1.Trichobezoar dectected by ultrasonography: case report.

The authors experienced a case of gastroduodenojejunal trichobezoar detected by ultrasongraphic examination. We thought that tricholbezoar had relatively specific ultrasonographic findings such as a broad hyperechogenic superficial bandlike rim with a complete posteior sonic shadowing and multiple linear echogenic strands on its surface representing hairs, which were better seen after water ingestion. So we were able to get the correct diagnosis of trichobezoar prior to conventional barium studies in a patient who could not be suspected suspected clinically.

2.A Case of Paroxysmal Atrial Fibrillation before Induction of General Anesthesia in an Elderly Patient.

Atrial fibrillation(AF) diagnosed by fibrillatory waves of atrial activity and an irregular ventricular response on ECG is a common arrhythmia associated with increased cardiovascular morbidity and mor- tality. The incidence of AF increases markedly with advancing age and the presence of congestive heart failure. Most patients with AF can be attributed to organic heart disease such as rheumatic, atherosclerotic and hypertensive heart disease. But there is an increasing awareness of so-called lone AF which has no underlying cardiac or metabolic abnormality. We report a case of paroxysmal AF developed just before the induction of anesthesia in a 73-year-old patient who had no history of cardiovascular disease.

3.A Case of Relapsed Acute Disseminated Encephalomyelitis.

Acute disseminated encephalomyelitis(ADEM) and acute relapsing disseminated encephalomyelitis(ARDEM) are representative demyelination diseases that occur among young children with a fulminant onset similar to encephalitis or meningitis. The diseases often occur after some viral infection of immunization and the etiology of these diseases is considered to be an autoimmune response because of the similarity in pathologic findings to experimental allergic encephalomyelitis. Cerebral computed tomography(CT) findings of demyelination in ADEM or ARDEM show normal to low density areas in the white matter. In cerebral MRI findings, a scattered distinct high intensity lesion considered to be demyelination is observed in 72-weighted imaging even in the early stages. ADEM is usually monophasic, but recurrent episodes may occure. When ADEM is reccurent, the distinction from multiple sclerosis becomes difficult. We report here a case of acute relapsing disseminated encephalomyelitis(ARDEM) in a 9 years old male child who experence ADEM, 3 times.

5.Percutaneous Balloon Mitral Valvuloplasty in Pregnancy.

BACKGROUND: Since percutaneous mitral valvuloplasty(PMV) using balloon was introduced by Inoue in 1984, this procedure has been accepted as a new non-surgical therapeutic modality for the treatment of selected patients with mitral stenosis. Pregnant women with mitral stenosis has suffered from life threatening complication due to altered hemodynamic changes and heart failure. Surgical valvotomy has been performed after failure of medical therapy with high risk of fetal mortality and teratogenicity. PMV can be an adequate alternative to surgical valvotomy in pregnant women. METHODS: After precise echocardiographic examination of mitral valve and its surrounding structures including thrombi in left artrium was performed, antegrade transseptal procedure was done in all 3 cases with abdominal shield using Inoue balloon technique. RESULTS: Mitral valve area increased over 1.5cm2 and hemodynamic variables improved immediately after PMV. Symptoms subsided soon after PMV in all 3 cases. Healthy normal babies were delivered transvaginally without difficulties in all 3 patients. CONCLUSIONS: Percutaneous mitral valvuloplasty is a safe and effective therapeuteic alternative to surgical valvotomy in a pregnant women with mitral stenosis who failed medical therapy, in a high risk or symptomatic pregnant women.

6.Neuroblastoma: Computed Tomographic Findings.

