Objective To investigate the roles of cell apoptosis and the gene expressions of Fas, FasL, bcl-2 and bax in acute rejection of pancreaticoduodenal transplantation and to evaluate the function of duodenum biopsy for early detection of rejection in rats. Methods Wistar and SD rats were divided into two groups: ①Wistar rats that underwent allogenic pancreaticoduodenal transplantation from the organs of SD rats; ②Wistar rats that received homogenic transplantation. The grafts were then harvested on day 3, 5 and 7 after the transplantation, and all graft samples were observed with HE staining and TUNEL was also used to detect apoptotic cells. The expressions of Fas, FasL, bcl-2 and bax were measured by immunochemical method. According to Nakhleh’s score, pathologic features of transplanted pancreas and duodenum were ranged from one to three scores in order. Results The percentage of same or different scores between the pathological scores of pancreas and duodenum in allogenic pancreaticoduodenal transplantation group were 61.1% (11/18) and 38.9% (7/18) respectively, and there were 6 specimens of pancreatic tissue got higher scores with only one higher score for duodenum. Objective To investigate the effect of gene silencing of Y-box binding protein-1(YB-1) by RNA interference on the proliferation and migration in prostate cancer cells lines PC-3 cells .Methods YB-1 siRNAs(pGenesil-1-YB-1-1 and pGenesil-1-YB-1-2) were synthesized and transfected into cloned into the the PC-3 cells by liposome .The expressions of YB-1 were measure by RT-PCR and Western blotting .The proliferation and migration were respectively detected by MTT and Transwell method . Results ThemRNA and protein expressions of YB-1 were significantly decreased by pGenesil-1-YB-1-1 and pGenesil-1-YB-1-2 (P<0 .05) ,compared with the control group ,the inhibition ratio of mRNA expression was 36 .23% and 39 .42% respectively and the inhibition ratio of protein expression was 41 .56% and 55 .33% respectively .The proliferation and migration were significantly decreased by pGenesil-1-YB-1-1 and pGenesil-1-YB-1-2(P<0 .05) .Conclusion YB-1 gene silencing by RNA interference inhibits the proliferation and migration in prostate cancer cells lines PC-3 cells .

ObjectiveTo observe the effect of nerve block combined with sequential analgesia in peri-operative knee arthroscopy.Methods Two hundred patients undergoing knee arthroscopy,were performed lumbar plexus and sciatic nerve block anesthesia by nerve stimulator with ultrasound guidance.Celecoxib was combined for sequential analgesia.The vital signs,the effect of anesthesia in the surgery,the duration of postoperative analgesia,and patient's satisfaction to analgesia were observed to evaluate the clinical effects.ResultsThe vital signs of the patients in the surgery were stable,the effect of anesthesia was good,180 cases of excellent,16 cases of good,4 cases of poor.The duration of postoperative analgesia was 6-10 (8.2 ± 0.5) h.Combined with sequential analgesia program,there was high patient's satisfaction [ excellent 75％ (150/200),good 10％ (20/200),general 10％ (20/200),poor 5％ ( 10/200 ) ].Conclusion The nerve block combined with sequential analgesia is safe and effective in knee arthroscopy,it achieves high patient's satisfaction,this maybe a routine in knee arthroscopy.

