1.Lichenoid Drug Eruption due to Ethambutol.

A 73-year-old female patient, who had been treated with antituberculous drugs for 4 months, was referred to our department because of generalized macules and papules present for 15 days. Clinical examination revealed polygonal, flat papules of erythematous to violaceous hue on the entire skin. Histopathologic findings were similar to those of lichen planus. In a provocation test with ethambutol, the skin lesions were aggravated and the new lesions developed. We considered this case to be a lichenoid drug eruption due to ethambutol. The skin lesions gradually cleared after ethambutol was eliminated from the patient's tuberculosis therapy.

2.The Effect of Left Ventricualr Mass on the Transmitral Blood Flow.

Measurements of mitral flow velocity by pulsed Doppler echocardiography are very useful in evaluating left ventricular diastolic filling properties. In hypertensive patients, abnormalities of diastolic function may precede systolic abnormalities and may serve as a more sensitive marker of end organ damage. We estimated left ventricular nass by 2-D echo short axis area-length method and compared with peak mitral flow velocity in early diastole(PFVE)and during atrial systolic(PFVA). There was a significant increase of LV mass and LV mass indices in the hypertensive patients and PFVE/PFVA ratio was decreased in them. Aithough there was no relationship between blood pressure and PFVE/PFVA ratio, a significant relationship was demonstrated between LV mass index and PFVE/PFVA ratio in the hypertensive patients.

3.Stability of Pin Fixation of Displaced Supracondylar Fractures of Humerus in Children

Fifty two cases of displacecd supracondylar fractures were divided into three groups according to the methods of fracture stabilization. Cast immobilizations without pin fixation were included to the group 1 (12 cases), fixation with lateral pins to the group 2 (22 cases), and lateral and medial cross pins to the group 3 (18 cases). In each group we attempted a radiological analysis for the stabilization failure through measurements of the horizontal rotation and the mediolateral tilting in their preoperative, postoperative and follow-up X-rays. Stabilization failure during the follow-up period was presumpted to be present in those cases that show (1) a development or increase of the horizontal rotation in follow-up lateral X-ray, and (2) accompanied change in Baumann's angle of more than 5 degrees. The failed stabilizations were in three cases (25%) in the group 1. In the group 2 the horizontal rotations after initial fixation were developed in four fractures. However, only two (9%) of the four cases accompanied significant changes in the Baumann's angle. Loss of the initial fixation in the cross pin group was found in one case. These results demonstrate that the cross pin fixation is the most stable form. Fixation in only lateral side sometimes shows rotational instability, but this method seems to be still useful because singificant coronal tilting develops in only a few occasions of those with horizontal rotation.

5.A Positive Association between the Atherogenic Index of Plasma and White Matter Hyperintensity

Background:
White matter hyperintensity (WMH) is a risk factor for dementia and ischemic stroke. The atherogenic index of plasma (AIP) is a simple and cost-effective marker for the prediction of various vascular diseases. In this study, we evaluated the relationship between AIP and WMH in adults without cerebrovascular accidents.
Methods:
We analyzed the data of 281 adults, aged ≥26 years, who underwent brain magnetic resonance imaging (MRI) at the health promotion center of an education hospital between January 2014 and December 2018. Participants were divided into three categories according to tertiles of the AIP scores (T1: <0.20; T2: 0.20–0.48; and T3: >0.48). WMH was defined as a modified Fazekas scale score of 1–3 on brain MRI. A cubic spline curve was used to determine the linearity of the relationship between AIP and WMH. Multiple logistic regression analysis was used to evaluate the relationship between the AIP and WMH.
Results:
The prevalence of WMH was 45.7% in T1, 57.0% in T2, and 66.0% in T3 (T3 vs. T1, P for post-hoc analysis=0.005). The increased odds of WMH were associated with increased AIP. The odds ratio (OR) with a 95% confidence interval (CI) for WMH of T2 and T3 compared with T1 were 1.57 (0.88–2.80) and 2.30 (1.28–4.14), respectively. After adjusting for confounding variables, the OR with a 95% CI for WMH in the T2 and T3 groups vs. the referent T1 were 1.55 (0.76–3.13) and 2.27 (1.06–4.84), respectively.
Conclusion
AIP is independently and positively associated with WMH in a healthy population.
6.A Case Report of Symptomatic Torus Palatinus.

