Objective To explore the effect of azithromycin sequential therapy on inflammatory cytokines and humoral immune function in the treatment of children with mycoplasma pneumonia.Methods 342 children with mycoplasma pneumonia were divided into control group(168 cases)treated with erythromycin sequential therapy and observation group (174 cases)treated with azithromycin sequential therapy based on the random number table.The changes of inflammatory cytokines and humoral immune function were recorded and compared.Results The levels of interleukin 4(IL -4),interleukin 6(IL -6),C -reactive protein(CRP)and tumor necrosis factor -α(TNF -α) had no significant differences between the two groups before accepting different treatments (P >0.05).The decreased levels of IL -4,IL -6,CRP and TNF -αof the observation group after treatment were greater than the control group (t =10.913,11.047,9.919,53.207,all P <0.01).The levels of immunoglobulin M(IgM),immunoglobulin G(IgG) and immunoglobulin A(IgA)had no significant differences between the two groups before accepting different treat-ments (P >0.05).After treatment,the decrease of IgM and IgG between the two groups had significant differences (t =3.178,3.034,all P <0.01 ).Conclusion Azithromycin sequential therapy for the treatment of children with mycoplasma pneumonia has better clinical efficacy than erythromycin sequential therapy,which can improve inflamma-tory cytokines index and regulate humoral immune function.It has considerable value in the clinical application.

Objective To observe the changes in help T cell 1/2 cytokines and transcription factors in murine cytomegalovirus (MCMV) infected newborn mice and to determine the effectiveness of intervention with thymopentin.Methods One hundred and twenty newborn BALB/c mice were randomized into a blank group,model group and thymopentin group,with 40 mice in each group.Mice in the model and thymopentin group received an intraperitoneal injection of MCMV suspension within 4-6 hours after birth at the dosage of 20 μ L to and tissue culture infections dose of 10431 U/0.1 ml,establish a systemic infection model,and the same volume of normal saline was injected into mice in the control group.Mice in the thymopentin group also received thymopentin 0.3 mg/(kg · d).On Day 3,5,7,10 and 14,8 mice in each group were sacrificed and their splenic tissues were harvested.Interferon (IFN) γ and interleukin (IL)-10 expression in the supernatant of a splenic lymphocyte culture was assessed by enzyme-linked immunosorbent assay.T-bet/GATA-3 mRNA in splenic tissues were analyzed using reverse transcription-polymerase chain reaction.One way ANOVA,including LSD and Dunnett T3 methods,was used for statistical analysis.Results (1) IFN-γ expression in the model and thymopentin groups peaked on Day 3,and was higher than that in the control group [(280.73 ± 14.88),(286.03 ± 15.44) and (149.42±5.43) pg/ml,respectively,F=183.532,P=0.000].Expression in the thymopentin group decreased after Day 3,and increased by Day 7.At Day 14,the IFN-γ expression level in the thymopentin group was higher than that in the control and model groups [(252.33±8.33),(149.07±7.05) and (148.57±4.53) pg/ml,respectively,F=385.487,P=0.000].(2) IL-10 expression in the model group gradually increased.By Day 14,the expression became obviously higher than that in the control and thymopentin groups [(71.19± 1.50),(36.67 ± 2.55) and (40.01 ± 1.28) pg/ml,respectively,F=523.670,P=0.000].IL-10 expression in the thymopentin group increased after Day 3 and decreased by Day 7.On Day 14,the expression in the thymopentin group was lower than that in the model group,but higher than that in the control group.(3) T-bet mRNA expression was obviously increased in the model and thymopentin groups.On Day 3,the expression was higher than that in the control group (relative value of gray-scale:0.74±0.02,0.71 ±0.04 and 0.30±0.01,respectively,F=741.630,P=0.000).By Day 3,the expression in the thymopentin group decreased,and gradually recovered on Day 5,and on Day 14 it was higher than that in the control and model groups (relative value of gray-scale:0.45 ± 0.01,0.30±0.01 and 0.30±0.01,respectively,F=257.571,P=0.000).(4) GATA-3 mRNA expression in the model and thymopentin groups increased on Day 3,and was higher than that in the control group (relative value of gray scale:0.48±0.02,0.53±0.01 and 0.33±0.01,respectively,F=345.167,P=0.000).On Day 14,the expression in the model group was higher than that in the thymopentin group,which was higher than that in the control group (relative value of gray-scale:0.99 ± 0.02,0.55 ± 0.02 and 0.34 ± 0.01,respectively,F=1 767.505,P=0.000).Conclusions A Th1/Th2 shift may be induced in MCMV infected neonatal mice,which manifests as a state of predominant Th2 response.Thymopentin can ameliorate this situation.

