1.A Case of Solitary Fibrous Tumor of the Nasal Cavity.

Solitary fibrous tumor (SFT) belongs to a group of mesenchymal tumors, first described as a primary spindle cell tumor of the pleura in 1931. Recently, SFT has been reported in various locations with no relation to serosal surface, such as the mediastinum, head and neck, orbit and urogenital system. Histopathologically, the tumors have a disorganized or "patternless" arrangement of spindle cells in a collagenous background and prominent vascular channels of varing size. To date, only 22 cases of SFT arising from the nasal cavity and paranasal sinuses have been reported in the world literature. We report a case of SFT localized in the right nasal cavity with extension to the right ethmoid sinus with a review of literature.

2.Comparative Study of Gastric Polypectomy by Various Methods.

The gastric polyps may be premalignant lesions and they should be removed as possible. The gastric polyps can be remoued by various endoscopic methods. The safety and therapeutic effectiveness of snare polypectomy, neodymium YAG laser therapy and electrocutery were compared in a total number of 106 patients who were diagnosed as benign polyps by means of endoscopic biopsy.Safety was evaluated by the rate of complications such as perforation and bleeding, and therapeutic effectiveness was evaluated by the average numbers of treatment session of each modality. Average numbers of treatment session of snare polypectomy, laser therapy and electrocautery were 1.04, 1.38 and l.44, respectively. Average numbers of treatment session of snare polypectomy were significantly (p<0.05) lower than those of other two methods. Perforation was not occurred in any group. Bleeding rates after snare polypectomy, laser therapy and electrocautery were 17.4%, 1.0% and 3.8%, respectively. Bleeding was significantly (p<0,05) more frequent after snare polypectomy than after other two methods. In conclusion therapeutic effectiveness was best in snare polypectomy but complication was most frequent after snare polypectomy. There was no difference between laser therapy and electrocautery for therapeutic effectiveness and safety. Therfore, laser therapy and electrocautery caa be used for the removal of flat adenoma, alternatively.

4.Cutaneous Müllerian Cyst on the Ankle: A Case Report

Cutaneous Müllerian cyst (CMC) is a rare benign cystic lesion that typically occurs on the lower extremity of young females shortly after puberty. They have been widely regarded as Müllerian heterotopias due to the morphological similarities of the cyst lining cells to the epithelium of the fallopian tubes. A 16-year-old female presented with a two-year history of a solitary, subcutaneous mass involving the left ankle. She had no specific symptoms associated with the skin lesion. However, the lesion had been growing in size. Histopathological findings revealed cystic structures lined by pseudostratified ciliated cuboidal to columnar cells, accompanying intraluminal papillary projection. The immunohistochemical staining profiles, including positive staining for estrogen and progesterone receptors, were characteristic of the fallopian tube epithelium. After being surgically removed, the lesion was cured without recurrence. Based on her typical presentation and histopathological findings, we present this patient as a case of CMC.
5.Surgical Laryngeal Preservation in Early-Staged Hypopharyngeal Cancer.

BACKGROUND AND OBJECTIVES: Hypopharyngeal cancer is aggressive, grows rapidly and often tends to spread through the submucosa combined with multiple "skip lesions" or tumor satellites. Therefore, even in the early stages, most patients require wide resection, usually including total laryngectomy followed by radiotherapy. However, it is obvious that, with consideration of quality of life, the laryngeal function and cancer control are equally important. The purpose of this study is to evaluate the oncologic and functional results of laryngeal conservation surgery for early-staged hypopharyngeal cancer. SUBJECTS AND METHOD: Fourteen patients with T1 or T2 carcinoma of the various subsites of the hypopharynx, including the medial wall of pyriform sinus (four patients), lateral wall of pyriform sinus (four patients), and posterior pharyngeal wall (six patients), who were treated surgically between 1992 and 2000, were studied. All except one patient who had skin graft underwent surgical resection of the primary tumor with laryngeal preservation and immediate reconstruction with forearm free flap transfer or primary closure. Follow-up period was 6-54 months. RESULTS: One patient died in the immediate postoperative period due to myocardial infarction. However, other postoperative complications were minimal. There was no evidence of disease for at least 1 postoperative year in 7of 13 patients (53.8%) with hypopharyngeal cancer who have been performed the conservation surgery. All patients except one who died of myocardial infarction, were decannulated. All but 2 patients achieved oral intake without continuous aspiration. Of these 2 patients, one patient had severe dysphagia due to the bulkiness of the reconstructed flap and he was fed through a gastrostomy tube. The other patient died of myocardial infarction before the trial of oral diet. CONCLUSION: Our data suggest that if the precise evaluation of the extent of the tumor and a careful selection of the well adjustable patients are done preoperatively, the laryngeal conservation surgery in early hypopharyngeal cancer may be valuable in terms of oncologic and functional aspect.

6.Antidepressant Prescribing Patterns in Korea: Results from the Clinical Research Center for Depression Study.

