2.The Impact of Organizational Justice, Empowerment on the Nursing Task Performance of Nurses: Focused on the Mediating Effect of Job Satisfaction and Organizational Commitment.

PURPOSE: The purpose of this study is to analyze the structural model on the Nursing Task Performance of Hospital Nurses. METHODS: Data collection was done by the 200 hospital nurses from May 1st to May 20th, 2013 in Seoul city. The sample variance-covariance matrix was analyzed using AMOS 19.0 and the maximum likelihood minimization function. The goodness of fit was evaluated using the SRMR, RMSEA and its 90% confidence interval, CFI, and TLI. RESULTS: First, hospital nurses' organizational justice and empowerment was not found to have a significant direct effect on nursing task performance. Second, organizational justice and empowerment had a direct effect on job satisfaction and organizational commitment. Third, organizational commitment was found to have a significant direct effect on nursing task performance. Forth, hospital nurses' empowerment was found to have a significant direct effect on nursing task performance. CONCLUSION: Nurses' nursing task performance was influenced by organizational justice, empowerment, and organizational commitment. In order to increase the degree of nursing task performance in hospital nurse, it is necessary to develop the nursing program and its application considering hospital nurses' organizational justice and empowerment and organizational commitment.

5.A Case of Cutaneous Matestasis Originating from Thymic Carcinoma.

We report a case of cutaneous metastasis of thymic carcinoma in Korean. A 44-year-old female, who had been diagnosed as thymic carcinoma and treated with total thymectomy and post-operative radiation therapy, presented grouped erythematous papules on the left chest. CT scans revealed metastatic lesions in the left pleura and abdominal cavity. Histopathologic examination of skin biopsy showed extensive infiltration of malignant cells in dermis and subcutis. The morphology of infiltrating cells was similar to that of tumor cells from primary thymic carcinoma specimen. The results of immunohistochemical study for skin biopsy were consistent with the features of non-neuroendocrine thymic carcinoma. Histopathologic and immunohistopathologic similarity between cutaneous lesion and previous tumor suggested that malignant cells in the skin lesion could originate from progressing thymic carcinoma.

6.Two Cases of Pigmented Fungiform Papillae of the Tongue.

Pigmented fungiform papillae of the tongue(PFPT) is a rare benign pigmentary disorder of the tongue. In dark-skinned individuals, PFPT appears to be relatively common, whereas this entity is seldom reported in fair-skinned people and Asians. Although the relevance to systemic conditions has been suggested in several reported cases, the etiology and pathogenesis of this entity are not yet clearly defined. We report two cases of PFPT in young Korean females. Clinically both patients presented with asymptomatic pigmentary lesions confined to fungiform papillae of the tongue, which had been detected several months ago. Histopathologic findings revealed increased melanophages in the subepidermal area within the fungiform papillae.

7.A case of Nutcracker Syndrome Associated with Proteinuria.

We evaluated the availability of toluidine blue stain in body fluids, such as peritoneal and pleural fluid and urine. Nine hundreds specimens, i.e., 400 pleural and 400 peritoneal fluids and 100 urine samples, respectively, from Jan. 1995 to May 1996 were included. We obtained the result of high sensitivity and high specificity in toluidine blue stained body fluid in comparison with Papanicolaou stained result. Additionally, we found the diagnostically important crystals in chylothorax and some urine samples, which can not be seen in routine Papanicolaou stain. We thought the toluidine blue stain in body fluid is one of very useful diagnostic methods.

9.The Clinical Characteristics of Initial Drug Resistance in MDR-TB Patients.

BACKGROUND: Multidrug-resistant tuberculosis(MDR-TB) in patients is mainly caused by acquired drug resistance, However, a small proportion of MDR-TB is caused by initial drug resistance(IDR), which may be somewhat different from acquired drug resistance. This study analyzed the clinical characteristics of IDR in MDR-TB patients to use the results as basic data in managing the disease. METHODS: A retrospective study of 30 IDR cases in MDR-TB patients from Jan. 1995 to Dec. 1998 was perormed. In order to analyzed the clinical charcteristics, the age, sex, family history, duration of negative conversion, number of resistant drugs, treatment regimens, duration of treatment, extent of disease and cavitary lesion on the chest X-ray was examined. In order too analyzed the level of improvement, the extent of the disease and cavitary lesion on the chest X-ray, tested by Wilcoxon signed rank sum test, and the disease free interval rate of 1-year and 4-year was examined using the Kaplan-Meier method. RESULTS: The mean age of the patients was 46.6 years and the sex ratio 1:1. Six(20%) patients had a family history. The mean negative conversin of the sputum AFB stain was 2.6 months. The number of resistant drugs was 7.6 and the number of used drugs 3.6. Twenty-three(67%) patients were treated for less than 12months and 28(93%) patients were treated with first-line drugs. The extent of the disease and the cavitary lesion on the chest X-ray improved after treatment(p<0.05). Among 13 patients who were followed up for 22.6 months, 2(15%) patients relapsed and the disease free interval rate of 1-year and 4-year was 85%. CONCLUSION: It is recommended that the duration of treatment of IDR in MDR-TB with first-line drugs be 9-12 months even if the extent of disease and cavitary lesion on the chest X-ray improves.

10.Ureteral Stricture from Retroperitoneal Fibrosis Caused by Isolated Common Iliac Artery Aneurysm .

Retroperitoneal fibrosis sometimes causes urological problem involving the ureter, but the mechanism is uncertain. An aortic aneurysm, including an iliac artery aneurysm, is thought to be one of the mechanisms of retroperitoneal fibrosis. However, cases caused by an isolated iliac artery aneurysm are very rare, and symptoms tend to be non-specific; therefore, no definitive treatment has been established. Herein, we report our recent experience of a patient with a left common iliac artery aneurysm involving the ureter, who underwent successful surgical therapy.
