1.Clinical significance of early monitoring of serum PCT and IL-6 in patients with lower limbs long bone open fractures

Objective To explore the clinical significance of combined detection of serum procalcitonin (PCT)and interleukin 6 (IL-6 )in patients with lower limbs long bone open fractures before and after emergency debridement fixation and postoperative reg-ular antibiotics treatment.Methods Patients with lower limbs long bone open fractures within 8-24 h after injury were divided in-to wounds fester group,wounds contamination group and wounds non-contamination group.Serum PCT and IL-6 levels were detec-ted and compared between patients group and control group.Results Before treatment,PCT and IL-6 levels in wounds contamina-tion group and wounds fester group were higher than control group (P<0.05 or P<0.01),but those in wounds non-contamination group were not different with control group (P>0.05).After treatment,PCT and IL-6 levels in wounds fester group and wounds contamination group were decreased,but those in non-contamination group were with a trend of fluctuation.In the first day of treatment,PCT and IL-6 levels in contamination and fester group were higher than control group (P<0.05 or P<0.01),but there was no significant differences,compared with the levels before treatment(P>0.05).PCT and IL-6 levels in non-contamination group,compared with control group and levels before treatment,were not significantly different (P>0.05).In the third day of treatment,PCT and IL-6 levels in the three patient groups were higher than control group (P<0.05 or P<0.01).PCT and IL-6 levels in fester and contamination group were not significantly different with the levels before treatment (P>0.05).PCT and IL-6 levels in non-contamination group were significantly higher than the levels before treatment (P<0.05).In the fifth day of treat-ment,with the improvement of disease condition,PCT and IL-6 levels decreased significantly.PCT and IL-6 levels in fester and contamination group were significantly higher than control group (P<0.05),but lower than the levels before treatment (P<0.05).PCT and IL-6 levels in non-contamination group were higher than control group and the levels before treatment (P>0.05). Conclusion Wounds infection could onset in patients combined with lower limbs long bone open fractures and soft tissue open inj u-ry.Dynamic monitoring of PCT and IL-6 levels might be significant for early prediction of bacteria infection,rational use of antibi-otics and promote healing of wounds and fractures.
2.Performance Evaluation of Alarms and Parameters Generated by Hematology Analyzer during Detection of Circulating Sezary Cells of Peripheral Blood Smears

Objective To evaluate the performance alarms and parameters generated by hematology analyzer during detection of circulating sezary cells of peripheral blood smears.Methods Blood samples from patients with Sezary syndrome (n=21) were studied with Sysmex XN-9000 analyzer,and compared to patients with benign or tumoral skin lesions (n=48) and patients with chronic lymphoproliferative B-cell diseases (n=55) and normal person (n=45) used as control.Results In present series,the value of structural lymphoid parameters (ly-x and ly-z) and the alarm Blast/Abn Lympho were statistically higher in Sezary cases than in control cases (U=37,40,44,55,38,41,all P<0.05).Screening Sezary cells critical values of ly-x and ly-z determined by ROC curve were greater than or equal to 86 and was more than or equal to 68.The combination of above three indexes up (i.e.alarm Blast/Abn Lympho> 100,ly-x=86 and ly-z>68) sensitivity and specificity respectively for 100 % and 92.1 %,in order to get the best screening Sezary cells diagnostic efficiency.In addition,the value of ly-x was associated to the count of circulating Sezary cells and value of ly-z to the presence of large Sezary cells,both parameters described as prognostic factors.Conclusion The combination of alarm Blast/Abn Lympho and structural parameters (ly-x/ ly-z/ly-y) may allow to define rule of blood slide review to screen circulating Sezary cells.
3.Performance Evaluation of the CellaVisionTM DM9 6 Analyzer During Detection of Intracellular Parasites of Peripheral Blood Smears