Objective To investigate the effect and mechanism of hydrogen sulfide (H2S) on kidney injury induced by urinary-derived sepsis. Methods Rabbits were randomly divided into Control group, Sham group, Sepsis group, NaHS 2.8μmol/kg group and NaHS 8.4μmol/kg group. Upper urinary tract obstruction and acute infection was induced to estab-lish Sepsis model. At 24 h before surgery, and 24 h, 48 h, 72 h after surgery, blood was taken to examine white blood cell count (WBC), creatinine (Cr) and blood urea nitrogen (BUN). At 72 h after surgery, morphological changes were observed by HE staining and transmission electron microscopy. Immunohistochemical staining was used to detect TNF-α, IL-10 and NF-κB expression. Blood H2S concentration was measured by deproteinization and Cystathionine-γ-lyase (CSE) activity us-ing spectrophotometric methylene blue method. Results At 24 h, 48 h, and 72 h after surgery, Levels of WBC, Cr and BUN were all elevated in Sepsis group compared with the other four groups. Levels of WBC and BUN in NaHS 8.4μmol/kg group were lower than those in NaHS 2.8μmol/kg group. At 24 h, 48 h after surgery, there is no significant difference be-tween levels of Cr in NaHS 8.4 μmol/kg group and that in NaHS 2.8 μmol/kg group , but Cr level in NaHS 8.4 μmol/kg group was marked lower than that in NaHS 2.8μmol/kg group 72 hour after surgery. Pathological features of kidney injury were also alleviated by intravenous administration of NaHS. TNF-α, NF-κB expressions in NaHS 2.8μmol/kg group and NaHS 8.4 μmol/kg group were lower than those in Sepsis group, IL- 10 expression was higher than that in Sepsis group. TNF-α, NF-κB expressions in NaHS 8.4μmol/kg group were lower than that in NaHS 2.8μmol/kg group, whereas IL-10 expression in NaHS 8.4μmol/kg group was higher than that in NaHS 2.8μmol/kg group. Compared with Control group and Sham group, H2S content and CSE expression in kidney were decreased in Sepsis group. After intravenous administration of NaHS, H2S content increased, but the CSE activity has no obvious change. Conclusion Exogenous H2S reduced kidney in-jury induced by urinary-derived sepsis through inhibiting NF-κB, decreasing TNF-αand increasing IL-10.

Objective To explore a suitable method to estimate the quantity of removing skin in double-fold eyelid operation.Methods Upper eyelid skin gently pulled towards eyebrows was flattened with the eyelash being slightly curved,the double-fold eyelid line that was parallelled to blepharitis was marked at 6-7 mm far from blepharitis.Then opening eyelid and looking upwards,the suprapalpebral fold was formed,and several dots were marked on the fold margin. After slightly closing eyelid the suprapalpebral fold margin line was formed through connecting the dots.The skin between the two line was the quantity to remove in operation.If the two line overlaped,removing skin did not need.Results A total of 89 cases were enrolled in this study.The patients were followed up from three months to one year,and the results showed that 67 cases were satisfactory,22 cases were basically satisfatory,and no one was not satisfactory.Conclusions The designing method for removing skin according to double-fold eyelid line and suprapalpebral fold margin line is a good method to estimate the quantity of removing skin in double-fold eyelid operation.

Objective To observe erythropoietin (EPO) and its mRNA expression changes in rats cortical neurons when suffering hypoxia and investigate the endogenous EPO protective effect of hypoxia neuronal cells.Methods Cultured cortical neurons were prepared from hypoxia rats and divided into control,hypoxia 12,24,48,72 h group.Using immunohistochemistry,reverse transcriptase polymerase chain reaction (RT-PCR) and Western blot,we observed EPO and its mRNA expression in cells,and also observe the culture medium containing lactate dehydrogenase to evaluate the activity of neurons in the whole process.Results Immunohistochemistry,RT-PCR and Western blot analysis showed:the EPO (20.79 ± 2.98) and its mRNA (0.78 ± 0.05) at 12 h had a basic expression in hypoxia group,compared with the control group (EPO:17.12 ± 1.99; mRNA:0.39 ± 0.05),and the difference was statistically significant (t =2.51,P ＜ 0.05 ; t =13.51,P ＜ 0.01) ; the strongest expression was observed at 48 h (EPO:28.88 ± 3.41,mRNA:1.45 ± 0.07),the difference was statistically significant (t =7.29,P ＜ 0.01 ; t =33.24,P ＜ 0.01) ; and neuronal activity was strongest.Lactate dehydrogenase activity was significantly decreased after hypoxia 72 h,and also a statistically significant difference was found between the groups at each point.Conclusions The EPO and EPO mRNA expression are increased after hypoxia in neuron cells,and may enhance the activity of neurons.Our study suggests that EPO might be involved in the development process of neuronal hypoxia and play an important role in neuronal hypoxia process.

Objective To investigate the safety,diagnostic value and clinical impact of double balloon enteroscope (DBE) in elderly patients (aged ≥75 years).Methods Clinical data and DBE findings of elderly patients in the Department of Gastroenterology in Remin Hospital of Wuhan University from January 2010 to January 2013 were retrospectively analyzed and compared with those of younger patients.Results The diagnostic rate,complication rate and clinical impact rate were 69.0％ (29/42),0％ (0/42) and 76.2％ (32/42) in elderly patients.There were no statistically significant differences on those parameters between elderly patients and younger patients (all P＞0.05).For elderly patients,the leading causes of obscure gastrointestinal bleeding (OGIB) were tumor (30.0 ％,9/30) and ulcer (16.7 ％,5/30),while angiectasis (6.7 ％,2/30) was uncommon.Conclusions DBE is an effective and safe method for diagnosis of small bowel disease in elderly patients.Tumor is the most common cause of OGIB,while angiectasis was uncommon.

