1.(99m)Tc-MDP Scintigraphy of Femoral Head Necrosis Following Femoral Neck Fracture

Secondary ischemic necrosis of femoral head due to loss of blood supply following to femoral neck fracture is well known. The regional distribution of bone.seeking radiopharmaceuricals in the skeleton can depend on a number of facto rs, but bone blood flow is a major physiological determinant of regional skeletal uptake of Tc-99m polyphosphate and bone imaging may thus be used for the evaluation of vascularity of the femoral head. The authors made a comparative study of scintigraphic findings and operative findings of 28 cases of femoral neck fracture treated at Kyung Hee University Hospital from April ′80 to May ′84. The results were as follows: 1. In 16 cases of proven avscular necrosis of femoral head, scintigraphy showed absent or decreased activity in 14 cases (87.5%), while radiography showed increased density in 10 cases (62.5%). 2. In 12 cases of proven vital femoral head, scintigraphy showed increased activity in 9 cases (75%) and radiography showed decreased density in 9 cases (75%). 3. 99mTc-MDP scintigraphy was an excellent and useful method for assessing bone vitality of femoral head.

4.Acute Bacterial Meningitis after Continuous Epidural Analgesia: A case report.

Recently, continuous epidural catheter insertion is common practice not only in anesthesia but also in pain clinic. Meningitis is rare but serious complication of epidural analgesia. Acute bacterial meningitis occurred in a 82-year-old female patient with thoracic herpes zoster after continuous thoracic epidural analgeia. An MRI of thoracic region did not show an epidural abscess or granulation tissue. Gram stain and culture of the epidural catheter tip and the CSF yielded Staphylococcus aureus. Immediate treatment with antibiotics and supportive therapy was instituted and then the patient discharged without neurologic sequalae.

5.Surgical treatment of congenital melanocytic nevus in the face of the children.

During the last 10 years, we have experienced 169 cases of congenital melanocytic nevi of the face at Seoul National University Children's Hospital. In these patients, various modalities of treatment have been used, including excision, staged excision, skin graft, local flap, and composite graft. Dermabrasion, electrocoagulation, and laser therapy were excluded because these were not definitive treatments. We reviewed over 169 cases based on the involved on the aesthetic units of the face and the length of the defect perpendicular to wrinkle lines. We have attempted to create a stadard for the selection of the appropriate treatment modality. Cases which involved orbital unit(38 cases), cheek(30 cases), and parotid-masseteric unit(18 cases) were analysed statistically. To avoid the statistical error, units of sufficient number of cases and involving only one unit were included. In each unit, we used Receiver Operator Characteristic Method to identify the turning points of the length of the defect perpendicular RSTL which decided the treatment modality. And we used t-test to show the statistical difference between the lengths of the defect perpendicular to RTSL in each modality. The turning point were 0.9-1.0 cm between excision/staged excision, 2.0 cm between staged excision/FTSG, in orbital unit, 1.3 cm between excision/staged excision in cheek unit, 1.6-1.9 cm between excision/staged excision in parotid-masseteric unit. And it was identified by ANOVA test and t-test that the above groups were statistically different. Till now, the treatment modality of the congenital melanocytic nevus was based on the experiences of the surgeon. But now, we have presented the stadard of the treatment according to the size in each aesthetic unit. Therefore we can choose the approproate treatment modality among the severals in accordance with the standards.

7.Radionuclide scintigraphy of the scrotum

Radionuclide scrotal scintigraphy with 99m Tc-pertechnetate is an easy, well established, sueful and readilyavailable technique for evaluation of acute scrotum. We studied 41 cases of radionuclide scrotal scintigraphy andthe results were as follows; 1. The over all diagnostic accuracy of scrotal scintigraphy waas 93%(38/41 cases). 2.Scrotal scintigraphy was very useful and accurate in differential diagnosis of epididymo-orchitis and testiculartorsion in patient with acute scrotal pain and swelling, while there was some limitation in differential diagnosisof hematoma from acute epididymo-orchitis or torsion. 3. scintigraphy of epididymoorchitis showed increasedperfusion and radiocativity in the epididymis and/or testis and its diagnostic accuracy was 90%(19/21 cases) 4.Acute testicular torsion showed normal flow in perfusion and cold defect occupying affected testis in staticimage, while missed torsion showed slightly increased flow in perfusion image and cold defect surrounded by anuniform rim of hyperactivity (halo sign). Diagnostic accuracy of testicular torsion was 86%(6/7 cases).

8.A Case of Salmonella Osteomyelitis in Lumbar Spine.

Since Maisonneuve mentioned osteomyelitis by typhoid fever for the first time in 1835, numerous investigators had reported salmonella osteomyelitis. But these are very rare in its incidence ; only 0.82% of patients with typhoid infections have skeletal involvement. The authors recently experienced a case of salmonella osteomyelitis in lumbar spine respectively caused by salmonella typhi D group. A 48 year-old female was admitted to our Dept. of Neurosurgery with 4 months history of severe lumbago. On hospitalization her emperature raised upto 39 degrees C once and widal test showed salmonella typhosa OH 640x. Lumbar spine X-ray films demonstrated irregular bony destruction of L2 and L3 with mild degree of narrowing of intervertebral space of L2-3. Pus evacuation and currettage of involved vertebrae surgically were performed. Salmonella osteomyelitis involving intervertebral space was confirmed by culture of pus obtained from pathologic lesion and by biochemical test. Satisfactory result is obtained by surgical and antibiotic therapy.
