1.A case of lichenoid contact dermatitis caused by a color film developer.

Color developers(CD) have been noted to produce lichen planus like eruption as well as eczematous contact dermatitis. The chemicals involved are derivatives of paraphenylenediamine or aniline, i. e., Kodak CD 2, Kodak CD 3, Agfa TSS. We report a 28-year-old male who showed a feature of lichenoid contact dermatitis after exposure to color developer. Patch test revealed positive reactions to CD2, CD3 and CD4. Skin biopsy from a patch site showed lichen planus-like tissue reaction.

2.A Case of Molluscum Contagiosum Treated with Diphenylcyclopropenone Immunotherapy.

We are reporting a case of molluscum contagiosum in a 3-year-old boy who showed a good response to diphenylcyclopropenone (DPCP) immunotherapy. The lesions were multiple, 2 to 7mm in size, centrally umbilicated, normal skin-colored papules of 7 months' duration which were distributed on the penoscrotal area, extremities and trunk. In spite of several treatments using extraction and curettage, new lesions developed continuously. The patient was sensitized with 0.1ml of 1% DPCP solution in acetone on his right shoulder and there-after challenged with solution of varying concentrations (0.05 to 0.1%) on his left shoulder once or twice a week to maintain a mild eczema. Clinical improvement was noted 1 week after sensitization and almost all of the lesions cleared after 8 week's treatment.

4.A Case of Desmoplastic Trichoepithelioma.

We report herein a case of desmoplastic trichoepithelioma in a 73-year-old female, who is the oldest among the patients of the disease reported in Korea. She had an asymptomatic, hard, annular lesion, measuring 0.6 x 0.7 cm with an elevated border and a depressed center on the left nasolsbial fold. Histopathological findings showed narrow strands of basaloid tumor cells, horn cysts and a desmoplastic stroma.

5.The Role of Ultrasonography in Identifying Testicular Rupture after Blunt Scrotal Trauma.

To determine the value of ultrasonography for the diagnosis of testicular rapture due to blunt trauma, we reviewed 69 patients of blunt scrotal trauma, which were evaluated by ultrasound before treatment. Thirty patients were managed conservatively, of which 3 cases were explored lately due to persistent pain or mass, and 39 explored immediately. Of the 30 patients treated conservatively the injury was resolved in 27. Surgical exploration of the 42 cases revealed testicular rupture in 29 and simple hematocele in 13. Analysis of the 29 cases with testicular rupture demonstrated that orchiectomy rates were 20% in early exploration and 53% in delayed exploration. in the 27 cases the testicular rupture was correctly diagnosed by ultrasonography, and there were 4 false-positive and 2 false-negative diagnoses of rupture. The specificity and sensitivity for the diagnosis of testicular rupture are 70% and 93%, and the positive and negative predictive values are 87% and 82%, respectively. Ultrasonography used in conjunction with a thorough physical examination is highly sensitive in identifying testicular rupture, and can provide objective information supporting the need for early surgical exploration in patients with blunt scrotal trauma.

6.Preservation of peritoneal membrane in CAPD patients.

Peritoneal membrane shows progressive thickening, fibrotic changes and neovascularization with time on PD. Peritonitis, uremia and bioincompatible dialysate solution are important causes of peritoneal fibrosis in CAPD patients. Epithelial to mesenchymal transition (EMT) is one of the important etiologic factors for the peritoneal fibrosis in CAPD patients. Low GDP group showed less EMT than conventional PD group. Adjustment of peritoneal growth factor for dialysate CA125 revealed significant association with EMT suggesting that fibroblastoid transition from HPMC could be affected by the amount of intraperitoneal VEGF per unit mass of HPMC. There was significant improvement in both degree of EMT and D-CA125 at 6th and 12th months after switching from high GDP solution to low GDP solution. Application of icodextrin solution showed preservation of the peritoneum, like less EMT and high mesothelial bulk mass. In conclusion, therapy with low GDP solution including icodextrin may positively impact on preservation of the peritoneal membrane via reduced EMT.

7.Epithelial to Mesenchymal Transition in CAPD Patients.

Epithelial to mesenchymal transition (EMT) is an important etiologic factor for the development of peritoneal fibrosis in CAPD patients. Mesothelial cells are main source of trans-differentiated fibroblasts under stress from the bioincompatible peritoneal dialysate. In our study there was no difference in dialysate TGF-beta and VEGF between the low and high GDP groups during an initial 12 months. However, after adjusting with D-CA125, the low GDPs group showed a significantly lower D-TGF-beta/D-CA125 and D-VEGF/CA125 during the initial 12 months. Among the adjusted peritoneal growth factors for CA125, VEGF/CA125 and TGF-b/CA125 were factors significantly associated with greater EMT in this study. Adjustment of the peritoneal growth factor for effluent CA125 (surrogate for mass of HPMCs) revealed significant association with EMT suggesting that the fibroblastoid transition from HPMCs could be affected by the amount of intraperitoneal growth factors (TGF-b, VEGF) per unit mass of HPMCs. There was significant improvement in both cell score and D-CA125 at the sixth and 12th months after switching from a high GDPs solution to a low GDPs solution. Use of icodextrin solution in patients who had average peritoneal transport showed not only better systemic effects such as decreased glucose absorption via dialysate but also preservation of the peritoneum, including less EMT and high mesothelial bulk mass. In conclusion, Therapy with low GDP solution including icodextrin may positively impact preservation of the peritoneal membrane integrity and prevention of peritoneal fibrosis with time on PD.

9.A Case of Congenital Generalized Linear Porokeratosis.

Linear porokeratosis is a rare variant of porokerstosis and usually occurs after childhood. We report herein a case of linesr porokeratosis in an 18-year-old female. The lesions had developed since birth and were found on the nearly whole body, including the face, neck, axilla, chest, abdomen, back, and upper and lower extremities. Palms and soles were also affected. Biopsy specirnens from the left forearm and right sole showed typical cornoid lamellae. We could not see any improvement with a course of oral etretinate for 6 weeks.
