1.The Clinical effect of T-cell CD40 ligand,IL-6 and hs-CRP expression on rheumatoid arthritis patients

Objective To investigate clinical significance and the variation of serum interleukin -6(IL-6) ,high-sensitivity C-reac-tive protein(hs-CRP) and T-cell CD40 ligand(CD40 L) in patients with rheumatoid arthritis (RA)(included activity group and stable group) .Methods 90 cases of RA patients in the observation group ,90 cases of healthy volunteers were selected as control group . Enhanced immunoturbidimetric assay was used in measurement of serum hs-CRP .Levels of CD40 L ,IL-6 were detected by enzyme-linked immunosorbent assay .Results BPC ,RF ,ESR ,hs-CRP ,IL-6 and CD40 L in peripheral serum of observation group were sig-nificantly higher than control group ,the differences were statistically significant (P<0 .05) .BPC ,ESR ,hs-CRP ,IL-6 and CD40 L in peripheral blood of patients in RA activity group were significantly higher than patients with RA in stable group ,the differences were statistically significant(P<0 .05) .RF ,ESR ,hs-CRP ,IL-6 and CD40L in peripheral serum of RA patients whose platelet ele-vated were significantly higher than RA patients whose platelet stay normal level ,the differences were statistically significant (t=6 . 070 ,3 .858 ,3 .272 ,3 .217 ,2 .253 ,P<0 .05) .Conclusion Serum hs-CRP ,IL-6 and CD40 L relate to RA inflammation closely .Com-bined detection can observe the condition changes and treatment effect in patients with RA .
2.Optimization of Chitosan Flocculation Purification Process of Zhigancao Decoction by Orthogonal Test

Objective:To optimize the chitosan flocculation purification process of Zhigancao decoction. Methods:With the total content of ginsenoside Rg1 , Re and Rb1 and the impurity removal rate as the evaluation indices, an orthogonal test was used to investi-gate the effects of concentration of solution, amount of chitosan and standing temperature on the purification process. Results:The op-timum conditions of chitosan flocculation clarification process of Zhigancao decoction were as follows:the concentration of extraction so-lution was 0. 25 g·ml-1 , the standing temperature was 25℃, and the amount of chitosan was 12% . Conclusion:The optimum con-ditions of purification process are promising with high impurity removal rate and active ingredients retention rate, which provide refer-ence for the dosage form reformation of Zhigancao decoction.
3.Impact of Holly Leaf Extract on Lipid Metabolism and Hemorheology in Hyperlipidemia Rats

Objective:To explore the effects of holly leaf extract on blood lipid regulating function and blood rheology in hyperlipi-demia rats to provide experimental basis for the development and application of holly leaf. Methods:The rats were fed with high-fat di-et for 4 weeks to establish the model of hyperlipidemia. The rats were randomly divided into the control group, the model group, the positive group, high-dose group (0.60 g·mL-1), medium-dose group (0.30 g·mL-1) and low-dose group (0.15 g·mL-1) of Holly leaf with gastric perfusion for 9 weeks. TC, TG, HDL-C, LDL-C, AST, ALT, SOD and MDA in serum of the rats were deter-mined, and various indices of hemorheology were measured. Results:When compared with the model group, holly leaf extract at dif-ferent doses could decrease the content of TC, TG, LDL-C, ALT, AST, MDA and apo B in varying degrees, and increase the content of HDL-C, the SOD activity and lipoprotein levels in varying degrees at the end of the gastric perfusion, and the effects of high-dose of holly leaf were the most significant (P<0. 05,P<0. 01). Holly leaf extract at different doses could decrease plasma viscosity, whole blood viscosity, erythrocyte aggregation index and hematocrit, and increase red blood cell deformability index, and the effects of high-dose group of holly leaf were the most significant (P<0. 05,P<0. 01). Conclusion:Holly leaf has obvious regulating effects on blood lipid function and blood rheology in hyperlipemia rats, which can reduce blood lipid levels.
5. Anterior cervical decompression technique via intervertebral space under microscope

Objective To discuss the technique of the anterior cervical decompression via intervertebral space under microscope. Methods A total of 271 patients with cervical disc herniation underwent anterior microsurgical decompression and intervertebral fusion from Jun. 2000.6 to Dec. 2005. The patients included 178 cases with single level herniation, 71 with double level ones, and 22 with three level ones. MRI images showed slight or medium anterior compression with slight abnormal changes within spinal cord in all cases. The main equipment included operation microscope for neurosurgery and high speed bur. The annular fibrous and osteophytes were removed by high speed bur to expose and clean the degenerative nucleus pulpous. We lifted the posterior longitudinal ligament (PLL) with tension using a hook-like stripper and cut it; then the residual PLL and posterior bony edges were removed. Results There were no severe complications such as injuries of the spinal cord, nerve root or blood vessels. The average intraoperative blood loss was (126 + 29. 4) ml and the mean operative time was (62+12. 4) min. The average time for the patients to get off bed was (1. 8 + 0. 4) days. The nerve functions were improved in all patients after operation, with the excellent nerve function found in 69 cases, good in 157 cases, fair in 27 cases, and poor in 18 cases according to JOA scores. Conclusion Anterior cervical decompression via intervertebral space under microscope is suitable for treatment of pathological changes of the intervertebral space; delicate manipulation under microscope and high speed bur can contribute to a successful operation; and the anatomy of intervertebral space is more suitable for the biomechanics of the cervical vertebrate.
6.Clinical analysis of six cases of juvenile primary fibromyalgia syndrome

