, should be implemented. The indicator of 'ratio of crossmatching to transfusion' (C/T ratio) has been focalized on in this regard. The authors investigated C/T ratio for eight hospitals, evenly distributed in their geographical location as well as size. Only elective surgeries operated from March 1 to May 31, 1995 were included for analysis. Standardized survey format was distributed, and retrospective reviews of medical records were performed by volunteer surveyors from each hospital after two sessions of pre- survey education. The results were as follows. Average C/T ratio, for all hospitals and all surgeries, was 1.76. Differences in C/T ratio by sex, months, clinical departments, blood components were not significant. However, the ratio showed increasing tendency with ages. The C/T ratio on the average was not higher, compared with the recommended guideline. However, due to study' s limitations ini standardization of survey method and verification of data, we could not conclude that current status of appropriateness of blood use is satisfactory. In addition, to pervasively use the C/T ratio as a quality indicator for blood use management, supplementary measures, such as standardization of data, should be adopted.

5.Measuring the Burden of Major Cancers due to Premature Death in Korea.

OBJECTIVE: To estimate the burden of diseases in Korea especially caused by major cancers using the YLL(years of life lost due to premature death) measurement. METHODS: First, we determined the parameters: such as age-specific standard life expectancy, age on death, sex, cause of death by analyzing the national death certificate data and life table collected during 1996 provided by the National Statistical Office. Secondly, we estimated the age group-specific YLL by employing standard expected years of life lost(SEYLL). Thirdly, final burden of disease due to premature death was estimated by using YLLs measurement which developed by global burden of disease study group. RESULTS: The burden of premature death by cancer for male was attributed mainly to liver cancer(514.5 person-year), stomach cancer(436.4 person-year), and lung cancer(367.7 person-year). Each of these cancers was responsible for the loss of over 100 person-year based on our YLL measurement. The burden of premature death by cancer for female was attributed mainly to liver cancer(135.1 person-year), stomach cancer(252.1 person-year), and lung cancer(121.8 person-year). Each of these cancers was responsible for the loss of over 100 person year based on our YLL measurement. CONCLUSION: We found the YLL method employed in this study was appropriate to quantify the burden of premature death. Thereby, it would provide a rational bases to plan a national health policy regarding premature death caused by cancer.

6.Future Supply of and Demand for Diagnostic Radiologists in Korea.

PURPOSE: To project the supply and demand of diagnostic radiologists in Korea. MATERIALS AND METHODS: With the year 2010 in mind, the 'in-and-out moves' method was applied in order to estimate the supply of diagnostic radiologists. We also used the health need approach and the physician population ratio method for the projection of demand. As sources of information, administrative and demographic statistics were analyzed. RESULTS: If the current situation in terms of the supply of newly qualified diagnostic radiologists is maintained, the projected number will be in short supply by as many as 790 in 2000 and by 370 in 2005. There will, however, be an oversupply of about 110 in 2010. CONCLUSION: To accomplish a balanced state of supply and demand in 2010, the numbers of newly qualifying diagnostic radiologists should be maintained at the current level.

7.Case report of Budd-Chiari syndrome

The authors have studied the findings of radionuclear scanning, inferior vena cavography and ultrasonographyin 2 cases of Budd-chiari Sundrome experienced diffuse inhomogeneous uptake throughout enlarged liver without colddefects. Slightly increased activity in the central portion of liver in case I and markedly increased activity inthe caudate lobe in case II were observed. In inferior vena cavography, the dilated hepatic veins formingintrahepatic collaterals were seen in enlarged caudate lobe. Marked stenosis of I.V.C. at the level of confluenceto right atrium, intraluminal thrombosis of I.V.C., partial obstruction of hepatic veins by thrombi at theconfluent portion and systemic collaterals were noted in case I. In case II, abrupt conical obstruction of I.V.C.at the orgin of hepatic segment, which is consistent with membrane or web. was seen. Prominent systemiccollaterals were developed through the serpinginous margedly dilated azygos and hemiazygos veins, and splenorenalshunt. In ultrasonography, diffuse narrowing and obstruction of hepatic veins at eh confluent level by echogenicnodules were seen in 2 cases. Prominent intrahepatic venous collaterals were observed in case II.

10.Current Status of Subspecialists Training Programs and Factors Affecting Subspecialists' Job Selection after Training.

PURPOSE: This study was designed to identify current status of the subspecialist training programs and related factors affecting subspecialists' job selection. METHODS: The study subjects were 5,569 subspecialist trainees in 61 hospitals between 1989 and 1999. Among them, 1,260 subjects were selected to identify employment status after training. Also we analysed factors affecting career selection for 863 subspecialists on which basic information was available. RESULTS: About 26.6% of all subspecialist trainees trained in 1999 was for subspecialties in internal medicine, the largest majority, and 89.3% was in metropolitan areas. Also 91.1% were trained in teaching hospitals. Among subspecialists completed training, 79.5% selected career to work at general or teaching hospitals as of 1999, but 13.5% practised at clinics. The factors affecting career selection after training were gender, ownership of medical school, and specialty. CONCLUSION: The fact that not a few subspecialists work at primary care clinics means there are unreasonable human resource allocation and planning, with probable poor quality of primary care. Therefore, it is necessary to have a human resource plan at the national level for appropriate number of subspecialists, based on each specialty, in particular. Any structural factors affecting destination of subspecialist trainees, such as gender and graduated medical school, etc, should be dealt with in the long run.
