Liver is one of the most predilection sites of hematogenous metastasis of a variety of malignances,especially for gastrointestinal tumors.Surgical resection was the first choice for the treatment of liver metastases,while it could not apply to patients who had multiple metastatic lesions or other organs involvement.Interventional technique has been widely recognized for the advantages of minimal trauma,little pain,quick recovery and obvious efficacy.
2.Strategies of strengthening the cultivation of medical graduate students' ability of clinical scien-tific research

Graduate education is the maln approach to cultivate advanced medical talents. However, the current clinical research ability tralning for postgraduate students is poor. This article discusses about four possible reasons: the misunderstanding of the medical research, system defects of the endowment of scientific research fund, drawbacks of evaluation criteria, and deficiency of grad-uate student curriculum. In order to improve the clinical research ability of medical graduates, this article also discusses the possible solutions: clarifying the understanding, strengthening policy support, im-proving the evaluation methods, and perfecting the tralning course of the clinical medical research.

5.Role of microphthalmia-associated transcription factor in melanoma metastasis

In recent years, the function of microphthalmia-associated transcription factor (MITF) is a hot field in melanoma.The abnormal expression of MITF is closely related to the occurrence and metastasis of melanoma.In addition, the down expression of MITF promotes its invasion.Studying the regulation of MITF and its related molecules and signaling pathways will let us further understand the molecule mechanism of malignant melanoma metastasis and provide help to exploit novel molecular targeted drug.
6.Optimization of hydrolysis process of linarin using response surface methodology and research about ARI activity of glycosylation-acacetin.

OBJECTIVETo optimize the hydrolysis process of linarin by response surface methodology, and to use the model of aldose reductase to study the acacetin's activity of aldose reductase inhibitory.
METHODThe model of acacetin enzyme in vitro was established by the determination of fluorescence absorption of NADPH, the inhibition rate of acacetin aldose reductase was calculated, and then the IC50 of hydrolysis was obtained. The hydrolysis process of linarin hydrolysis condition was optimized by using response surface method.
RESULTThe results indicated that the IC50 of acacetin (2.74 mg x L(-1)) was less than the IC50 of linarin (3.53 mg x L(-1)). Hydrolyzation time of 7.4 h, sulphuric acid concentration of 0.54 mol x L(-1) and the ratio of material to liquid of 3 : 1 were the optimum conditions.
CONCLUSIONHydrolyzate acacetin has preferable inhibitory activity of aldose reductase. The optimized hydrolysis condition of linarin is convenient to use with good predictability.

7. Giant left ventricle complicated with severe aortic regurgitation in Marfan patients with functional mitral regurgitation: A clinical study

Objective: To analyze the incidence of functional mitral regurgitation (FMR),post-operation characteristics and operation indications of Marfan patients with severe aortic regurgitation and giant left ventricle. Methods: The clinical data and the follow-up data of 19 Marfan patients with severe aortic regurgitation (≥10 ml) and giant left ventricle (LVEDd≥7.0 cm), who were treated from Jan. 2000 to Dec. 2007,were analyzed retrospectively. Results: All patients received Bentall operation and there was no perioperation death. Follow-up was 72 patient-year (ranging 3 months-7 years). The degree of FMR decreased obviously after operation, with an improvement rate of 87.5%, and the FMR almost disappeared 6 months to 1 year later. Conclusion: Bentall achieves good outcome in Marfan patients with severe aortic regurgitation and giant left ventricle. Mild to moderate FMR(< 10 ml) disappears with the decrease of the left ventricle size. For patients with severe FMR (≥ 10 ml) complicated with dysfunction of left ventricle, managed ventricular pacing is suggested.
8. Short-term effects of Sipulin combined with radiotherapy on local advanced non-small cell lung cancer and its influence on immune function

