2.A Clinical Study of Squamous Cell Carcinoma Arising in Chronic Osteomyelitic Foci

Squamous cell carcinoma in chronic osteomyelitic foci is uncommon malignant tumor although well documented. This article deals with clinical study of 9 cases of squamous cell carcinoma arising in chronic osteomyelitic foci during past 9 years from April 1962 to March 1969 at the Dept. Orthopedic Surgery, Taegu Presbyterian Hospital. The diagnosis was confirmed by clinical symptoms, history, tissue and lymph node biopsy. Brief analysis of age and sex distribution, symptoms, physical findings and clinical management were recorded and summarized as follow: 1). This tumor may occur in any age group but all reported cases were after the age of 30. 2). Only 1 outof 9 cases was female. 3). The common clinical manifestations at the time of admission were draining sinus with foul odorous discharge, fungating mass and enlargement of regional lymph node. 4). More than 80% of our series occurred in the lower extremity, tibia acouting 55.6%. 5). 6 casses had epen fracture and duration of osteomyelitis was 28 years in average. 6). Biopsy included tissues from all portions of sinuses, a fungating mass and the tissues from the marrow space in case of deep extension. 7). The choice of treatment consisted of early adequate amputation or radical resection of the focus and irradiation.

3.Trichobezoar dectected by ultrasonography: case report.

The authors experienced a case of gastroduodenojejunal trichobezoar detected by ultrasongraphic examination. We thought that tricholbezoar had relatively specific ultrasonographic findings such as a broad hyperechogenic superficial bandlike rim with a complete posteior sonic shadowing and multiple linear echogenic strands on its surface representing hairs, which were better seen after water ingestion. So we were able to get the correct diagnosis of trichobezoar prior to conventional barium studies in a patient who could not be suspected suspected clinically.

4.Two Cases of Generalized Granuloma Annulare.

Two cases with generalized G.A. were described. The first case was 42 years, old farmer representing slightly erythematous multiple papulonodular lesions on the face, neck, scapular areas, arms and dorsal hands. Coalescence of lesions in some areas of the dorsal hands and neck produced circinate or polycyclic pattern. The second case was 4 years old girl who developed purple to brown muliple papulonodular lesions and polygonal or round plague on the arms, dorsal hands, palms, neck, iliac crests, lower legs, dorsal feet and sole. The plague lesions mimick lichen planus.

5.Effect of Sequential Embryo Transfer in vitro Fertilization.

OBJECTIVE: The objective of this study is to nfluence of sequential embryo transfers in an invitro fertilization was examined. METHOD: After in vitro fertilization, a maximum of 6 fertilized oocytes was enrolled in this study. At day 3 after an oocytes retrieval, embryos with good quality were transferred (mean 4.9), remaining embryos (mean 2.0/cycle) were cryopreserved at blastocyst stage (Group 1). At day 5 after oocytes collection, second a embryos had reached the blastocyst stage (Group 2) using P1 supplemented with 10 SSS and 30% Follicular fluid. No statistical difference in the pregnancy rate could be seen between the group without a second embryo transfer (n=21; 28.6%) and the group with a second transfer (n=52; 28.8%). RESULTS: The incidence of multiple pregnancy rate per embryo transfer was not statistically different between both group and no high-rank multiple pregnancy (greater than triplete) were observed (o.9%, 15.4%, respectively, p=0.74, chi2). Out of 114 cycles (506 embryos) cultured embryos in group 2, 52 cycles (159 embryos, 29.8%) reached the blastocyst stage. CONCLUSION: The second transfer did not have a significant effect on the pregnancy rate. The most important factor for the pregnancy seems to be the quality of the embryos transferred on day 3 following oocyte retrieval. We recommend embryo transfer is performed only one, day 2~3 or D5.

10.The Significance of the Early Electroencephalographic Findings in Severely Asphyxiated Newborn Infants .

PURPOSE: Perinatal asphyxia occurring in newborn is one of the major causes of acute mortality and chronic neurological disability in survivors. We have studied the relationship between early electroencephalography(EEG) findings and clinical course and neurologic outcome in severe asphyxiated neonates. METHODS: Between the period of July 1999 and June 2002, 25 neonates who were diagnosed with severe perinatal asphyxia(1-minute Apgar score of < or =3 and initial pH is less than 7.2) at NICU in Dongsan Medical Center were enrolled. An EEG was recorded and analyzed within three days of life and divided into two groups - group 1(normal or focal change on EEG) and group 2(generalized abnormal EEG). Between the two groups, clinical courses and neurologic outcomes were compared. RESULTS: Fifteen infants(60%) were group 1 and ten infants(40%) were group 2(polyspikes, burst- suppression, generalized low voltage). Associated maternal disease, days of hospitalization, need for ventilator support, delay of oral feeding and convulsion duration are significantly higher and longer in group 2. Also, poor neurologic outcome(expire, developmental delay) was significantly higher in group 2(60%) than group 1(13.3%). CONCLUSION: Thus, the early neonatal EEG in asphyxiated newborn can be a predictable diagnostic tool in assessment of neurologic outcome.
