1.Studies on Platelet Aggregability in Thrombotic Disease and Hypercholesterolemia and Effects of Aspirin and Dipyridamole.

Although platelet have been implicated in the pathogenesis of the thrombotic disease, the platelet aggregability was not well studied in Korea. Author measured platelet aggregability in 103 clinical cases including 30 healthy volunteers to evaluate the platelet function and the effect of Aspirin and Dipyridamole on aggregability in Korean. 24 patients with cerebral thrombosis, 24 patients with ischemic heart disease and 25 patients with hypercholesterolemia were included for this study. Aggregation tests were performed at three final concentrations of epinephrine(10microM/L) and ADP(4 microM/L, 10 microM/L) with platelet aggregometer which was made by Chrono-Log Corp. in all cases. Platelet aggregations were measured in patients who were treated with Aspirin, Dipyridamole and combined treatment of Aspirin and Dipyridamole respectively. The following results were obtained. 1) The mean maximal platelet aggregability in the normal subjects induced by 10 microM/L epinephrine was 59.3+/-24.26%, 66.6+/-14.00% in Bm and 62.5+/-19.30% in B5 in induction by 4 microM/L ADP, and 77.2+/-8.99% in Bm and 76.6+/-9.83% in B5 in induction by 10microM/L ADP. 2) The mean maximal platelet aggregability in patients with cerebral thrombosis induced by 10 microM/L epinephrine was 89.2+/-7.33%, 78.8+/-9.41% in Bm and 78.5+/-9.93% in B5 in induction by 4 microM/L ADP, and 86.4+/-7.69% in Bm and B5 in induction by 10 microM/L ADP. The results showed significantly elevated platelet aggergability than that of normal subjects(p<0.01). 3) The mean maximal platelet aggregability in patients with ischemic heart disease induced by 10 microM/L epinephrine was 88.1+/-11.99%, 78.2+/-12.50% in Bm and B5 in induction by 10 microM/L ADP. The results showed significantly elevated platelet aggregability than that of normal subjects(P<0.01). 4) The mean maximal platelet aggregability in patients with hypercholesterolemia induced by 10 microM/L epinephrine was 86.8+/-15.99%, 82.7+/-11.19% in Bm and 82.0+/-12.87% in B5 in induction by 4 microM/L ADP, and 88.5+/-11.47% in Bm and B5 in induction by 10 microM/L ADP. The results showed signifcantly elevated platelet aggregability than that of normal subjects(P<0.01). 5) The mean maximal platelet aggregability in patients with thrombotic disease was studied by Dipyridamole administration. The platelet aggregability induced by epinephrine before administration was 90.9+/- 8.52% and after administration it was 78.9+/-15.68%, and the results showed that Dipyidamole lowered aggregability significantly. The platelet aggregability induced by 4 microM/L ADP before administration was 84.0+/-11.90% in Bm and B5 and after administration it was 78.0+/-11.44% in Bm and B5, and the results showed that Dipyridamole lowered aggregability but not significant. The platelet aggregability induced by 10 microM/L ADP before administration was 89.2+/-10.39% in Bm and B5 and after administration it was 80.5+/-8.44% in Bm and B5, and the results showed that Dipyridamole lowered aggregability significantly. 6) The mean maximal platelet aggregability in patients with thrombotic disease was studied by Aspirin administration. The platelet aggregability induced by epinephrine before administration was 91.0+/-4.79% and after administration it was 47.6+/-17.72%. The platelet aggregability induced by 4 microM/L ADP before administration was 84.6+/-10.37% in Bm and B5 and after administration it was 72.6+/-11.85% in Bm and 65.3+/-15.97% in B5. The platelet aggregability induced by 10 microM/L ADP before administration was 84.9+/-6.30% in Bm and B5 and after adminstration it was 77.7+/-8.60% in Bm and 75.0+/-8.89%. The results showed that Aspirin lowered aggregability markedly. 7) The mean maximal platelet aggregability in patients with thrombotic disease was studied by combined administration of Aspirin and Dipyridamole. The platelet aggregability induced by epinephrine before administration was 86.7+/-13.77% and after administration it was 36.7+/-14.01%. The platelet aggregability induced by 4 microM/L ADP before administration was 81.5+/-12.93% in Bm and 80.6+/-14.15% in B5 amd after administration it was 54.7+/-17.27% in Bm and 44.6+/-21.17% in B5. The platelet aggregability induced by 10 microM/L ADP before administration was 87.8+/-10.11% in Bm and B5 and after administration it was 65.7+/-13.59% in Bm and 62.0+/-16.42% in B5. The results showed that combined administration of Aspirin and Dipyridamole lowered aggregability significantly and the results were lower than that of normal subjects. 8) The effects of combined treatment of Aspirin and Dipyridamole showed marked reduction of platelet aggregability than that of single treatment of Aspirin or Dipyridamole in thrombotic disease.

