1.The Impact of Health Behaviors and Social Strata on the Prevalence of Cardio-Cerebrovascular Disease.

OBJECTIVE: The study aimed to determine which factors influence the occurrence of cardio-cerebrovascular diseases. The researchers analyzed differences in health behaviors and social strata according to each type of cardio-cerebrovascular disease, examined regional characteristics and compared the study's results with the past research data. METHODS: In statistical analysis, multiple logistic regression was conducted by utilizing the raw data obtained from the 2010 Chungcheongnam-do Community Health Survey. RESULTS: The results showed that cardio-cerebrovascular diseases were influenced by alcohol consumption, obesity and monthly income. CONCLUSION: First, education programs and campaigns that maintain an appropriate level of alcohol consumption are needed. Second, in addition to education programs on managing obesity, government policies should encourage physical exercise. Third, government policies should expand health care services and prevention programs for the low-income class, and strengthen health management networks.

2.Vitamin D insufficiency is associated with food sensitization in children under 2 years with atopic dermatitis.

PURPOSE: Vitamin D levels are known to be associated with development of allergic diseases. However, existing data are controversial. The aim of this study was to investigate whether vitamin D levels are associated with food sensitization (FS) in young children with atopic dermatitis (AD). METHODS: We performed a medical record review of all patients under 2 years presenting to pediatric allergic clinic in Hallym University Sacred Heart Hospital for atopic dermatitis between March 1, 2012 and February 28, 2013. Serum 25-hydroxyvitamin D (25(OH)D)levels and six specific immunoglobulin E (IgE) to common food allergens were measured in 132 young children with AD. We defined vitamin D insufficiency was 25(OH)D <30 ng/mL, and FS as specific IgE > or =0.35 IU/mL to any of six common food allergens. Associations between serum 25(OH)D levels and FS were examined by using multiple logistic regression, adjusting for potential risk and confounding factors. RESULTS: The mean age was 10.1 months. Among the 132 children with AD, 65.9% had FS. In this group, there was a negative correlation between 25(OH)D levels and serum eosinophil percentage (correlation coefficient [r]=-0.335, P<0.01), but not correlated with serum total IgE levels. Children with vitamin D insufficiency were more likely to have FS (adjusted odds ratio, 17.1; 95% confidence interval, 1.36 to 215; P=0.028) than those with adequate vitamin D levels. CONCLUSION: Vitamin D insufficiency is associated with FS in young children with AD. Further research is needed to confirm these findings.

3.A Trend Analysis of the Prevalence, Awareness, Treatment, and Control of Hypertension by Age Group.

OBJECTIVES: We examined the trends in prevalence, awareness, treatment, and control of hypertension in Korea as a basis for improving hypertension control programs. METHODS: 12 598 participants of the Korean National Health and Nutrition Examination Survey were included in this study. Weighted linear regression to test time trends from 2007 to 2011 was performed. RESULTS: The prevalence of hypertension was 20.7% in 2007, 29.4% in 2009, and 26.2% in 2011. Awareness of hypertension was 64.8% in 2007 and 61.1% in 2011. Awareness in those aged 65 and over was greater than in younger groups (age 19 to 44 and 45 to 64; p<0.001). The treatment rates were 58.4% in 2007 and 56.8% in 2011. The treatment rate was greater for those 65 and over than for younger age groups (p<0.001). The percentage of controlled hypertension was 37.6% in 2011. The percentage of controlled hypertension in those 65 and over was higher than in younger age groups (p<0.001). The treatment-control rates were 65.9% in 2007 and 67.7% in 2011. The prevalence of hypertension was higher in the males (p<0.001), while the awareness (p<0.001), treatment (p<0.001), and control (p<0.001) rates were higher in the females. CONCLUSIONS: The prevalence of hypertension was increasing and the hypertension awareness, treatment, and control rates were low in younger age groups and males.

4.Study for the Sensitization Rate of Garlic and Onion Using Multiple Allergen Simultaneous Test-Immunoblot Assay.

OBJECTIVE: Onion and garlic are a commonly consumed food in the Korean population. But these food materials have rarely been studied for their allergenic potentials. We analyzed the sensitization rate of garlic and onion using multiple allergen simultaneous test (MAST)-immunoblot assay. METHODS: From January 2009 to December 2013, we analyzed the results of the MAST immunoblot assay performed in 2,691 allergy patients at Soonchunhyang University Cheonan Hospital, retrospectively. RESULTS: Among 2,691 patients, 1,063 were under 18 years old, and 1,628 were over 18 years old. In the all age groups, 29 patients (1.08%) were positive to garlic, 54 patients (2.01%) were positive to onion and one patient was positive to garlic and onion and others. A total of 84 patients (3.12%) were positive to garlic or onions. In infants under the age of 2 years, garlic was the seventh and onion was the nineteenth common food allergen, and onion was more common than rice. While getting older, the sensitization rate of garlic decreased (P<0.001), but onion increased (P<0.01). CONCLUSION: Using one of the semi-quantitative methods, MAST immunoblot assay, we realized that the sensitization rate of garlic was higher than rice or wheat in infants. We presume further studies on the role of garlic and onion in food allergy are needed.

5.Lung Cancer Screening with Low-Dose CT in Female Never Smokers: Retrospective Cohort Study with Long-term National Data Follow-up.