Objective To establish a method for the determination of phthalates in water by solid phase extraction-gas chromatography-mass spectrometry(SPE-GC-MS).Methods The water samples were extracted and enriched by solid phasse extraction,using 10%methanol-methyl tert-butyl ether as eluant,dimethylphthalate(DMP),diethylphthalate(DEP),dibutylphthalate (DBP)and diethylhexylphthalate(DEHP)were determined by gas chromatography-mass spectrometry with selected ion monitoring mode(SIM).Results The concentration had a good linear range in the range of 0.00~10.00?g/ml,the correlation coefficient was above 0.998.The detection limit of DMP,DEP,DBP and DEHP were 2.7,2.1,1.6 and 2.5 ng/L respectively,the average recovery rate and the RSD of the method were 78.0%～102.5%and 3.3%～7.8%respectively.Conclusion This method,with small amount of solvent,is simple,fast,accurate and is applicable to the determination of phthalates in water.

Objective To investigate the factors causing exudation of drugs by intravenous contrast medium in CT enhanced scan, and to analyze the corresponding measures. Methods From January 2016 to June 2017 in the First People's Hospital of Jiande City 61 cases of drug extravasation enhanced CT scanning in patients as the research object, through the observation of the drug leakage factors to agents in this group of patients with intravenous contrast and record, in order to find out measures. Results The statistics show, this group of patients with drug extravasation factors: 12 years of age, the incidence rate was 63.93％ (39/61) injection velocity; the incidence rate of 3.0～4.0ml/s was 70.49％ (42/61);small vein injection of vascular forearm 80.33％ (49/61); osmotic pressure, the incidence rate of 300mOsm/kgH2O was 83.61％ (51/61). Conclusion CT scan causes intravenous contrast agent oozing drugs are various, according to the specific selection of patients with drug exudation measures, can effectively improve the patients clinicalcharacteristics of blood stasis, swelling, pain, and has important practical value.