3.The mRNA Expression of Phosphoinositide-specific Phospholipase C-beta Isozymes in Postnatal Rat Brains.

Phosphoinositide-specific phospholipase C(PLC) is known as a key enzyme which produces two major second messengers: diacylglycerol and inositol 1,4,5 trisphosphate. Although it has been suggested that PLC beta isozymes have important roles in nervous system, less is known about the function of PLC beta in development of nervous system. We have localized the mRNA expressions of PLC beta isozymes in the postnatal rat brains by id firm hybridization histochemistry. In the postnatal rat brains, each isozyme of PLC beta showed differential expression pattern. The expression of PLC beta1 mRNA was found in various areas including olfactory bulb, cerebral cortex, caudate putamen, hippocampus, dentate gyrus, and cerebellum. In general, the expression in these areas was gradually increased after birth (PO) until postnatal day 21 (P2l) and slightly decreased to adult level. The expression of PLC beta2 mRNA was not found in postnatal rat brains. The expression of PLC beta3 mRNA was found from P0, peaked at Pl4, and decreased to adult level in the purkinje cells of cerebellum. PLC beta4 mRNA was strongly expressed in the thalamus, cerebellum, cerebral cortex, and olfactory bulb. In these areas, the expression was gradually increased after birth, peaked at P2l, and decreased to adult level. In whole body parasagittal sections of 18 day old rat embryo, PLC betal mRNA was exclusively expressed in nervous tissue, PLC beta3 and PLC beta4 were expressed in various tissues, and the expression of PLC beta2 was not found in any kind of rat tissues. From the different spatiotemporal mRNA expression patterns of PLC beta isozymes in the postnatal rat brains, it is suspected that each PLC beta isozyme may have specific role in signal transduction for postnatal development of rat brain.

4.Relationship Between Initial Biochemical Bone Markers and Change of Bone Mineral Density of Postmenopausal Women with Short-term Hormone Replacement Therapy.

OBJECTIVE: This study is proposed to examine the relationship between bone loss after hormone replacement therapy for one year and initial bone markers. METHODS: One hundred postmenopausal women were studied for one year. At first visit, measurements were made of bone mineral density(BMD) at the lumbar spine and femoral neck, and of serum osteocalcin and urine deoxypyridinoline. After hormone replacement therapy was done for one year, BMD was rechecked. RESULTS: Serum osteocalcin was correlated with the rate of bone change(r=0.150, 0.262) and urine deoxypyridinoline was weakly correlated(r=-0.003, 0.038). The changes of femoral BMD in women with higher concentration of osteocalcin and deoxypyridinoline were significant different from those in women with normal concentration. At the lumbar spine, no significant differences were showed. In women with higher concentration of osteocalcin, the incidence of subjects with increased spine BMD was increased up to around 71.1%, which showed not significant increase compared with normal concentration of osteocalcin. And the incidence of increased spine BMD in women with higher concentration of osteocalcin was marked increased compared with the incidence of normal concentration. The level of the osteocalcin with increased spine BMD was 11.5+/-6.6 ng/ml, and the level with decreased spine BMD was 9.2+/-4.7 ng/ml, respectively. And the level of the deoxypyridinoline was 7.2+/-4.2 pmol/ mol cr in women with increased spine BMD and 7.2+/-2.7 pmol/ mol cr in women with decreased spine BMD, which was not statistically different. The concentration of deoxypyridinoline with increased and decreased in BMD in spine and femur was not statistically different. but the concentration of osteocalcin was 12.2 +/-5.5 ng/ml in significantly increased in BMD and 8.5 +/-4.5ng/ml in significantly decreased in BMD, which was showed significant different. CONCLUSION: Present study indicates that the serum osteocalcin more reflcets bone turnover changes at the menopause than deoxypyridinoline, however, the clinical utility of measurements of osteocalcin only to response to therapy is uncertain.

