4.Primary Bone Tumors in Hindfoot.

PURPOSE: Primary bone tumors of hindfoot are uncommon compared with other locations, and there have been few large-group studies. This study was designed to analyze the characteristics and the clinical results of the primary bone tumors of hindfoot. MATERIALS AND METHODS: Forty five cases in 44 patients who have been diagnosed from 1989 to 2011 were reviewed. The minimum follow-up period was 1 year. We retrospectively reviewed the medical records and images. RESULTS: Twenty six cases were male and 18 cases were female. Mean follow-up period was 33.1 months and mean age was 25.1 years. Forty four cases were benign and 1 case was malignant. Thirty six cases occurred in calcaneus and 9 cases were in talus. The most common benign bone tumor was simple bone cyst (20 cases), followed by intraosseous lipoma (12 cases), and chondroblastoma (4 cases). In calcaneus, there were 18 cases of simple bone cyst, and 12 cases of intrasosseous lipoma. In talus, there were 3 cases of chondroblastoma, 2 cases of simple bone cyst, and 2 cases of intraossesous ganglion. Many patients with hindfoot bone tumors presented with pain, but some were found accidentally. Patients received surgical procedures, such as curettage and bone graft, open reduction and internal fixation, tumor resection, and below knee amputation. CONCLUSION: Primary bone tumors of hindfoot are rare and can be misdiagnosed as ankle sprain or contusion. Although most are benign, malignant tumors cannot be ruled out, so early diagnosis and appropriate treatment is important.

6.Detection of Major bcr/abl mRNA from Stored Bone Marrow Aspirate Smears Using the Reverse Transcriptase Polymerase Chain Reaction.

BACKGROUND: The verse transcriptase polymerase chain reaction (RT-PCR) has been widely used to analyze the bcr/abl fusion mRNA in chronic myelogenous leukemia (CML). Fresh or cryopreserved cells may not always be available for molecular diagnosis. So we investigated the value of stored bone marrow aspirate smears as the sources of material for the detection of bcr/abl mRNA. METHODS: We extracted RNA using modified Chomczynski method, and amplified bcr/abl mRNA by RT-PCR from the 70 cases of bone marrow smear slides stored from 7 days to 7 years, which were comprised of 49 CML, 11 other chronic myeloproliferative disorders (CMPD) and 10 acute lymphoblastic leukemia (ALL). Sensitivity of RT-PGR was tested using the slide smears prepared with 10(0)-10(6) K562 cells, and RT-PCR results losing each fresh bone marrow cellular suspension and slide smears in 24 patients were compacted. For major bcr/abl rearrangement, RT-PCR was performed by nested PGR afters GDNA synthesis losing downward primer and beta2-microglobulin was used as RNA controls. RESULTS: The sensitivity of RT-PCR for detecting bcr/abl mRNA was l02 cells per slide. Sixty one cases (86%) of 70 bone marrow aspirate smears showed positive results of beta2-micyoglobulin cDNA as an indicator of intact RNA. Thirty nine cases of 42 beta2-microglobulin cDNA positive CML bone marrow aspirate smears showed 29 b3a2 type mRNA and 10 b2a2 type mRNA. Nine cases of 11 bone marrow aspirate smear with other CMPD showed negative results of bcr/abl mRNA. Two cases of 10 ALL bone mallow aspirate smears had b2a2 type mRNA and b3a2 type mRNA, respectively. The results for detection of bcr/abl mRNA with fresh cell suspensions of 24 patients were same as the bone marrow aspirate smears storied for 7 days to 1 year. CONCLUSIONS: These data indicated that RNA obtained from bone marrow smears storied for less than 1 year was valuable as the source of RT-PGR for the detection of bcr/abl mRNA in CML and the bone marrow smears stored for much longer period ould be assailable as the specimens for retrospective analysis of specific gene alter-ation in other hematologic malignancy.

7.Multiple Rice Body Formation in Subacromial and Subdeltoid Bursal Spaces.

We encountered a rare condition of rice body in subacromial and subdeltoid spaces in a patient with unremarkable medical history. Although it is uncommon, there have been continued reports on its formation in certain type of infective and inflammatory arthritis. However, except for a traumatic event, evaluation yielded no known and conceivable cause for his chronic inflammatory bursitis. Relatively typical findings for rice body on magnetic resonance imaging have been described, and in our case the imaging prompted us to schedule early removal, which is generally accepted as the management of choice to prevent further progression of symptoms. The symptoms of the shoulder showed significant improvement, and a close follow-up schedule has been recommended for observation of recurrence and development of any foreseeable underlying cause.

10.The effect of decorinas an antagonist of tgf-betain fetal rat wound healing.

Adult wounds heal with scar-tissue formation, whereas fetal wounds heal without scarring and with a lesser inflammatory and cytokine response. The unique fetal wound repair process is not dependent on the sterile, aqueous intrauterine environment. The differences between fetal and adult wound healing appear to reflect processes intrinsic to fetal tissue, such as the unique fetal fibroblast, a more rapid and ordered deposition and turnover of tissue components, and, particularly, a markedly reduced inflammatory infiltrate and cytokine profile. Among these cytokines, the transforming growth factor-beta(TGF-beta) is a growth factor which plays an important role in the regulation of cell growth and differentiation. The fibrosis characteristic of adult wound repair may be associated with TGF-betaexcess. Recent experimental studies have focused on the specific anti-TGF-beta strategies for scarless wound healing. Decorin, a proteoglycan, is known to regulate TGF-beta. This factor antagonizes the action of TGF-betain tissues. However, little is known about the functions of this factor in vivo. The objects of the present study were to analyze the effects of TGF-beta, an important regulatory molecule in adult healing events, and the effects of decorin, known inhibitor against TGF-beta, on the fetal tissue response following wounding. Fetal cellular and extracellular matrix response to injury were evaluated by treating the wound with TGF-beta and decorin in fetal rat at 14 days gestation (term = 21 days). Histologic response and histomorphometric analysis two to eight weeks later were compared between TGF-betaonly treated wound and TGF-betawith decorin treated wound.The histologic finding of the TGF-beta treated wound was characterized by an early acute inflammatory response : by week 6 fibroblasts and collagen were predominant. In contrast, TGF-beta with decorin treated wound had no remarkable histologic evidence of acute inflammation or fibroblast penetration and few collagen was deposited. These observations demonstrate that the fetal response becomes adultlike with fibroblast proliferation and collagen accumulation when TGF-betais added, thus documenting the responsiveness of the fetal system to adult repair signals. Such responsiveness thus suggests a critical difference in the fetal wound environment. Fetal repair may proceed in the absence of trophic factors like TGF-beta, thus accounting for optimal "healing" in the absence of excessive fibrosis. And these observations also confirmed the inhibitory action of decorin against TGF-beta in rat fetus model. We can suggest that the decorin minimize the inflammatory response and subsequent cellular proliferation in wound healing process, thus eventually prevent collagen deposition and scar tissue formation.