Objective To study the clinical features of juvenile primary fibromyalgia syndrome (FMS). Methods Six patients with juvenile primary FMS were registered in the department. Their clinical data were assessed and compared with 36 patients with adults FMS. Results Abdominal pain was the first symptom in five of six juvenile primary FMS, diffuse aching and left knee pain were the first symptoms in one of six patients. All were misdiagnosed prior to their rheumatological evaluation. Diffuse aching, fatigue, sleep disturbances, illness changes with weather and feeling worse with exercise were existed in six juvenile FMS patients (100%), the mean pain score was 8.8 and the mean initial tender point (TP) was 13.7. Arthrodynia, subjective joint swelling, abdominal pain, irritable bowel symptoms and urinary urgency were existed in five of six patients (83%). Dysmenorrhea was present in 4(67%), depression in 3 (50%), morning stiffness in 2 (33%), paresthesias in 2 (33%) and anxiety in 2 (33%) respectively. There was no difference compared with adults FMS. Conclusion Juvenile primary FMS is a common disease and clinicians should pay more attention to it to avoid misdiagnosis.
7.STUDIES ON TEH UTILIZING THE PHOTOSYNTHETIC BACTERIUM FERMENTATION FOR BIOTRANSFORMATION ON CORN STRAW

The physiological character,functions and applications of photosynthetic bacterium had been discussed,during biotransformation of teh com straw treated with ammonia in aerobic,anaerobic and natural conditions,Comparing the concentration of teh reductive sugar and teh protein in teh fermented borth,we suggest a good way of biotransformating teh corm straw by photosyntehtic bacterium.In the photosynthetic bacterium fermentation in which the com straw treated substrate,teh concentrations of the reductive sugar and the transformative reductive sugar and teh protein in teh fermented borth were higher thean those without ammonia. Analysis of the results shows theat the transformative reductive sugar and protein were the nourishment of photosynthetic bacterium,so theere may be a way that we harness the corn straw by teh photosynthetic bacterium.
8.Risk factors of erectile dysfunction in patients with cardiovascular diseases.

OBJECTIVETo investigate the penile erectile function of hospitalized male patients with cardiovascular diseases, the incidence of erectile dysfunction (ED) in this cohort, and the relationship of ED with cardiovascular diseases and its risk factors.
METHODSUsing a self-designed questionnaire, we conducted an investigation among the hospitalized patients in the Department of Cardiovascular Diseases of the First and Second Affiliated Hospitals of Xi'an Jiaotong University. We measured their body height, body mass index (BMI), waist circumference, hip circumference, and blood pressure, obtained their personal data, past history, metabolic indexes, and erectile function scores by IIEF-5, and analyzed the risk factors of ED using univariate and multivariate logistic regression and OR analyses.
RESULTSTotally, 225 valid questionnaires were included in this investigation, which showed a 66.7% incidence of ED, 15.8% mild, 27.0% mild to moderate, 17.6% moderate, and 6.3% severe. The incident rates of ED in the 18-35 yr, 36-49 yr, 50-65 yr, and > 65 yr age groups were 13.6%, 39.1%, 89.2%, and 91.2%, respectively. Univariate logistic regression analysis manifested that the risk factors of ED in the patients with cardiovascular diseases included age (OR = 3.122, 95% CI 2.040-4.779), smoking (OR = 1.768, 95% CI 1.209-2.584), BMI (OR = 1.261, 95% CI 1.114-1.427), total cholesterol (OR = 1.77, 95% CI 1.339-2.340), TC/HDL (OR =1.715, 95% CI 1.349-2.181), hypertension (OR = 1.717, 95% CI 1.110-2.658), and coronary heart disease (OR = 2.235, 95% CI 1.169-4.275), while multivariate logistic regression analysis showed the risk factors to be age (OR = 4.99, 95% CI 2.264-10.998), financial condition, (OR = 2.804, 95% CI 1.127-6.976), smoking (OR = 2.109, 95% CI 1.179-3.772), BMI (OR = 1.414, 95% CI 1.136-1.760), and TC/HDL (OR = 2.001, 95% CI 1.016-3.943).
CONCLUSIONThe incidence of ED is high in hospitalized patients with cardiovascular diseases and rises with the increase of age. Age, smoking, financial condition, BMI, and TC/HDL are the risk factors of both ED and cardiovascular diseases, and financial condition is closely associated with ED.

10.Experimental study on lauromacrogol in sclerotherapy of endometrial cyst models

Objective To investigate the value of the lauromacrogol in sclerosis therapy of endometrial cyst models of SD rat.Methods 45 SD rats were randomly divided into lauromacrogol treatment group,dehydrated alcohol treatment group and physiological saline blank group after being built cysts models.Then corresponding medicine was injected into the cysts,and the changes of pathomorphology and VEGF expression of ectopia endomembrane were observed a week later.Results Microscope observation indicated that endometrial cyst glandular epithelium and gland were inactived by both lauromacrogol and dehydrated alcohol.The effective rate of lauromacrogol group was 87.5%,and that of dehydrated alcohol group was 85.0%.There was no significant difference between these two methods (P =1.000).The ectopia endomembrane glandular organs of physiological saline group were not destructed obviously.Besides,VEGF expression of lauromacrogol and dehydrated alcohol group were lower than physiological saline group (P =0.003 and 0.006).There was no significant difference between VEGF expression of lauromacrogol and dehydrated alcohol group (P =0.926).Conclusions Compared with dehydrated alcohol,lauromacrogol had the same therapeutic effect on sclerosis therapy of endometrial cyst models.