6.A therapeutic Guideline of Gallbladder Polyps in Laparoscopic Era.

BACKGROUND/AIMS: The nature of gallbladder polyp is obscure before operation, and surgical indication is still controversial. The aim of this study is to suggest the therapeutic guideline for patients with the gallbladder polyps in the laparoscopic era. METHODS: Among 49 patients who received cholecystectomy because of gallbladder polyp from January 1, 1992 to December 31, 2000 in Department of Surgery, Seoul Municipal Boramae Hospital, we studied 42 histopathologically confirmed gallbladder polyp patients retrospectively. These patients were divided into two groups as true polyp(adenoma, carcinoma) and pseudopolyp(cholesterol, inflammatory, hyperplastic), and clinical characteristics were compared between the groups. RESULTS: The mean age was 48.19+/-11.09 years and male to female ratio was 17:25 in all patients. Of 42 patients, 32 pseudopolyps(76.2%) and 10 true polyps(23.8%) were confirmed by pathologic examination. Of 32 pseudopolyps, cholesterol polyps(29cases, 90.6%) were most common type. 4 carcinomas and 6 adenomas were included in 10 true polyps. Of 6 adenomas, coexistence with dysplasia was present in two cases. 1 case of carcinoma was present as arising from underlying tubular adenoma. Compared with pseudopolyp, true polyp showed significant larger size in diameter than psedopolyp(p=0.014). Despite of these statistical difference, 44% of pseudopolyps was more than 10mm in maximum diameter. 2 cases of true polyp(20%) was less than 10mm. However all of the other clinical features such as, the patients's age, sex, symptom, presence of gallstones, and number of polypoid lesion were not significantly different between the groups. All 4 carcinoma patients received laparoscopic cholecystectomy are disease free state with mean 68.5 months follow-up period. CONCLUSION: Although true polyps showed statistically significant larger than pseudopolyps in maximum diameter, there was no clear cut point dividing the two groups. All of the other clinical characteristics could not differentiated true polyps from pseudopolyps. Because of the low morbidity of laparoscopic procedure, premalignant potential of gallbladder adenoma and the adenoma-carcinoma sequence are unquestionable, an aggressive surgical intervention should be proposed in the era of laparoscopic surgery.

8.Comparison of Five Sampling Methods of Cervical Cytology in Premaligant Lesions of the Uterine Cervix.

Fourty three patients known to hnve cervical intraepithelial neoplasia were assigned to Papanicolaou smear in five different ways, i, e, A) cotion tipped wood applieator at the pasterior vaginal fronix and the ectocervix, B) cotton tipped wood applicator at the ectocervix and the endocervical canal, C) Cytohrush at the endocervical canal, D) Cervexbrusk and E) Cyto-spatula. At the completion of cytologic study, all the subjects were done colposcopically directed biopsy and/or ECC. To evaluate the false negative rates of the cytologic test, cervical cone biopsy or hysteectorny was performed on all the patient. The false negat,ive rates were 53.5% in A) 39.5% in B), 14,0% in C), l8.6% in D), 25.6% in E) and statistical differences occurect between each group, It was also found that bleedings were most frequently endountered in E) and the encocervical cells were most frequently appeared in C). With these results, it can he stated that the rate of negative encocervical cells in the srnear were correlated with the false negative rates of cervical cytology.