Objective To assess the variations in different thyroid stimulating hormone(TSH) and free thyroxine (FT4) detection kits for evaluating thyroid function during pregnancy and to establish the corresponding normal reference ranges.Methods This study was based at the International Peace Maternity and Child Health Hospital affiliated to Shanghai Jiaotong University School of Medicine.A total of 200 pregnant women who visited the hospital between June,2011 and September,2012 were recruited in this study according to the National Academy of Clinical Biochemistry (NACB) criteria.Blood samples were sequentially collected from the women at the first (T1,9-12 weeks),second (T2,16-24 weeks) and third (T3,32-36 weeks) trimesters to determine the serum TSH and FT4 levels using four different detection kits (Siemens-C,Siemens-Ⅰ,Abott and Roche).A linear trend test was used to analyze serum TSH and FT4 levels with four different kits.A percentile range of P2.5 to P97.5 was used to establish the normal trimester-dependent reference ranges of TSH and FT4 levels for different detection kits.The Bootstrap method was used to compare the differences in the four reference ranges.Results Similar dynamic changes in TSH and FT4 levels during pregnancy were detected among the different kits (F=0.950,P=0.595; F=11.640,P=0.081,respectively).Among the four reference ranges of TSH,the Roche kit showed the most remarkable fluctuation during pregnancy,while Roche kit in the first trimester and Siemens C kit in the second and third trimesters showed larger fluctuations in reference ranges of FT4.More importantly,the reference ranges of TSH and FT4 showed significant variations among the four different kits in each trimester (TSH:T1:F=2 945.390,P ＜ 0.01; T2:F=2 826.260,P ＜ 0.01; T3:F=1 698.360,P ＜ 0.01.FT4:Tl:F=1 145.440,P ＜ 0.01; T2:F=2 260.240,P ＜ 0.01; T3:F=1 439.920,P ＜ 0.01).Conclusions TSH and FT4 measurement using four different commercial kits showed similar trimester-dependent dynamic changes.However,it is necessary to establish trimester-dependent and detection kit dependent normal reference ranges of TSH and FT4 for thyroid function evaluation for pregnant women.

Aim at the two problems in the field of traditional Chinese medicine (TCM) mechanism elucidation, one is the lack of detailed biological processes information, next is the low efficient in constructing network models, we constructed an auxiliary elucidation system for the TCM mechanism and realize the automatic establishment of biological network model. This study used the Entity Grammar Systems (EGS) as the theoretical framework, integrated the data of formulae, herbs, chemical components, targets of component, biological reactions, signaling pathways and disease related proteins, established the formal models, wrote the reasoning engine, constructed the auxiliary elucidation system for the TCM mechanism elucidation. The platform provides an automatic modeling method for biological network model of TCM mechanism. It would be benefit to perform the in-depth research on TCM theory of natures and combination and provides the scientific references for R&D of TCM.

Objective To preliminarily screen the neoplastic stem cell (NSC) related surface markers combination and to under-stand the NSC distribution in hepatoblastoma(HB) .Methods The children cases of HB undergone the surgical therapy in the Jian-gxi Provincial Children′s Hospital were selected .The immunohistochemical method was adopted for observing the expression and distribution of NSC-related markers CD34 ,Thy-1 ,c-kit ,CD56 and stem cell factor(SCF) in the HB tissue and the normal hepatic tissue away from the edge of tumor tissue 3cm outside .Results Thy-1 and c-kit were sporadically distributed in the HB tissue and mainly focued on the portal area ,but did not exressed in the normal liver tissue ;the expression of CD34 and SCF in HB was signifi-cantly higher than that in the normal liver tissue(P<0 .05) ,in which CD34 was mainly distributed in the endothelial system and SCF was mainly expressed in the portal area ;CD56 was expressed in the tissue of the clustered nerve fibers ,the expression differ-ence between them had no statistical significance (P>0 .05) .Conclusion The different NSC related surface markers are distributed in the HB tissue and focus on the specific areas .The positive cells of Thy-1/c-kit expression may play a role in the HB occurrence .