PURPOSE: Because of growing concerns about lung cancer in female never smokers, chest low-dose computed tomography (LDCT) screening is often performed although it has never shown clinical benefits. We examinewhether or not female never smokers really need annual LDCT screening when the initial LDCT showed negative findings. MATERIALS AND METHODS: This retrospective cohort study included 4,365 female never smokers aged 40 to 79 years who performed initial LDCT from Aug 2002 to Dec 2007. Lung cancer diagnosis was identified from the Korea Central Cancer Registry Database registered until December 31, 2013. We calculated the incidence, cumulative probability, and standardized incidence ratio (SIR) of lung cancer by Lung Imaging Reporting and Data System (Lung-RADS) categories showed on initial LDCT. RESULTS: After median follow-up of 9.69 years, 22 (0.5%) had lung cancer. Lung cancer incidence for Lung-RADS category 4 was 1,848.4 (95% confidence interval [CI], 1,132.4 to 3,017.2) per 100,000 person-years and 16.4 (95% CI, 7.4 to 36.4) for categories 1, 2, and 3 combined. The cumulative probability of lung cancer for category 4 was 10.6% at 5 years and 14.8% at 10 years while they were 0.07% and 0.17% when categories 1, 2, and 3 were combined. The SIR for subjects with category 4 was 43.80 (95% CI, 25.03 to 71.14), which was much higher than 0.47 (95% CI, 0.17 to 1.02) for categories 1, 2, and 3 combined. CONCLUSION: Considering the low risk of lung cancer development in female never smokers, it seems unnecessary to repeat annual LDCT screening for at least 5 years or even longer unless the initial LDCT showed Lung-RADS category 4 findings.

PURPOSE: Pulmonary function test (PFT) plays a key role in the diagnosis and management of asthma in adolescents. But, it is not clear whether adolescents with asthma have significantly reduced lung function when compared with adolescents without asthma. The purpose of this study was to determine the effect of atopy and allergic diseases on pulmonary function and bronchial hyperresponsiveness (BHR) in Korean adolescents. METHODS: A questionnaire survey was conducted on 647 middle school students (male, 264; female, 383) from Seoul city to determine the prevalence of symptoms and diagnosed allergic diseases. We also performed the PFT, methacholine challenge test, skin prick tests and serum total immunoglobulin E. Current atopic dermatitis was diagnosed by doctor's medical examination. RESULTS: Female showed higher values of forced expiratory volume in 1 second/forced vital capacity (FEV1/FVC) and higher numbers of BHR (PC20 less than 25 mg/dL) compared to male (P<0.01). BHR was more common in atopy group (P<0.01), but PFT was not significant difference between atopy and nonatopy. Mean values for all spirometric parameters for asthmatic adolescents were in the normal range. Adolescents with asthma symptoms had slightly lower FEV1/FVC and forced expiratory flow 25%-75% than that of adolescents with no history of wheeze ever, asthma diagnosis and current asthma, but there was no statistically significance. CONCLUSION: The majority of adolescents recruited from the general population who reported having asthma symptoms or other allergic diseases had normal lung function. Sex, atopy, and current atopic dermatitis may affect BHR in Korean adolescents.

7.The clinical and microbial characteristics of healthcare-associated pneumonia.

BACKGROUND/AIMS: Healthcare-associated pneumonia (HCAP) occurs outside hospitals, but its characteristics are similar to those of hospital-acquired pneumonia (HAP). We evaluated the clinical and microbial characteristics of HCAP in Korea. METHODS: Of 130 subjects with suspected pneumonia, 49 were classified as HCAP and 81 as HAP. We retrospectively examined the clinical presentations, outcomes, pathogens, and drug resistance rates of Pseudomonas aeruginosae in both groups. RESULTS: The clinical presentations, including the symptoms and laboratory findings, at the time of hospitalization were comparable in both groups. The hospital mortalities of HCAP (28.6%) and HAP (34.6%) did not differ significantly; the length of the hospital stay was similar for all of the survivors (14 vs. 17 days, respectively). Of the identified pathogens, methicillin-resistant Staphylococcus aureus was significantly less common in HCAP than in HAP (two vs. 18 cases, respectively, p<0.01), whereas Klebsiella pneumoniae was more common in HCAP (five vs. zero cases, respectively, p<0.01). The frequency of other Gram-negative rods was similar in both groups. The rate of resistance to antibiotics in P. aeruginosae in both groups was substantial, with the highest resistant rate to ciprofloxacin (50% and 61.5% in HCAP and HAP, respectively). CONCLUSIONS: Although the clinical features and outcomes of HCAP were comparable to those of HAP in the study population, the frequency of methicillin-resistant S. aureus was significantly lower in HCAP compared to HAP.

8.Prescription Trends of Psychotropics in Children and Adolescents with Autism Based on Nationwide Health Insurance Data.

Children with autism are often medicated to manage emotional and behavioral symptoms; yet, data on such pharmacotherapy is insufficient. In this study, we investigated the Korean National Health Insurance Claims Database (NHICD) information related to autism incidence and psychotropic medication use. From the 2010–2012 NHICD, we selected a total of 31,919,732 subjects under 19 years old. To examine the diagnostic incidence, we selected patients who had at least one medical claim containing an 10th revision of International Statistical Classification of Diseases and Related Health Problems (ICD-10) code for pervasive developmental disorder, F84, not diagnosed in the previous 360 days. Psychotropics were categorized into seven classes. Then, we analyzed the data to determine the mean annual diagnostic incidence and psychotropic prescription trends. Diagnostic incidence was 17,606 for the 3 years, with a mean annual incidence per 10,000 population of 5.52. Among them, 5,348 patients were prescribed psychotropics. Atypical antipsychotics were the most commonly used, followed by antidepressants. An older age, male sex, and the availability of medical aid were associated with a higher rate of prescription than observed for a younger age, female sex, and the availability of health insurance. Psychotropic drugs were used for less than one-third of patients newly diagnosed with autism, and prescription differed by sex and age. Increased diagnostic incidence is associated with an increased prescription of psychotropic drugs. Therefore, medication-related safety data and policies for psychotropic drugs in autism should be prepared.
