2.Circadian Variation of Non-Sustained Ventricular Tachycardia.

BACKGROUND: Circadian rhythms have been described for many cardiovascular phenomenon, both physiologic and pathologic. However, its frequency of onset in nonsustained ventricular tachycardia(NVT) has rarely been studied. Hence, this study was performed to examine the occurence of circardian rhythm in patients with NVT and to access underlying factors affecting it. METHOD: Among about 2000 patients who had undergone 24-hour Holter recordings between 1988 and August 1992, 46 patients with one or more ventricular tachyardia(VT) episode were selected. In this group, subjects receiving antiarrhythmic drug, including beta blockers were excluded. RESULTS: The frequency of onset of circadian rhythm in NVT peaked between 10 to 11AM and 6 to 7PM. In patiens younger than 60 years, its frequency was highest between 9 AM to 12 noon and 6 to 8 PM, whereas peak occurred around 9 AM in the morning in other age groups. In addition, circadian rhythm was more frequent in female NVT patients between 9 to 11 AM, while peaking between 6 to 8 PM in male patients. Moreover, higher frequency was shown during the day in patients with underlying heart disease, although the peak occurred in 9 AM and 8 PM in patients without any heart disease. CONCLUSION: In NVT patients, the frequency of onset of circadian rhythm varied with time of the day, showing nearly equal peaks in the morning and in the evening. This is similar to rnythms described for acute myocardial infarction and sudden death. Furthmore, the occurrence was affected by age, sex and presence of underlying disease.

3.Clinical Analysis of Factors Influencing the Development of Delayed Ischemic Neurologic Deficit after Aneurysm Surgery.

OBJECTIVE: This study shows the factors influencing the development of postoperative delayed ischemic neurologic deficit. METHODS: The authors analyzed 120 patients had performed aneurysmal neck clipping. Eleven variables were examined as to relationship to delayed ischemic neurologic deficit and classified as non- surgical and surgical variables. Data were analyzed by the univariate analysis and significant variables were entered into multiple logistic regression model in order to draw out Odds ratio. RESULTS: Delayed ischemic neurologic deficit after aneurysm surgery developed in 23 patients(19.2%). Significant non-surgical variables were clinical grade at admission(p=0.032), CT grade(p=0.005), and degree of preoperative angiographic vasospasm. Surgical variables were timing of surgery, brain retraction time, and duration of temporary clip application. The application time of temporary clip was significant (p=0.032) in development of delayed ischemic neurologic deficit. However, in multiple logistic regression analysis, duration of temporary clip application was excluded. With the clinical grade I - II and the CT grade I - II as the reference, the Odds ratio were 2.358(95% CI 0.87-6.37) and 4.041(95% CI 1.06-15.37), respectively. CONCLUSION: Delayed ischemic neurologic deficit after aneurysm surgery developed in poor clinical grade and high CT grade with high risk. The effect of surgery on the development of post-operative vasospasm was not significant.

4.Removal torque of bicortically stabilized RBM(resorbable blast media) pin implants in rabbit tibia.

STATEMENT OF PROBLEM: The use of small diameter implants having less than 3 mm in diameter were restricted because of lack of bonding strength to bone. PURPOSE: The purpose of this study was to observe how much resorbable blast media pin implants increase the binding force to the bone compared to machined transitional pin implants by measuring removal torque, and whether they can be used as final implants for replacement of small diameter teeth. MATERIAL AND METHOD: Fifteen rabbits were used in this study. Two kinds of implants (resorbable blast media pin implants and machined transitional pin implants) were inserted in each tibia bicortically. After healing time of 2, 4 and 8 weeks, the removal torque values were recorded and the rabbits were sacrificed for histological analysis. Linear finite element method analyses were conducted to compare bicortical fixation with monocortical fixation. RESULT AND CONCLUSION: Within the limitation of this in vivo study, the following conclusions were drawn: 1) The removal torque value of RBM pin implants showed statistically significant increase compared to machined pin implants at 2, 4, and 8 weeks respectively (p<0.05). 2) The removal torque value of RBM pin implants at 2, 4, and 8 weeks was increased statistically significantly with time (p<0.05). 3) Bicortical fixation showed better stress distribution compared with monocortical fixation in a linear finite element method analysis. 4) RBM pin implants are not recommended as transitional implants because they showed a lot of bone fracture in histologic specimens.

6.A simple, sensitive and non-destructive technique for characterizing bovine dental enamel erosion:attenuated total reflection Fourier transform infrared spectroscopy