Objective: To observed the short-term effects of Sipulin combined with radiotherapy on local advanced non small-cell lung cancer (NSCLC) and its influence on the immune function. Methods: Ninety patients with local advanced NSCLC were randomly divided into trial group (Sipulin plus radiotherapy) and the control group (only radiotherapy). The patients in trial group were given Sipulin (10 mL/d, 25 mg peptide) by Bolus infusion combined with radiotherapy for 4 weeks. The patients in control group received only radiotherapy at the same dosage. All the patients were followed up for 2 years. The follow up ratio was 100%. Results:(1) The response ratio was 66.7% and 33.3% in trial group and control group, respectively (P < 0.05). The one-year survival rate was 71.1% and 44.4% respectively (P <0.05). (2) The life quality of the patients in trial group was much better than that in control group (P<0.05). (3) The population of CD3+ cells, CD4+ cells and the ratio of CD4+/CD8+ were increased but the population of CD8+ cells were decreased after combined treatment. The population of CD3+ cells, CD4+ cells and the ratio of CD4+/CD8+ were decreased but the population of CD8+ cells were increased after single radiotherapy (P < 0.05). Compared with control group, combined treatment induced a marked rise in the population of CD3+ cells and CD4+ cells and the ratio of CD4+/CD8+ and a fall in the population of CD8+ cells (P < 0.01). Conclusion: The short-term effect of Sipulin combined with radiotherapy on local advanced NSCLC is better than that of single radiotherapy. Combined therapy could improve the life quality and enhance the immune function of the NSCLC patients.
9. Systematic research of secondary development of Shufeng Jiedu Capsule

The study of secondary development of major Chinese materia medica varieties is the important content of Chinese materia medica (CMM) innovation research, and it is also the important approach of inheriting and developing traditional Chinese medine (TCM) theory and breaking the bottleneck restricting of the development of TCM theory and CMM industry. In this paper, the secondary development of Shufeng Jiedu Capsule (SJC) was systematically studied. The chemical components of SJC were elucidated by the identification and characterization of herbs, SJC and components ingested into the blood. The main effective components were further screened and cleared by integrating UPLC-Q/TOF-MS, NF-κB double luciferase reporter gene system, binding experiments of G protein coupled receptors, network pharmacology analysis, and spectrum-efficiency screening method. Based on the traditional efficacy, the mechanism of SJC was explained in terms of anti-inflammatory immunity, antipyretic effect, pharmacokinetics and tissue distribution of the main components based on the methods of animal model and genomics. The formula features, compatibility regularity were clarified and the action properties, comparative advantages and core clinical value were found and refined by the research of decomposed recipes and the comparison with the similar CMM recipes and chemical drugs. The quality control systems of SJC and related herbs were established by content determination of multi-target ingredients and multi-batch samples, and the establishment of the characteristic mode of HPLC fingerprint. The original quality standards were promoted comprehensively to ensure the uniformity, stability and controllability of product quality. This study not only provided important theoretical and experimental basis for clinical application and guidance of clinical practice, but also provided research ideas and modes for the secondary development of other major CMM varieties.
10.Therapeutic efficacy of moxibustion plus medicine in the treatment of infertility due to polycystic ovary syndrome and its effect on serum immune inflammatory factors

Objective: To explore the effects and related mechanism of heat-sensitive moxibustion plus clomifene citrate capsules for infertility due to polycystic ovary syndrome (PCOS). Methods: A total of 70 patients were randomized into a control group and an observation group by the random number table method, with 35 cases in each group. The control group was treated with clomifene citrate capsules, while the observation group was treated with heat-sensitive moxibustion on the basis of the medications in the control group. The treatment course lasted for 6 menstrual cycles. The endometrial thickness and ovarian volume of the patients were measured before and after treatment. The levels of serum sex hormones, tumor necrosis factor-α (TNF-α) and nuclear factor-κB (NF-κB) were detected. Follow-up was performed for 1 year after treatment, and the pregnancy rate was recorded. Results: After treatment, the endometrium of the patients in both groups was significantly thickened, the ovarian volume was significantly reduced, and the intra-group differences were statistically significant (all P<0.05). The endometrium thickness and ovarian volume in the observation group were significantly different from those in the control group (both P<0.05). After treatment, the levels of serum testostrn (T) and luteinizing hormone (LH) in both groups decreased significantly, the level of estradiol (E2) increased obviously, and the intra-group differences were statistically significant (all P<0.05). The levels of serum T and LH in the observation group were lower than those in the control group, and the E2 level in the observation group was higher than that in the control group, and the between-group differences were statistically significant (all P<0.05). After treatment, the levels of serum TNF-α and NF-κB of the patients in both groups decreased, and the intra-group differences were statistically significant (all P<0.05). The levels of serum TNF-α and NF-κB in the observation group were significantly lower than those in the control group (both P<0.05). After 1-year follow-up, the pregnancy rate was 20.0% in the control group, versus 51.5% in the observation group. The pregnancy rate in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion: The treatment of heat-sensitive moxibustion plus clomifene citrate capsules can increase the endometrium thickness, reduce the ovarian volume, improve the status of serum sex hormone disorders, and increase the conception rate in patients with infertility due to PCOS, which may be related to the adjustment of the levels of immune inflammatory factors such as serum TNF-α and NF-κB.