Objective To study the clinical efficacy and safety of oxymatrine in auxiliary treatment of hepati-tis B virus(HBV) related primary hepatocellular carcinoma (PHC).Methods 345 patients were divided into treat-ment group (174 cases) and control group (171 cases).The control group was given hepatic artery chemoemboliza-tion therapy.The treatment group added with oxymatrine.The near future curative effect of solid tumor and patients'immune function,liver function,the improvement of the quality of life change,and adverse reaction were observed. Results KPS score of the treatment group was better(Z =-3.296,P <0.05),more significant in the near future curative effect of solid tumor (χ2 =4.676,P <0.01).The immune function of the treatment group was significantly improved [ Within group: t(treatment group:CD3) = 2.544, t(treatment group:CD4) =2.446, t(treatment group:CD8) = 2.745, t(treatment group:CD4 /CD8) =2.873,t(treatment group:NK) =2.542,t(control group:CD3) =2.614,t(control group:CD4) =2.337,t(control group:CD8) =2.545,t(control group:CD4 /CD8) =2.336, t(control group:NK) =2.672, P <0.05; Between groups: t(after treatment:CD3) =2.947, t(after treatment:CD4) =2.846,t(after treatment:CD8) =2.943,t(after treatment:CD4 /CD8) =2.879,t(after treatment:NK) =2.798,P <0.01].In the treatment group,the effect was more significant in the improvement of ALT,HBV -DNA,AFP[Within group:t(treatment group:ALT) =2.676,t(treatment group:HBV -DNA) =2.682,t(treatment group:AFP) =2.611,P <0.01,t(control group:ALT) =2.556, t(control group:AFP) =2.523,P <0.05,t(control group:HBV -DNA) =1.216,P >0.05;Between groups:t(after treatment:ALT) =2.417, t(after treatment:AFP) =2.432,P <0.05,t(after treatment:HBV -DNA) =2.674,P <0.01].Two groups appeared adverse reaction after chemotherapy,digestive system and hematopoietic system anomaly,the incidence rates of adverse events in the treatment group were lower than those in the control group (χ2 2 2 white blood cel s decreased =46.969,χthe pain =46.977,χheating =22.499,χ2nausea,vomiting =88.749,P <0.05).Conclusion The oxymatrine auxiliary treatment for HBV related PHC has curative effect,safe and reliable,and can significantly improve the patients quality of life,it is worth promoting.

Objective To investigate the role of andrographolide (AP) in protection of carbon tetrachloride (CCl4)-induced acute liver injury in mice and the possible mechanisms. Methods The mice were randomly divided into five groups, including two groups with different doses of AP (50 mg/kg and100 mg/kg), a control group, a CCl4 model group, and a silymarin group. Serum levels of alanine aminotransferase (ALT), aspartateminotransferase (AST), hepatic malondialdehyde (MDA), and glutathione (GSH) were examined. Pathological changes in the liver were observed. RT-PCR was used to detect the expressions of tumor necrosis factor-a (TNF-α) and heme oxygenase-1 (HO-1) mRNA. Results As compared with CCl4 model group, serum levels of ALT and AST and hepatic MDA activity were significantly decreased in AP group (100 mg·kg-1), along with a remarkable increase in hepatic GSH content. Pretreatment with AP at a high dose alleviated histopathological changes induced by CCl4. A markedly increased level of TNF-a induced by CCl4 was reduced by AP, while HO-1at transcriptional level was dramatically elevated following AP pretreatment. Conclusions AP plays a role in protection of CCl4-induced acute liver injury by inhibiting lipid peroxidation and reducing formation of free radicals, the mechanism may be involved in inhibition of TNF-αand activation of HO-1.

Objective To explore the medication rules of Professor WEI Pin-kang in treating gastric cancer; To provide references for clinical medication. Methods Gastric cancer cases treated by Professor WEI Pin-kang in TCM outpatient department of Shanghai Changzheng Hospital from January 2008 to December 2015 in WEI Pin-kang TCM Studio Outpatient Medical Cases (including electronic database and paper medical cases database). TCM Inheritance Support System V2.5 was used to build database. Frequency analysis, association rule analysis, complex system entropy clustering and other data mining methods were used to analyze frequency, medicine association rules and core medicine combination. Results Totally 305 clinical prescriptions including 185 kinds of Chinese materia medica were collected. The Chinese meteria medica with the high frequency were Pinellinae Rhizoma Praeparatum, Arisaematis Rhizoma Preparatum and Galli Gigerii Endothelium Corneum, etc. High frequency Chinese materia medica were with the properties of warm and cold, and with the flavors of bitter, spicy and sweet, to Stomach Meridian of Foot-Yangming, Spleen Meridian of Foot-Taiyin, and Liver Meridian of Foot-Jueyin. The medicine combinations with high frequency were Pinellinae Rhizoma Praeparatum and Arisaematis Rhizoma Preparatum,Pinellinae Rhizoma Praeparatum and Galli Gigerii Endothelium Corneum, and Galli Gigerii Endothelium Corneum and Arisaematis Rhizoma Preparatum. The common medicine pairs were Aurantii Fructus and Aurantii Fructus Immaturum and Scorpio and Scolopendra. Analysis showed 36 association rules of the confidence > 0.99. 70 core medicine combinations and 11 new prescription were obtained. Conclusion Professor WEI Pin-kang treats gastric cancer based on phlegm theory. The dissolving phlegm and eliminating stagnation medicine, regulating qi-folwing for harmonizing stomach medicine and heat-clearing and detoxifying medicine are commonly used.