PURPOSE: Torus palatinus is a bony prominence at the middle of the hard palate. The size varies from barely discernible to very large, from flat to lobular. This oral exostosis is not a disease or a sign of disease, but if large, may be a problem. So, we present the clinical and histopathologic features and applied therapy and provide a comprehensive review of the rare case of the symptomatic exostoses. METHODS: A 37-year-old woman had slowly growing exophytic nodular mass of the bone that arises the midline suture of the hard palate. The patient was concerned about discomfort associated with movement of her tongue and about frequent irritation of the palatal mucosa during mastification of the hard food. The patient had a large, unilobulated torus palatinus. It extended from the area adjacent to the canine to a point beyond the junction with the soft palate. The mass was oblong in shape, measuring about 3cm long, 2cm wide, and 0.8cm in height. RESULTS: Before surgical intervention a CT was obtained for the sake of estimating the thickness of the bone between the exostoses and the maxillary antrum and floor of the nose. The surgical procedure was performed with the patient under general anesthesia. Removal of the exostosis was performed after midline mucoperiosteal incision with osteotome and diamond burr. Histologic finding revealed decalcified dense bony tissue, the presence of lacunae, and normal osteocytes. CONCLUSION: Surgical removal is recommended when one or more of the following condition exist: interference with the construction of prosthesis, interference with oral function, irritation or pathology of the overlying tissue, inability of the patient to maintain proper oral hygiene, and fear of malignancy or other psychologic trauma. We report a rare case of the torus arising in hard palate with symptoms.

7.Occupational Injury Statistics in Korea.

OBJECTIVES: The occupational accident rate was officially reported to be 0.77 per 100 workers in 2001 and 0.70 in 2009. The stagnant decrease in accident rate raises a question about the effectiveness of prevention activity because there have been active prevention efforts in the past 10 years. It is also necessary to know the exact status of occupational injuries to direct a prevention strategy. METHODS: The author re-analyzed occupational injury statistics to find the reason for stagnant decreases in occupational injuries. Compensated occupational injuries cases were used to calculate fatal and non-fatal injury rates. Injuries from commuting accidents and sports activities were excluded as well as occupational diseases. The number of workers was adjusted to that of full time equivalent employees. RESULTS: The fatal injury rate excluding injuries associated with commuting accidents, sports activities, and occupational diseases decreased from 12.59 in 2001 to 8.20 in 2009. In 2007, 67.5% of accidents that involved being caught in objects, which are mostly caused by machines and equipment, occurred in the manufacturing industry; this type of incident has decreased since 2001. The fatal and non-fatal injury rates in the manufacturing industry have continuously decreased while the rates in the service industry have not changed from 2001 to 2009. Non-fatal injuries might not be reported in many cases. The number of insured workers was underestimated as long working hours were not adjusted for in the reporting system. CONCLUSION: The occupational fatal injury rate has decreased and the non-fatal injury rate might have decreased during the last 10 years, although the statistics show stagnancy. The decrease of the injury rate was countervailed by various factors. Hence, the current accident rate does not reflect the actual situation of accidents in Korea. Korea needs to develop an improved system to more accurately calculate occupational fatal and non-fatal injury rates.

8.A Case of Stenotrophomonas Maltophilia Endophthalmitis after Cataract Operation.

It has been reported that the cause of the bacterial endophthalmitis following cataract extraction and intraocular lens implantation was G(+) organisms in most cases. However, the endophthalmitis caused by G(-) organisms has worse prognosis than that by G(+)organisms. We experienced a case of Stenotrophomonas maltophilia endophthalmitis which has not yet been reported in Korea. A 77-year-old female presented with blurred vision, conjunctival injection and ocular pain 5 days after cataract extraction and intraocular lens implantation. Ocular findings such as corneal edema, hypopyon and thick posterior capsular opacity were consistent with infectious endophthalmitis. Culture of vitreous aspirate isolated Stenotrophomonas maltophilia 15 days after cataract extraction.

9.Evaluation of the CellaVision Advanced RBC Application for Detecting Red Blood Cell Morphological Abnormalities

Background:
The Advanced RBC Application of the CellaVision DM9600 system (CellaVision AB, Lund, Sweden) automatically characterizes and classifies red blood cells (RBCs) into 21 morphological categories based on their size, color, shape, and inclusions. We evaluated the diagnostic performance of the CellaVision Advanced RBC Application with respect to the classification and grading of RBC morphological abnormalities in accordance with the 2015 International Council for Standardization in Haematology (ICSH) guidelines.
Methods:
A total of 223 samples, including 123 with RBC morphological abnormalities and 100 from healthy controls, were included. Seven RBC morphological abnormalities and their grading obtained with CellaVision DM9600 pre- and post-classification were compared with the results obtained using manual microscopic examination. The grading cut-off percentages were determined in accordance with the 2015 ICSH guidelines. The sensitivity and specificity of the CellaVision DM9600 system were evaluated using the manual microscopic examination results as a true positive.
Results:
In pre-classification, > 90% sensitivity was observed for target cells, tear drop cells, and schistocytes, while > 90% specificity was observed for acanthocytes, spherocytes, target cells, and tear drop cells. In post-classification, the detection sensitivity and specificity of most RBC morphological abnormalities increased, except for schistocytes (sensitivity) and acanthocytes (specificity). The grade agreement rates ranged from 35.9% (echinocytes) to 89.7% (spherocytes) in pre-classification and from 46.2% (echinocytes) to 90.1% (spherocytes) in post-classification. The agreement rate of samples with withinone grade difference exceeded 90% in most categories, except for schistocytes and echinocytes.
Conclusions
The Advanced RBC Application of CellaVision DM9600 is a valuable screening tool for detecting RBC morphological abnormalities.
