1.Safe margin of breast conserving surgery for breast cancer

Breast conserving surgery has become the first choice for the treatment of early breast cancer.However,the safety margin is still one of the important issues for surgeons in the implementation of breast conserving surgery.Because variety of clinical factors have some effects on the safety margin,at present,no consensus on safe margin of breast conserving surgery is reached,and the controversy still goes on.
2.Effects of axial stress on healing of lengthened callus

AIM: To explore the effects of axial stress regulation on healing of lengthened ca llus. METHODS: Tibiofibulars of 30 rabbits were lengthened 2 cm with t wice plane sreminobicular crib external fixator. Experimental group continues to lengthen 3 mm, and then were drawn and shortened 0.5mm once every 24 h separate ly for 10 days. Ten days later, tibiofibulars were axial short-stressed 1 mm wee kly with three times. Animals were killed 58 days after operation and X-ray, bon e density and histology tests were performed with lengthened bone. RESULTS: X-ray examination had no significant difference between two group animal when le ngthen ended, but X-ray showed lengthened callus was different on day 58. In con trol group, lengthened center had a serial callus and cortex had no formed, in experimental group, lengthened center had serial cortex and transfixial medullar y cavity expect serial callus. The syndrome of insulin-dependent diabetes mellitus(IDDM)includes not only abnor-mal glucose metabolism but also specific microvascular complications that include nephropathy,retinopathy and neuropathy.In the short term,improvement in the quality of life and possible prevention of further morbidity associated with diabetes makes pancreas trans-plantation an important therapeutic option,particularly when combined with a kidney trans-plant,in appropriately selected diabetic patients.The focus of the early management of the pancreas transplant recipient is the prevention of acute rejection.In this article we review the methods for clinical diagnosis of the early acute rejection due to pancreas transplanta-tion.
4.Pharmacokinetics of gastrodin from Tiangou Jiangya capsule in rats.

The paper aims to study the pharmacokinetic parameters of gastrodin in rats effected by compound compatibilitiy and different doses of Tiangou Jiangya capsule. The extracts from Gastrodiae Rhizoma( equivalent to gastrodin 16.82 mg x kg(-1) and Tiangou jiangya capsule (equivalent to gastrodin 8.410, 16.82, 33.64 mg x kg(-1)) were oral administrated to rats respectively. The plasma were taken at various time points and treated with acetonitrile to measure the contents of gastrodin by HPLC method. The mean plasma concentration-time data were analyzed by 3P97 pharmacokinetic software and the pharmacokinetic parameters between groups were treated by SPSS 16.0. The results showed that gastrodin in rat was fitted to one-compartment model, Cmax and AUC of Tiangou Jiangya capsule were in direct proportion to oral administration, and t1/2Ka had nothing to do with doses, which indicated that gastrodin was fitted first-order rate transfter process in vivo. Morever, comparison with the Gastrodiae Rhizoma extract, isodose gastrodin in Tiangou Jiangya capsule showed a significant decrease for Cmax, Ke and increase for t1/2Ke, V/Fc, this indicated that compound compatibility can delay the absorbtion of gastrodin, prolong the resident time and promote the distribution in vivo, but its bioavailability is not significantly effected.

6.Guide and prediction value of partial pressure of end-tidal carbon dioxide during cardiopulmonary resuscitation

Objective To study the clinical significance of end-tidal carbon dioxide partial pressure (PetCO2 ) during cardiopulmonary resuscitation (CPR) and seek the fixed value according which to decide if we should and when to give up.Methods This was a prospective,observational study.A total of 124 patients with cardiac arrest in or out-of-hospital from may 2003 to March 2009 in emergency department of our hospital were selected.All of them had definite etiological factors.Changes of PetCO2 in 124 cardiac arrest patients during CPR were tracked.Results The gender,age,rescue time in seventy-one patients with the return of spontaneous circulation (ROSC) after endotracheal intubation have a significant difference with that in fifty-three patients without ROSC (P <0.01 ).The PetCO2 of the survival were higher than that of patients without ROSC or with ROSC,but finally died (P <0.01 ).A fixed point 14.4 mmHg of PetCO2 after 20 minutes' CPR can be used as a reference value to guide CPR or predict prognosis.Conclusions Monitoring PetCO2 during CPR has a predictive value on the success of resuscitation.

BACKGROUND:No particularly effective medicine could treat human immunodeficiency virus 1(HIV 1) associated dementia(HAD) at present,which mainly because the mechanism of HIV 1 infection induced neural damage and necrosis is still not completely clarified. OBJECTIVE:To investigate the effects of human immunodeficiency virus I type(HIV 1) enveloped protein(EP) gp120 on the synaptic transmission and plasticity in hippocampal CA1 area of rat to clarify the mechanism of HAD generation. DESIGN:A paired design. SETTING:Department of Pathophysiology of Medical College of Jinan University. MATERIALS:The study was conducted in the Department of Pathophysiology(a tertiary national key laboratory of National Administration for Traditional Chinese Medicine,registration number: TCM 03 131) of the Medical College of Jinan University from January to October of 2003.Male SD rats aged between 2 and 5 weeks were used in the study. INTERVENTIONS:Brain slice perfusion and recording technique was employed. MAIN OUTCOME MEASURES:To record the excitatory postsynaptic potential(EPSP) in hippocampal CA1 area in rat;to investigate the effects of gp120 on long term potentiation(LTP) induced by high frequency electric stimulation in Schaffer lateral branch. RESULTS:gp120 had inhibitive effect on LTP in hippocampal CA1 area in rat [the average amplitude of LTP decreased from normal (216.1± 14.0) % to(90.8± 6.0) % ,n=12,P< 0.01],but had no effect on basic EPSP.PKA/PKC protein kinase(PK) inhibitor H7 could reverse this inhibitive effect [the average amplitude of LTP was(198.8± 16.2) % ,n=8,P< 0.01]. CONCLUSION:gp120 might participate the generation of HAD through inhibiting LTP in hippocampal CA1 area.
9.Atrial myxoma: a clinical and pathologic analysis

Fifteen cases with atrial myxoma in our hospital from May 2001 to Dec 2008 were retrospectively analyzed.Patients presented with a variety of symptoms mimicking mitral valve disease,including palpitation, shortness of breath and activity intolerance.Pulmonary and systemic embolizations occurred in three patients and sudden syncope in one.Radiograph and echocardiography can help to reveal atrial myxoma.Definite diagnosis was made based on histological examinations.In 6 months - 6 years'follow-up one case recurred.Surgical treatment should be prompted once diagnosis established.Tumor pedicle must be completely removed to prevent recurrence.
10.Inhibitory Effects of Circadian Gene Per2 on Human Pulmonary Carcinoma Cell

Objective To reveal the effects of circadian gene Per2 on growth of human lung cancer cell A549 cell and its mechanism. Methods pcDNA3.1(+)-Per2 was transfected into A549 cells and controlled with the vector pcDNA3.1(+). The expression of Per2 was confirmed with RT-PCR and western blot. The Inhibitory effect of Per2 on cell growth and proliferation was demonstrated with MTT assay and colony-forming assay. Apoptosis was detected with flow cytometry. Results The circadian gene Per2 exhibited a growth-inhibitory and apoptosis-inductory effect on A549 cells. Conclusion The results indicate that Per2 may inhibit A549 cell proliferation,which mechanism might involve cellular apoptosis.