Although many techniques are available to assess enamel erosion in vitro, a simple, non-destructive method with sufficient sensitivity for quantifying dental erosion is required. This study characterized the bovine dental enamel erosion induced by various acidic beverages in vitro using attenuated total reflection Fourier transform infrared (ATR-FTIR) spectroscopy. Deionized water (control) and 10 acidic beverages were selected to study erosion, and the pH and neutralizable acidity were measured. Bovine anterior teeth (110) were polished with up to 1 200-grit silicon carbide paper to produce flat enamel surfaces, which were then immersed in 20 mL of the beverages for 30 min at 37 °C. The degree of erosion was evaluated using ATR-FTIR spectroscopy and Vickers’ microhardness measurements. The spectra obtained were interpreted in two ways that focused on the ν1, ν3 phosphate contour: the ratio of the height amplitude of ν3 PO4 to that of ν1 PO4 (Method 1) and the shift of the ν3 PO4 peak to a higher wavenumber (Method 2). The percentage changes in microhardness after the erosion treatments were primarily affected by the pH of the immersion media. Regression analyses revealed highly significant correlations between the surface hardness change and the degree of erosion, as detected by ATR-FTIR spectroscopy (Po0.001). Method 1 was the most sensitive to these changes, followed by surface hardness change measurements and Method 2. This study suggests that ATRFTIR spectroscopy is potentially advantageous over the microhardness test as a simple, non-destructive, sensitive technique for the quantification of enamel erosion.
7.Evaluation of Lung Preservation by Using of Canine Bilateral Sequential Lung Tranplantation.

BACKGROUND: Numerous studies of safe, long term preservation for lung transplantation have been performed using ex vivo models or in vivo single lung transplantation models. However, a safe preservation time which is applicable for clinical use is difficult to determine. We prepared LPDG solution for lung preservation study. In this study we examined the efficacy of LPDG(low potassium dextran glucose) solution in 24-hour lung preservation by using a sequential bilateral canine lung allotransplant model. MATERIAL AND METHOD: Seven bilateral lung transplant procedures were performed using weight-matched pairs(24 to 25kg) of adult mongrel dogs. The donor lungs were flushed with LPDG solution and maintained hyperinflated with 100% oxygen at 10C degrees for a planned ischemic time of 24 hours for the lung implanted first. After sequential bilateral lung transplantation, dogs were maintained on ventilators for 3 hours: arterial resistance were determined if the recipients hourly after bilateral reperfusion and compared with pretransplant-recipient values, which were used as controls. After 2hours of reperfusion, the chest X-ray, computed tomogram and lung perfusion scan were performed for assessmint of early graft lung function. Pathological examinations for ultrastructural findings of alveolar structure and endothelial structure of pulmonary artery were performed. RESULT: Five of seven experiments successfully finished the whole assessments after bilateral reperfusion for three hours. Arterial oxygen tension in the recipients was markedly decrased in immediate reperfusion period but gradually recovered after reperfusion for three hours. The pulmonary artery and pulmonary vascular resistance showed singificant elevation(p<0.05 versus control values) but also recovered after reperfusion for three hours(p<0.05 versus immediate period value). The ultrastructural findings of alveolar structure and endothelial structure of pulmonary artery showed reversible mild injury in 24 hours of lung perservation and reperfusion. CONCLUSION: This study suggests that LPDG solution provides excellent preservation in a canine model in which the dog is completely dependent on the function of the transplanted lung.

8.External Traction Technique for the Adjuvant Treatment of Zygomaticomaxillary Complex Fracture.

The fixation of zygomaticomaxillary complex fracture is accomplished by the various internal placement of interosseous wiring, miniplates, microplate and even absorbable miniplate across properly reduced fracture lines to resist torsional rotation and masticatory force. However, in cases of comminuted compound fracture which makes zygomaticomaxilary complex fixed unstably, asymmetric malar height due to incomplete intraoperative reduction and predictable re-depression of zygoma postoperatively, we can hardly solve the malar asymmetry only by internal fixation device. So we employ the new external traction device which is made of hard board paper, cast, Steinmman pin, and rubber band. It is easy to make and apply it to the patient and morever, it is a dynamic traction method because it can control of traction force by rubber band and control of traction vector variously depending on degree of placement of fracture segment and fracture location, respectively. And it has less limitation to patient's hospital activity. Whereas it makes tiny traction pain during the traction device applied and the patient can feel uncomfortable head tightness. In summary, The authors' external rubber tration device can be used as a simple and convenient method in combination with internal fixation method in the postoperative management of malar symmetry.

9.Regional Lymph Node Involvement by Renal Angiomyolipoma.

Renal angiomyolipomas are benign mixed mesenchymal tumors, which are composed of atypical blood vessel, smooth muscle and fat cells. A 34-year-old woman presented with a palpable right abdominal mass, accompanied with discomfort. A CT scan showed a large right perinephric mass, with heterogeneous density, surrounding the kidney, with enlargement of the regional lymph nodes. The presumptive diagnosis was one of a malignant tumor of the retroperitoneum, with lymph node metastasis or an angiomyolipoma, involving both the kidney and lymph nodes. A right radical nephrectomy, including the tumor, was performed. The final diagnosis was an angiomyolipoma arising from the kidney, with the dissected lymph nodes having the same pathology. There was no evidence of a tumor recurrence during the six month follow-up period. This was an unusual case of a multicentric angiomyolipoma.

10.A Case of Huge Left Ventricular Thrombus Associated with Hypereosinophilic Syndrome.

Cardiac manifestations of hypereosinophilic syndrome rarely include left ventricular thrombosis leading to peripheral emboli. And the cases of thrombectomy in patients with left ventricular thrombus and hypereosinophilic syndrome are extremely rare. Recently we experienced a 58-years-old woman with hypereosinophilic syndrome, the history of thalamic infarction and a huge thrombi in left ventricle. We report this case with literatures.