OBJECTIVE: This study aimed to investigate antidepressant prescribing patterns, including initial choice, switching and combining, and concomitant use of non-antidepressant agents, for depressive disorders in naturalistic clinical care settings in Korea. METHODS: Patients with depressive disorder were recruited from both outpatient and inpatient settings in 18 hospitals from all over Korea. Treatment was performed in naturalistic patterns based on each clinician's decision. Data were collected on the prescription of antidepressants and concomitant agents from baseline to 12-week follow-up. RESULTS: Selective serotonin reuptake inhibitors (SSRIs) were the most commonly prescribed initial antidepressant (48.9%), followed by newer dual-action antidepressants (45.8%). When an SSRI was the initial antidepressant, 46.2% of patients whose medication was changed were moved to newer dual-action antidepressants, and 67.4% of combination cases were combined with newer dual-action ones. When a newer dual-action antidepressant was the initial antidepressant, 70.6% of patients whose medication was changed were moved to SSRIs, and other antidepressants including tricyclic antidepressants were most commonly added for combination treatment (50% of combination cases). During the treatment period, 20.6% of antidepressants prescribed were augmented by non-antidepressant agents, and 75.1% were used concomitantly with anxiolytics or hypnotics. The most commonly used concomitant non-antidepressant agent was quetiapine. CONCLUSION: The selection of antidepressants and the concomitant use of non-antidepressant agents are becoming increasingly diversified, and the results of this study reflect changes in the prescribing pattern in actual Korean practices.

7.Supraglottic Partial Laryngectomy: Oncologic and Functional Results.

BACKGROUND AND OBJECTIVES: Supraglottic partial laryngectomy allows the removal of selected supraglottic tumors, preserving a functioning larynx and avoiding a permanent tracheotomy. The purpose of this study was to evaluate our experience with supraglottic partial laryngectomy and to review the functional and oncologic results of the operation. SUBJECTS AND METHOD: We retrospectively reviewed the medical records of 40 patients with squamous cell carcinoma of the supraglottis and 1 patient with sarcomatoid cancer ; they were either treated with supraglottic laryngectomy (n=31) or extended supraglottic laryngectomy (n= 10) from May 1991 and December 2001. Fifteen patients had tumors in T1, 25 patients in T2, and 1 patient in T3. The primary lesion of the tumors were as follows: suprahyoid epiglottis (n=12), infrahyoid epiglottis (n=13), aryepiglottic fold (n=10), false vocal cord (n=4), and pyriform sinus (n=2). A Kaplan-Meier, Willcoxon and Fisher's exact test was performed to obtain the survival rate and the prognostic factors. The evaluations of postoperative function were performed with regard to decannulation, oral diet, and average time taken to decannulate and to initiate oral intake. RESULTS: The disease-specific 3-year survival rate was 87%. Pathologic lymph node metastasis and the invasion of tumor to the preepiglottic space were significant clinical prognostic factors affecting survival. Local recurrence was developed in only one case (2.6%). Decanulation was possible in 93% of our patients. Ninety-five percent of our patients could ultimately take oral diet. The average decannulation time was postoperative 28.3 days and the average time taken to begin oral feeding time was 23.2days. CONCLUSION: This study suggests that supraglottic partial laryngectomy may be used with acceptable oncologic and functional results for supraglottic cancers.

8.How to enhance students’ learning in a patient-centered longitudinal integrated clerkship: factors associated with students’ learning experiences

Purpose:
Longitudinal integrated clerkships (LICs) have been introduced in medical schools, as learning relationships with clinical faculty or peers are important components of medical education. The purpose of this study was to investigate the characteristics of student-faculty and student-student interactions in the LIC and to identify other factors related to whether students understood and acquired the program’s main outcomes.
Methods:
The study was conducted among the 149 third-year students who participated in the LIC in 2019. We divided the students into groups of eight. These groups were organized into corresponding discussion classes, during which students had discussions with clinical faculty members and peers and received feedback. Clinical faculty members and students were matched through an e-portfolio, where records were approved and feedback was given. A course evaluation questionnaire was completed and analysed.
Results:
A total of 144 valid questionnaires were returned. Logistic regression analysis showed that relevant feedback in discussion classes (adjusted odds ratio [AOR], 5.071; p<0.001), frequency of e-portfolio feedback (AOR, 1.813; p=0.012), and motivation by e-portfolio feedback (AOR, 1.790; p=0.026) predicted a greater likelihood of understanding the continuity of the patient’s medical experience. Relevant feedback from faculty members in discussion classes (AOR, 3.455; p<0.001) and frequency of e-portfolio feedback (AOR, 2.232; p<0.001) also predicted a greater likelihood of understanding the concept of patient-centered care.
Conclusion
Student-faculty interactions, including relevant feedback in discusstion classes, frequency of e-portfolio feedback, and motivation by e-portfolio feedback were found to be important factors in the LIC program.