Glycosaminoglycans (GAGs) are a group of important large biological molecules containing a hexosamine and an uronic acid component. Glycosaminoglycans-binding cytokines, such as vascular endothelial growth factor, midkine, osteopontin, heparanase, and basic fibroblast growth factor, can be found on cell surface and extracellular matrix. Inaddition to various biological functions, GAGs-binding cytokines are also closely related totumor development and progression. GAGs-binding cytokines are gaining increasing attention in molecular diagnosis and targeted therapy of tumors.

Objective: To study the phenotypes of T cell in patients with Hashimoto thyrioditis (HT) and to analyze its relationship with the impairment of follicles, so as to provide evidence for clinical diagnosis and treatment. Methods: Totally 136 HT specimens were obtained by fine needle aspiration biopsy (FNAB) and were divided morphologically(L, P, O type). The serum parameters (TPOAb, TGAb, TSH) were tested, and 20 cases with nontoxic goiter were taken as control. Immunohistochemical assay was performed with 60 specimens and FCM (Flow cytometry) was used to study 23 specimens. The T cell infiltration and the distribution of CD3+, CD4+, and CD8+ T cells were compared between all types of HT tissues. The correlationship between the atrophic degree of follicle with T lymphocytes infiltration and phenotype was analyzed. Objective: To explore the effects and related mechanism of heat-sensitive moxibustion plus clomifene citrate capsules for infertility due to polycystic ovary syndrome (PCOS). Methods: A total of 70 patients were randomized into a control group and an observation group by the random number table method, with 35 cases in each group. The control group was treated with clomifene citrate capsules, while the observation group was treated with heat-sensitive moxibustion on the basis of the medications in the control group. The treatment course lasted for 6 menstrual cycles. The endometrial thickness and ovarian volume of the patients were measured before and after treatment. The levels of serum sex hormones, tumor necrosis factor-α (TNF-α) and nuclear factor-κB (NF-κB) were detected. Follow-up was performed for 1 year after treatment, and the pregnancy rate was recorded. Results: After treatment, the endometrium of the patients in both groups was significantly thickened, the ovarian volume was significantly reduced, and the intra-group differences were statistically significant (all P<0.05). The endometrium thickness and ovarian volume in the observation group were significantly different from those in the control group (both P<0.05). After treatment, the levels of serum testostrn (T) and luteinizing hormone (LH) in both groups decreased significantly, the level of estradiol (E2) increased obviously, and the intra-group differences were statistically significant (all P<0.05). The levels of serum T and LH in the observation group were lower than those in the control group, and the E2 level in the observation group was higher than that in the control group, and the between-group differences were statistically significant (all P<0.05). After treatment, the levels of serum TNF-α and NF-κB of the patients in both groups decreased, and the intra-group differences were statistically significant (all P<0.05). The levels of serum TNF-α and NF-κB in the observation group were significantly lower than those in the control group (both P<0.05). After 1-year follow-up, the pregnancy rate was 20.0％ in the control group, versus 51.5％ in the observation group. The pregnancy rate in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion: The treatment of heat-sensitive moxibustion plus clomifene citrate capsules can increase the endometrium thickness, reduce the ovarian volume, improve the status of serum sex hormone disorders, and increase the conception rate in patients with infertility due to PCOS, which may be related to the adjustment of the levels of immune inflammatory factors such as serum TNF-α and NF-κB.