Objective To evaluate the performance of the CellaVisionTM DM96 analyzer during detection of intracellular para-sites of peripheral blood smears.Methods This study compared the detection rates of Plasmodium and Babesia species on peripheral blood smears utilizing the CellaVision DM9 6 with the rates for a routine red blood cell morphology scan.A total of 307 slides were analyzed,consisting of 142 slides positive for Plasmodium or Babesia species and 165 negative controls. Slides were blinded,randomized,and analyzed by CellaVision 9 6 and microscopy for red cell morphology scans.The technolo-gists were blinded to prior identification.Results The examination of common technician and blood professional technician, whether using optical microscope or CellaVision DM9 6 could accurately identify all the negative control,its specificity was 100%.The sensitivity of blood professionals using CellaVision DM96 was not statistically significant in the sensitivity of the laboratory and the use of light microscopy for the detection of Plasmodium parasites (χ2=0.24,0.03,0.43,P>0.05).The higher percentage of Plasmodium in blood smear,the higher detection rate of CellaVision DM9 6 .When the blood smear Plas-modium white rate greater than 1%,only 2 contain Plasmodium falciparum samples were negative for the miscarriage of j us-tice.The blood smear percentage was higher than 2% CellaVision of Plasmodium DM96 will all the blood smear 100% cor-rect interpretation.Conclusion This study demonstrates that CellaVision had the potential to detect intracellular parasites in routine screening of blood smears resource for storing images to be used for educational purposes,including training technol-ogists in the detection of intracellular parasites.
4.Reliability evaluation of the blast/abnormal lymphocyte alarm tested through white precursor cell (WPC) channel on the Sysmex XN-9000 analyzer

Objective To evaluate the reliability of the white precursor cell (WPC) channel on the Sysmex XN-9000 hematology analyzer (XN-9000) in giving the abnormal white blood cell (WBC) alarm.Methods A total of 520 blood samples with abnormal WBC alarm from the Sysmex XE-2100 (XE-2100) and XN-9000 were performed the manual peripheral blood smear review,and their results were analyzed.Results The specificity of the XN-9000 WPC channel in the blast alarm was significantly higher than that of XE-2100 (x2 =6.98,P<0.05),while the false positive rate of the XN-9000 (36%) significantly lower than that of XE-2100 (64%,x2 =6.18,P < 0.05).The overall efficiency of the XN-9000 WPC channel in identifying blasts and abnormal lymphocytes (90.4% and 94.4%) was significantly higher than that of the XE-2100 (72.2% and 80.1%,x2 =5.6 and 6.33,P < 0.05).Under the automatic reflex analysis mode of WPC channel,the specificity and overall efficiency of the WPC channel in identifying blasts and abnormal lymphoeytes of all the detected samples were significantly higher than those of the WDF channel (x2 =6.15,4.14,6.02 and 4.88,P <0.05).The total review rate of the XN-9000 (WDF/WPC) was significantly lower than that of the XE-2100 (x2 =4.33,P < 0.05).Similarly,the review rate of the XN-9000 for the blast/abnormal lymphocyte alarm (84/260,32%) was significantly lower than that of the XE-2100 (107/260,41%) (x2 =4.38,P<0.05).Conclusion Compared with the XE-2100,the XN-9000 WPC channel can improve the overall efficiency of the blast/abnormal lymphocyte alarm,reduce the false positive rate and the review rate,and then improve the efficiency of clinical laboratories.

8.Clinical application of CCND1 gene expression detection with fluorescent quantitative polymerase chain reaction in the diagnosis of breast cancer

Objective To use fluorescent quantitative polymerase chain reaction (FQ-PCR) todetect the level of cyclin D1 gene (CCND1), and to investigate its clinical application in the diagnosisand monitoring of breast cancer. Methods FQ-PCR was applied to detecting the expression level ofCCND1 gene in peripheral blood from 15 healthy females, 30 patients with benign breast disease and81 patients with breast cancer with β2-microglobin as the internal control. Results There were no sig-nificant difference in the level of CCND1 expression between healthy control group and benign breastdisease group (P>0. 05). The level of CCND1 expression was significantly higher in breast cancergroup than that in healthy control group and benign breast disease group (P<0.05). The difference inβ2-microglobin was statistically insignificant among three groups (P> 0. 05). The positive rate was45.7% in breast cancer group, and 0 in benign breast disease group. Conclusion FQ-PCR is a rapid,sensitive and specific method for quantitative determination of CCND1 gene, which may he used as anavailable tool for the diagnosis, monitoring of therapeutic effect and metastasis as well as prognosis e-valuation of breast cancer.

Objectives To study the effect of Lactulose oral solution combined with polyethylene glycol electrolyte powder on the intestinal cleanness.Methods One hundred and twenty patients undergoing electronic colonoscopy were divided into the experiment group and the control group,60 cases in each group.The experiment group orally took Lactulose oral solution and polyethylene glycol electrolyte powder 1d before the check,while the control group polyethylene glycol electrolyte powder 1d before the check.The two groups were compared in terms of intestinal cleanness and adverse reactions.Results The experiment group was significantly better than the control group in intestinal cleanness(P>0.05)and there were no significant difference has in adverse reactions(P>0.05).Conclusion The combined use of polyethylene glycol electrolyte powder and Lactulose Oral Solution may improve the intestinal cleanness and reduce the incidence of adverse reactions.