PURPOSE: To evaluate the characteristic CT findings of neuroblastoma, we studied neurobalstomas. METHODS AND MATERIALS: We analysed CT findings of available 25 cases among pathologically proved 51 neurobalstomas from Jan. 1983 to Sept. 1990. RESULTS: The most frequent site of origin is adrenl gland (40%) and the second is retroperitoneum (32%) and the third ismediastinum (16%). Characteristic CT findings are as follows:Calcifications within the tumor is detected in 86% of abdominal neuroblastomas and 50% of mediastinal origin. Hemorrhagic and necrotic changes within the tumor is noted at 86% in the tumor of abdominal origin and 25% in mediastinal neuroblastomas. Contrast enhanced study showed frequently septated enhanced appearance with/without solid contrast enhancement. Encasements of major great vessels such as aorta and IVC with/without displacement by metastatic lymphnodes or tumor are frequently seen in 90% of abdominal neuroblastomas. Multiple lymphadenopathy are detected in 95% of abdominal neurobalstomas and 25% of mediastinal neuroblastomas. The most common organ of contiguous direct invasion is kidney in 6 cases and the next one is liver but intraspinal canal invasion is also noted in 2 cases. CONCLUSION: We concluded that diagnosis of neuroblastoma would be easily obtained in masses of pediatric group from recognition of above characteristic findings.

7.Wilms' tumor:Changes of CT findings after chemotherapy.

When the tumor is advanced with distant metastasis or unresectable initially, preoperative chemotherapy could be applied in the treatment of Wilms' tumor We experienced 6 cases of favorable type of Wilms' tumor, 1 case of clear cell sarcoma and 1 case of renal cell carcinoma. They were treated with preoperative chemotherapy and underwent CT Scans before and after the therapy. Pathologic changes after chemotherapy in Wilms' tumor were known from previous reports as subtotal hemorrhagic necrosis, cystic change, clusters of foamy histiocytes, granulation tissue formation, primitive nephrogenic tissues and peripherally remained focal areas of blastemal infiltration. Changes of CT findings after chemotherapy were internal necrosis(6/6), decrease in size(5/6), decrease and absence of regional lymph node enlargement(4/6) and improved or disappeared metastatic lesions(3/3). Although our study had some limitations such as small numbers of cases and all cases were favorable types, we thought that there were good correlations between change of CT findings and subtotal hemorrhagic necrosis after preoperative chemotherapy in Wilms' tumor.

8.An Unusual Case of Pure Motor Variant of Guillain-Barre Syndrome Initially Presenting with Hemiparesis.

The pure motor variant of Guillain-Barre syndrome (PMGBS) has features of rapidly progressive, symmetric limb paralysis and areflexia, and lacks any sensory symptoms or signs. The PMGBS initially presenting with hemiparesis is very rare. We report a 59 year-old-male who presented with left-sided weakness 1 day prior to admission, and with right-sided weakness about 2 days following admission. In our case, clinical features and electrophysiological tests were consistent with an atypical PMGBS.

10.Immunohistochemical Characterization of Dendritic Cells after Systemic Administration of Staphylococcal Enterotoxin B.

This study was performed to investigate the effects of Staphylococcal enterotoxin B (SEB) on dendritic cells (DCs) and other immune cells in the major lymphoid organs. A single dose of SEB (25 microgram/kg) was administered to BALB/c mice by intraperitoneal injection. After the mice were sacrificed in groups of three at 2 h, 6 h, 1 day, 2 days, 3 days, 1 week and 2 weeks, the spleen, lymph node and thymus were removed. The immunocytochemical characterization of the cells was carried out using various monoclonal antibodies in cryostat-cut sections. We demonstrated in this study the distribution patterns of DCs and their major costimulatory and adhesive molecules in the murine spleen, lymph node and thymus after SEB administration. We obtained the evidence for maturation of DCs in vivo in response to SEB. DCs were found in increased number in the periarterial lymphatitc sheath (PALS) of spleen, paracortex of lymph nodes and thymic medulla. CD86, ICAM-1 and MHC class II molecules were upregulated on the activated and matured DCs after SEB injection. The most salient feature of the present study was the differential expression pattern of the costimulatory and adhesive molecules on the activated DCs. In addition to DCs, T cells expressing T cell receptor Vbeta8 were increased in number after SEB treatment. In conclusion, SEB exhibited a potent and effective stimulative effect on DCs in vivo.
